News: Case Management Information

Planning Ahead (Excerpt reprinted from the Alzheimer’s Association Caregiver Center, www.alz.org)

It's important for everyone to plan for the future, but legal plans are especially important for a person with Alzheimer's disease. The sooner planning starts, the more the person with dementia may be able to participate. Why plan ahead? Making legal plans in advance is important for several reasons: Early planning allows the person with dementia to be involved and express his or her wishes for future care and decisions. This eliminates guesswork for families, and allows for the person with dementia to designate decision makers on his or her behalf. Early planning also allows time to work through the complex legal and financial issues that are involved in long-term care. Legal planning should include: · Making plans for health and long-term care · Making plans for finances and property · Naming another person to make decisions on behalf of the person with dementia Legal capacity Legal capacity is the ability to understand and appreciate the consequences of one's actions and to make rational decisions. In most cases, if a person with dementia is able to understand the meaning and importance of a given legal document, he or she likely has the legal capacity to execute (to carry out by signing) it. The requirements of legal capacity can vary from one document to another. A lawyer can help determine what level of legal capacity is required for a person to sign a particular document.

4

Before a person with dementia signs a legal document: · Talk with the person. Find out if the person with dementia understands the legal document and the consequences of signing it. · Make sure the person knows what is being explained and what he or she is being asked to do. · Ask for medical advice. If you have concerns about the person's ability to understand, ask for medical advice. A doctor may be able to assist in determining the level of a person's mental ability. · Take inventory of existing legal documents. Verify whether living wills, trusts and powers of attorney were signed before the person was diagnosed. The person may no longer remember having completed them. Even if legal documents were completed in the past, it is important to review them with another person for necessary corrections and/or updates. Meeting with a lawyer You can complete certain legal documents without a lawyer, but getting legal advice and services from an attorney who specializes in elder law can be especially helpful. If you already have a lawyer, he or she may be able to refer you to an attorney that specializes in elder law. Otherwise, there are several resources available to help you locate elder law services. If you meet with a lawyer, be sure to discuss these three key issues and any other concerns you may have: · Options for health care decision making for the person with dementia · Options for managing the person's personal care and property · Possible coverage of long-term care services, including what is provided by Medicare, · Medicaid, veteran benefits and other long-term care insurance For more information, please see the complete article online at https://www.alz.org/care.

News: Case Management Information Planning Ahead continued… A note from Senior Center staff We at the Stoughton Area Senior Center work with both caregivers and their loved ones who have been diagnosed with Alzheimer’s disease or a related dementia. We see a lot of people struggling with tough situations and caregivers getting increasingly fatigued and overwhelmed. Often times, the health of the caregiver deteriorates as a result of the stress. As soon as a loved one receives a diagnosis of any kind of irreversible dementia, planning and preparing for the future is essential. We cannot overemphasize this enough. The more prepared people can be for future care and legal planning, the better the outcomes for all. For more assistance, please call any of the phone numbers noted below: Alzheimer’s Association www.alz.org, phone number 608-203-8500 or 1-800-272-3900 Alzheimer’s and Dementia Alliance of Wisconsin www.alzwisc.org, phone number 608-232-3400 or 1-888-308-6251 Aging and Disability Resource Center (ADRC) of Dane County 608-240-7400. What a Trip to the Emergency Room Means for an Older Person Reprinted by permission from GWAAR Legal Services

When we’re young, a trip to the emergency room is usually for an unexpected illness or injury, and most of us take getting back to normal for granted. For older individuals, however, a trip to the ER can be a much different experience. Consider these facts: · 2/3 of seniors who go to ER are never admitted to the hospital –they return directly home. · 6 months after visiting ER seniors are 14% more likely to have acquired a disability (i.e. no longer being able to bathe, dress, shop, use stairs, or manage finances by themselves). · The extent to which an older person gets up and about after an ER visit is reduced drastically for at least a year without a full recovery. · The leading cause of ER visits in older persons is a fall and injury. One theory of why ER visits become so critical for older patients is that many may have been coping adequately, but an ER visit acted as a tipping point and now they need more help. Another theory is that seniors who fall and injure themselves may become afraid of falling again and limit their activities, which leads to deterioration. Finally, underlying vulnerabilities (depression, dementia, or delirium) may go undetected, leaving older adults susceptible to the ongoing impact of these conditions. Starting in Feb. 2018, the American College of Emergency Physicians is starting a program to certify ERs in geriatric competence to make sure seniors leave the ER safely. Medical staff will be educated in the principles and practice of geriatric care; assessing seniors to determine their degree of risk; screening older adults deemed at risk for cognitive concerns, falls and functional limitations; performing a comprehensive medication review; making referrals to community resources such as Meals on Wheels; and supplying an easily understood discharge plan. Kitchen Corner Senior Snacks To Have On Hand Cherry tomatoes -- High in antioxidants. Celery sticks -- High in vitamin C, good for blood pressure, cholesterol, and preventing cancer. Avocado -- High in vitamins, minerals and lutein, which is recommended to prevent vision problems and macular degeneration. Hummus -- A flavorful chickpea dip/spread often made with olive oil, lemon juice, garlic, or salt. Dried apples -- Get them without preservatives, and lowor no-sugar. They're also fun to toss in cereals, or even in rice or other cooked grains. Raw nuts -- Unprocessed maintains nutrients; many have high antioxidants. Best nuts: almonds, brazil nuts, cashews, hazelnuts, macadamias, pecans, pine nuts, pistachios, walnuts. Nut butters -- Get unprocessed, fresh nut butters that maintain their nutrients. Natural pumpkin seeds -- Raw seeds are very high in vitamins and minerals. Granola bars -- Natural granola bars are low in fat, and have a good balance of health carbs, protein and sugars, and other nutrients.. Whole grain crackers -- Try to find low-fat and reduced salt. Whole grain tortilla chips -- Baked is always best. And serve with a little salsa, of course. Popcorn -- low salt, and low- or no-butter. Trail mix -- low or no sugar types are ideal. Brown rice cakes -- Brown rice is much healthier than white rice because it is unprocessed and contains more nutrients. String cheese sticks -- Low-fat, low-salt version Yogurt -- Plain, unsweetened, which you can then dress up with berries or natural jam, or natural granola on top. Spreadable cheese -- You can get natural cheeses in small tubs. Great to put on vegetables! Natural popsicles -- Make them at home by pouring natural juice with no added sugar, into a popsicle form, add a stick, and freeze. You can also add in a piece of fruit.
