The newsletter may be mailed to you for a cost of $8.00/year, or viewed on our website or picked up at no cost. __________________________________________

File of Life Emergency Packets Free wallet-sized and/or magnetic File of Life folders for your refrigerator are available at the Senior Center to make lifesaving information accessible to emergency personnel. __________________________________________

Resource Library The Senior Center has a library of resource publications, including books, brochures, DVDS and other media available to be rented out by the public.

Loan Closet

Serving: Stoughton, Towns of Albion & Dunkirk

Equipment such as wheelchairs, commodes, walkers and other items may be checked out on a short-term basis for a minimum donation of $5.00 to those residing in our case management service area (Stoughton, Town of Albion and Town of Dunkirk).

The Case Management program links older adults with services available to help them remain as independent as possible in their homes and community. Our Case Managers coordinate, evaluate and advocate for such services (e.g. medical, legal, housing, financial, food and transportation options) to meet each individual’s needs and preferences. Referrals to Case Managers remain confidential. Case Managers share information only with necessary provider agencies for the purpose of providing services and only with the signed consent of the care recipient. Persons may remain anonymous when making a referral for another person. An appointment may be made at the Senior Center or in your home. If you live in Dunn, Pleasant Springs or Rutland case management services are provided by the McFarland Senior Center, (608) 838-7117. _________________________________________

Nutrition Site The Senior Center serves lunch every weekday at 12:00 PM on a donation basis to people over 60 years of age. Participants must make a reservation by 1:00 PM the day before the intended visit.

Foot Care Clinic Stoughton Hospital Home Health offers monthly foot clinics at the Senior Center. Check our newsletter or website www.ci.stoughton.wi.us/senior for specific dates and rates. Participants must contact the Senior Center to make an appointment. _________________________________________

Blood Pressure Screens Blood pressure screening is available each month. Check our newsletter or website www.ci.stoughton.wi.us/senior for specific dates and rates. No registration needed. _________________________________________

Support Groups The Senior Center hosts the following support groups: Caregiver, Grief, Low Vision, Multiple Sclerosis, Older Adult Alcoholics Anonymous, Al-Anon and Parkinson’s disease. Please see pages 20 – 21 for more information, along with other area groups.

Home Delivered Meals Meal delivery is available to homebound older adults in the Stoughton School District every weekday. Participants must call before 1:00 PM the day before to request meals to be delivered at home.

FINANCIAL SERVICES_______________________________________________ Care Wisconsin’s Partnership Program coordinates health and long-term care, including all medical care, for eligible Dane County seniors. Partnership covers all prescription drugs, nursing care, transportation, in-home care, hospitalizations and other services. The Partnership care team—including a care manager/social worker, a registered nurse, and a nurse practitioner—coordinates with local doctors and service providers to support area enrollees. Funding is provided by Medicaid and Medicare for eligible enrollees. Caring for the Caregiver The Area Agency on Aging of Dane County coordinates the National Family Caregiver Support Program. Community Action Coalition The Community Action Coalition (C.A.C.) provides financial assistance to residents in Dane County in need of security deposits or eviction prevention funds. C.A.C. only assists those households seeking new housing due to eviction, non-renewal of lease, a change in family size, or another emergency situation.

An appointment may be held at the Senior Center or in the resident’s home.

Volunteer Income Tax Assistance Stoughton’s Volunteer Income Tax Program (VITA) assists older adults and low income individuals and families in filing their state and federal income taxes. Assistance with claiming Wisconsin Homestead Credit is also included. There is no cost for this program. Stoughton Hospital Cardiac Rehabilitation & Wellness Center 900 Ridge Street, (608) 873-2314 Phase 3 Cardiac Rehab Program Once participants have finished their Cardiac Rehab (Phase 2 Program), they can continue to exercise at Stoughton Hospital under the supervision of medical professionals through the Phase 3 Cardiac Rehab Program. The Community Action Coalition (C.A.C.) provides financial assistance to residents in Dane County in need of security deposits or eviction prevention funds. Home Health (608) 873-2366 Registered nurses are on call 24 hours/7 days a week to provide care for acute and chronic disease conditions, case management of care, medication monitoring, wound care, IV therapy and education regarding chronic disease management. Other services offered include personal care, social services and physical, occupational and speech therapy.

Rehabilitation & Sports Medicine Serves patients requiring services of physical therapy, occupational therapy and speech therapy.

Inpatient Rehabilitative Care (608) 873-2217 Skilled nursing care or rehabilitation after a three day or longer hospitalization. To be eligible, a patient can be hospitalized at Stoughton Hospital or another hospital and then enter the inpatient rehabilitative care program. Customized care plans are designed to meet individual needs, enabling patients to return home as soon as possible. Journey Mental Health Center

Agrace HospiceCare Inc. is a nonprofit, community-based hospice dedicated to providing physical, emotional and spiritual support to patients and families dealing with life-limiting illnesses. Agrace offers comprehensive inhome, inpatient, respite, and residential care, and also provides hospice services in skilled nursing and assisted living facilities throughout Agrace’s service area. Mobile Outreach to Seniors Team (M.O.S.T) M.O.S.T counsels older adults who are experiencing changes and emotional stresses. The goal is to improve life satisfaction and to resolve areas of unrest or conflict. M.O.S.T. also offers the following services:  Care plans that focus on the individual’s concern  Supportive counseling and education  Advocacy  Support for the caregiver  Partnership with senior centers for community services An appointment may be held at the Senior Center or in the resident’s home from Mondays –Thursdays, 8:30 AM -4:30 PM. Independent Living Telephone Reassurance This program is staffed by volunteers who want to promote interaction and reduce loneliness through regular telephone calls to older or disabled adults. Volunteers can also be a reliable source for making medication or appointment reminder calls based on your personal need for socialization or safety. Peer Support Program The Peer Support Program is a proactive approach intended to engage older adults who are experiencing isolation and/or loneliness. The project empowers trained volunteers to use their skills to interact with at-risk older adults, while offering compassionate outreach. They are good listeners who bring a little of the outside world into that of their peer, enrich their lives via friendship, and may even foster an increase in independence. Disabled American Veterans (DAV) (608) 280-7019 Volunteers escort veterans, who have no other means of transportation, to their doctor appointments. Passengers must be ambulatory and a veteran. There is no charge, but reservations are at on a first come, first serve basis. Appointments must be made between 9:00 am – 2:00 PM. ___________________________________________
