Medical and Prescription Drug Insurance Helpful Information The CMC medical and prescription drug program is designed to offer employees choice of quality health care options. We encourage employees and dependents to establish a relationship with a primary care provider (PCP) or practice to oversee their health and coordinate their care.

Medical Insurance Premiums The employee premiums that are deducted from your paycheck are based on the coverage level and your base annual salary. However, premiums will not differ based on the plan you choose.

Preventive Care Services

As we focus on helping employees maintain good health, we continue to offer in-network medical preventive care services covered at 100%, with no deductibles, co-pays, or co-insurance. Examples of preventive covered services include routine: mammograms, colonoscopies, labs, immunizations, well-child care for babies and children, and physicals for adults. For a more complete list of covered preventive care services, please refer to the Summary Plan Description. The list is also available at IRS.gov or by contacting Health Plans, Inc.

Prescription Drugs

You must enroll in a CMC medical plan to participate in the prescription drug coverage. The cost of prescription drugs continues to increase year-after-year, particularly the cost of specialty drugs. Employees may save money by purchasing a 90-day supply of maintenance drugs. The 90-day supply of drugs may only be purchased through the DHMC Pharmacy (in-person or mail-order) or in person at CMC Pharmacy. If not available through DHMC or CMC, 90 day prescriptions may be obtained through Optum Rx mail order. Employees may find additional savings by utilizing prescription coupons made available at various websites such as GoodRx.com, Target.com and Walmart.com.

Preventive Drug List

Preventative drug coverage is an integral part of a comprehensive health care plan which includes preventive drug therapy. Preventive medications are those prescribed to prevent the occurrence of a disease or conditions for those individuals with risk factors, or to prevent the reoccurrence of a disease or condition for those who have recovered: conditions such as high blood pressure, high cholesterol, diabetes, asthma, osteoporosis, and heart attack. In our ElevateHealth HSA plan and our Choice Health Plan, drugs on the Preventive Drug List will by-pass the deductible and only require you to pay co-insurance. Optum Rx manages this program and the list of drugs included in the Preventive Drug Program.

Prescription Advantage

The prescription Advantage Program provides discounted rates on certain approved prescription drugs (those on the Prescription Advantage Drug List) purchased at the DHMC Pharmacy, whether purchased in person or through the mail. The DHMC Pharmacy will mail these prescriptions to you at no cost. This is a special program offered through the DHMC Pharmacy. Employees and dependents are eligible for this program, regardless of participation in a CMC Medical Plan.

Diabetes Management Program All employees and their dependents who are on diabetic medications and enrolled in a CMC medical plan will have zero copay for any diabetic drug that is covered on the OptumRx formulary and is filled at the DHMC Pharmacy or Cheshire Medical Center Retail Pharmacy. This includes all diabetic test strips, lancets, insulin, metformin and other diabetes medications. In addition, diabetic medical supplies obtained through a participating Durable Medical Equipment (DME) provider will be covered by the CMC medical plans with a zero cost to the employee. This includes supplies for insulin pumps such as reservoirs and infusion sets.


Low Premium Medical/Prescription Drug Plans ElevateHealth is an insurance partnership of Dartmouth Hitchcock, Elliot Health System, and Harvard Pilgrim Health Care. Employees who enroll in one of the two ElevateHealth plans may expect care from a high quality, close-knit network of providers focused on keeping you healthy and well. ElevateHealth Providers and facilities are located where most CMC employees and their families are seeking care today; primarily in New Hampshire and Vermont. (Providers outside of the ElevateHealth Network are not covered, except in case of Emergency Care). Dependents living outside of the ElevateHealth Network have expanded in-network coverage using the Harvard Pilgrim National Network once the Out-of-Area Dependent Form is completed and submitted to Health Plans, Inc. Since we understand that the ElevateHealth Plans are not a “one size fits all�, we have the Choice Health HSA Plan offering to provide a full network with national coverage for employees and their family members.

Additional Premium May Apply Tobacco Use Premium The Cheshire Medical Center medical plans require employees to affirm whether they, or any of their covered dependents, use tobacco. Covered dependents include spouse or any children enrolled in the plan. Employees may be subject to a tobacco surcharge of $15.00 per bi-weekly pay period. It is important to note that the surcharge will be $15.00 per pay period regardless of the number of family members who use tobacco. If, at any point throughout the year, the tobacco user(s) stops using tobacco products and are tobacco free for 30 calendar days, the covered employee will be required to sign an affidavit. If an employee or his/her covered dependents are unable to achieve tobacco-free status for medical reasons, physician documentation is required in order to waive the surcharge. Tobacco products include: cigarettes, pipes, cigars, snuff or chewing tobacco, and electronic cigarettes.

Spousal Surcharge If you choose to cover your spouse under a Cheshire Medical Center medical plan, and your spouse has access to group-sponsored health insurance coverage through his/her own employer, your health care contributions will be twice the standard premium for the coverage you choose to enroll in.

Health Savings Account (HSA) The HSA is a tax-free savings account that can be used to pay for qualified out-of-pocket medical, dental or vision care expenses, as well as to save for future health care costs. CMC provides a base contribution to each eligible employee’s HSA account enrolling in the ElevateHealth HSA Plan or the Choice Health HSA Plan. Employees may opt out of the HSA contribution. The Health Savings Account will be administered by HealthEquity, Inc. For a list of eligible expenses visit HealthEquity.com.

HSA Eligibility 1. Neither you or your spouse are enrolled in a Health Reimbursement Account (HRA) or full purpose Healthcare Flexible Spending Account (FSA) 2. Not claimed as a dependent on another’s tax return. 3. Not enrolled in Medicare. 4. Not insured under any other medical plan that is not a High Deductible Health Plan (HDHP). 5. Have not received VA benefits (Medical or Rx) during the past 3 months. Upon turning age 65 and enrolling in Medicare A, or being in receipt of Medicare due to one of Medicare’s qualifying eligibility rules, an employee is not eligible to receive or contribute to the HSA. Any funds still in the account are accessible for qualified expenses. However, if a member defers his/her enrollment in Medicare s/he is able to participate in the HSA benefit. During the Open Enrollment period employees must attest that they meet eligibility requirements in order to receive a contribution.

Contributions to Your HSA Account CMC Contribution To help you pay for some of the expenses associated with the ElevateHealth HSA Plan or the Choice Health Plan, CMC will make a lumpsum contribution to each eligible employee’s HSA, based on the coverage level elected and the employee’s annual base salary. CMC will contribute the following tax-free contribution for 2020: Base Annual Salary HSA Single/HSA Family $0 - $34,999 $800/$1,600 $35,000 - $74,999 $600/$900 $75,000 + $400/$700 Employees beginning employment after January 1 will receive a pro-rated lump-sum contribution in the first pay period of the month following your benefits eligibility.

Your Contributions In addition to CMC’s contribution, you may elect to make additional pre-tax contributions to your HSA through payroll deductions, subject to IRS annual limits. For 2021, the limit is $3,600 for employee only coverage and $7,200 for all other coverage levels. These amounts represent the total amount (your contributions plus CMC’s contribution) that can be deposited into your account in 2021. Employees who are age 55 or older may also make additional “catch-up” contributions of up to $1,000 to their HSA. Remember, the amount you contribute to your HSA is pre-tax and the dollars that are deposited in your HSA belong to you, whether deposited by you or CMC. The HSA does not have a “use it or lose it” rule, so you can carry-over your balance from year-to-year and you can take the account with you if you leave or retire from CMC. You may start, stop, or change your HSA payroll contribution amount at any time during the year through the benefit portal.

Health Savings Account (HSA) continued Penalties for Non-Eligible Health Care Expenses under an HSA The IRS governs the provisions of the HSA and provides guidelines on what is considered an eligible health care expense. If you use your HSA funds to pay for non-eligible health care expenses, you may be subject to a 20% penalty and your non-eligible expense will be treated as taxable income. In the event of an IRS audit, employees must show receipts of eligible health care expenses reimbursed. For more information, please refer to IRS.gov/pub969. Remember: If elected, it is your responsibility to keep accurate records to prove to the IRS that distributions were for qualified medical, dental or vision expenses. Save all documentation for IRS auditing purposes. IRS Form 8889 is required to be filed with your taxes. Forms are available online at IRS.gov.

Limited Purpose Flexible Spending Account (LP-FSA) In addition to the HSA account, employees who elect the Elevate Health HSA Plan or the Choice Health HSA Plan may also participate in the LP-FSA. The LP-FSA may be used to pay for eligible dental or vision care expenses for you and your eligible dependents. Medical expenses may not be applied towards the LP-FSA. The CMC 2021 maximum limit you may contribute to your account is $2,750.

Health Reimbursement Account (HRA) The HRA is a savings account that can be used to pay for eligible out–of-pocket health care, dental, or vision expenses. Only CMC can contribute to an HRA and will provide a contribution for employees who enroll in the ElevateHealth HRA Plan. Once your HRA is activated through HealthEquity, you will receive a personalized debit card to use when paying for your eligible expenses. You will also have access to Health Equity’s on-line system to help you manage your HRA. The balance in your HRA account accumulates and rolls-over each year if you continue to enroll in the ElevateHealth HRA Plan. HRAs are employer based accounts so you will not have access to the HRA if you enroll in a different plan, leave, or retire from CMC.

Contributions to Your HRA Account CMC Contributions: To help you pay for some of the expenses that will be subject to your health care plan deductible and co-insurance, if you enroll in the ElevateHealth HRA plan and are eligible for HRA contributions, CMC will make a contribution to your HRA based on the coverage level you elect and your annual base salary. Flexible Spending Account (FSA) Full Purpose Medical FSA (FP-Medical FSA)

The FP-Medical FSA is a pre-tax savings account available to employees who waive health care coverage, enroll in the Cheshire Medical Center ElevateHealth HRA Plan, or are ineligible to participate in the HSA contribution. The FP-Medical FSA can be used to pay for eligible health care, dental, or vision care expenses that are not covered by insurance for you and your eligible dependents. The maximum amount you may contribute to your FSA is $2,750 for 2021.

Limited Purpose Medical FSA (LP-Medical FSA)

The LP-Medical FSA is a pre-tax savings account available to employees who are enrolled in the Elevate Health HSA or Choice Health Plans. The LP-Medical FSA can only be used to pay for eligible dental or vision care expenses but cannot be used to pay for eligible health care expenses. This allows for additional pre-tax savings for those who will be incurring eligible dental and vision expenses in the calendar year while allowing the HSA funds to be preserved for medical expenses only. The same annual limits and deadlines for submission that apply to the FP-Medical FSA apply to the LP-Medical FSA as well as the “qualified life event” requirements to make a change to the LP-Medical FSA.

Dependent Care FSA (DC-FSA)

The DC-FSA is a pre-tax savings account that may be used to pay for eligible elder and/or child care expenses so that you can be gainfully employed. You may only use this account if you utilize child care or elder care services so that you and your spouse can work, obtain gainful employment, or attend school full-time. It is important to note that your child care or elder care provider must furnish you with his/her Social Security Number (SSN) or Tax Identification Number (TIN) in order to receive reimbursement for your expenses. IRS regulations limit the amount you may contribute to a DC-FSA to $5,000 for your family (or $2,500 if married and filing separate tax returns) per calendar year. In order to qualify for reimbursement, expenses must be incurred for the care of “eligible dependents” who the IRS defines as: • A child under the age of 13, or a child of any age who is physically or mentally incapable of self-care. • Any other dependent (including your same sex spouse) who is physically or mentally incapable of self-care and who meets specific criteria. For more details about who qualifies as eligible dependents under the DC-FSA, refer to IRS Publication 503, titled Child and Dependent Care Expenses, which is available by calling the IRS at 800-829-1040 or at IRS.gov. Note: You can only participate in the DC-FSA if you earn less than the highly compensated limits. For the 2021 plan year, an employee who earns more than $130,000 in 2020 is considered a highly compensated employee.

Health Equity administers both the medical and dependent care Flexible Spending Accounts. Participants in the Medical FSA will receive a personalized debit card from Health Equity to use when paying for eligible health care expenses.

Dental Insurance Cheshire Medical Center offers two Dental Plan options for you and your eligible dependents through Northeast Delta Dental: • Basic Dental • Enhanced Dental Dental hygiene can impact your overall physical health. Practicing good oral care, such as brushing and flossing daily and visiting your dentist on a regular basis, are important steps toward keeping your mouth free from the build-up of bacteria that can lead to oral infections. Preventive screenings (dental check-ups and cleanings) can also help detect signs of other diseases.

Enhanced Dental Carryover Benefit Northeast Delta Dental offers a “Double-Up Max” carry-over benefit feature on the High Option Dental Plan.

To Qualify • Members are enrolled in the Enhanced Dental Plan. • Northeast Delta Dental must have paid a claim for either an oral exam or a cleaning during the 2019 calendar year. If enrollees don’t receive one of these services, they will be ineligible to accumulate dollars for future use. • A member’s paid claims during the calendar year cannot exceed a threshold of $500. • Accumulated amounts may only be used after the current annual maximum has been exhausted. • Carryover benefit amounts cannot be used toward lifetime orthodontic benefits.

How it Works With this feature, enrollees may double their annual maximum by accumulating $250 a year in additional benefits for use in future coverage periods. When a dental procedure is needed that costs more than the allowable annual maximum, the accrued carryover benefit dollars can help make up the difference.

Cheshire Medical Center Tax Sheltered Annuity Plan - 403(b) Eligible employees will be enrolled in the 403(b) plan which is intended to provide employees an incentive to save for retirement through a matching contribution, as well as offer retirement benefits that are consistent and spread evenly over their career with Cheshire Medical Center. You may save up to 100% of your income, subject to IRS annual maximums. If you are age 50 or older, you may also make “catch up” contributions, subject to IRS annual maximums. Your contributions and any associated investment earnings are vested immediately. • Employees are automatically enrolled with a 4% savings rate • Employees can opt out or reduce/increase his/her savings rate at any time • Each year employees’ savings rate will increase by 1%, up to a maximum of 10%, unless the employee opts out Cheshire Medical Center contributions provided each pay period: CMC base contribution of 4% of pay

Disability Insurance Short-Term and Long-Term Disability Insurance Cheshire Medical Center will provide Short-Term Disability (STD) and LongTerm Disability (LTD) insurance at no cost to you through Sun Life Financial. Both STD and LTD provide income protection for non-work related injuries or illness. STD and LTD disability payments may be offset by Social Security Disability, Workers’ Compensation, pension payments, or any other group disability insurance payments that you are eligible to receive. Both STD and LTD payments are considered taxable income. Sun Life Financial determines eligibility and medical necessity for STD and LTD. CMC will provide the following STD and LTD benefits for eligible employees at no cost to you:

Short-Term Disability (STD) Insurance • Staff: 70% of your base salary, for a maximum of 24 weeks • Advanced Practice Providers: 100% of your base salary, up to 26 weeks

Long-Term Disability (LTD) Insurance • 50% of your base salary, for a maximum of $15,000 per month Staff STD benefits begin following a 14-day elimination period. You will use available earned time to provide income during this elimination period. In order to be eligible for STD coverage, you must begin STD on the 15th full day of missed work. Sun Life Financial will determine eligibility and medical necessity for LTD. If approved, LTD payments begin immediately following the date STD payments end.

Long-Term Disability (LTD) Insurance “Buy-Up” Options In addition to the LTD benefits provided to you at no cost, CMC’s benefit program offers two “buy-up” options. If you choose one of these options, you are responsible for paying the entire cost of the “buy-up” coverage through after-tax payroll deductions. In addition, Evidence of Insurability (EOI) may be required. The buy-up options are as follows:: • 60% of your base salary, to a maximum of $15,000 per month • 66 2/3% of your base salary, to a maximum of $15,000 per month

Life Insurance Staff Life and AD&D Insurance Benefits* CMC provides Basic Life and Accidental Death & Dismemberment (AD&D) insurance which is administered by Sun Life Financial. This protection provides a benefit for you and your family if you die or become seriously injured while you are employed at CMC. CMC provides the following Life and AD&D Insurance benefits for eligible employees at no cost to you:.

Basic Life Insurance • 1 x annual salary up to $200,000.

Accidental Death and Dismemberment (AD&D) Insurance • 1 x annual salary up to $200,000. *under the plan provisions, employee Basic Life/AD&D insurance coverage reduces by 50% at age 70.

Staff Supplemental Life and AD&D Insurance** In addition to the Basic Life and AD&D insurance provided to you at no cost, CMC’s Benefits program offers you the opportunity to purchase additional Supplemental Life and AD&D insurance for yourself and your eligible dependents. Any changes you make to your dependents’ Life Insurance while on Leave of Absence will not take effect until you are actively at work. It’s important to know that some levels of coverage will require you to provide evidence of your good health or Evidence of Insurability (EOI).

Staff Supplemental Life Coverage Options • Up to five times your annual salary, in one-half salary increments, up to a maximum of $1,500,000 Each Open Enrollment period, you may increase your coverage by one times your annual salary, without EOI, up to a maximum of $500,000. Newly hired or newly benefits eligible employees may elect up to $500,000 of Supplemental Life/AD&D with no EOI.

Spouse Life/AD&D Insurance Coverage Options • Up to $100,000 of coverage, in $5,000 increments, subject to EOI Each Open Enrollment period, you may increase your coverage for your spouse in increments of $5,000, to a maximum of $50,000, without EOI. Any amount elected over $50,000 is subject to EOI. Newly hired or newly benefits eligible employees may elect up to $50,000 of coverage for their spouse with no EOI.

Additional Benefits Earned Time Employees who are scheduled to work 20 hours per week, as reflected in their personnel file, are eligible for the Earned Time benefit. Earned time is used for vacation, holiday, and sick time. The amount of earned time an employee accrues is based on various factors including, but not limited to, length of service and hours paid. Earned time accrual can be carried over from year-to-year up to the allowable cap.
