'The Wait is Over' at Aventura Hospital’s Emergency Room ER wait times now average 5 minutes

"When seconds count… count on us." The pledge Aventura Hospital and Medical Center's Emergency Room is living up to, is helping to save lives and prevent complications. Aventura Hospital CEO Heather J. Rohan is proud to report, "We improved from an arrival-to-bed time of 104 minutes in December of 2006, to a current average of just 5 minutes." To speed up patient care, Aventura Hospital wasn't slow to act on implementing positive initiatives spearheaded by Jullie B. Millar, R.N., Director of Emergency Services and Dr. Roman Skylar, Emergency Room Medical Director. According to Dr. Skylar, "change for the better" is evident on a myriad of levels, including staffing, patient processes and technology combining to create a "Rapid Medical Evaluation" system. "When patients arrive at the ER, they're brought back to an available room;" Dr. Skylar said. "At the time a patient is being triaged, they're also being seen by a physician/mid level provider; blood is being drawn and an EKG is being done. By having the ability to carry out all these steps at the same time, the patient's wait time is shortened and valuable minutes are saved." "Teamwork and dedicated staff are key in effectively implementing change;" stated Julie Millar. "Our nurses, ancillary staff and medical providers all work together in achieving a common goal, providing safe quality patient care and improving customer satisfaction." Aventura Hospital's state-of-the-art imaging equipment is also making it possible to obtain more accurate diagnoses in a shorter timeframe. "Images can be shared in an integrated manner so different hospital departments can view them at the same time," Dr. Skylar said. "The images can be sent electronically to specialists connected to the hospital network, even though they may be at a remote location." At Aventura Hospital, heart attack patients are being treated in record time thanks to the hospital's participation in the STEMI Program. Paramedics will call the Emergency Room and activate the Cardiac Alert team before the patient arrives to the hospital. "We've reduced our door to balloon time, which is the time from the moment a patient arrives at the door of the emergency room with chest pain to the moment the

Sweetening the bailout with pork

Taxpayers are justifiably wary of seeing $700 billion of their hard-earned tax dollars bailing out the bad debts of Wall Street bad apples. When most Americans invest poorly, they have to accept the financial consequences. It seems fundamentally unfair to take care of Wall Street before the rest of us. At the same time, people don't want to see their retirement accounts, 401(k) plans, or other investments lose value; they want their kids to be able to get student loans and car loans; and they don't want to see the credit system collapse. Very few people are happy about the bailout bill, and the options are difficult to parse. Worse, the Senate, in the midst of a debate over the historic bailout package, pulled out an old bag of tricks: piling billions of dollars of unrelated legislative provisions into the package and daring the House to reject the bailout again. Many of these provisions are tax breaks that have been waiting in the wings for months, hoping for a legislative train to leave the station. So instead of an enormous $700 billion bailout, Congress has now passed a bill with more than $800 billion in spending, including tax benefits for arrow manufacturers, rum producers in Puerto Rico and the Virgin Islands, and film and television producers. With the addition of the tax break legislation, Congress is using this national economic emergency to smooth the way for controversial legislation. Throughout the process, the House and Senate have had very different approaches to this bill. The House demanded that the bill's costs be offset. The Senate has not.
In the first attempt by a foreign country to break the blockade of Gaza, a Libyan freighter carrying 3,000 tons of essential humanitarian aid set sail for the impoverished coastal strip. On the shore, Gazans assembled to welcome its arrival, a muchneeded gesture of hope and relief for Gaza's 1.5 million residents, caged in on all sides in one of the most densely populated areas on earth, allowed only the most basic foodstuffs and regularly bombarded with ordnance from the skies. As it approached Gaza's coastal waters on December 1, the ship was intercepted by the Israeli navy and forced to turn back. The Libyan shipment was an effort to follow the lead of three prior ships led by international peace activists that successfully broke the siege of Gaza. Israel allowed the peace activists through after defense officials calculated that obstructing their passage would have amounted to a public relations disaster. But as the siege of Gaza enters its 18th month, and the situation there becomes increasingly dire with no sign of abatement, most Western media no longer view it as newsworthy, save for the occasional reporting on periodic United Nations appeals for action each time Israel tightens the shackles an additional notch. Compounding the problem, Israel has severely restricted media access to Gaza. When the most severe closures are imposed, journalists are barred from entering the Strip to cover its effects. After more than four decades of occupation, Palestinians have become accustomed to the normalization of the absurd. They have grown used to waiting for hours under the sun at checkpoints on their way to work and school, to circumventing roadblocks, to living under curfew, to arbitrary arrests and military raids. But the international silence on this man-made humanitarian crisis indicates that the rest of the world has also come to assume that a certain level of suffering is an acceptable norm for Palestinians. After failing to break the will of Gazans during 40 years of occupation, Israel is now resorting to a war of attrition, or occupation on the cheap. As Israel strangles the livelihood of the Gazans, the international community foots the bill, provid-
ing just enough aid to keep the Gazans alive, albeit malnourished. The entire population of Gaza has been reduced to a mere subsistence existence, half of them reliant on U.N. food handouts that have been slow in coming, their aspirations trampled on and their future clouded out by existential concerns of mere day to day survival. How is it that the international community can sit by idly while an entire population is enclosed in a ghetto and starved into submission? Israel says that the objective of its siege is to deter by punishment Palestinian militants from firing Qassam rockets at Israeli towns. But this strategy has failed to bring security to Israel. Qassam rockets continue to be launched and Hamas is strengthening its hold on the Gaza Strip, not reducing it. What sort of peace and security can this policy of starvation and economic strangulation bring Israel? According to the United Nations, the majority of Gaza's children suffer mental distress due to abnormal living conditions. Men face degradation, as they are unable to provide for their families during the economic standstill. The next generation, denied opportunity, hope and recognition of their political aspirations are learning hate first hand. In the words of senior U.N. humanitarian affairs coordinator Jan Egeland, Gaza is “a human time bomb ticking.” The longer this manufactured humanitarian catastrophe is allowed to continue, the further away any possibility of peace will become. If the situation is to improve, the international community must pressure Israel to lift the siege of the Gaza Strip and to engage its adversaries through negotiation. As Israel's greatest ally, the United States can play an important role in this regard. With a new U.S. administration taking office in January, there will be an opening for a new approach. Some of the questionable items: Tax break for certain wooden arrows designed for use by children: The legislation suspends a 39-cent excise tax on arrows made from natural wood, a provision that has been backed by Senators Ron Wyden (D) and Gordon Smith (R) of Oregon; the tax relief would benefit Rose City Archery in Myrtle Point, Ore., and a handful of other companies. The cost is estimated at $2 million. Race Track Tax Break: A $100 million provision that allows NASCAR racetracks to write off capital investments, such as new concession stands and repaving tracks, over a seven-year period. Film and Television Productions: Hollywood seems to be one sector of our economy that continues to prosper these days, but film and television production facilities won an extension of a special tax break. This provision will cost taxpayers $478 million over the next ten years. Perhaps senators had a little too much rum in the daiquiris they were sipping while considering this legislation - most likely rum from the Virgin Islands or Puerto Rico that will also receive a tax break extension at a taxpayer cost of $192 million. The $700 billion bailout itself is hard for taxpayers to stomach, but adding $110 billion dollars of unrelated tax breaks and spending to this legislation was irresponsible and unnecessary. In these times of historic economic turmoil, lawmakers had an obligation to give the public a clean bill. She wants more openness Q: My boyfriend and I are in our late teens, and we were both each other’s “first time.” Before we first had sex, we talked about it and made sure that it was something that we both wanted to do, and we also talked about all the dangers that went along with it. It was great! But now, almost a year later, he still isn’t comfortable talking about what he likes or wants sexually. How do I get him to feel comfortable talking to me about this? I love him very much and don’t want to pressure him into doing anything that he’s not ready for, but I would really like that kind of openness. A: I know that you want to be open with each other, but have you ever thought about why he might not want to talk in detail about some intimate matters? Let’s say he wanted to have oral sex, and at some point you said it was something you’d never do. He knows there’s no point in bringing it up, and he doesn’t want to pressure you, so he’d

rather not get into such discussions. That’s just one example. There could be dozens of others. So my advice is not to pressure him into being more “Open.” Sometimes you do regret getting what you wish for. Q: How closely related are sex and stress? My girlfriend is about to move away and start medical school. She’ll only be about two hours away, so we’ll be able to see each other on weekends, for the most part. It’s just that lately we can’t seem to get in the mood. We’ve talked about it, and we think it’s the stress of the change that is about to take place. It’s not that we don’t have sex, but we don’t have it or the desire as often anymore. It is possibly the stress causing this? A: Stress can absolutely ruin a couple’s love life. Your stress could be the result of just not knowing what the future holds, which has created a situation where you are both keeping your emotions in check, afraid that you might get hurt. My hope for the two of you is that once she moves away and you develop a routine, you’ll be able to work things out. But that also might not happen. The distance, combined with the rigors of medical school, might mean that this relationship will not last. While you’re waiting for the move to take place, don’t put pressure on yourselves to have sex. Instead, just enjoy being together, and then you’ll see what the future has in store for you.

When considering your holiday shopping this year, here’s a little advice. Don’t be tempted to overspend like you may have in years past. Sure, we’ve been hearing about all those “deals” since Halloween. It even seemed like the holiday shopping season kicked off a few weeks early this year. Retailers couldn’t wait to start their “Tricks or Treats” to get us in the mood to shop, even if most of us can’t afford to rack up much anymore debt. Holiday shopping is expected to be very soft this year. Think of it like an ice cream cone that’s been sitting on the heat for several hours. That’s why we’re seeing more hype than ever about “Deep Discounts and Special Bargains.” Yes, retailers need us to “Spend Our Way” out of this recession. Yes, this will be a critical shopping season for too many businesses, which may face shutting their doors for good after the holidays. Yes, we all have to do our part to reverse our current economic meltdown. But NO, we don’t have to go deeper in debt and threaten our family’s financial future doing it. Here’s my advice: Budget. Don’t overspend. Use cash and demand cash-only discounts, rather than putting everything on plastic. We also have to be very careful about using gift cards this year. Too many retailers may shut down after the holidays which would make unused gift cards worthless. On-line shopping also will be down. And since you can’t demand “cash-only discounts” online, you may actually be able to wrangle better deals face-to-face with local merchants. And you won’t be facing all those extra shipping charges. So where are the deals? Home electronics, clothing and big-ticket appliances are seeing deep discounts this year. There are still great deals out of big “overstock” liq-
uidators offering older merchandise. Most manufacturers also offer deeply discounted “refurbished” products at their online websites. You can also look for great deals on “refurbed” home electronics like computers and cameras. But hold onto all the packaging and receipts to make sure they really work before throwing it all out. And remember, homemade presents can be great gifts too. Baked goods, such as fresh breads, cakes and cookies, can be a very personal present that doesn’t cost a lot of money, but still carries a lot of “love.” Home sauces and soups can be a real treat too that won’t break your budget. Taking a friend or coworker out for a quick lunch or breakfast snack is a nice holiday treat too. What’s more, this holiday season, there are a lot of changes taking place in the credit card industry. And most of them are not consumer-friendly at all. Some of the big name credit cards have quietly been notifying customers, with tiny-type mailings, that they’re changing their “Consumer Agreements.” What that means in simple language is that they’re cutting your spending limits and increasing your interest rates on unpaid balances. They’re also betting you won’t notice how expensive those credit cards really are until it’s too late and the bill comes due in January. By then it will be too late and you may find yourself paying bigger credit card bills than you never even expected. That’s a “Holiday Shopping Secret” you can never forget during this year’s shopping season. Remember the old adage: “Never go to the grocery store without a list or when you’re hungry.” Chances are you’re likely to buy more than you need and spend more, too. The same advice applies when you’re shopping for the holidays. Whether you’re shopping at the mall, online, or by phone or mail, the Federal Trade Commission (FTC), the nation’s consumer protection agency, says that a little planning and know-how can help you deliver a holiday season that’s on budget and maybe even a little less stressful. “Don’t put all your eggs in one basket.” If this everyday piece of advice comes to mind when thinking about your investments, then you may already understand the importance of a diversified portfolio. But even the most carefully composed investment portfolios can get out of balance from time to time. To make sure your portfolio stays on track to helping you reach your goals, it’s important to take the time regularly to rebalance your portfolio to make sure it’s still in line with your investment objectives. Some of the most common reasons your portfolio could get out of balance would involve a change in the ratio of the asset classes in your portfolio, or a change in the value of your various assets. Asset classes refer to the general types of investments that make up your overall investment mix: stocks, bonds, cash, mutual funds and ETFs, to name a few. As the respective values of those various investments change, the proportions in your portfolio will change as well. That’s why it’s important to check in on your investments to make sure the division of your assets doesn’t stray too far from the allocation you want. To help understand this problem, let’s take a look at how your portfolio can get out of balance. Consider a very basic portfolio made up of stocks, bonds and cash. Out of these three asset classes, stocks are likely to see the biggest price fluctuations. If the price of your stock position rises significantly, the overall percentage of stocks in your portfolio grows in relation to the percentage of cash
and bonds. The proportion of cash and bonds is therefore decreasing. This imbalance may increase the volatility of your portfolio as a greater percentage of your assets face a higher level of risk, due to the fact that stocks tend to carry a greater level of risk. At the other end of the spectrum, if your stock prices drop, the percentage of equity assets in your portfolio decreases as well. The risk is lowered, but so is your opportunity for growth. Another way to look at it is to consider a hypothetical example with numbers. Let’s say you invest $10,000 in bonds and $10,000 in stocks at the beginning of the year. By year end, you see that your stake in bonds has grown to $10,475 (for a return of 4.75 percent on the year) while your stock holdings are now worth $11,560 (or a 15.6 percent return). While that’s a nice overall return for your portfolio, you’ll notice that your investment mix in stocks and bonds has strayed from the even 50-50 you started with at the beginning of the year. At this point, your portfolio is 52.5 percent stocks and 47.5 percent bonds, and at this pace the difference could get much bigger in just a few more years. Rebalancing helps put you back in line with your original allocation. When your portfolio gets out of balance, it’s important to realign your investments by making adjustments in accordance with your long-term strategy. Meeting with your financial advisor on a regular basis to discuss your asset allocation can help you spot significant changes and make the necessary adjustments to get your portfolio back on track. LET’S TALK CARS Mercedes-Benz is staking a lot of capital on its new BlueTEC diesel technology, and with good reason. BlueTEC has been winning industry awards and accolades from all corners since the German automaker launched the program in an E-Class sedan two years ago. It was timed to coincide with the introduction of lowsulphur fuel in the United States. For those who haven’t read about it, BlueTEC is Mercedes’ new formula for powerful and clean diesel engines. Gone are the days of the noisy, smoky diesel; BlueTEC is highly efficient and quiet, and sharply reduces harmful nitrogen oxide and particle emissions. Just about 70 years ago, Mercedes-Benz became the first auto manufacturer to use the combustion-engine principle pioneered by Rudolf Diesel in a passenger car. Diesel technology even then had a
much higher efficiency than the traditional gasoline spark-ignition engine, and the diesel has been steadily improved over the years. A giant leap came in 1997 with the introduction of common-rail direct injection (CDI) technology in combination with the development of the four-valve engine system. Since then, the CDI abbreviation has meant both excellent fuel economy and a huge increase in torque, or towing power. With the introduction of the maintenance-free particulate filter in 2003, Mercedes-Benz overcame the last big objection to diesel power. The filter virtually eliminated soot particles and nitrogen oxide emissions. The first Mercedes passenger vehicle to get the new technology was an E320 in 2006. It was a success from the start and was voted “World Green Car of the Year” in 2007. Mercedes then launched the R320, ML320 and GL320 BlueTEC vehicles in the U.S. Most recently, the ML320 won the Southern Automotive Media Association award for being the best “Green” vehicle displayed at the South Florida International Auto Show. Mercedes now claims that its high-tech diesel engines meet the world’s toughest
