Michael’s New Smile

W

OW! What a difference. I’ve been waiting my whole life to do this. I can’t believe it was that easy. And these are only the temporaries!!!” Those were the first words we heard when Michael looked in the mirror. A couple of weeks have passed since we prepared Michael’s teeth and placed them in temporaries. Of course, like the cobbler’s kids, we haven’t had time to finish his case; but at least he’s not toothless. The plastic temporaries are a good representation of what the final ceramic teeth will look like and wearing the temporaries for a short time has given him an opportunity to get used to his new smile and his new bite. “I can’t remember the last time I could bite and chew on my back teeth and I haven’t had a headache since we did all the work. This really feels good and my jaws don’t hurt anymore. Also I can eat foods I haven’t been able to chew in a long time. My gums don’t hurt anymore.” When asked, what kind of difference has it made in your life so far, Michael stated, “Well, that’s a good question. I guess I’d have to say, it’s given me more confidence in who I am. Now I don’t shy away from people when I talk with them and

especially when I laugh (Michael really has a great laugh…and now a great smile). “Now, I WANT to talk to people to tell them about the change in my smile and how much better I feel about my appearance. I can’t believe what a change it made in my whole facial appearance, I feel younger”. Another question we asked is, moving forward, how do you feel it will help you deal with some of the issues of PTSD. “With PTSD, it’s easy to become a recluse. You want to hide from people and avoid conflict. My bad teeth didn’t help that because I was always conscious of my diseased teeth and gums and wanted to hide my smile. One of the issues I have always dealt with, even before my Army injuries, is fear of rejection, or fear that people wouldn’t take me seriously because they were looking at my teeth and not hearing what I had to say. I remember, in high school, seeing all the kids in braces having their teeth straightened and wishing I could do the same. I know that it will give me more confidence to break out of my shell and talk with people I have not met before. A lot of my anxiety has centered around how I felt about my teeth…and now I have a new start!”

What We Did The first step after taking the models and bite registration is to do what we call a diagnostic wax-up. The stone models of Michael’s teeth are mounted on a jaw simulator and then the stone teeth are prepared to receive the veneers, crowns and implants which I sculpt from wax to simulate Michael’s final smile. This helps me understand how to prepare Michael’s teeth the day we do the live treatment.

What’s next?

The day of the appointment, Michael dressed comfortably and came in excited to finally get his new smile. Some patients may opt to have some light sedation in the form of a sleeping pill, but Michael wanted the full experience. Being a dental assistant, he felt the experience would help him to relate to future patients going through the same experience. It took us about four hours to complete Michael’s case and place the temporaries. Michael was impressed with how comfortable the whole experience was and how little pain he had that evening and the next day.

In the next few weeks, as time allows us, I will segmentally replace the temporaries with his final crowns. This will take about three one hour appointments for Michael and lots of my personal time designing Michaels final smile on the computer and then customizing the shape and color by hand. magine a hospital unit filled with nurses and physicians who are vigilant around the clock, who selflessly rush from patient to patient, carrying in their hands the smallest medical tools and equipment you could ever fathom, and the cries of babies so tiny they are barely audible – this is the Level III Neonatal Intensive Care Unit (NICU) at St. Francis Medical Center. The expert NICU nurses and physicians have seen it all: babies as young as 23-weeks at birth defy all the odds, fight their way through surgeries, infections, countless procedures, and immeasurably high stakes. They hold the hands of mothers, fathers, and family 14

members as their babies are carefully nestled in incubators for days, weeks, and months in some cases. Neuropathy and the B Vitamins Neuropathy affects 20 percent of Americans, yet many people have never heard of it. By: Katharine Leppard, MD

P

eripheral neuropathy is a dysfunction of the nerves, typically affecting the feet first, that can cause a variety of symptoms including numbness, pain, and balance problems. This condition affects 20 percent of Americans, yet many people have never heard of it. Vitamin B supplementation is often recommended to treat neuropathy, and often the thought, “If a little is good, then, even more, is better” has been applied to the B vitamins. However, there are some critical pieces of information about the B vitamins that are important to know. Peripheral neuropathy symptoms can vary widely and can include pain, numbness, burning, stinging, uncomfortable tingling, shooting sensations, weakness, or balance difficulties. Typically, these symptoms affect the longest nerves first, the nerves in our feet, followed by the nerves in our hands. Nerve pain can be especially severe at night, and often does not respond to standard pain medications such as anti-inflammatory and narcotic medications. It is hard to understand how a numb foot can also feel severe pain. One way to look at a peripheral neuropathy is that it is a “short circuit” of the nerves. The injured nerves cannot feel the normal sensations that we want to feel, causing numbness in the feet and sometimes balance difficulties. Yet those same injured nerves may register a normally nonpainful stimulus as painful - something as simple as a sheet touching a foot. I have had patients tell me they feel as if they are walking on river rocks or sand. Others report they feel as if there is tape or leather on the bottom of the feet. The “bunched sock” sensation is classic for neuropathy. Patients feel as if there is a sock bunched under the foot, when the sock is perfectly fine, or even when there are no socks on the feet. Other patients notice balance problems as the first symptom and have no pain, and because the onset of numbness was so gradual, don’t even notice numbness. There are many causes and types of peripheral neuropathies. The most common cause in the U.S. is 16

diabetes. Other known causes include thyroid disease, vitamin B12 deficiencies, hereditary forms, alcohol abuse, renal disease, toxins such as heavy metals, and damage from medications including some forms of chemotherapy. Over 30% of peripheral neuropathies have no identifiable cause and are called idiopathic neuropathies. Idiopathic neuropathies can be very frustrating and hard to understand when diagnosed in an otherwise healthy person. Peripheral neuropathy is diagnosed by EMG, a specialized test that measures how well the nerves carry an electric stimulus and looks for abnormality in the electrical activity in the muscle. Small fiber neuropathies affect the microscopic nerves in the skin, in this case the EMG is normal, and a skin biopsy can be done for diagnosis. Once a diagnosis of peripheral neuropathy is made, there are lab tests that need to be done to look for treatable causes of peripheral neuropathy. These labs include tests for diabetes, thyroid dysfunction, Vitamin B12 level, folate, and a specialized protein test called a serum protein electrophoresis. Vitamin B12 deficiency is common. It is estimated that 10-25% of people over the age of 80 have a B12 deficiency. B12 deficiencies can cause anemia, optic nerve issues, spinal cord degeneration. and peripheral neuropathy. Vitamin B12 is found in red meat, dairy, fish, and eggs. The recommended daily dose varies depending on the resource. The Mayo Clinic recommends a 1,000-2,000 mcg daily dose for B12 deficiency. The tricky thing about B12 is that it requires a chemical made by the stomach called the intrinsic factor to be absorbed by the body. As we age, the production of this intrinsic factor can drop off. Many people have excellent diets or take B12 supplements, and they simply are not absorbing the B12. The only way to check Vitamin B12 is a blood test that measures the B12 level. Normal levels average 200-900 pg/ ml, however, there are many studies that report that neurologic symptoms can develop with B12 blood


levels less than 400 pg/ml. If people do not absorb B12 normally, they need to do B12 injections. Vitamin B6 can actually be toxic to nerves in doses above 100 mg/day. Vitamin B6 is included in prescription medications such as Metanx, and in many health supplement drinks, protein powders, and tablets. You need to add how much Vitamin B6 is in your daily supplements. I have had patients who took megavitamins and health supplements in an attempt to be healthy and developed foot numbness. They come to the office for evaluation and are diagnosed with peripheral neuropathy. They are shocked to learn they have actually been doing harm with excess Vitamin B6. Once nerve damage has occurred, the medical treatment options are limited. If no underlying condition is found to treat, it is a symptom management situation. There are medications to help with neuropathic pain such as Lyrica, Cymbalta, and Neurontin, but these medications do not improve nerve function or reverse nerve damage. Q: Why was Strata Integrated Wellness Spa founded? A: In the past, all of the partners have experienced

limitations with the traditional medical approach. Because of this, Strata Integrated Wellness Spa was launched on the premise that health and wellness are dependent upon an integration of all best medical practices. While so-called allopathic or traditional medicine is what many people imagine when thinking of health and wellness, there is a whole new broad field labeled "integrative medicine" which encompasses not only traditional medicine but areas of complementary medicine including naturopathic, chiropractic, functional, holistic, energetic, and other areas of care. Altogether these create a true proactive, allencompassing wellness approach of mind, body, and spirit with the thought being that balance in all three of these areas is necessary for true wellness to be present.

Q: What inspired you to leave a successful traditional practice and join forces with integrated and preventative medicine? 18

A: As the medical director for Strata Integrated

Wellness Spa, I am a traditionally trained physician (MD) in the areas of internal medicine and cardiology (also a PhD in cardiovascular physiology). I have spent over 30 years practicing traditional medicine and while I received a great deal of satisfaction in treating patients in this manner, I began to feel something was missing. In traditional medicine we, unfortunately, have a "sick care" system in lieu of a "health care" approach to patients. This results in a very reactive approach to treatment. Several years ago, I began to understand that a proactive (preventative) approach not only made sense clinically for patients but also made sense teleologically, intellectually, and financially overall for the healthcare system. I began to incorporate more "nontraditional" therapies in the management of my own patients and noted almost immediately improved success rates. This drove me to pursue more educational and professional opportunities in the area of integrative medicine, focusing upon developing a markedly different medical paradigm. As I transitioned from my traditional cardiology practice to the medical director at Strata Integrated Wellness Spa, I wanted to be on the leading edge of knowledge to run an integrative medicine program. Based upon this, I enrolled in the first cohort of the Duke University Leadership Program in Integrative Medicine and am


Q: When it comes to health and wellness, what trends are you seeing among patients— in terms of treatments they’re pursuing, preventative measures they’re taking, etc.? A: Consumers are extremely informed whether it be in

the areas of health, wellness, consumable goods, or other items. The Internet provides a valuable, though not always accurate, resource which allows people to investigate potential alternatives for the diagnosis and management of medical conditions. Many people have had issues in the traditional medical system and, as a result, are searching for alternatives to so-called standard care. Additionally, in this age of information many people now are pursuing options to their health that include approaches for potentially preventing or slowing the development of diseases. While most diseases in the United States are chronic in nature and progress over a number of years - if not decades - lifestyle management and preventative measures can significantly impact these problems.

Q: What do clients (patients?) get at Strata Integrated Wellness Spa they don’t get from their traditional physician? A: The single most important aspect of Strata

Our only objective is to comprehensively and accurately evaluate and diagnose the ‘whole person’ for current or potential future medical issues. We evaluate our clientele utilizing approaches described by the National Institutes of Health as "precision medicine." Precision medicine is considered by many to be the future of medicine but here at Strata Integrated Wellness Spa it is the "now.”

Q: Do you take insurance or do your clients pay out of pocket? A: We are considered an out-of-network provider for

most forms of insurance and therefore we do provide appropriate diagnosis and professional service coding for each condition. This allows our clients to submit claims to their insurance for consideration of reimbursement of their expenses. We are not a Medicare or Medicaid provider and therefore our services cannot be submitted to these entities for reimbursement. ’ve always heard that the first step to recovery is to admit there is a problem. Design help isn’t necessarily a situation of life or death, but I can say with confidence that a well-appointed home does add contentment, happiness, and, I’m sure, years to your life! All joking aside, sometimes it’s important that we reach out and ask a professional.

Many today believe that just by investing in countless hours in front of a television, watching HGTV or the Design Network, it’s the same as receiving a degree in Interior Design from an institution of higher education. The reality is that often it’s made to look so simple that it appears as if anyone can throw up some shiplap and everything is right with the world . . . Not so fast! Shiplap and an overly “nail-headed” linen chair won’t solve the fact that you have dated furniture or furniture that you inherited from your mother. You find your entire home is full of heirlooms or antiques, along with many other treasured items, and you have no idea how to arrange or display them that makes sense in your not-so-formal home. 20

Nonetheless, a seasoned designer should be able to come up with the solutions to make your home a prized backdrop for your cherished past and the things you hold most dear. Going about the process of finding help may be a bit daunting, but it’s certainly not impossible. Here are some ideas from a couple of my friends in the design industry.

The client and designer need to mesh. • Trust is paramount. • Accomplishments and experience in the type of project are helpful to the outcome. • The client should ask to see a portfolio of similar projects that conveys the “look” you are wanting. • The client should consider the Designer’s qualifications, aesthetic, and the level of trust they have for them equally.

Construction with design often represents more site visits, as well as more total work, as opposed to only furnishing an existing space. Most designers offer an initial complimentary site visit to look at the job and discuss budgets and time frame in order to determine pricing.

Stephen Shutts, Stephen Shutts Design, Morgantown, West Virginia, says, “When it comes to large-scale projects, be it new construction or a significant renovation, I find that all too often a client will come to me far too late in the project, once many of the crucial decisions have already been made. People often think the time to hire an interior designer is towards the end of a project, when it’s time for furnishings, window treatments, accessories, etc. But for a project to be truly successful and cohesive, the interior designer, as well as all associated trades, should be considered and hired from the project’s inception. I liken the built environment to a machine, where all the parts and pieces must function together in just the right way to achieve the desired outcome.” Winter 2018

Hopefully, your challenge today turns into an amazing client/designer relationship, by just having thoughts and ideas become fluid in regards to your immediate needs. I have had the great fortune of working for years with the same clients and friends on the interior of their homes, or in some cases, multiple homes. Our relationship is predominant to the trust of being allowed to do what I know will work. Often, the “reveal” is the first time my client will experience the overall revision of an interior space. If the client hopes to obtain goods at wholesale prices from the designer, the client may have missed the purpose of engaging in a relationship with the designer. The bottom line of the designer’s position is to accomplish your project, and most designers generally will match your best price, if it is at all possible. Keep in mind, items found on the internet are often not able to be price matched, especially when considering overhead costs that are incurred when dealing with a brick and mortar operation. Screen Time and Poor Vision, What Can You Do? By: Dr. James Cutler

S

creen time may be adversely affecting your child’s vision. According to the recent CLEERE Study, myopia has increased 66% in the last 30 years in the U.S. This number has rocketed up 30% in only the most recent 10 years in the U.S. and approximately 40% in China. Recent studies suggest that the cause is due to increased near activity that children are involved in. Clinical and animal studies now give us a better understanding of how the eye changes and elongates. The retinal image and appropriate focus along the surface of the back of the eye greatly influences the changes that occur to the shape of the eye and resultant vision. Excessive screen time and poor focus can lead to rapid unwanted changes to the eyes of growing children. Unfortunately, kids are increasingly faced with more screen time in the classroom, with homework, and at play with personal pads, devices, and computer games. These activities draw the child’s attention and vision increasingly closer causing extra stress on their visual systems. The amount of focus stress on the eye triples when shifting focus from arm’s length to one foot. This increased near stress causes the growing eye to undergo permanent changes, including elongation of the globe, stretching of the internal optic nerve retinal fibers and blood vessels, and changes in the cornea. These changes will make your child’s vision blurrier, and create higher risks for ocular damage including glaucoma, retinal tears, and retinal degeneration, according to the Flitcroft study. The calculated health risk of this increased myopia is comparable to hypertension, smoking, and cardiovascular disease.

So, what can you do about it? For starters, get your child a professional eye examination. Vision screenings are not enough to assess changes in the ocular structure, accommodative ability, and risk for myopic changes. Your eye care professional can help you set a course of action for prevention or slowing the changes that eyeglasses alone cannot provide. There are now non-surgical means that can give your child better vision and a better quality of life. Corneal reshaping and Orthokeratology (specialty contact lenses) that act as braces for the eye can provide your child with adequate vision and will slow the progression of blur. There are also new soft contacts that just came to the U.S. market that have been shown to slow myopic changes as well as some new medication that is delivered as an eye drop. You can also find ways to reduce screen time. I often suggest to parents to use the 20/20 rule after 20 minutes of screen time, get up and walk around for 20 seconds. Also, move your child further away from the computer and set time limits on personal devices. Child Abduction or Parental Kidnapping The legal consequences of child custody litigation are often times extremely complex and difficult.

A

Child Abduction or Parental Kidnapping occurs when a parent violates a Court Order. Ordinarily, parents who have no court orders that define where the child lives and what kind of parenting time either parent has will not be impacted when a parent violates an informal agreement between the parties. The importance of having parenting agreements approved by a judge is vital, so that if a parent violates the parenting agreement, the guilty parent can be held responsible per the orders of the court.

There are specific state and federal laws that apply to child abductions and parental kidnapping: Uniform Child Custody Judicial Enforcement Act; International Parental Kidnapping Crime Act; The Fugitive Felon Act; Hague Convention on the Civil Aspects of International Child Abduction; International Child Abduction Remedies Act, etc. A critical provision of federal law concerning interstate and custody determinations and disputes can be found in the Parental Kidnapping Prevention Act (PKPA). The applicable Colorado law is the Uniform Child Custody Jurisdiction Act (UCCJEA), which can be found in the Colorado Revised Statutes in Title 14. Without orders from a judge that spell out parenting time and other 28

necessary provisions, a Colorado court will not be able to assist you if there is a child abduction or parental kidnapping. Here is an example: Beth and Tom have two (2) children but were never married to one another. Beth and Tom never had any court ordered parenting orders to address parenting time and/or child support because as Beth said, “They always agreed about the kids and got along.” Tom was named on both children’s birth certificates and the parents always made decisions affecting the children, jointly. Beth had always been the primary parent while she lived in Denver and Tom had parenting time every other weekend from Fridays after school until he dropped the children off at school on Monday mornings. When the children reached the age of 6

the children that they had packed up all of their things and were now living in Arizona. When Tom found out that his children were living somewhere else, he contacted Beth who refused to respond to phone calls, text messages, or Tom’s emails. Later that day, Tom made a 911 call to the Denver Police Department to report that his kids had been abducted. The police officer met with Tom and said that there was nothing that they could do because this was a civil matter, and police only investigated crimes. The officer told Tom he should get a real good custody lawyer. The following day, Tom made an appointment to immediately see a Family Law Attorney, and was told that since no parenting orders existed, the only thing that could be done was for

Beth and Bob became involved in a serious relationship and Bob invited Beth and the children to move in with him, in his Flagstaff house, while the kids were on their Christmas break from school. When Tom called the children to wish them a Merry Christmas, he was told by one of

him to file an Allocation of Parental Responsibilities case in the District Court where the children had last lived in Colorado. Tom’s lawyer also told him that Colorado was the children’s “Home State,” and that he would have to file an action before he could expect to have his parenting rights enforced. It was at that moment that Tom found out


that his plans to take the children skiing in Vail over the second half of their Christmas break was not going to happen. Because of the congestion of the court’s docket, Tom had to wait more than 6 weeks to have a Temporary Orders Hearing before the judge. At the Temporary Orders hearing, the judge noted Beth’s role as the children’s primary parent since the children’s births, and allowed the children to stay in Arizona, thus making Tom’s every other weekend parenting time moot. Tom was not happy with this recent turn of events and ultimately ran out of money to fight for custody and agreed to a Parenting Plan with Beth for shared parenting time every year over Thanksgiving, Christmas, Spring Break, and summers. Additionally, Tom was ordered to pay Child Support to Beth. An experienced Colorado Family Law Attorney should possess the kind of specialized legal and trial skills and knowledge to give parents appropriate legal advice. The list of parental responsibilities is far greater than just determining how holidays will be divided. Another important parenting abduction and kidnapping situation occurs when a parent has custody orders in another state and leaves that state. In some circumstances, the court, which originally decided custody and parenting, will no longer enforce those orders because the children have been

away from that state for such an extended time. When a court will not take action to enforce its parenting orders and to prevent the children from being taken to another state, it is important to talk to a Family Law Attorney regarding the registration of the court orders from the originating state. In Colorado, this is a process that must begin with the registration of the foreign decree and foreign parenting orders of the other state.

orders from another state’s court judge in the U.S., it is clear to see that the court’s view of children being taken or withheld against orders of another court are treated in an extremely serious manner. Many situations occur when children are wrongfully taken from one or both parents, both throughout the United States and internationally. Some of these cases involve immigration laws and require that kind of specific expertise. The legal consequences of child custody litigation are often times extremely complex and difficult. The parties become engaged in a tug of war to see which one has the fight to prevail and at what cost. In a recent case that I handled, a parent who had the child for her summer parenting time visit, decided to keep the child in Colorado instead of returning the child to her father who had parenting orders in Texas. Not only did the Colorado court allow for the emergency registration of the Texas court orders, it also conducted an emergency hearing before ultimately ruling that the child be turned over to the father within one hour after the hearing had concluded. With specific court

Hormone Replacement Therapy Why I Chose to Pursue HRT

M

y name is Samuel Johnson (name changed to protect confidentiality). I am 45 years old, and I haven’t felt this good since I was in my twenties! Let’s face it, aging isn’t a cakewalk. I have been an athlete for most of my life, and I’ve enjoyed playing basketball and soccer with friends and colleagues throughout my adulthood. Turning 30 wasn’t as dramatic as I thought it would be, but for some reason, things started to change for me when I turned 35. Over time I started to notice that my body didn’t feel as alive and energetic as it felt when I was younger. I would work out and play sports with my friends, and immediately afterward, I would feel drained and my body felt sore to the point it was difficult for me to walk after a workout. My energy level was horrible, and I started to lose motivation to do the things I used to love doing. I can’t say I ever felt clinically depressed, but I did start to feel sadness at times, and I started to develop a pessimistic view of myself, my body, and my future. This wasn’t like me at all . . . On top of that, I started to gain weight, lose muscle mass, and my desire to have sex started to decrease. Honestly, I never imagined that wanting to have sex would be a problem for me! I was embarrassed to talk about the way I felt with my wife, and unfortunately, I isolated myself in ways that weren’t healthy for myself or for my marriage. But enough of the sob story. I’ve never been one to sit back and let life happen to me. After a year or two of feeling this way, it dawned on me that I was too young to be feeling this old! That was when I decided to get help and find out what was driving this funk. I remember seeing a commercial on TV about hormone replacement therapy and the effects of low testosterone. I looked at the screen, and I realized that I had almost every symptom that characterized low testosterone in men. Decreased libido, check. Increased body fat and decreased muscle mass, check. Looking Into Your Heart: Cardiac MRI at Penrose-St. Francis Health Services Cardiac MRI, or MRI of the heart, is a test that uses a large magnet, radio signals, and a computer to make images of the heart.

eart disease is the leading cause of death in the United States, according to the Centers for Disease Control and Prevention (CDC). In the U.S., 1 in every 4 deaths is the result of heart disease – that’s about 610,000 people who die from the condition each year. Heart disease does not discriminate and it’s the most fatal condition for several populations, including Caucasians, Hispanics, and African-Americans. Almost half of all Americans are at risk for heart disease and the numbers are rising, so having a good way to diagnose and direct treatments is a big deal. magnetic fields around the body, which line up the hydrogen protons in the body. Radio waves knock the protons out of position and as these realign, they send out radio signals. A computer receives the signals and converts them into images of the body. “Cardiac MRI can provide detailed information on the type and severity of heart disease to help decide the best way to treat heart problems, such as coronary heart disease, heart valve problems, pericarditis, cardiac tumors, or damage from a heart attack,” explained Dr. Harshal Patil, who specializes in Cardiovascular Disease and Cardiac MRI with Colorado Springs Cardiology, a Centura Health Clinic. Dr. Patil is the only cardiovascular physician offering Cardiac MRI in Colorado Springs and southern Colorado. To prepare for a Cardiac MRI, patients must communicate if they have a pacemaker or other implanted devices in their bodies because the MRI machine can damage the devices. Any metal inside the body must also be specified and any metal on the body, such as jewelry, must be removed prior to the MRI. “There are no risks for a Cardiac MRI and very few side effects if any. Cardiac MRI does not use ionizing radiation and there have been no documented ill effects from the


radio and magnetic waves it uses,” said Dr. Patil. “The MRI machine looks a little intimidating, but as long as patients follow instructions to prepare appropriately, they will be completely safe.” Dr. Patil is excited about the accuracy provided by the Cardiac MRI and its ability to diagnose coronary artery disease, as well as determining who should consider more invasive treatments to fix their condition. Cardiac MRI is also a great way to track the effectiveness of treatments, according to Dr. Patil, such as whether a stent or angioplasty restored enough blood flow to rescue the suffering cardiac tissue. This cutting-edge technology offered at Penrose-St. Francis can provide a critical 6-month or 12-month follow-up to assess a patient’s heart health after treatments. “There is increasingly strong evidence that Cardiac MRI is very good for complex cases and for diagnosing conditions where other tests have not been clear. Pelvic Exams and Pap Smears Why They are Still a Key Measure in Maintaining Women’s Health

I

n 2017, the Center for Disease Control estimated that each year there are about 42,700 new cases of cancer found in areas of the body where the Human Papillomavirus is often found, with 33,700 of those being caused by HPV itself*. Those figures are only seen in those who have chosen to get themselves tested, and the CDC expects more go undiagnosed each year. With the number of undiagnosed cases of HPV that the CDC expects to circulate each year, a woman may find comfort in scheduling an appointment with their primary care or OB/GYN provider to ensure she is in good health and discuss ways to limit the chances of contracting the Human Papillomavirus.

The American College of Obstetricians and Gynecologists (ACOG) recommend women receive a pelvic exam when previous medical history or symptoms deem it to be necessary*. In regards to pap smears, women aged 21 to 65 only need to have one done every three years, but women aged 30 and older could contemplate getting one every five years if combined with HPV testing*. Women who have HIV, a previous pap smear that had shown precancerous cells, a weakened immune system, or any other abnormalities, may need to be seen more frequently by their primary care or OB/GYN provider. When a woman

comes to the clinic for a gynecology exam, the primary care or OB/GYN provider will be able to discuss with the patient what is included in the exam and whether or not a pap smear will be necessary that year. Whether she has been experiencing any symptoms or not, a pelvic exam is recommended for a general assessment of the woman’s gynecological health and to assess and diagnose any possible medical conditions that may have appeared. Besides the triennial Pap smear, an external visual exam, internal visual exam, and a bimanual exam are generally included in the annual pelvic exam. Both the external and internal visual exams allow the primary care or OB/GYN provider to look for and inspect any abnormalities. The bimanual portion of the exam checks the uterus and ovaries for any irregularities such as growths or tenderness the woman may have. The primary care provider or OB/GYN should take the time to explain what they are doing during the exam and why, and after the exam is finished, should be able to discuss the results of the exam and any next steps. Some medical professionals suggest differently when it comes to how often a patient should receive both pelvic exams and Pap smears. The best time to determine if one is needed is when the patient and primary care or OB/GYN provider discuss the individual woman’s history. Making an appointment to have an exam can help with early detection of any potential problems and may possibly give the woman peace of mind about their health.

I

t was a rainy, chilly afternoon this past October, about noon mountain time, as I patiently waited in my office for my interview phone call with Brooke Shields. Right on time, a friendly voice came on the other line and cheerfully said: “Hi, it’s Brooke.” Immediately I felt the warmth and hospitality in her voice and instinctively knew this was going to be a fun conversation. We feel privileged to feature this vivacious actress and businesswoman on our cover for the winter edition of Colorado Living Well--the one and only Brooke Shields! Literally modeling since she was an infant, her first job was for Ivory Soap when she was only 11 months old. In 1978, she stepped onto the acting stage at the young

age of 12. Known for her incredible beauty, inside and out; captivating smile; and disarming wit, Brooke has continued to capture our hearts. Born in New York City, Brooke has been nominated for two Golden Globes, has a degree from Princeton University in French Literature, and is now a busy mother of two, who still finds time to be a clothing designer, actress, wife, soccer mom, and spokeswoman. Spending her time between three major cities, Brooke is a grounded, hospitable, strong leader who, in just a few minutes by phone, made me feel as if I was just on a casual call with one of my good friends. Q: Our readers always love it when we give them a day in the life of . . . what’s your schedule like right now and where do you call home? Right now, I’m flying back and forth to Los Angeles to film Jane the Virgin (Brooke plays a renowned actress and supermodel named River Fields). When I’m not in New York City, where I live, or in L.A., I travel back and forth to Philly to film with QVC, where I’ve recently debuted my own clothing line. That’s the business part - the rest of my time is spent with family. My husband and I have two daughters. One in junior high and one just starting high school.

Q: Wow that sounds like a packed schedule, how do you find the balance? Honestly, like anyone at this stage of life, it’s really hard to find balance, but the reality is that our family is used to passing the baton back and forth. My husband is usually also busy in L.A. editing, but this year he’s been home a lot, which makes it easy for me to accommodate the travel schedule. For QVC, I only go once or twice a month to be on the show so that’s not too bad. Also, it’s not uncommon to catch a red-eye flight back from L.A. The producers of Jane the Virgin are great to work with my schedule and book multiple episodes. This allows me to get many of the takes done in one trip.

The important thing for my husband and I is to not let our kids control the show. We remind and teach our kids to ask, not demand or tell us what they want to do. This attitude keeps our kids and our family life balanced. We consistently ask them if we are meeting their needs and keeping balance within our family a top priority. Between music, tutoring, and sports they’re both so busy too. It’s just a hurried season of life, but we wouldn’t have it any other way.

Q: You starred in your first movie at age 12, that’s pretty young to have that kind of pressure and attention. How do you think that experience at such an early age helped shape who you are today? My girls have such a routine with their life. That was not my experience at all. I was raised by a single mother in a very bohemian type of lifestyle. Remembering my life at that age, I often think, “How did I manage to get my schoolwork done and get into a good university?” I really had to be the one who self-regulated, which built in a discipline and selfconfidence that serves me well today. Growing up, I had a very loving dad. I never knew my parents together, so that never stressed me. This led me to think I’d always be a single mom—”like I can do that.” It’s still somewhat foreign to me to share the responsibility with my husband. Being so independent, I have to be cautious to include him and not overrun the show. One thing I will say is that there was never a lack of love growing up with my parents.

The day I went into the QVC offices for the first time, I was honestly uncertain that I had the energy to keep up with the hosts. They sat me down and said, “Ok Brooke, we are going to start with a video about us.” The video ended up being nothing but bloopers. How genius! This was a reminder to not take yourself too seriously. No one likes stiff, nonauthentic people.

Q: How do you define your role as a mother? What are your goals for your daughters? I love being the fun mom and the mom the girls want to talk to . . . the house everyone wants to come to, but my role as a mother is not to be their friend. I’m big on manners and strict on how we talk to each other. I’m not trying to be cool with them. My responsibility is to guide and direct them. I realize that as my husband and I are getting older, that we’ve already lived such full lives. However, I also realize that our daughters’ lives are just starting. It’s a great responsibility to nurture and guide them so they will be ready to live their life.

Q: What other projects are you currently working on? What’s going to be new for 2019? I still love doing TV. Currently I’m working on Murphy Brown, QVC, Jane the Virgin, and more.

Q: How do you stay fit and what does a workout session look like for Brooke Shields? First off, I’ve never been the person who can just eat whatever they want. I eat well, but food is not so much my issue. It’s tequila and wine that really adds the weight, so I’m careful to not indulge too often. My whole thing is that I have to work out – I don’t necessarily enjoy it but love how the endorphins and serotonin make me feel. If I don’t force myself to exert the energy to work out, I don’t sleep well and don’t work well. So, for me, it’s not about vanity, but more about functioning at my best. What I do that works for me is to find the workout that makes me happy. I don’t like running – I can’t run. Cycling I love, horseback riding I love – Pilates I’m… eh starting to love. At 53 years old working out is a necessity. I remember after college doing a Broadway show. It was so intense. We would do eight shows a week, four to five hours a day of dancing and the weight would just fall off. When this time of life was over, I didn’t know what to do – I was so used to pushing my body to extremes, so I had to learn to look at exercise from a different perspective. Now my workouts are about happiness and health and living longer!

Q: Let’s talk about your clothing line for QVC - Who is your target? You said, 48

“There is a huge market of women who have money and don’t want to look prepubescent, but don’t want to look like old ladies either.” There are so many things going on when I’m on QVC. It’s live TV and it’s actually super hard. You wear an earpiece in one ear, so you have the producer talking into your ear giving instructions, you’re looking at multiple TV’s, you’re trying to talk about the product, you move to different cameras for different angles, you’re interacting with the host – your brain has to fire back and forth so rapidly, it’s always a challenge and I love it! For my clothing line, I’m a part of every aspect of design from start to finish. This is really important to me. I’m putting my name on these clothes and I want to have the knowledge to speak to the quality, colors, length, seams, buttons, pleats, and all the details about the clothing. This is my story and my brand . . . I’ve truly lived it, so by the time a garment gets to air it’s been through rigorous testing and approval and I know I can fully believe in what I’m selling on air. Women and their bodies are intimate. I’m having a conversation with women simultaneously all over the United States and I’m building a level of trust. This isn’t a quick sale, it’s lifestyle, it’s personal. February of this year was the first time I appeared on QVC with my clothing line Timeless, and so far, I am very pleased and the garments are being very well received.

Q: With this being our winter holiday issue, how do you celebrate the holidays? What is your fondest memory as a child and as an adult? We spend every Thanksgiving in Florida with my stepmom, since my Dad passed away. My husband loves to BBQ, so he smokes a brisket each year. Normally in mid-December, everyone comes to NYC for a major family affair. It’s quite a full house and planning fiasco let me tell you. Then usually the day after Christmas we take the girls somewhere for New Year’s. This is really our time to travel with our girls before they head to college. This Christmas, however, we are actually going to Africa. I don’t think our girls are too happy about that because they love waking up to the tree and Dad’s breakfast, but Africa has been on my family bucket list for some time and I want our girls to experience it with us, especially with no electronics. I want us to be stuck together – somewhere where there’s no internet.

Good bathing practices: • Shower at least twice a week. The more active you are, the more often you will need to shower. But, the more you shower the more problems you will have with dry and irritable skin. • Showers should be 5-10 minutes long instead of 20 minutes long. The longer the shower, the more dry and irritable your skin will be. I often recommend getting in the shower and getting wet. Then, turn off the water and apply your shampoos and cleansers. Turn the water back on and rinse off. It is not only a “green” and responsible way to consume water, but it will also help your skin stay moisturized. • Pat dry your skin. Rubbing your skin only causes problems. People often do it to “exfoliate;” but, your skin naturally exfoliates and the more you try to force it to exfoliate by bad products or bad practices, the more problems you will have. • After you bathe, apply lotion from the neck down while skin is still slightly damp. This helps “seal in” the moisture and often times makes all the difference in your routine. If your skin is still dry, apply lotion more often.

Bathing mistakes: • Do not make the water hot while showering. Hot water just rips the moisture out of your skin. You do not need to have the water so cold that you shiver during your shower. But, if the water is so hot that your bathroom mirror is fogged over, that the paint is coming off the walls from the steam, and your skin is so red you look like a lobster, then the water is way too hot and you are damaging your skin. • Don’t use soaps as they can be too irritating to the skin. Use good cleansers in the shower instead. • Do not apply your cleanser with a wash cloth, a loofah, or a sponge. These products are too abrasive, are breeding grounds for mold and bacteria, and cause damage to the skin. Put

your cleanser on with your hands. Do not scrub it into your skin. Let it rest on your skin for a minute and then rinse it off. Again, unless you have a select few medical conditions that need medical intervention to help your skin exfoliate, you should not be manually exfoliating your skin. • Do not overdue how often or how long you are in a hot tub. Most people should be able to handle going into the hot tub for 5-10 minutes once or twice a week. If you are in the hot tub for one hour every day, your skin will have problems. Rinse off after getting out of the hot tub and apply your lotion.

Recommended Products: The following products can be purchased without a prescription online or at your local drug store. • For cleansers I recommend using Dove Sensitive Skin Body Bar, Cetaphil® Restoraderm® Body Wash, CeraVe® Cleanser, or Vanicream™ Cleansing Bar. • For moisturizers my recommendation will depend on if you mind the “feel” of a moisturizer on your skin. For those who do not like to “feel” the moisturizer, I recommend CeraVe® Lotion or Vanicream™ Lite Skin Care Lotion. Since these are not as “heavy” feeling, you will probably need to use them more often than the “thicker” ones that have more of a “feel” to them. For those who do not mind the “feel” of a moisturizer, I recommend Cetaphil® Restoraderm® Moisturizer, or Vanicream™ Moisturizing Skin Cream. The above tips should keep your skin moist and happy this winter. ibroScanÂŽ, also known as transient elastography, is an accurate, non-invasive, painless aid for managing patients with liver disease. FibroScan is FDA-approved for clinical management of patients with liver disease and is covered by most insurance carriers. FibroScan works by emitting a pulse of energy, which you may feel as a slight vibration on your skin. FibroScan measures the speed of this energy and immediately provides this information to your physician, who then uses it as part of a broader evaluation of your liver health. In essence, FibroScan measures liver stiffness and translates this into an estimation of liver damage. Traditionally, physicians have used a liver biopsy to determine the severity of liver disease, but a liver biopsy is invasive, carries a risk of complications (bleeding), is expensive (total costs are typically more than $1,000), and requires time off from your daily activities. In contrast, FibroScan can allow your physician to get the needed information concerning the extent of liver damage (scar tissue, also called fibrosis) much easier, with the only preparation being to fast for three hours before the test. Most liver conditions have been studied using FibroScan, and more than 500 scientific articles have been published using the technology. While there are other non-invasive methods to estimate liver damage, using blood tests or magnetic resonance imaging (MRI), FibroScan offers high accuracy and is the least expensive of all the available tests.


In addition, using the same machine, and during the same examination, physicians can determine the amount of fat in the liver (called steatosis in medical jargon). This is very important for those suffering from fatty liver disease. This common condition affects millions of Americans. The severe form of this condition, called Non-Alcoholic Steatohepatitis (NASH) will become the leading cause of liver disease requiring liver transplantation in the US in the next several years. Using FibroScan, we can make better choices to determine the best treatment, and follow the progress of that treatment, with repeat FibroScan testing.

he holidays are a particularly difficult time for someone who is grieving. Experiencing the loss of a loved one can be disorienting at any time; and for most of us, the grieving process is a complex journey that continues long after a significant person in our lives has passed. Although it might seem incomprehensible, it’s important to remember that recovery from the grief you feel is possible. With time and the right support, healing from the pain of losing a loved one can be achieved.


Pikes Peak Hospice & Palliative Care’s Center for Grief & Loss is available year-round to provide support and offers these tips for helping someone who is grieving during the holidays: Help those who are grieving by talking about the person who died and reliving cherished memories. Don’t expect the bereaved to do everything the same this holiday season. Lighten the load of those who are grieving by offering to help them with their grocery and/or holiday gift shopping. Offering to take their children shopping can also be a great way for you to help them cope with loss during the holiday season. Understand that all people grieve differently. Give them space and be very understanding of their situation.

Make sure to include the person who is grieving in your plans but understand that they may back out at the last minute. Make it a point to call, write or visit with those who are grieving. Knowing that others are thinking about them will make them feel better during this time of loss. Be a good listener and ignore the urge to give advice unless asked. Remember, someone doesn’t ‘get over’ the loss of a loved one.

The Center for Grief & Loss offers monthly support groups and events that are available to anyone, whether or not a family member has received care from Pikes Peak Hospice & Palliative Care. The support groups are free of charge, but donations are welcome to help support Pikes Peak Hospice & Palliative Care’s programming.
