hat’s the number of years we have been publishing this buyers guide issue. And what changes have taken place over the years. The amount of functionality featured in the systems back then pales in comparison to what’s offered today. Pharmacy has changed over the years, and so has the technology being offered.

Because pharmacy is a dynamic profession, and because our federal government continues to pass legislation that requires adaptation to new rules, the companies behind the technology are always busy keeping up. One example is having their e-prescribing software audited for compliance with the DEA requirements for electronic prescribing of controlled substances. With front-store POS systems it is SIGIS, IIAS, and PCI compliance that comes into play. Another example would be 340B programs and the special inventory management requirements. Next up is the conversion to ICD-10 from ICD-9 codes, come October of next year. And in addition to that will be the new track-and-trace requirements in order to keep the supply chain free of bogus drugs.

Then there are the biotech drugs to contend with. The emergence of these drugs has spawned a new type of pharmacy called specialty pharmacy, where there’s particular emphasis on ensuring that the patients receiving these drugs are taking them as prescribed and getting the expected outcomes. Because of the expensive nature of biotech drugs, this places new demands on the inventory management of these drugs, and requires more rigorous documentation and reporting by the pharmacies managing the patients on these drugs. Biotech drugs will eventually eclipse the drug spend on traditional drugs by plan sponsors.

I see specialty pharmacy as providing more evidence of the importance of pharmacists as therapy management specialists. Slowly but surely pharmacy will be a recognized healthcare provider under Medicare. There has been a push for this recognition for a number of years now, but recent events are showing how pharmacists need to be recognized. To this point, there was an interesting article in the Sunday, Feb. 16, issue of The New York Times Magazine, where a woman was in and out the E.R. several times with acute diarrhea. The doctors were having a hard time pinpointing a diagnosis. Numerous tests were done, as well as a CT scan of her abdomen and pelvis. They thought she had celiac disease. It was a pharmacist, on the team making the rounds, who suggested that her condition might be tied to Benicar, the medication she was taking for her high blood pressure. In researching the drug the pharmacist found an F.D.A. warning that described patients who had the same symptom that looked like celiac disease. The patient was taken off the drug and her condition has since improved — end of story. CT

Rx.com Community Healthcare Network Outcomes-Based Healthcare Rx.com Community Healthcare Network, LLC (CHN), is a newly formed Texas company chartered by Rx.com, Partners, L.P., and affiliates PDX, Inc., and National Health Information Network, Inc. (NHIN). By licensing and enhancing derivative products of the PDX intrapharmacy electronic pharmacy record (EPR) and the PDX Care Rx clinical services platform — as well as NHIN’s standardized data files and reconciliation services — Rx.com CHN has launched the largest patient-centric, portable, interoperable electronic pharmacy record in the world. Today, the EPR contains well over 97 million uniquely identified patient records and in excess of two billion prescription transactions attached to the patients’ clinical and billing profiles in 54 chain and independent pharmacy databases in the PDX Intra-Pharmacy Electronic Pharmacy Record Data Center — housed in two mirrored, active-active data centers in Dallas and Forth Worth, Texas. Historical Reflections
E
very now and then it is a good idea to stop and think about where we were and where we are. Thinking about how we got from there to here is worth pondering, if only to get a better understanding of ourselves and our surroundings. I hope the following reflections stir some memories of yours that need revisiting.
Electronics I have been reading a book about the
evolution of electronics over the last 50 years or so. It is by T. R. Reid and is titled The Chip: How Two Americans Invented the Microchip and Launched a Revolution. The two Americans in the title, Bob Noyce and Jack Kilby, both figured out how to make the microchip at about the same time — but went about it in opposite ways. The book also covers many side stories and has clear, layman-friendly explanations about how these things work. My interest in what can now only be called the history of computers is strong for two big reasons: 1) I was fortunate to be a friend of Bob Noyce for a couple of years, and he was an amazing man; 2) When my brother and I put together the first stand-alone pharmacy computer system in 1972, we created it using one of the first microchips. Just a few words about Bob Noyce. We both served on the University of California Board of Regents. He and I were the only members who had any science understanding. Things like light-years and nanoseconds flew right over the heads of some intelligent, in other disciplines, members. He was a kind, gentle, forceful, and brilliant gentleman. I was a fledgling pilot; he flew his own private jet. I was trying to keep financially solvent; he was a multimillionaire. We were good friends. If he had lived long

Now I see changes I used to dream about. Pharmacists are now performing, and getting paid for, more and more clinical functions. We’re using our education and brains for more than counting to 100 by 5s. enough he would have received the Nobel Prize. (He was also one of the founders of Intel.) When I graduated from pharmacy school the most sophisticated calculating device was a slide rule. The primary manufacturer of slide rules gave the last one it made to the Smithsonian in 1976. Electronic calculators totally replaced slide rules and were faster, more accurate, and easier to use. My father was a gadget nut. In his preteens he was one of the first ham radio operators in California. My brother Chad and I inherited his love of gadgets. My brother patented several electronic devices, all of them using the cutting-edge technology of the time. I am not the inventive type. I just like to use the gadgets. We got together and decided that many pharmacy functions could be handled by a computer. Chad knew how to make the computer and how to program it. I knew what went on in a pharmacy. He proceeded to design and manufacture the computer components. I went to work charting out all of the things that happen between receiving the Rx and handing the completed prescription to the patient.
His tasks soon turned out to require much more time than he had, so we hired a programmer. I started out with an 8-1/2-by-11-inch sheet of paper and soon found that I needed long sheets of butcher paper to list and relate all of the things that go on in a pharmacy. And that was when the only third party was the Medicaid program. Things have changed.

I remember making capsules that had Dexamyl granules (the little beads emptied out of the manufactured capsule) mixed with thyroid powder and dextrose. We mixed the ingredients and punched out the

things were a lot different. The name of the drug was never on the prescription container. No patient profiles. We could not refill an Rx without the patient giving us the Rx number. Manual typewriters. If you typed the wrong letter, you had to back up and hit the right one harder. When I paused to think about what I would type next, I always rested my hand on the keyboard, until I began to use an electric typewriter. Resting my hand resulted in the keys all flying at once and getting tangled up trying to reach the paper. Throughout life it seems little habits need to be modified to accommodate new technology. If there were any questions from the patient: “Ask your doctor.” “What’s it for?” “Ask your doctor.” Prices for prescriptions were chaotic. Cost of the drug, plus a percentage, plus a small fee. Prices were from $1.49 to $15.95. Notice they always ended in 9 or 5. If someone asked for a price quote, you would write your price quote on the prescription using the code PHARMOCIST. $1.49 was coded PRS. That way the next pharmacy could be friendly and quote the same price or be hungry and quote $1.39. One day our programmer asked if he would have to program the computer to price prescriptions that were over $40.96. I said: “No. If one does come up the pharmacist will want to handprice it instead of letting the computer do it.” Boy was I wrong. Do you

capsules. How many pharmacies even have the empty capsules on the shelf? We also dispensed a lot of placebos. You could do that because the name of the drug never appeared on the label. We had placebos for most of the Schedule II drugs. The physician would call us and ask if we had placebos for Percodan. We would say yes, and he would say, “Give her 20 tabs; 1 prn pain.” Can’t do that today. I think the current AMA position on placebos is that the prescriber needs to tell the patient that he or she is prescribing a placebo. That sounds self-defeating to me. There were no technicians. There were no computers. And most importantly, there were nowhere near as many drugs and therapeutic options. We have come a long way. Drug interactions were just beginning to be thought about. But that’s another long story. Now I see changes I used to dream about. Pharmacists are now performing, and getting paid for, more and more clinical functions. We’re using our education and brains for more than counting to 100 by 5s. I frequently meet with pharmacy students. I tell them that I wish I was just starting. There is so much to look forward to. Changes in Healthcare Demand Different Thinking
T
here’s an old joke about an extension agent who approaches a farmer and tells him that a $10 book on how to farm better will make him twice the farmer he is now. The farmer scratches his head and says, “Heck, son, I don’t farm half as good as I know how to now.” Times are changing in healthcare, and it’s still early enough in the year to add a few resolutions. Think about each of these changes and challenge yourself as to whether you have aligned your practice accordingly.
Less Provider Centric and More Patient Centric In an
environment where better outcomes and cost containment are still dominant, healthcare is finally realizing that without patients who are engaged and participating in their own health, the whole thing goes thud. As a practitioner who has more contact with patients, this should give you an opportunity to be actively involved in participatory healthcare. You can identify, measure, and make interventions to help patients achieve healthcare goals that are important to them and to the healthcare system in which you practice. This isn’t to say that you will have to figure out a way to be competitive in the distribution aspect of your practice. Accountable care organizations (ACOs) are looking to partner with providers who can deliver efficient and effective transactions, but they really need to identify providers who can help manage patients between encounters. ACOs are predicted to double in number during 2014. Check the Internet to see if there is an emerging ACO in your area with which you can affiliate. We believe that pharmacists are uniquely positioned to help patients when they exit hospitals and doctor appointments, and when they begin new therapies. Adherence has been addressed by former Surgeon General C. Everett Koop, who said, “Drugs don’t work in patients who don’t take them.” Adherence is also closely associated with your medication therapy management service offering. The takeaway is whether or not your practice can legitimately claim that you can deliver a patient-centric, observable, and measurable outcome that engages patients and their nonprofessional caregivers whenever possible.

ONC and HITECH Anniversaries

014 marks the anniversary of two profound industry-changing events: the establishment of the Office of the National Coordinator for Health Information Technology (ONC) in 2004 and the passage of the Health Information Technology for Economic and Clinical Health (HITECH) Act in 2009 as part of the American Recovery and Revinvestment Act. ONC is the primary federal entity that coordinates the nation’s efforts to implement and use the most advanced health information technology (HIT) and the electronic exchange of health information. The position of national coordinator was created in 2004, through an executive order, and legislatively mandated in the HITECH Act. Physician and venture capitalist David Brailer served as the first national coordinator. In January 2014 the nation’s fifth national coordinator, Karen B. DeSalvo, M.D., M.P.H., M.Sc., was appointed. Other national coordinators have included Harvard Medical School professor David Blumenthal, psychiatrist Robert Kolodner, and physician and public health expert Farzad Mostashari. ONC is at the forefront of the administration’s health IT efforts and is a resource for the entire health system in supporting the adoption of health information technology and the promotion of nationwide health information exchanges to improve healthcare. ONC’s efforts received a great boost in 2009 with the passage of HITECH, when $49 billion was earmarked to expand the use of HIT. Money was set aside to promote adoption of electronic health records (EHRs) and their meaningful use, as well as for health information exchanges (HIEs). That investment has been successful in moving the nation’s HIT agenda forward, according to DeSalvo. In her recent blog post on HITECH’s anniversary (www.healthit.gov/buzz-blog/category/from-the-onc-desk), she notes these figures:

Eighty-three percent of the nation’s eligible professionals and 94% of eligible hospitals have signed up for the Medicare and Medicaid Electronic Health Records Incentive Programs. Slightly more than 435,000 professionals and 4,690 hospitals are registered for the programs. Over 335,000 professionals and 4,400 hospitals have received incentive payments for adopting certified EHRs or demonstrating meaningful use. They showed: Almost 80% of office-based physicians used some type of EHR system, an increase of 60 percentage points since 2001 and nearly double the percent in 2008 (42%). About half of office-based physicians surveyed said they use a system that qualifies as a “basic system,” up from just 11% in 2006. Almost 70% of office-based physicians noted their intent to participate in the EHR incentive program.

HITECH also set aside nearly $600 million to establish HIEs. I had the opportunity to give an update on HIE status to participants at the annual conference of the American Society for Automation in Pharmacy (ASAP) this past January. Progress continues, and in February plans were announced by 16 state and regional HIEs to collaborate in an effort to build trust and remove barriers to patient data sharing. The effort moves forward a 10-year-old plan to create a national HIE network. The group’s hope is to ensure that important patient information is available at the point of care. What’s Coming with Track and Trace: Part Two
T
he Drug Quality and Security Act (DQSA) was signed into law on Nov. 27, 2013, and in the last issue of ComputerTalk we discussed the requirements to be implemented under the act in the first three years. Within this time frame, guidance documents will be issued, standards for information exchange will be published, wholesalers will become licensed, and transaction information, history, and statements will be provided for each product transaction. However, the law is slated to take 10 years to fully implement. Here, we will discuss the longer-term conditions and implementation issues expected from 2017 through 2023, by which time the act will be fully operational. Starting Nov. 27, 2017, drug products must contain a product identifier — a two-dimensional data matrix barcode that contains information on the standard numerical identifier, lot number, and product expiration date. The standard numerical identifier will comprise the product’s NDC and a unique alphanumeric code of up to 20 characters. The product identifier should be both human and machine readable. The product identifier will facilitate product recalls because it will contain individual lot numbers of a drug. At this same time, manufacturers will be required to respond to verification requests regarding whether a particular product identifier corresponds to that manufacturer’s product. Manufacturers may satisfy this requirement through an electronic database that could be developed and operated either in-house or by another entity, the latter of which may be the most likely course of action. The development and operation of these manufacturers’ databases could potentially be done by one outside entity, but it remains to be seen if this will be the case. If so, this could add an additional security component, since all product information would be centralized. By November 2020, pharmacies will only be allowed to engage in product transactions if the product is encoded with a product identifier. At this same time, pharmacies must be able to check the lot numbers in their inventory to

determine if potentially counterfeit drugs are present. This will require pharmacies to keep track of the product identifiers in their inventory, and pharmacies may need internally developed electronic databases or have an outside firm design such a database. These requirements lend themselves to the adoption of barcode scanning by more pharmacies, which could be used to track product identifiers. Since many drug products have an expiration date that extends beyond three years, additional issues may be raised if older drugs are still left in the supply chain from before the November 2017 product identifier implementation deadline. Although the act states that exemptions may exist for these products, this introduces another layer of complexity to the act’s implementation. By Nov. 27, 2023, the act specifies that transaction information and statements will be exchanged in a secure, interoperable electronic manner. drug package from the manufacturer to the dispenser. With this new technology, the entrance of counterfeit drugs into the supply chain will become much more difficult and likely require collusion. Wholesalers and third-party logistics providers will be required to obtain licenses through the government and be subjected to inspections, making it more difficult for questionable parties to enter and participate in the supply chain. Furthermore, a database of authorized wholesalers will be published by the FDA and will be available to the public on the Internet. Not only will the new track-and-trace legislation help to reduce the entrance of counterfeit drugs into the marketplace, it will also facilitate product recalls for contaminated drugs. The addition of the product identifier that contains lot numbers means that manufacturers and/or pharmacies will be able to directly identify patients who have received the recalled product. The Pharmaceutical Distribution Security Alliance was a proponent of the passage of this act and is made up of a diverse group of stakeholders, including biopharmaceutical manufacturers, distributors, pharmacies, and third-party logistics providers. Furthermore, the National Community Pharmacists Association, National Association of Chain Drug Stores, and American Society of Health-System Pharmacists all publicly supported the legislation. The support from many different stakeholders is not difficult to understand. This legislature will increase patient safety and ensure that counterfeit drugs do not enter the market to compete with legitimate pharmaceuticals.
