INVITATION IN NV NV TO ATTEND On b On behalf of the organising committee we invite you to attend Medicines Management 2010, the 36th SHPA National Conference, will be held in Melbourne from 11-14 November 2010. This year’s central theme of ‘Staying Alive’ will showcase innovation and developments in hospital pharmacy practice, with particular focus in the areas of chronic disease management, pain control and infectious diseases. A dedicated technician program will feature speakers from around Australia demonstrating novel approaches and expanding roles of pharmacy technicians. The contributed paper and poster sessions will allow delegates to showcase recent advances and updates in hospital pharmacy practice. The Conference will be held at the recently re-developed Melbourne Convention Exhibition Centre, with state of the art facilities and stunning waterfront views.

Centrally located, delegates will have convenient access to the city’s hotels, public transport and all of Melbourne’s famous attractions. Delegates are strongly encouraged to use the social program as an opportunity to network and socialise with colleagues. For accommodation cancellations made within 30 days of your arrival, the hotel/apartment may impose a 100% cancellation fee for the duration of your entire stay.

EARLY ARRIVAL OR LATE DEPARTURE

Please note that the check in time for the Conference hotels is from 2.00pm. Please indicate or notify the Conference Secretariat in writing if you will be arriving early, as it may require you to book the night before to guarantee immediate access to your hotel room. Please also indicate or notify the Conference Secretariat in writing if you will arrive at your hotel after 6.00pm, as failure to do so may mean your room will be released. CHANGE OF BOOKING

Any change to a reservation must be notified to the Conference Secretariat in writing and not directly to the hotel.

PAYMENT OF ACCOMMODATION

No booking will be confirmed without a credit card guarantee or a pre-payment. You need to pay the balance of your accommodation account and any incidental expenses incurred during your stay (for example, mini bar, telephone or dry cleaning) when checking out of the hotel. CREDIT CARD GURANTEE

DRESS

CANCELLATION AND REFUND POLICY

The Conference dress is smart casual for sessions, Wine and Nibbles on Thursday evening and the Welcome Reception on Friday evening. The Gala Dinner on Saturday evening is optional 1970s theme dress up.

Registration cancellations received in writing at the Conference Secretariat by 30 September 2010 will be accepted. Registration fees will be refunded less an A$100 administrative fee. Cancellations received after this date cannot be accepted and will not be refunded. However it is acceptable to transfer your registration to another person. The full name and details of the person that will replace you must be advised in writing to the Conference Secretariat prior to the Conference. No refunds will be made for nonattendance to the Conference.

INSURANCE

Registration fees do not include personal travel insurance of any kind. It is strongly recommended that, when registering for the Conference and booking travel, a travel insurance policy be taken out to cover loss, cancellation, medical cover etc for any reason. The Conference Committee and Conference Secretariat do not take responsibility for any delegate failing to insure.

NAME BADGES

Your official meeting name badge must be worn at all times, as it is your entry to all sessions, the exhibition hall and functions. Security staff will be asked to refuse entry to anyone not wearing their name badge. PRIVACY

Information necessary for your attendance at the Conference will be gathered, stored and disseminated in accordance with the National Privacy Legislation. A participant list with name, organisation and state will be supplied to all delegates and exhibitors at the Conference.
