Social Care Passport (North Wales) NAME:
Social Care in Partnership - North Wales Gofal Cymdeithasol mewn Partneriaeth - Gogledd Cymru
North Wales Social Care Passport The North Wales Social Care Passport has been designed to provide guidance on core training for the workforce working in the social care sector as well as providing a record of CPD and refresher training. This Passport is the responsibility of the individual Care Practitioner. The Social Care Passport provides: the opportunity for managers to ensure a consistent and standardised approach to training and development to meet National Minimum Standards support for the workforce in commencing the Continued Professional Development process providing employees with a portable training record provides guidance for refresher training helps with linking training with QCF units The Social Care Passport acts as a pivotal tool in the development of the workforce. Managers are required to sign and date each course box on completion. The passport is an ‘active’ tool and not a vehicle for gathering dust. The standards within the passport should be applied to the work setting. The passport is a part of the supervision, CPD and probationary processes.

Cyfrifoldeb yr Ymarferydd Gofal Unigol yw’r Pasbort hwn. Mae’r Pasbort Gofal Cymdeithasol yn darparu: y cyfle i reolwyr sicrhau ymagwedd gyson a safonedig at hyfforddi a datblygu i gwrdd â Safonau Gofynnol Cenedlaethol cymorth i’r gweithlu wrth ddechrau’r broses Datblygiad Proffesiynol Parhaus yn cyflenwi gweithwyr â chofnod o hyfforddiant cludadwy yn darparu canllawiau ar gyfer hyfforddiant gloywi yn helpu gyda chysylltu hyfforddiant ag unedau’r Fframwaith Cymwysterau a Chredydau (FfCCh) Mae’r Pasbort Gofal Cymdeithasol yn gweithredu fel erfyn hollbwysig yn natblygiad y gweithlu.
