Open Enrollment October 28 — November 15, 2020


OPEN ENROLLMENT Open enrollment is your annual opportunity to review your benefit options for the next year. During open enrollment you can decide to keep the coverage you currently have or make changes. Any changes you make to your benefits coverage during open enrollment become effective January 1, 2021. If you miss the open enrollment deadline your next opportunity to enroll will be in the fall of 2021 for open enrollment 2022 unless you experience a qualified change of family status.

When is open enrollment? Open enrollment begins October 28-November 15, 2020.

How do I enroll? You will enroll using Dayforce anytime between October 28–November 15th. Click on the benefits icon and choose open enrollment. You may log in as many times as you like during the enrollment period–just remember to save and submit!

What if I want to make changes? Log in to Dayforce/Benefits/Open Enrollment to review your current elections and make changes. Remember that certain changes to voluntary life elections may require evidence of insurability.

What if I do not want to make any changes? If you are satisfied with your current elections, you are not required to re-enroll as all elections except for flex spending accounts (see below) will carry over to 2021. We do recommend that you log in and review and update your life insurance beneficiary information.

Will my flex spending account election roll over to next year? If you are currently enrolled in a flexible spending account (health or dependent care), your coverage will not roll over to 2021. You must make a new election each year for these plans to continue.

Will my PTO automatically roll over on January 1? You are eligible to roll over 50% of the amount you were awarded on January 1 to the next year. This process happens automatically in Dayforce. Below is an example on how the rollover amount is calculated: Jane is awarded 96 hours on January 1, 2020 and has a year end balance of 56 hours. Jane can roll over 48 hours (50% of 96) to 2021.

Where can I learn more? Open enrollment information is available on the Open Enrollment tile in Connect. In lieu of formal meetings we are also offering Benefit Office Hours at all locations during open enrollment where you can drop in and get your questions answered. There also are voluntary virtual meetings for those who may want a benefit refresher!


Consumers 2021 Open Enrollment | Benefits At-A-Glance MEDICAL PLAN

HEALTH CLUB REIMBURSEMENT

Eligibility: Full-time

Eligibility: All employees

Change for 2021: Slight increase in rates.

Eligible employees receive reimbursement of health club or gym membership dues including a one-time initiation reimbursement. Amount received is based on years of service and full- and part-time status.

The Consumers medical plan offers comprehensive care for you and your family through Priority Health. The Priority Health PPO plan offers a broad network of providers, prescription and 100% preventive care coverage for eligible in-network services. DENTAL PLAN Eligibility: Full-time Good dental care is an important part of your well-being. Administered by Delta Dental, this plan allows you to see the dentist of your choice and provides up to $1,500/person in annual benefits (after deductible/ excluding orthodontia). Be sure to log on to www.deltadentalmi.com for more information, resources and tools.

VISION PLAN Eligibility: Full-time Regular eye exams and good vison are important. Consumers offers a comprehensive and flexible vision care plan administered by EyeMed. Through EyeMed you can access a large network of providers and receive yearly well vision exams, glasses or contacts in lieu of glasses.

FLEXIBLE SPENDING ACCOUNTS (FSA)

TEAM CONSUMERS Eligibility: All employees For $4 a pay period, employees who opt in to Team Consumers are eligible for up to $150 reimbursement for paid registration to any competitive or charitable fitness event or club sport that has a fee to participate. You also have the opportunity to volunteer at variable charitable events! Bonus—you also receive awesome TEAM Consumers wear.

ONLINE WELLNESS Eligibility: All employees Sonic Boom is our platform to complete challenges, find wellness tips, take a health risk assessment and so much more. 2021 EMPLOYEE BI-WEEKLY PLAN COSTS

Eligibility: Full-time, Part-time (20+ hours per week) Flex spending accounts allow you to pay for eligible health and dependent and childcare expenses on a pre-tax basis reducing your taxable income. The Consumers FSA plans are administered by BASIC and provide for multiple options to pay and submit expenses. You can contribute up to $2,750 to the health care account and $5,000 ($2,500 if married and filing separately) to the dependent care account. You are eligible to roll over $500 of unused health FSA dollars to the next year.

TELADOC Eligibility: All employees Teladoc provides 24/7 access to U.S. board-certified doctors through the convenience of your phone, email, video or mobile app. Teladoc doctors can treat many medical conditions, and the service allows you to ask questions for non-emergency medical issues or request a prescription or refill. Consumers pays the full cost of the Teladoc benefit.

EMPLOYEE ASSISTANCE PROGRAM (EAP) Eligibility: All employees The Consumers EAP program through New Directions is available to all employees and/or eligible family members. The EAP provides employees with the free, confidential, short-term counseling and referral services. Employees may receive up to eight sessions per issue/ per year at no cost.

2021 Open Enrollment | Benefits At-A-Glance Continued LIFE INSURANCE & DISABILITY PLANS

PAID TIME OFF (PTO)

Eligibility: Full-time

Eligibility: Full-time, Part-time (20+ hours per week)

Basic Life/ADD Consumers pays the full cost of Basic Life/ADD coverage up to $25,000 and you are automatically enrolled in this coverage.

PTO is awarded on January 1 of each year. The annual amount you receive is based on your years of service and full- or part-time status. At the end of each calendar year you are eligible to roll over up to half of your annual amount awarded to the next year.

Voluntary Employee Life You can elect voluntary life coverage up to five times your base salary up to a maximum of $500,000. Evidence of insurability is required if you are increasing your current coverage by more than $20,000 during open enrollment. Voluntary Spouse Life You may elect coverage for your spouse from $5,000 to $100,000. The amount you elect in voluntary spouse insurance must be at least equal to your own voluntary life insurance coverage. Evidence of insurability is required if you are newly enrolling in this plan or increasing coverage by more than $20,000 during open enrollment. Dependent/Child(ren) Life Insurance You may choose up to $20,000 dependent life coverage without evidence of insurability. The amount you elect in dependent life insurance must be at least equal to your own voluntary life insurance coverage.

SHORT-TERM DISABILITY (STD) Short-term disability provides income replacement to eligible employees if you become disabled and unable to work for reasons that are not work-related. After a seven-day waiting period, the benefit provides 70% base pay replacement up to $1,000 for a total of 13 weeks. Consumers pays the full cost of short-term disability coverage.

LONG-TERM DISABILITY INSURANCE

PAID MEDICAL LEAVE (PML) Eligibility: Full-time, Part-time (20+ hours per week) Consumers provides eligible employees 40 (FT) or 20 (PT) hours of paid medical leave per calendar year. PML can be used for sick time, health care appointments, short-term disability waiting period and other health-related issues. PML hours do not roll over from year to year.

Long-term disability benefits begin after a 90-day disability period. This benefit provides eligible employees 60% base pay replacement to a maximum of $10,000 per month. Consumers pays the full cost of long-term disability coverage.

You choose how to save and invest through before-tax and/or Roth after-tax contributions and a variety of investment options through Principal Financial. Consumers matches 100% on the dollar up to a 10% contribution. While you are always 100% vested in your own contributions, you become fully vested in the Consumers match after six years of 1,000 hours of service a year.
