CONFERENCE SCHEDULES PRECONFERENCE: THE MANAGEMENT OF ACUTE-ON-CHRONIC ILLNESS

Exacerbations of acute-on-chronic conditions may have major short- and long-term implications on the patient and healthcare system. Early recognition, timely and effective treatment of exacerbations are crucial to improve outcomes and mitigate the burden of these conditions. This preconference focuses on strategies to managing acute-on-chronic illnesses in Primary Care. Presenters will discuss how to recognize warning signs and establish aftercare protocols pertinent to the Primary Care practice.

OBJECTIVES: • Recognize common acute-on-chronic scenarios in different organ systems. • Understand how underlying chronic illness can predispose to acute exacerbation. • Focus on aftercare of individuals with certain chronic illnesses to manage and prevent future exacerbations.
