Once the user enters their login and password, they are taken to this screen which provides them access to all of the website features. The look-up features in the center of the screen allow the user to search for an individual participant already in the database by their case number, social security number, driver’s ID number, name, or date of birth. This screen can also be used to create a list of all referrals, participants, or all persons in the database. In addition, a case manager can create a roster of their caseload of either open or closed cases. To add a new referral, the user would click on “add referral” on the left side of the screen.
After hitting “Add Referral� the user is taken to the referral management form where all information can be entered about the prospective participant. The user can enter as much or as little information as they have, and can return at any time to add additional data.
The user will indicate whether the person was eligible for the program (yes/no) and if eligible, did they enroll (yes/no). If the person is ineligible, the referral will automatically close when “submit” is hit. If the person is eligible but opts to not participate, their case will also be closed. If the referral is both eligible and opts to enrolls, they will automatically become a participant upon hitting “submit.” When a referral becomes a participant, all of the data entered on the referral form will automatically populate on the participant form. The main participant form also allows the user to enter other pertinent information about the new court entry. This data can be added and edited at any point while the case is open.
When a case is open, there are many screens that can be accessed for entering data. When using the search features to find a participant, a screen will come up where the user can select the data screen they would like to visit. Once in a participants case file, there will always be a column on the left that allows the user to select any of the data screens (both to enter new data and to view/edit existing data).
Participant Form: This form contains basic data on the participant including demographic data, assessments, and criminal history information.
Contacts: The contact form is designed to be a case management tool where both case managers and stakeholders can enter data about interactions with participants.
Intake Evaluation: This form is completed when a participant is enrolled into the program to provide data on their interactions with the criminal justice and healthcare systems 12 months prior to enrollment. This will be critical data for conducting outcome evaluation to compare their behavior pre and post their participation in the court.
Treatment Plan: This form allows the case manager to lay out the participant’s plan while enrolled in the court. The plan can be edited as often as needed while the case is open.
Treatment Court Services Form: This provides a place for case managers to track all the services that a participant has received through their time with the court. It can be edited as needed while the case is open.
Drug Tests: This form captures the results of all drug tests administered during a participant’s time in the court program.
Status Report: This form allows case managers to create regular status reports (i.e. monthly, quarterly) to keep stakeholders abreast of participant performance.
Conclusion Form: This upper portion of this form is completed when the participant leaves the court (either successfully completing the program, or upon an unsuccessful removal from the program). The latter portion of the form is completed 12 months after graduation/removal for the purposes of comparing pre-program performance with post-program performance.
Follow-Up Form: This form provides a place for case managers to record all interactions with a participant after they have left the court.
