I am calling to share with you a great opportunity for << youth name >>. I assume you are aware that << youth name >> was cited on <<date>> (or name the day, such as “last Friday”) for <<law violation>>? -or- Are you aware that a truancy referral for << youth name >> was sent to the Douglas County Attorney’s Office? I’m calling to invite you and << youth name >> to a team meeting to discuss an opportunity so that we can address << youth name >>’s overall well-being. The purpose of this one-half hour long meeting will be to determine if we can avoid prosecution of these charges and further court involvement for your family and for << youth name >>. Does this sound like something that you think could benefit << youth name >>. I would like to schedule time with you and << youth name >> to the next scheduled meeting which is at <<time>> next Thursday <<date>> at the Juvenile Assessment Center. Do you know where this is located? (I would confirm the appointment before going into more details about the process.) The team will include people who have supported or worked with your family currently or in the past. We welcome others that you or << youth name >> identify to be part of this scheduled meeting. If there is someone who is a very supportive person in << youth name >> ‘s life, such as a case worker, a therapist, a mentor, or anyone like that, they can come to support <<youth name >> as well. Please invite them and pass along all the detailed information I will provide to you on this call. Here is a list of people who will be at this meeting: Myself, and representatives from NFC (your Family Permanency Specialist), Project Harmony, and the Douglas County Attorney’s Office. <<Provide details regarding JAC address, directions, parking- we will have a one page map/ directions/ info ready to fax, etc.>> Would you like me to fax or email you a map and directions to our office, my contact information, and some additional information about the meeting? (will have brochure and map/ directions available in electronic/scanned and hard copy versions) <<if “no” ask them to write down name & number >> I have a couple of quick questions that will help us stay within the 30 minute time frame at << youth name >> ‘s team meeting:
1) What school does << youth name >> currently attend, and what is his/ her current grade level? (this will be known on truancy cases) 2) What programs or services is << youth name >> currently involved in? This can include counseling, after school programming, anything like that.
Again, << youth name >>’s team meeting will be: <<time>> next Thursday <<date>> at the Juvenile Assessment Center. Please make sure to arrive on time, as there will be other team meetings occurring just before and just after << youth name >>’s. Do you have any other questions I can answer today?
