To house individuals in the New Haven community who have a history of incarceration and shelter stays by participating in a multiagency collaborative that includes resources from DMHAS, DSS, CSSD, DOC, OPM, Connecticut Coalition to End Homelessness and the Corporation for Supportive Housing. Identified frequent users have struggled with substance abuse, mental health, homelessness, lack of employment, lack of social networks, lack of basic needs and trauma. Bachelor’s degree preferred or Associates with 2-3 years’ experience working as a social services case manager. Valid Driver’s License, CADAC or CADAC eligible preferred, computer literate and physically able to move through the community; knowledgeable of the greater New Haven substance abuse, mental health and social services network of providers; empathetic engagement skills; cultural competencies; life training and group skills. Must be reliable, punctual and able to work independently while maintaining client confidentiality and respecting proper relationship boundaries; must be willing to accept direction and guidance from supervisors. Pathways to Independence Manager Job

Provide leadership and oversight to the Pathways program serving homeless adults participating in the following services: housing stability, employment services, benefits counseling, and peer support services.
