Look no further, because medically enrolled employees can achieve just that by completing the health assessment, biometric screening and certifying a tobacco-free status. If you are a tobacco user, you will be required to sign up for and complete the Lockton Nurse Advocate Tobacco Cessation Program to avoid a surcharge in 2025.
A wellness surcharge of $75 per month will apply to premiums in 2025 if you do not complete the biometric screening and online health assessment.
A tobacco surcharge of $50 per month will apply to premiums in 2025 if you do not complete the tobacco affidavit.

If your benefits effective date is on or after Sunday, June 2, 2024, you are not required to participate in the Wellness Program. No surcharge will apply for 2025!

Body mass index (BMI) less than or equal to 30, or a waist circumference of less than or equal to 35 inches for females and less than or equal to 40 inches for males
Blood pressure less than or equal to 135/85 mmHg
HDL cholesterol higher than or equal to 50 mg/dL for females and higher than or equal to 40 mg/DL for males
Triglycerides less than or equal to 150 mg/dL

Follow the steps to log in to Your Asset Health Portal on the web or app, then when asked for your login credentials, click “Forgot password?”. You will then receive an email to reset your password.
If you have an Apple Watch, please download the Asset Health Mobile app first and activate HealthKit* within the app to sync.
*Apple HealthKit is an app that allows all health and fitness data collected from your iPhone, Apple Watch, etc., to be stored in one place. It can help to simplify your process of syncing activity data for future wellness challenges.

By syncing your mobile or steps tracking device with the Asset Health portal, you can automatically record your steps or activity data so you can participate in the numerous challenges available without having to do a thing! Self reporting is also available.
You can authorize one device per account on the Asset Health portal. Please follow the instructions below.
1 Once you are logged in, click View All on the corner in the right and select Settings from the fly out menu.
2 Once on the Settings page, click Authorize Device on the left side of the page. Then, click the blue Authorize button.
3 Scroll through the list of approved devices until you find yours. Then, click Connect below your device.
4 You will now be taken to the device account login page. Here, you must log in using the username and password you created during the setup of your device. This device account username and password are not associated with the Asset Health portal.
5 Please verify that you are logged in to the correct account (not the account of your spouse or another family member), then click Allow* to give Asset Health permission to pull activities from your device.
6 Now that your device is authorized, be sure to sync your device often with its parent app to ensure your data is up to date. (Example: Sync your Fitbit device with the Fitbit app.)
*Attention Fitbit users: Please do not uncheck any of the boxes before clicking Allow. Although Fitbit requires all boxes to be selected in order for your device to properly sync, DFA only receives minutes of physical activity and steps.
Where do I go for on-site biometric health screenings?
Your location may be offering on-site screenings in partnership with Quest Diagnostics as an option to complete your biometric screening in 2024. Please check with your local HR representative for more information.
How long will it take to get my biometric screening results?
If your Physician Results Form is complete with the required risk factors, you will receive your results online within 10 days and a paper report in the mail within three weeks of submitting the form. Please allow 14 business days for your biometric screening activity to be marked complete within the Asset Health portal.
If it’s been more than 10 days since you uploaded or faxed your form and you haven’t received a Results Are Ready email (indicating your form is processed) or a Physician Form Rejected email (indicating there is an issue with your form), call the Quest Diagnostics’ Service Center at 1-855-623-9355 for a status update.
Can I access the Asset Health portal in Spanish?
Currently, you may only access the Asset Health portal in English. Please contact your local HR representative if you need assistance.
How do I determine if I’ve met all the Wellness Program requirements?
Log in to the Asset Health portal and view the Wellness Program progress tile on the homepage to check your completion status. You’ve completed the program when all your activities are marked as complete. Check your program completion status.
How do I access the health assessment on the Asset Health portal?
For medically enrolled employees with a benefits-effective date prior to Sunday, June 2, 2024: Log in to the Asset Health Portal, click on the Know Your Numbers tab and complete the health assessment activity. Once you finish the short quiz, this will be marked as complete.
What sections must my physician fill out on my Physician Results Form?
Your physician must complete the health care provider section of the form, including signature, date, test(s) performed and Unique Physician Identification Number (UPIN)/National Provider Identifier (NPI). Your physician will need to fax the completed form to 1-844-560-5221, or you may upload the completed form to my.questforhealth.com using the Upload Form button on the dashboard.
When do I need to submit my Physician Results Form to Quest?
You are responsible for ensuring your form is returned to Quest Diagnostics by Saturday, Aug. 31, 2024.
» My Physician Results Form was rejected. What do I do now?
If you submit an incomplete form and your email address is on file, you will receive an email notifying you that your Physician Results Form is incomplete. If there is no email address on file, you will receive a notification letter in the mail. Then, you will need to re-submit the completed form following the same submission steps again.
Can I talk to a Lockton Nurse Advocate even if it is not required?
Yes, anyone is welcome to participate on a voluntary basis even if you are not encouraged to complete the Lockton Nurse Advocate TCP because of your tobacco attestation results. You can schedule an appointment with a Lockton Nurse any time by calling 1-888-251-2260, through the resource tab on the Asset Health portal, or by scanning this QR code to link to the online scheduler.
What happens if I am unable to make my scheduled Lockton Nurse Advocate call?
If you are unable to make your scheduled appointment, you must notify a Lockton Nurse Advocate at least 24 hours prior to the scheduled call. If there are no appointments available, you will be prioritized as soon as there are new appointments available. If you do not show for your scheduled appointment, you will receive an automated email letting you know that you will need to reschedule your appointment. If there are no available appointments and you did not provide the necessary 24 hours’ notice, you will not be prioritized once new appointments are available.
How do I know if I need to register for the Lockton Nurse Advocate Tobacco Cessation Program (TCP)?
If you attest to being a tobacco user when completing the tobacco affidavit on the Asset Health portal, you will be required to complete the Lockton Nurse Advocate TCP to avoid a tobacco surcharge in 2025.
How do I register for the Lockton Nurse Advocate Tobacco Cessation Program (TCP) and what are my options?
You must register for the Lockton Nurse Advocate TCP on the Asset Health portal by Monday, Sept. 30, 2024, and complete it by Wednesday, Nov. 27, 2024. The program consists of three calls and six educational modules.
If you choose to complete the Lockton Nurse Advocate TCP, you also have the option of participating in two Tobacco Cessation webinars in place of the three calls. This will consist of two live webinars reviewing the TCP. Webinar 1 of 2 will go over modules 1-3 of the TCP packet, and webinar 2 of 2 will cover modules 4-6. If you are interested in doing this instead of the three 1:1 calls, please register for two webinar options by Monday, Sept. 30, 2024.
Tobacco use refers to any form of tobacco consumption, including smoking cigarettes, cigars, pipes, hookahs and vaping. Smokeless tobacco products, such as chewing tobacco and snuff, are also considered tobacco use.
Are nicotine gum, lozenges, pouches and patches considered tobacco use?
Nicotine replacement therapy, such as gum, pouches and patches, is considered a form of nicotine use but not tobacco use, as it does not involve the inhalation of smoke or the use of tobacco products. With that being said, the Lockton Nurse Advocate TCP does not provide free nicotine replacement therapy products, but the program does offer other forms of support and resources to help individuals quit tobacco and nicotine use.
