Hospice of the Western Reserve

Most Compassionate Palliative Care Providers 2019 - Cleveland Headquartered in Cleveland, Ohio, Hospice of the Western Reserve (HWR) provides hospice services, palliative care and bereavement support to patients and families throughout Northern Ohio. As one of the original legacy providers of hospice in the U.S., HWR is celebrating 40 years of compassionate care. We profile the firm to learn more about the vital work it undertakes. Hospice and palliative care have been Hospice of the Western Reserve’s (HWR) specialties since 1978. Over the past 40 years, the hospice has worked to enhance its service and meet the diverse needs of its patients. As part of this, it has created a range of initiatives, all of which are designed to improve the quality and scope of care.

For example, the Western Reserve Navigator palliative care program complements curative care provided by the patient’s own doctor. When offered concurrently, it provides extra support to manage symptoms, reduce hospital readmissions and enable individuals to be more independent. Alongside this, HWR has also designed a special model of care to integrate palliative care at the point of service within dialysis

centers. Community partnerships have been formed with the dialysis centers and the Kidney Foundation of Ohio. Specialty care for patients with chronic conditions can help alleviate symptoms, improve quality of life and decrease hospitalizations. With the help of medications, clinical support and other interventions offered by a trained staff, patients can benefit much earlier in their illness, beginning with palliative care through hospice care, if that is their choice. The organization’s Moments to Remember Program focuses on enhancing the quality of life

for hospice patients and their families by fulfilling wishes. Specialized care for Veterans is provided through the Peaceful & Proud program. Since its inception, the non-profit agency has cared for more than 120,000 Northern Ohioans. Today, specialized care is provided for cancer, renal disease, COPD, HIV/AIDS, neurodegenerative diseases, heart failure and more. Additionally, a Pediatric Palliative Care team supports children with life-threatening illness. Seeking to provide patients with the highest possible standard of care, the organization advocates for dignity, quality and choice

at the end of life nationally and globally. Together with its sister hospice, Helderberg Hospice in Somerset West, South Africa, Hospice of the Western Reserve recently received the 2018 Global Partnership Award from Global Partners in Care. The award recognizes the exceptional impact the international partnership has played in the delivery of palliative care. It is the organization’s staff that have driven it to the success it enjoys today. HWR offers the region’s largest team of certified hospice and palliative care professionals. Care is provided in private residences, nursing facilities, assisted living

communities and hospitals. Three In-Patient Care Units (hospice houses) offer intensive symptom management in a homelike setting as well as care for the final days. Many nurses have earned the distinctive credential as a Certified Hospice and Palliative Care Nurse (CHPN). All physicians are Board Certified in Hospice and Palliative Medicine, and as such, they are able to offer patients the expert care they need at this vital time in their lives. As part of its drive to support staff and ensure that they are able to provide the exceptional standard of care that patients deserve, HWR’s internal culture

encourages honest dialogue, respect for the feelings, values and cultures of others, teamwork and commitment to mission. A harmonious work environment is fostered by following covenants established by the founders. This culture ensures that the hospice attracts and retains the region’s best skilled hospice professionals. Currently, a comprehensive dementia initiative is underway to reach individuals earlier in the disease process to communicate their long-range healthcare choices. Through collaboration with other dementia experts and community organizations, HWR is working to explore innovative new ways to provide family caregivers with the education,

support and tools they need to provide loved ones with a better quality of life. This development is one of many that will ensure that the hospice remains able to address the ever-growing need for hospice and palliative care over the years to come. By 2030, it is estimated that the number of seniors in Northern Ohio will be almost equal to the number of people under the age of 20. Longer lifespans also mean more seniors are living with chronic disease, creating a growing need for hospice and palliative care services, and as such, HWR will remain committed to providing cutting-edge hospice services and care as it looks towards a bright future.

, Mindme

Best Vulnerable Personal Alarm Development Company – UK & Best Bespoke GPS Tracking Device: Mindme Locate Thanks to over 10 years’ experience in the market, Mindme is an innovative GPS tracking device provider supporting a range of corporate and private clients who wish to ensure safety for vulnerable family members, friends or patients. We profile the firm to find out more. Over the past decade Mindme has played a leading role in the development of GPS personal alarms for vulnerable people. The firm’s Founder and Chief Executive has been involved with GPS since it first became commercially available over twenty years ago, and as such is able to draw on a vast wealth of experience to supply innovative solutions that support vulnerable people across the UK. In the very early days Mindme asked Chichester Careline to act as its Response Centre and so began a relationship that continues to this day. When Mindme launched Locate it was not greeted with open arms. The company was accused of breaching Human Rights and/or Privacy issues. Matters came to a head when Sussex Police decided to trial some units and the Daily Telegraph ran a front page headline ‘GPS tags for dementia patients’.

The news story went viral and was covered by all the UK television news channels, most local radio stations and newspapers from around the world. Carers from all over the country rang national and local radio talk programmes and many told horrendous stories of how their loved one went missing and the trauma it caused the family. Today Mindme Locate is the market leader in dementia tracking and is acknowledged to be an important aid in the welfare of people with dementia who wander. It gives device users the freedom to go out alone whilst it gives their carers peace of mind. Looking ahead, Mindme will continue to innovate and adapt to ensure that its solutions remain at the forefront of emerging developments within both the GPS tracking technology and social care markets.

, N.ableD C.I.C

Best Exercise & Nutritional Therapy Providers 2019 - Southern England N.ableD is a home-based health and wellbeing service for those who are looking to get their health back on track following an injury, illness or a recent hospitalisation. We spoke to Founder Nadine Denneth to find out more about the unique range of services her firm has to offer in this vital sector. Drawing on Founder Nadine’s experience with the UK’s health and social care system, N.ableD works to provide an innovative range of solutions, which includes exercise therapy, nutritional therapy, physiotherapy, yoga therapy and massage therapy. Supporting a range of clients of all ages and abilities, the firm works collaboratively with clients to offer them a unique service offering, as Nadine highlights.

“N.ableD is the only home-based health and wellbeing service that works collaboratively across multiple professional disciplines to support and empower vulnerable people to achieve great health and improve their quality of life. Our focus is on increasing our clients’ physical activity levels,

so we can help them to reduce or avoid unnecessary social care arrangements further down the line. “For example, strength and balance training can reduce the risk of falls in the home, yet people often mistakenly believe that exercising with a long-term condition will make things worse. More rather, the more conditions someone has, the more they need to improve their strength, stamina, flexibility and skill, which is why the work we do with our clients is vital in helping to prevent further deterioration and escalation of health and social care needs.” Looking ahead, Nadine is keen to drive the social care industry towards a greater focus on preventative measures to reduce the burden that many disabilities have on the NHS, as she explains in her concluding comments.

“Moving forward, at N.ableD we would like to see more being done to prevent healthcare problems, such as frailty and cognitive decline - both of which can be significantly improved with exercise and nutritional therapy, to help reduce the costly, and often unnecessary, burden of social care. Our focus will remain on supporting our clients in this area, and we are thrilled to be recognised as one of the pioneering firms leading the way forward to address one of the biggest health and social care issues of our time. We will continue to lead the market in terms of recovery therapy and are excited for what the future holds for us and our clients.”

Best Specialist Dementia Care Home 2019 - Southern England The New Grange is a residential dementia care home located on the south coast in Worthing, West Sussex and welcoming a wide range of residents to enjoy its renowned hospitality. Having recognised the establishment in this year’s Social Care Awards we profile it to find out more about the services it provides. Caring for up to 45 people aged over 65 and over, the New Grange specialises in supporting those living with dementia. Part of a small independent care group with five homes around the south east of England, all of which care for people living with dementia, the New Grange draws on this expertise to offer its residents the best possible care. In addition to providing specialist care the group invests heavily on the infrastructure of the homes to create comfortable and wellappointed living environments.

Grange faces stiff competition, but thanks to its dedication to quality and reputation for excellence in dementia care the establishment has flourished over the years. Looking ahead, the New Grange’s future depends on the government’s attitude as to how it

supports local authorities, which support residents and often pay for their care. , UK Healthcare Market

Recruitment Key Issue For UK Healthcare Market As Brexit talks continue with little chance of a definitive deal in sight, the UK’s healthcare market’s core challenge over 2019 will be attracting and retaining qualified staff. While there are still many details to be ironed out over the UK’s impending departure from the European Union, one key focus remains on freedom of movement. The general lack of information has led many private and public healthcare organisations in the country to question whether they will be able to retain and attract the staff they need throughout 2019 and beyond. Niall Dickson, chief executive of the NHS Confederation, which represents organisations across the healthcare sector, highlighted how Brexit could affect the NHS

“Patients must not suffer as a result of the Brexit process. Even if there is no deal we cannot have no agreement about how to protect patients in the UK and across Europe.

“While we welcome no-deal preparations made by the UK Government, this is an issue for both London and Brussels. We have seen the EU is willing to make arrangements in a number of areas – including finance. We now need to see the same urgency from both sides on health.

“We are therefore calling on both the UK government and the European Commission to guarantee patient safety whatever happens. That means free movement of medicines, free movement of medical devices and supplies for as long as it takes to resolve these concerns.

“This is an unprecedented situation for the health service. Of course, the NHS can adapt to emergencies, but that doesn’t mean we shouldn’t seek to avoid them. No deal and no arrangements to protect patients is simply not acceptable and could put lives at risk.”

and how it would need to be protected against healthcare staff shortages.

In response to the staffing challenges which the NHS and wider healthcare market could face over the coming months, the Minister of State for Care recently helped to launch off a home care company’s latest campaign to encourage more people to consider a career in care. MP Caroline Dinenage has pledged her support for the Home Instead Senior Care ‘You Can Care’ campaign which raises awareness of the opportunities to work in care and highlights how fulfilling a role it can be.

“Care work is a hugely rewarding career which has a real impact on people’s lives, supporting them to enjoy their later years,” she said. “It’s great to see a home care business which is so passionate about helping older people age well. Quality care like this plays a crucial role in enabling older

people to stay living healthily and happily at home for longer.” The rollout of ‘You Can Care’ 2019 follows the success of last year’s campaign which saw hundreds of people using the hashtag #YouCanCare on social media to share inspirational stories about caring. It culminated in the first UK awareness event called ‘You Can Care’ Week which aimed to dispel the myths around working in care and celebrate those working in the sector. Since launching the campaign in January 2018, Home Instead Senior Care has recruited 5,500 CAREGivers across its 195-office strong franchise business. The company specialises in caring for older people at home and is expert in dementia care plus end of life and respite care. Currently, the charity Skills for Care estimates that 8% of adult social care roles are vacant, which equates to approximately 110,000 vacancies at any one time. Looking ahead, whatever happens with Brexit, the UK healthcare industry will continue to focus on recruitment over the coming months. The NHS began its 2019 recruitment campaign with a range of adverts designed to entice new candidates, but moving forward both the private and public sectors will have to band together to lobby the government for greater clarity on movement rights so that they can retain and attract the best possible health and social care staff over the years to come.
