What if we could give Tennessee babies the best possible start in life? We can.

There may be nothing a new mother can do for her baby that is more healthful or critical to early development than breastfeeding. If you know a new or expecting mother, there’s nothing like a solid circle of support by family, friends, and health professionals to encourage breastfeeding. So get on board. Breast milk. Babies know it’s perfect. Visit www.healthytennesseebabies.com to see for yourself just how perfect. New mothers can find support and encouragement by calling the Tennessee Breastfeeding Hotline at 855-423-6667 (855-4BFMOMS).

Tips for Air Travel with Your Infant

Y

ou can be a cool, collected traveler with your infant on a plane with these easy tips:

• RESERVATIONS: Make them early to get the bulkhead section so you have more room. Children 2 and younger fly for free as long as they're on their parent's lap, but your infant will still need a boarding verification document to get through security.

• SPEAKING OF SECURITY:

D

on’t just have a baby shower — have a fun one! Let laughter and joy flow with a few, silly games:

the other guests to wail like a baby. Fastest one wins! When the game is over, give the contents of the bag to the new mom.

FAMOUS PARENTS

DON’T STICK BABY!

As guests arrive, place a sticker on their back naming one half of a famous couple with children. The guests need to figure out who they are by asking each other questions and then go find their mate.

Purchase cloth diapers, diaper pins, and pink or blue round balloons. Blow up the balloons and add baby faces with markers. Test the diapering skills of your guests by asking them to put a diaper on the "baby" – without sticking it and popping the balloon. This is harder than it looks.

FIND THE BINKY Fill a diaper bag with items you'd expect to find inside (and maybe a few things you wouldn’t!) like bottles, wipes, a diaper pad, jar food, blankets, etc. Hide a pacifier deep inside the bag. Challenge guests to find the pacifier the fastest while coaxing

DON'T BREAK YOUR WATER This outdoor game is for casual parties! Fill several water balloons and have your guests compete in a relay race, carrying water balloons

between their knees and walking quickly from point A to point B without breaking your water. Team with the most filled water balloons wins!

DON’T SAY BABY Use ribbon and small baby pacifier charms to make necklaces. As guests arrive, give them a necklace and explain that the purpose of the game is to collect the most necklaces. Tell them they can't say the word "baby," or whoever hears them say it can take their necklace away. Let the game continue to the end of the shower! Order a pack of 100 multi-colored pacifier charms on Amazon for $7.50. — Pam Molnar

Car seats and all baby paraphernalia have to be scanned by security. Breast milk and infant formula are not limited to the 3.4 ounce rule. The Transportation Safety Administration states that parents may bring a “reasonable” amount with them through security, but it needs to be separate from other items and “declared” when you arrive. Do not put any kind of fluid for your baby through the X-ray machine.

• EXTRA CLOTHES: Aside from all of your wipes, diapers and such, have at least two changes of clothes for your infant packed in case of messes — as well as a change for yourself in case of spit up. Also pack gallon-sized zip-lock bags for soiled items.

• CHANGING BABY: There are teeny-weeny changing stations (albeit cramped) in the bathroom of most planes.

• CRYING BABY: If your infant begins crying on the plane, know that it's OK. Babies cry! Try to remain calm yourself. People who don't understand, don't get it. (please turn the page)

BABYBITS A Baby at 40?

J

an Watts remembers being 41 and pregnant with her second child. "I was at our swim club with my 3-year-old and following him quickly around the pool's edge," she says. Her friend, dangling her toes in the water, laughed and said, "Glad it's you and not me!" "She didn't mean any harm," Watts says, "but I've never forgotten it, either." Having a baby at 40 — or in your 4os — is much more commonplace than it used to be, and the majority of older mothers have normal pregnancies. If you’re physically fit, eat well and don’t have preexisting health conditions such as diabetes or hypertension, your overall risk of other pregnancy complications isn’t markedly higher than that of a woman in her

Post-Partum Doulas 20s or 30s, says Elizabeth Oldfield, M.D., with Tennessee Women's Care. “The main risk for pregnancy after age 40 is for fetal chromosome abnormalities such as Down syndrome,” says Oldfield. Women who are pregnant after 40 also have a higher risk of gestational diabetes, hypertension and C-section delivery. Pregnancy in your 40s is different depending upon whether it's your first baby or your fourth, but if it's your first, one thing makes a big difference for first-time older moms: the gap between you and the younger moms. "I am definitely more emotionally mature than I was in my 30s," says Watts. "But if there's any drawback at all to being an older mom, it's that I don't have a lot of younger mom friends, so I have to really work at making them because I want my boys to have lots of friends his age — and we moms make that happen." But would she change her experience if she could? "I wouldn't change a thing," she says. "I absolutely adore my little men and I am blessed to be their mommy!" — Susan Day

You're up all night, exhausted and long for extra help. A doula — a mother's helper — may be just what you need. Doulas provide in-home care for three to four hours a day to help your family (and especially you) adjust to life with your newborn. They provide what you need so you can focus on resting and bonding with your baby. Several options are in our area, too.

Beware Online Breastmilk New moms and dads can buy anything they want online, including human breast milk. And while the U.S. Food and Drug Administration recommends against providing infants human milk from unscreened donors, some mothers are nevertheless buying it online. According to the American Academy of Pediatrics (AAP), researchers recently tested 102 Internet samples of breastmilk and found 11 percent were contaminated with storebought cow’s milk or milk-based formula powder. Of the positive samples, 10 were high enough to rule out incidental contamination. Cow’s milk can be problematic in infants with a milk allergy or intolerance. Because buyers can't test the milk they purchase online, the AAP says parents should be aware the milk they are buying might not be 100 percent human milk and use extreme caution.

YOU

CAN HAVE A

DESIGNER NURSERY By Kiera Ashford

It's not just any room. Your baby's room can be a posh palace just for him with the right help. Here's what local experts have to say about getting the nursery you'll LOVE.

I

remember gazing out into the open space while gently and unconsciously rubbing my round, pregnant belly, leaning against the door jam of my soon-to-be baby's nursery wondering. Thinking. Planning. What's to become of this big, empty space in the months before his arrival? What do I really want to do for him? Will he like it when he gets older? Questions ran through my mind on a reel. I did what just about every other expecting mom would do; chose the easy way out and planned his room to be that stereotypical "boy" room decorating it in a sports theme. The nursery is one of the most exciting parts of having a baby. So, don't let it overwhelm you and don't overcomplicate it. Local experts can help you turn that ordinary room into something stylish and spectacular.

WHERE TO START? Coming up with a concept or theme for Baby's nursery is sometimes a difficult task. Years before, parents were decorating their baby's room in the newest Disney licensed gear or popular children's character. Nowadays, as parents are showing more of an interest in modern design and style, they're more inclined to polish up the standard nursery, but things can get tricky ... and fast.

YOU

CAN HAVE A

DESIGNER NURSERY "Stay away from commercialized themes," says Sara Ray, owner and lead designer of Sara Ray Interior Design (615-578-2090; sararayinteriordesign.com). Determine the best way to make the room feel like you and reflect how you use it. She notes that inspiration can come from just about anything from a cute baby outfit, a piece of art, something found in nature, etc. It doesn't have to all come from the same store or even manufacturer to fit just right.

PLANNING FOR THE FUTURE "A baby’s nursery should be very functional for all the users — baby and parents," says Ray. She encourages planning the space where everything's easily accessible. "Base your furniture arranging and selections on how you intend to use the space," says Ray. "Make the space baby and toddler friendly from the beginning so there’s no re-designing when he becomes mobile. Personally, I like spaces that can grow with the child, so my nurseries tend to incorporate sophisticated furnishings with layers of fun art, bed linens and décor that give the space an overall theme of sorts," she adds. "Collect things from different sources to create a much richer and interesting space," says Ray. She encourages you to incorporate at least one vintage item as she does in all of her designed spaces. "By using more sophisticated furniture pieces from the beginning there isn’t a need to continually buy furniture as the child gets older, the crib being the exception, of course," she adds. That is, unless you find the perfect crib that turns into a toddler bed and then a day bed.

CURRENT STYLE IDEAS When I was pregnant with my son, Pinterest wasn't around, and I didn't have a steady flow of inspiration to gawk at every day. Themes came by way of what the stores carried — limiting the creative thinking of soon-

14 • spring/summer 2015

to-be parents. However, as social media colors our creativity with inspiration from everywhere, those themes or design aspects are changing. What's popular now is evolving. Style's becoming a "thing." If you're wanting to stay on the path of trending themes or ideas, Ray suggests using themes such as woodland/camping, galaxy/space or even vintage floral. Use of an overall neutral color with pops of bright colors is trending, too, as well as bright, shiny, bold or even neon geometric shapes.

OFF-THE-WALL DESIGN Looking to add more than just a slab of color to the walls? Maybe a mural is what you need. D. Ellen Ingle, owner of Murals by Ellen (931-607-7652; delleningle.com), says, "With every mural I paint, I strive to make it feel timeless, as in it won't feel old or stodgy within just a few years. This means making the right design choices and can even be as simple as the color selections." You want to create an environment that's fun, inviting and allows plenty of room for the imagination to take flight. With that, she advises against adding human characters. "By leaving them out, the kids who are in the room can become the 'characters,'" adds Ingle. If you need encouragement coming up with an idea, Ingle's happy to help. Since the planning is such a long phase of this sort of project, be sure to allow at least two weeks for it. "Actual painting can take anywhere from one afternoon to three days to even a full week," says Ingle. However, that depends on the project. With these tips and ideas, you'll have more than just a nursery. You'll have a room that feels welcoming — a nursery that's functional and makes a bold statement, too. Kiera Ashford is associate editor for this publication and mother of two, ages 8 and 4.

The Baby Guide

A mural, like this one from D. Ellen Ingle of Murals by Ellen, could be the stroke of elegance your Baby's room needs.

BREASTFEEDING BASICS By Mary Helen Berg

Nursing your baby may be a little tough at first, but a few basic tips can help get you started on the path to an enjoyable breast-feeding experience.

S

ometimes the natural way of feeding your baby doesn’t come naturally at all. It surprises many new mothers how hard nursing can be at first. When the baby books say breast is best, mothers who struggle to breastfeed may feel they are failing their baby. What the books may not say is not all babies adapt easily to nursing. Some babies are fussy and others won’t latch on. With patience and persistence, new mothers and babies can learn about nursing together. Follow these basic tips and nursing can be the intimate, soothing experience you've heard about.

16 • spring/summer 2015

GET COMFY

GET READY

Find a comfortable, serene place to sit and feed your baby. The room doesn’t need to be silent. Every baby must become accustomed to the sounds around their home, however, the early days of nursing should take place in a spot where you can relax. Try lying in bed so you can rest while Baby nurses. If you sit in a chair, use a pillow to prop the arm where Baby will rest. Take deep breaths. Drink a cup of chamomile tea. Sometimes stress can inhibit milk flow. It's important to relax. Being calm helps your milk let down.

Position the baby so her entire body faces yours and her head rests in the crook of your arm near your breast. If Baby is lying on your lap face up and needs to turn her head to nurse, she may not be able to latch on correctly. If you hold her head in your hand instead of letting it rest on your arm, she may react to the pressure of your hand and push her head back instead of relaxing toward the breast.

LATCH ON Lift the breast and hold it toward the baby’s mouth to help her latch on. She will likely “root” around until she lands on the nipple. Make sure the baby

The Baby Guide

takes the whole areola in her mouth. If she catches just the nipple it will be painful for you and difficult for her to nurse. If a pinching pain occurs when she begins to suckle, don’t yank her off the breast. This can make the pain much worse. Take your pinky finger and put it in the corner of her mouth to break the suction. Back her off the breast and try to reposition her onto the nipple. If she is in the correct position, there should be no pain. After about five minutes, switch her to the other breast. Nursing from both breasts will encourage milk supply and help avoid engorgement and clogged milk ducts. As your

baby becomes a better nurser and your breasts become less sensitive, you can leave her on each breast for a longer amount of time.

TAKE CARE Some new mothers may experience cracked or bleeding nipples in the first few weeks of nursing. This usually occurs because the baby is not positioned correctly while nursing or has trouble latching on. To soothe cracked nipples, try these simple, inexpensive remedies: apply warm tea bags or compresses before and after nursing; smooth lanolin over the sore area; take a mild pain reliever about a half hour before nursing to ease the pain; change breast pads often to keep nipples dry. If problems with pain or bleeding persist, consult your doctor or lactation consultant. Breastfeeding is not always as easy as it looks. With determination and a little inside information, every mother can offer her baby the strongest possible start.

Lactation Consultants: What You Need to Know

Your body. Feeding your baby. Simple enough. Why would you need a specialist to show you how it all works? The first weeks of breastfeeding for some new moms can be exhausting, frustrating and painful. Nursing's not always easy, but it's a skill that — with a little patience and determination — anyone can learn ... maybe with even a little help from a lactation consultant.

WHO IS A LACTATION CONSULTANT? Lactation consultants come from a variety of backgrounds. The most highly trained are International Board Certified Lactation Consultants (IBCLC) certified by The International Board of Lactation Consultant Examiners. Their training includes 90 hours of lactation education and up to 1,000 hours of supervised clinical work as well as college level courses in biology, anatomy and nutrition. These professionals are committed to helping women breastfeed and have more classroom and clinical training than other breastfeeding counselors. Some registered nurses or other medical professionals also may take courses to augment their education so they can better assist women with breastfeeding.

WHAT THEY DO Your consultant may be based in a hospital or she may have a private practice. You may go to her office or she may make home visits. She will examine you and your baby to rule out obvious physical problems that may hinder breastfeeding. Next, she will talk to you about how to position your body, how to hold your baby and how to place him on your breast and help him latch on. Then, she will ask you to nurse while she observes your process. She can advise you on how long to nurse and how often. She will support you by confirming what works, coach you on alternative approaches and may provide a written plan for you to follow when she leaves.

LACTATION CONSULTANTS HELP WHEN … • You don’t live near female relatives who can show you the ropes, or you're the first of your friends to have a baby. • You're anxious about nursing. • You had a multiple birth and need advice on feeding two or more babies. • Your baby has nipple confusion and won’t take your breast after trying a bottle. • Your nipples are cracked and sore. • You have a low milk supply. • You need to learn how to express your milk. • You have physical issues such as inverted or flat nipples. • Your breasts are engorged and painful. • Your baby has trouble latching on.

HOW TO FIND ONE Some hospital facilities contain lactation centers that will send a specialist to your room to work with you before you are discharged and provide service for up to 10 days after Baby's

WHAT’S THE COST? Lactation consultants are trained healthcare specialists, so don’t expect a bargain. Fees can range greatly according to your location and your consultant’s experience, among other variables. Check with your insurance provider as it may cover some lactation services under obstetric or first year pediatric care. The good news is a lactation consultant can usually target your problem and quickly get you on the right track. Within just a few visits, you and your baby can be nursing like champions. Mary Helen Berg is a freelance writer.

healthy babies tennessee says breast is best!

E

vidence shows breast-fed babies are at lower risk for a variety of health issues. That's why a coalition of health-care organizations are working together on new effort, Healthy Tennessee Babies. The Tennessee Department of Health, Tennessee Initiative of Perinatal Quality Care, Tennessee Hospital Association and Tennessee Center for Patient Safety aim to boost awareness of immediate and long-term health benefits of breastfeeding. The coalition is doing so through a campaign in hospitals, doctors' offices, social media and beyond. planning a pregnancy? prep for it!

E

very woman planning a pregnancy wants a healthy experience for herself and her baby, and a new service in Middle Tennessee is here to help. Prep for Pregnancy — available at medical facilities in Nashville, Brentwood, Franklin and Smyrna — assists women in preparing for a healthy pregnancy long before conception. "We are a consultant practice," says Rosalyn Baxter-Jones, M.D., an OB/GYN with the program that also includes a nutritionist and health coach. "We are not here to replace a woman's OB; we're an extra arm to the OB," she adds. Prep for Pregnancy offers women an extensive evaluation process that includes pinpointing any vitamin or nutritional deficiencies as well as food sensitivities that can lead to infant allergies and digestive issues. Further evaluations include gut bacteria as well as hormonal levels and any medical issues that can affect a healthy pregnancy. Baxter-Jones recommends the primary consultation between six months to one year prior to getting pregnant, although she says women who are newly pregnant can benefit, too. The program assists with proper diet modification to ensure a healthy pregnancy. Some insurance companies cover the cost of the services, and the program offers installment plans for patients who need them. Learn more at 888-341-5556 or visit prepforpregnancy.com.

K

eep Baby safe in the back seat when you're behind the wheel. The Tennessee Child Restraint Law has specific requirements to ensure the safety of all child passengers, and the following criteria is specific to infants:

• Must remain rear facing until 1 year and 20 pounds, although the American Academy of Pediatrics recommends remaining rear facing as long as possible until the child reaches the upper weight limit of a convertible car seat. • In a rear-facing car seat, the shoulder straps should be at or below Baby's shoulders. • A rear-facing seat should never be placed in front of an active air bag. • The car seat should not move more than one inch side to side or front to back when you pull on the belt path area.

Children's Hospital at Vanderbilt performs free car seat safety inspections throughout the year. For dates and locations (along with more seat safety tips), visit vanderbiltchildrens.com and search "car seat safety."
