Babies Cry A Lot ... But, When is it Not OK?
N
ewborns cry in order to communicate: "I'm hungry;" "I'm tired;" "My tummy hurts;" and onward. New parents know their infant will cry, and they spend an awful lot of time trying to figure out why. The problem is, all babies are different, and some cry more than others. In fact, in a 2017 study published by the National Institutes of Health covering nearly 9,000 babies from around the world, it was found that: • Newborns tend to cry for around two hours a day. • Crying for more than two hours a day is more unusual. • If your baby cries for more than three-and-ahalf hours a day, it is considered "high." • Between a third and a fifth of babies cry for long stretches, without obvious reason, during the first three or four months of age. If your infant cries excessively, it may be colic — which is inconsolable crying around the same time of day (often late in the day) and marked by urgent, painful stress. Colic usually starts around 3 weeks of age, peaks at 6 weeks and improves by 3 months of age, says Laura Jana, M.D., author of Heading Home With Your Newborn (American Academy of Pediatrics). Soothing a colicky baby is practically impossible, so if you suspect it, call your pediatrician. And no matter what, never let your frustration or irritation with your baby's nonstop sobs get the better of you. Call a family member of friend to help with the baby and give yourself a break. Can You Take Medications and Breastfeed Safely?
our out of five new moms breastfeed their infants in the United States, according to the Centers for Disease Control and Prevention (CDC). And out of those moms, half take medications. That's nearly 2 million women taking medicine while breastfeeding, so the question of safety comes up a lot. Since limited information on the safety of many medications while breastfeeding is available to moms, you need to follow your doctor's advice. Some safe medications that are OK for babies during breastfeeding (according to the Food and Drug Administration) are acetaminophen (Tylenol), ibuprofen (Advil)

and amoxicillin (antibiotic). And you might think that common sedatives are OK during Tennessee's long allergy season, but some of them can cause breathing problems in infants, according to the CDC. These include chlorpheniramine (Advil Allergy Sinus), diphenhydramine (Benadryl Allergy and Vick's ZzzQuil) and doxylamine (Unisom). Non-sedating antihistamines are a better choice and include loratadine (Claritin), cetirizine (Zyrtec), and fexofenadine (Allegra). Pump and Dump Of course, if you're prescribed an unsafe medication during the course of your breastfeeding, don't just stop breastfeeding altogether. Instead, use the pump-anddump method. Breastfeed before taking your medication and pump and dump the breastmilk until at least two hours after your medicine intake. When in doubt, pump and dump, and if you have questions, consult with your doctor and/or lactation consultant to be on the safe side.

orey Engmann never considered hiring a postpartum doula until she learned she was expecting twins. Feeling overwhelmed and worried about how she would handle twin babies, along with the pressing needs of her 2-year-old, she turned to Teresa Marshall, a certified birth and postpartum doula, to help her after her twins arrived. Marshall spent five nights a week for four months at Engmann's home after the birth of the twins, providing support, encouragement and help. Her assistance enabled Engmann to get the sleep she needed to recover from a physically demanding pregnancy. (please turn the page)

"During the day I could be present, loving and enjoying all three of my children. It wasn't the endless cycle of fatigue and frustration as it sometimes was during the first few months with my first child."
What is a postpartum doula?
The first six weeks after a newborn arrives can be a mixture of happiness and anxiety as moms try to settle into a new routine — a routine that takes time to come. Many new mothers lack support from the start of postpartum because they feel they should be able to do things themselves, not to mention that many people are continuing to social distance when it comes to newborns. With COVID-19 still here, finding healthy support through a doula may be exactly what you need. "With postpartum depression a risk, it's so important for women not to be isolated," says Teresa Marshall, who in addition to her work as a birth and postpartum doula, is a facilitator for a pregnancy and postpartum depression support groups. The support of a postpartum doula can be especially helpful to mothers who: • Have a history of depression or post partum depression. • Don't have close friends and family nearby to rely on. • Are expecting multiples. • Have other little ones demanding attention. "I am convinced that Teresa saved my life," Engmann says. "As most moms have felt at one time or another, I was often wondering and questioning if what I was doing was the right thing for my children -- and with twins, the responsibility is unbelievably overwhelming," she adds. Mothering the Mother A mom who spends hours alone with her baby can easily spiral into emotional and physical exhaustion, which can put her at greater risk for postpartum anxiety or depression. During this challenging transition period -- often called the fourth trimester — a postpartum doula can provide calm reassurance and support to a new mama and her family. "When moms feel supported at home and they are getting what they need, the rates of postpartum depression are

reduced," says Kate Kripke, LCSW, an expert in perinatal mood and anxiety disorders and contributing writer for PostPartumProgress.com. "As soon as that mom starts to struggle and suffer, she will no longer have what is going to be required to care for her baby the way she wants to. Not because she isn't a good mom or doesn't love her child, but simply because she's human," Kripke says. B
e sure to keep Baby safe while riding in the back seat when you're behind the wheel: motor vehicle crashes remain the leading cause of death among children. The Tennessee Child Restraint Law has specific requirements to ensure that the safety of all child passengers and car seats in proper use will actually reduce the chance of death by 71 percent for infants and toddlers. Nearly 85 percent of safety seats in Tennessee are installed incorrectly, according to the Monroe Carell Jr. Children's Hospital at Vanderbilt. There are 29 fire stations and police departments that provide free car seat safety checks in the following counties: Bedford, Cheatham, Coffee, Davidson, Dickson, Franklin, Marshall, Maury, Montgomery, Putnam, Robertson, Rutherford, Sumner, Williamson and Wilson. The following car seat criteria is specific to babies: • Child must remain rear-facing until at least 1 year old and 20 pounds, although the American Academy of Pediatrics suggests children remain rear-facing as long as possible until they reach the upper weight limit of a convertible car seat. • In a rear-facing seat, shoulder straps should be at or below the child's shoulder level. • A rear-facing seat should never be placed in front of an active air bag. • Car seats should not move more than one inch side to side or front to back. • Harness clips should be level with the child's armpit. aking the decision to leave your baby in the care of others isn't an easy one, but if you go that route, do your homework. The American Academy of Pediatrics (AAP) suggests the following questions to help you make the best child-care choice: LICENSING/ACCREDITATION Is the center licensed or registered with the appropriate local government agencies? Are there any outstanding violations? Is the program currently accredited or in the process of becoming accredited? VISITING POLICY Can you visit the center before your child is enrolled? If your child is enrolled, can you visit the center anytime it's open? Can you see all the areas your child will use? Are visitors screened or is their ID checked so only approved adults can visit the center and pick up children? HEALTH PROFESSIONAL Is there a qualified health professional, such as a doctor or nurse, for the program? STAFFING Are there enough trained adults available on a regular basis? What happens if staff are ill or on vacation? What are the child-staff ratios? What education, training and experience do the child-care providers have, and are they CPR certified? neonatal intensive care unit (nicu) services key: Level I: Facility provides basic care for uncomplicated maternity and neonatal patients who are low risk. Level II-Obstetric: Facility provides a broad range of maternal-fetal services for normal patients and for those with mild or moderate obstetric illnesses or complications.

Level II-Neonatal: Facility provides specialty services for infants born at or beyond 32 weeks who have problems that are expected to resolve rapidly and are not anticipated to need subspecialty care on an urgent basis. These units provide care for babies convalescing after intensive care.
Level III-Obstetric: A broad range of maternal-fetal services for normal patients and those in need of intensive care. Can handle deliveries for all gestational ages. Level III-Neonatal: Provides care for infants born less than 32 weeks or with complex medical conditions. These units have the means to provide life support for as long as needed.
Baby Guide
Level IV-Neonatal: Has capabilities of Level III units with additional functions and considerable experience in the care of the most complex and critically ill newborns. Pediatric medical and surgical specialty consultants are on site 24 hours per day. Can provide surgeries for complex conditions.
