Colic 101:

Get Support if Baby Won't Stop Crying

Y

ou've heard about it and hope it doesn't happen to your baby. Colic. By definition, colic is a condition seen in infants younger than 3 months old marked by periods of inconsolable crying lasting for hours at a time for at least three weeks. It's hard on Baby and hard on parents, occuring in up to 40 percent of infants. “Babies can be sensitive and experience discomfort for various reasons,” says Jon E. Betts, M.D., of Old Harding Pediatric Associates. But before you decide your baby has colic, Betts advises you rule out constipation or other discomforts. “Constipation — characterized by firm stools rather than the frequency of stools — can cause pain and discomfort in infants,”

says Betts. “However, many infants grunt, groan and turn red in the face while trying to have a bowel movement, and this is normal and not constipation if the end product is a soft stool,” he adds. Babies can experience discomfort for many reasons, including hunger, an imbalance in their tummy, fatigue, excessive stimulation, or being too cold or overheated. Why babies develop colic is unknown, but it has certain qualities that differ from other forms of crying:

• Colic generally sets in between 2 and 4 weeks of age and resolves by 3 to 4 months of age.

DISTINGUISHING COLIC

• Lay your baby on his tummy over your knee or forearm. Pressure can help soothe an overactive tummy.

• While all babies cry (often up to three hours a day), colicky babies tend to cry more frequently and intensely.

• Colicky infants can cry throughout the day with it worsening throughout the day, peaking in evening hours. • Babies with colic tend to pull up their legs, have distended bellies and gas.

SOOTHING A COLICKY BABY • Hold and rock your baby. Babies often like gentle, rhythmic motion.

• Offer a pacifier. • Swaddle snugly.

• White noise can comfort a colicky baby. Some parents suggest hair dryer noise, vacuum cleaners and clothes dryers. White noise apps are available, too.

SOOTHING YOURSELF • Having a colicky baby is hard. Many parents can grow frustrated (more than 5 percent of babies are harmed for excessive crying, according to research). If you're feeling stressful because of your baby's inconsolable crying, put the baby in a safe place and walk away, even for a few minutes in order to regroup. • If Baby has colic, get support from a trusted friend, sibling or other relative. Let them help you by taking turns with the baby. Support will be a sanity saver. — Kiera Ashford

Mothers' Milk Bank of Tennesssee

new program to help nourish vulnerable newborns and pre-term babies is up and running in the state. Mothers' Milk Bank of Tennessee (MMBTN) is a not-for-profit providing safe donor human milk to benefit infants in need within Tennessee. In late March 2016, MMBTN opened its first Milk Bank Depot locally at Nashville General Hospital (a second location is in Memphis). The depot serves as a collection site for frozen human breast milk. Donated milk is sent off site for pasteurization before its distribution to Tennessee hospitals to prescribe to pre-term and critically sick babies. "Nashville General Hospital is pleased to join Mothers' Milk Bank of Tennessee as one of the first donation sites in Tennessee. Our hospital is located in one of the highest areas for infant mortality in the state, and by joining their efforts, we hope to make a difference in the lives of the babies served," says Joseph Webb, CEO of Nashville General Hospital.

Eligibility requirements for moms wishing to donate their breast milk include: being in overall general good health; non-smokers who consume less than two alcoholic drinks per day; willing to undergo a blood test (at the milk bank's expense); not regularly using medication or herbal supplements (with a few exceptions); willing to donate 100 ounces of milk initially and continue pumping to donate as you are able; nursing an infant who is younger than 1 year of age (bereaved and surrogate mothers can also donate); be able to arrange for transportation of your milk to the depot; and more. The application process includes an initial phone screening prior to the blood test screening to rule out HIV, RPR, HTLV, and hepatitis B & C. The approval process takes between one and four weeks. TOP PREGNANCY CONCERNS FOR MOMS-TO-BE By Kiera Ashford

Newly expecting, there's a lot on your mind. Here's help for some of the many things you're thinking about now.

T

he thought of adding another baby into my family mix makes me excited and nervous. My extended family is over-the-moon for us — except my father. His first words about my big announcement? "I'm really worried about you and preeclampsia," he said dryly. And I get it. I had preeclampsia with both of my former pregnancies ... but that also means I'm prepared to handle it again. Women can be overwhelmed by pregnancy worries large and small, so, we've tackled a few of them for you:

miscarried. It was then I realized I spoke too soon. So, what timing is best for breaking the news to everyone? "A lot of families tell everyone immediately; others wait until the second trimester out of fear of loss," says Heather Rupe, OB-GYN at Women's Group of Franklin and co-author of The Pregnancy Companion (Leafwood Publishing; 2011). "It's a personal decision based on the risks of loss. I counsel my patients that once you see the baby's heartbeat at 8 weeks, the risk of miscarriage drops to less than 5 percent. By 12 weeks, the risk is less than 1/200, so at that point the chance that everything will be OK is 99.5 percent."

what the law is regarding leave. According to the U.S. Department of Labor (pertaining to maternity leave), under the federal Family and Medical Leave Act of 1993 (dol.gov/ whd/fmla), employers must provide unpaid leave for up to 12 weeks for employees to care for a new child. And consider your work environment. If you work with chemicals or are exposed to radiation, tell your boss that you're pregnant immediately. It falls on your employer to assess workplace risks in order to keep your baby safe. Keeping your boss in the loop also lays the groundwork for a good relationship going forward.

TOP PREGNANCY CONCERNS FOR MOMS-TO-BE brother or sister is on the way. Sometimes younger children welcome the idea of a new sibling while older children (especially tweens or teens) may become angry. Take their feelings and different personalities into consideration. If your young child is upset about another baby on the way, give him time. Nine months is a long time for a child and he's apt to forget about your pregnancy until the delivery is close and he can see that mommy is big! As time moves along, let him help in picking out outfits, toys or anything else you may be needing for the baby. And for those older kids "embarrassed" by your "condition," ... again, give them time. When the baby finally arrives, love will melt it away.

MISCARRIAGE The thought of miscarriage may not cross your mind unless you've miscarried before. According to the March of Dimes (marchofdimes.org), "Miscarriage is when a baby dies in the womb before 20 weeks of pregnancy." But the risk of miscarriage may be lower than you think: for women younger than 35, it's 10 to 12 percent; for 35- to 39-year-olds it's 18 percent. It rises to 34 percent for women 40 to 44 years old, but you should still know that most miscarriages are due to a chromosomal abnormality. The March of Dimes says you can improve a healthy pregnancy by not smoking, drinking, or using street or prescription drugs.

HIGH-RISK & MEDICAL CONCERNS If a previous pregnancy was high risk, is your current pregnancy automatically high risk, too? Only if your first high-risk pregnancy was due to a pre-existing condition that you still have. Aside from that, your OBGYN may deem your pregnancy high risk if you develop complications or you have a high chance of developing them. Common high-risk conditions include being younger than 17 or older than 35; having a chronic medical condition such as obesity, diabetes or hypertension; developing gestational

diabetes; carrying multiples; a previous miscarriage; a previous premature birth; prenatal testing indicating a birth defect; or a history of fibroids or other gynecologic problems. Try to rest easy, though. Just because you're deemed high risk doesn't mean you're going to have problems. And during a high-risk pregnancy, you'll be monitored extra carefully, so that should give you some peace of mind.

MAKING ROOM FOR BABY Where's the new baby going to sleep? Maybe your home has an extra room, but if not, consider using your bedroom as a nursery also for the time being. It's increasingly popular to make room for Baby with what you have using Pinterest and new-mom blogs for ideas. If Baby's to share a room with a sibling, consider your older child's preference for the room arrangement, and do the best you can to make the transition comfortable for him.

TRAVELING WHILE PREGNANT There's no reason to stop traveling during pregnancy unless your doctor rules it out for some reason. If you're considering a long road trip, remember to factor in extra stops so you can get out and walk, stretch your legs and use facilities. "For any type of travel, including long car rides, frequent breaks to allow for walking around and staying well hydrated are recommended to decrease the risk of deep venous thrombosis (DVT) and/or pulmonary embolism (PE)," says Gina Hamrang, M.D. at Stonecrest Medical Center. "Pregnancy predisposes women to these two conditions as a result of elevated estrogen levels and venous stasis," she adds. And whenever buckling up, be sure the strap rests across your thighs under your abdomen — never across your belly. What about flying? "Occasional air travel during pregnancy is generally safe," Hamrang says. However, if you have high blood pressure, anemia or

have had a previous miscarriage, your doctor may advise against it. The Pregnancy Encyclopedia (DK; 2016) says "It's best to ask your doctor for advice that's right for you." Also know that most airlines — while policies may differ — will not accept pregnant flyers after 36 weeks, and most will request a letter from your doctor. 6 MONTHS+ Touch and Feel (Show and Tell!) Spend early playtime days showing your baby different objects. Show Baby anything that's safe! Spoons, whisks, books and magazines with pictures, record albums, colorful

fabrics or clothes, fruits and vegetables, and so on. Keep a little stash of objects beside you and sit with Baby. Hold an object about a foot from his face and stare at it together, telling him about it. Now it's reading time ... bring out a board book and let him touch. While he probably won't sit through a story right away, he eventually will. Then he'll develop favorite books and stories. Babies love looking at books and cuddling with you, and to keep him interested in books, use different voices and modify stories as needed!

Peek-a-boo! At about 6 months old (or sooner, depends on the baby), take a floaty scarf and fling it into the air, letting it settle on your baby's head. As he gets older, you may notice him starting to play with other items like blankets or towels. Variations on peek-a-boo: The classic: Hold up a towel between your face and your baby's, and ask, "Where's Jack? There's Jack!" over and over again. You can vary this game in a million ways. Hide behind a door and make your baby push it open to see you. Hide behind a chair and pop out first from above then from the sides. Go behind a corner with another person and alternate who gently coos, "Boo!" Keep a selection of hats and pop up wearing a different one each time. Another giggle-getter is to put a hat on your head, low enough to cover your eyes, and let your baby take it off, saying "Oh!" in surprise each time he does it.

Sing Songs It doesn't matter one iota if you don't like your singing voice; your baby does. Now's the time to sing away.

"The Wheels on the Bus," "Row, Row, Row Your Boat," "Head, Shoulders, Knees and Toes," and "PattyCake," to name a few. (If you don't remember the words to a favorite song, just look online.) You may feel silly at first, but keep at it. As your child gets into it, so will you. Try adding your baby's name to the song: "Old Mac Jackie had a farm," "Kate is my sunshine, my only sunshine," and so on. Try songs with silly sounds or animal noises in them, like "How Much Is That Doggie in the Window?"

Bubbles, Bubbles Everywhere There's something magical about bubbles, and at 6 months your baby can see far enough away to focus on them. Blow bubbles when he's getting fussy and watch the tears dry up. Blow bubbles in your backyard or at the park where other little ones may come along and entertain your baby in the process. Blow bubbles in the bathtub or out on the porch when it's late afternoon and your baby is cranky. Bubbles are cheap and easy to take along wherever you go.

This Little Piggy This little game goes way, way back. With a barefoot baby on your lap, wiggle one of his big toes and say, "This little piggy went to market," then move on to the toes next to it and say, "This little piggy stayed home," and move through each toe with, "This little piggy had roast beef," "This little piggy had none," And ... on the last teeny toe squeal, "This little piggy went wee-wee-wee all the way home!" As you say that last part, run your fingers up your baby's belly. Giggles all around!

Tummy Time It's important to have your baby spend time on his tummy, even if he protests vociferously.

-BOO!" your baby will love By Sandra Eby Get down on the floor with your baby. Look at him as you lie on your own belly. Lay your baby down on a towel and use it to gently roll him from side to side. Try saying, "Oops-adaisy, oops-a-daisy" as you roll him.

Fly, Baby, Fly! Now that your baby can hold his head up, it's time to hoist him into the air. Pretend he's a rocket ship, flying him over you and making realistic rocket noises. You can also play like your baby is in an elevator, which advances up floor by floor before sinking quickly to the bottom.

Touch It, Hold It, Bang It If your baby has one object, he'll bang it on the table. If he has two objects, he'll bang them together! Just make sure you give him bangable objects and do it with him. Bang them separately on the table, hit the table with both at the same time, see if the object sounds different when hit using the left hand rather than the right hand, and so on. Help him by handing over objects that make interesting sounds: hollow containers, metal spoons, bells. Pay attention to tactile sensations as well: Your baby will be fascinated by the way different things feel.

Roll Play Babies are fascinated by balls and how they move. You'll get a big laugh by juggling or tossing balls up in the air and letting them hit the floor while you make a silly sound effect: "Whoops!" Roll a soft ball toward your baby and watch him grab and squeeze it. Eventually, with encouragement, he'll roll the ball back toward you. Someday, he'll be able to kick and toss the ball or drop it into a big bowl or bucket. For now, bounce and roll.

show your baby how to pour water, sand or cornmeal from one to the other, or into a larger container.

Cruisin' Once your baby is up on his feet, you can encourage cruising by placing a favorite toy at the far end of the couch or over on the coffee table. Try enticing your baby by putting one of your toys, such as your phone or sunglasses, a distance away and cruising on your knees toward it. Your baby may find this amusing and attempt to join you. Encourage your baby to push an object around the room. Push toys and large empty boxes work well. Avoid folding chairs, which can fold up unexpectedly.

The Bath Is Fun No longer is your baby content to sit in the tub and be washed. Older babies want to stand up, splash, grab your hair, pat the shower curtain and so on. (Note: Never leave a baby unattended in the bath, not even for a minute.) Encourage the fun by adding lots of toys to the tub. Plenty of stuff around the house can be endlessly filled, drained, poured from or into, and floated. Poke holes in the top of a plastic bottle with a flat cap to make a homemade watering can. Let your baby feel the sensation of the water dripping onto him and show him how to cut off the flow by blocking it with his hand. Use your homemade toy to give his rubber ducky a shower. At the end of the bath, drain the toys in a plastic colander or a net bag suction-cupped to the side of the tub. Hopefully your baby is clean, happy and ready to sleep. Wasn't that fun? Give your baby a box that's easy to open (like a shoe box) and show him how to put things in and take them out. Another way to play this game: Get a bunch of cups (stackable measuring cups work great) and

hat was once a trend is now mainstream as more and more moms and dads embrace babywearing. The art of carrying your baby snuggly against your body has actually been practiced around the world for centuries, but it's only recent that it's become prevalent locally. Babywearing International (BWI) of Middle Tennessee — a chapter of the international organization — hosts free "Learning Library" meetings every month at various locations throughout the mid-state. Parents can learn all about BWI along with trying on different carriers. Lots of factors determine babywearing comfort, including Baby's age and height along with the parent's size and body.
