What are some things I can do at home to begin a healthy oral hygiene routine? Read your child books about going to the dentist. Sesame Street’s “Ready, Set, Brush” is a great example. You can also prepare your child for what’s to come by starting proper home oral care at an early age. Then they will be used to getting their mouth cleaned and taken care of, and won’t be surprised at the dentist. You can also show your child that oral care isn’t scary! Brush and floss your teeth in front of them, and act enthusiastic about it. Then, when you have to go to the dentist, talk about it excitedly. Avoid talking about pain or discomfort and emphasize how healthy their teeth and smile will be afterward. This will help ensure your child that it isn’t a scary place.

Can giving my baby a pacifier cause orthodontic problems later in life? Instinctive sucking is a very strong urge in infants during the first year of life. Not only does their survival depend on it, but it also provides them with physical and psychological comfort. Giving your infant a pacifier at this age is perfectly fine. After the first year however, the need for sucking decreases and is no longer necessary for survival. This is the ideal time to start weaning your baby off their pacifier. Dental studies have shown that pacifier use past two years of age can cause significant and undesirable changes to the upper jaw and the bite.

DAY BY DAY Born Free Once Baby starts feeding himself spoon to mouth, the growing away from you begins. Your clinging on does, too. After all, you’re charged with the care and upkeep of this small soul for years to come. You’ll want to manage his life for a long time, but the human experience has other plans. I know. I have four children, and three of their bodies are adult size. My fourth is growing tall and reedy and still getting taller. I adore my children. I have not had to force that, it just happens. It doesn’t matter if your kids are wild, sweet, funny, serious, outgoing or quiet — love opens your mother heart far and wide into places you never knew. As you watch them climb, swim, run, leap, crash, race, swing and catapult, your heart marvels and wonders, palpitates and panics. You’ll be baffled by how utterly different your kids are. They’ll pick on each other for their differences long before they celebrate them. You have no choice but to be steeped like a drenched tea bag right in the middle of their lives. It is marvelous … if you play it right. Huh? Play it right? Yes, you can cling too much. You can hasten their growth away from you if you’re a needy mom. They will sense that you won’t let them do this, you won’t let them do that — and they’ll do it anyway on the sly. Or, you can build a kid so accustomed to your deciding and doing everything for him that he can’t do anything for himself; he has to consult you first. If possible, simply become your child’s “bumper” in the bowling alley of life. I made a point with each of my kids to have a morning where I went with each of them to an open, safe field away from our home where no one was around. Where we’d just walk together. And then we’d start to skip and run. Then I’d let my little one go ahead of me, saying, “GO!” And my child would run, run, run, arms open wide to whatever, running toward nothing … in total freedom. Face flushed, they’d all eventually stop and look back. It’s true. More and more we need to take our children to safe places and to instill within them the idea that they ARE free, not captive, not living in a box, and definitely not owned by us. When you find yourself alone — not with your child’s arms all over you or your own arms heaving baskets of laundry or groceries or housework — go outside. Be aware of your freedom, your self-sufficiency. It’s weird, but take a spin like Maria does on that mountain in The Sound of Music. Arms wide, feel that freedom. Know your child needs that same feeling to come alive inside him, and you’re a part of that. One child at a time, you let them go. Mistakes will be made, but that’s how they will learn. Once your baby starts doing things for himself, it’s now your turn to monitor and to begin the unfolding ... not the molding. With your secure self ... he will mold himself. And if you let him go ... it WILL be beautiful.

MIND & BODY Careful! Medicine mistakes with the kids may happen more often than you realize. By Susan Day

n a New York University Department of Pediatrics study released last year, it was found that most parents mistakenly give their kids the wrong dose of liquid medicine — sometimes twice as much as they should — and that the culprit is most often the plastic measuring cup. You wonder: How can it be so hard? Instructions are printed on the box! Well, you know, parents are busy. Kid medicines are liquids, for the most part, and when parents have to decipher between printed abbreviations — milliliters, mL, etc. — it gets confusing. While the American Academy of Pediatrics now recommends liquid dosing be done only in milliliters across the board, plenty of us still have the little plastic measuring cups around. When it comes to dosing kids, take your time. Mark instructions out clearly with a Sharpie on the little cup if someone else is doing it for you, and stay on top of it. Safe med measurements aside, there are lots of different “mistakes” that can come up with kids. Here are a few more:

Waking Kids for Meds

Your child is sleeping through the night and missing a dose of amoxicillin for his strep. Should you wake him? “If your child is sleeping comfortably, I

would not wake him,” says Steve Riley, M.D., at The Children’s Hospital at TriStar Centennial. “Rest is important for fighting illness. However, if your child’s experiencing discomfort sleeping and the medicine is supposed to be taken on a routine schedule, you can wake him,” he adds.

Using Benadryl for Travel

Parents can be pretty cavalier about this one. You don’t want the hassle of a fidgety kid on the plane, so a quick dose of Benadryl should do the trick, right? Not so fast. Some kids can actually have an adverse reaction to Benadryl and become hyper instead. Research from Georgetown University Medical Center shows that Benadryl does not help young children sleep; some become more hyperactive. “If the purpose of taking Benadryl is to sedate your child, no,” says Riley. “Benadryl is used to relieve symptoms of allergies or colds. It’s not recommended to pre-medicate a child before flying or extended travel.” Now ... treats while traveling? That’s an absolute must — and think fun activities and patience!

One’s Sick ... And Now the Other It’s an ongoing problem when your kids are little that sick germs spread to each of them.

Your 7-year-old has strep and the next thing you know, she’s passed it to your 10-year-old ... or so you think. It’s tempting to just use the 7-year-old’s meds, right? Skip the doctor for the second child? Nah. Don’t treat a child with your other child’s medicine. It’s important when treating with prescription meds that a child complete the entire cycle of recommended medicines. “Please contact your pediatrician,” says Riley. “Although your other child may have symptoms, it is important to receive a diagnosis from a medical professional.”

Less kid-dosing mistakes are made when using an oral syringe as opposed to that little plastic measuring cup.

n too much of a hurry for your baby’s sunscreen protection? Don’t be. Build a little extra time into your day to completely cover Baby before going out. His skin needs a solid 30 minutes for that protective film to develop, so you might put it on during his morning diaper change. Just be mindful when dressing so you don’t accidentally rub it off. Meanwhile, it’s confusing to know what’s best for his skin with the vast variety of products out there. First things first: The American Academy of Pediatrics emphasizes babies younger than 6 months should be kept out of direct sunlight, protected with shade, shielded with sun hats and protective clothing when they are out,

rather than relying on sunscreen. “Sunscreen products are not recommended prior to 6 months of age,” says Kate Seymour, M.D., Internal Medicine & Pediatrics of Saint Thomas Medical Group. “Adequate clothing and shade are preferred measures for protection from the sun, but if these are not available and sun exposure is planned, a small amount of sunscreen could be applied to small areas of exposed skin such as the hands and face. The risk of sun damage to vulnerable immature skin is greater than the risk of limited, conservative use of sunscreen when used infrequently,” she adds. NEVER SAY SUNBLOCK Sunblock or sunscreen? Is there

really any difference? Seymour says manufacturers of the products are no longer allowed to say sunblock and adds that chemical sunscreens are the more common product and they filter ultraviolet rays to protect the skin from UVA and UVB rays. AVOID INHALING SPRAYS “The concern with spray sunscreens are the aerosolized particles which may be inhaled. This is especially dangerous for children with asthma or other chronic lung disease,” says Seymour. She recommends spraying it onto your hands and then rubbing it onto your child’s skin. Be sure to apply it evenly, too, so you don’t have patchy application. If it’s windy outside, you may not even reach the

skin or you risk bystanders inhaling it. Seymour notes that all sunscreens contain various ingredients, any of which can cause an allergic type response, too. If you’re unsure if your child will have an allergic reaction, do a spot test on the wrist where you apply a small amount and wait a day or so to see if a reaction occurs. Remember, too, that sunscreens do expire. “There is concern that the protective molecular bonds in the cream can break down over time and become less effective,” says Seymour. “Follow expiration dates listed on products.” Following these guidelines will ensure that you both have a happy and safe time in the sun.

BREASTFEEDING SUCCESS FROM THE START By Kiera Ashford

ll the world falls away as you relax in your big, comfy chair snuggling with your newborn on your first day home from the hospital. Everything seems to be going OK ... until Baby starts to cry when you try to breastfeed him. One of the keys to breastfeeding success is advance education before Baby’s arrival. “I always encourage my patients and their support persons to educate themselves about breastfeeding,” says Beth Bleecker, M.D., OB/GYN at TriStar Centennial Women’s Hospital. She also suggests you read reliable resources, attend breastfeeding classes and familiarize yourself with the lactation support available to you once Baby arrives. “It’s important to realize that every mom and baby is unique, and a good base of information can help moms start with confidence,” adds Bleecker.

START EARLY Experts recommend breastfeeding soon after Baby’s arrival. “A new baby has inborn reflexes that help jump start feeding,” says Kate Cropp, APRN, IBCLC, nurse practitioner and IBCLC with Nashville Birth & Babies who also offers lactation services in collaboration with Baby+Co., a natural, luxurious birthing center located in Nashville. “During the first one to two hours after birth, a healthy baby is alert and ready to eat, and we shouldn’t miss this window of opportunity!” Cropp says to place Baby on your abdomen to get him ready for his first meal because most babies make their way to the breast to start feeding in less than an hour.

KNOW BABY’S HUNGER CUES

Crying isn’t the only sign that Baby’s hungry. In fact, Bleecker says, “crying is actually a very late hunger sign.” So, learn the early cues so you don’t get to this point.

“In the beginning, skin-to-skin time is the first step, and then the first feeding as the baby shows interest,” says Bleecker. “Many times, but not always, the newborn will show hunger cues with mouth movements, move toward the breast or begin suckling motions.” “You may notice your newborn is turning his head toward your body and opening his mouth,” adds Cropp. “He may also make ‘head bob’ motions or shake his head from side to side.” Those are just the first cues when Baby is beginning to feel the urge to feed. Watch for stretching, more physical stirring and hand-tomouth movements next, says Bleecker. “Those are the next signs, before moving onto late cues such as crying and agitation. It’s ideal to start a feeding at the early cues, when baby is still calm.” If you’re nursing during his later cues, you may feel frustration, and Baby may not latch properly. Just relax.

RELAX & DE-STRESS Relaxation plays a big role when it comes to breastfeeding, and not just during feedings. Bleecker says to eat well, rest, practice mindfulness techniques and even try postnatal yoga in between feedings. Getting out of the house and going for a walk or a trip to a nearby coffee shop or store is also a good start. “Doing something routine might be just the thing to help you relax,” says Bleecker. If you’re stressed, you’ll have a more difficult time responding to your baby’s feeding cues says Cropp. “With fewer feeding sessions, milk supply drops. If you receive appropriate help though, things will typically return to normal,” she adds. When you’re in a calm, positive environment, breastfeeding is more enjoyable and a bit easier. “Moms put a lot of pressure on themselves to be ‘perfect’ at breastfeeding,” says Bleecker. “This really can take away from the whole experience, and can in

fact decrease supply, making for more stress!”

GET INTO POSITION Getting Baby to latch just right can be the trickiest part of it all. But, that can all change with position. Try to cradle Baby, which may be your first instinct. If that doesn’t work, try other positions such as cross-cradle and the football hold — those three positions, Bleecker says, are the most traditional positions to start with. You can even alternate positions. “Good positioning keeps your baby’s body in close contact with yours, usually with his shoulders and hips aligned, and with his nose right at your nipple,” says Cropp. You’ll know when you’ve achieved a comfortable position when Baby latches correctly. Bleecker adds, “It’s very helpful to have a lactation consultant check the baby’s latch, and for you to know what latch is correct.”

SUPPLY & DEMAND Your body makes as much milk as it needs to fulfill Baby’s hunger. Bleecker says breastfeeding is a supply-and-demand process and encourages early skin-toskin and latching to get things going. Looking to boost your supply because you seem to be lacking? Don’t assume you really are lacking. Now’s the time to reach out to a certified lactation consultant who can help you get on the right path. “A low milk supply can be caused by a wide variety of things, and, while nutrient-rich and lactogenic foods and herbs work well in boosting your milk supply, they’ll only be a Band-Aid covering up the larger problem underneath that has caused the low milk supply in the first place,” says Alicia C. Simpson, MS, RD, IBCLC, LD, author of Boost Your Breast Milk: An

All-In-One Guide for Nursing Mothers to Build a Healthy Milk Supply (Workman; 2017). A quick call to a certified lactation consultant can help you get on the right path. And, then there’s pumping. Nowadays, a hospital grade pump is waiting for you in your room. However, Simpson cautions against early use of them. “A breast pump is not a necessity for all mothers, and using a breast pump too soon can do more harm than good,” says Simpson. But, if you’re unsure, always ask your doctor or lactation consultant.

Breastfeeding support comes in many forms when troubleshooting is necessary. “I cannot stress enough how helpful lactation consultants can be in those first few weeks of breastfeeding,” says Bleecker. “Even as a professional who takes care of breastfeeding moms daily, I saw a lactation consultant for each of my kids for different reasons. It can be hard to trust yourself and your body to be doing the right thing for your baby ... but most of the time your instincts are correct,” she adds. Maybe it’s a support group that you need. Sharing your experiences with other mothers and hearing theirs in return can be helpful. Whatever the case, know that you’re not alone. Breastfeeding your baby can be the most enjoyable experience you can imagine. Let it happen.
