Y

DAY BY DAY ‘ Love the Kids You’re With

T

he supreme happiness of life, the late, great writer Victor Hugo said, “is the conviction that we are loved.” And it was Sigmund Freud — the legendary father of psychoanalysis — who said, “Love is the first requirement for mental health.” Boom. With how much I love my children — with how much you love yours — it’s hard to imagine a child not knowing he is loved, but there it is. For various reasons there are many, many people who go through life not knowing if they are loved or not, and if love is the first requirement for mental health, we should all step back a moment because, terribly, our children are facing an anxiety epidemic (see page 32). Does your child know he’s loved? Do you tell him you love him? Do you know kids actually question if they are loved? If a child’s not sure where you stand — if he doubts that you love him — he can start to suffer deeply. It’s easy to show love to little babies; they are putty in your hands and you can snuggle’em up and they only coo back. It gets tricky showing love to growing kids — they make messes, get in trouble, don’t perform in the world the way we want them to. Careful here: A kid who’s not 100 percent convinced of his parent’s love can begin to develop anxiety. Or, a kid who has an anxious parent can actually learn to be that way, too. These things are not love: Giving your child an iPad for hours on end because he wants it. Putting down a boy because he failed to make a tackle on the football team. Being unaware of your girl’s friends or of what your boy’s learning in school. Or just being plain, old selfish. I mean, shoot, I see moms push little hands away so they can quickly get back to their phones. How’s that for a message of THIS and NOT YOU?! You’re right that parenting is exhausting, but you know what’s worse? A kid who doesn’t get the attention or care he needs and now you don’t know what on earth to do or how to get things right again. But you can make things right again. No, you do not have to be a doormat or a parent who lives only for your kid, either — that’s no good, you need a life! But the good, healthy balance kids need is in the real world with you. Get away from the constant drone of screens and start having fun out there in person, in life. It’s officially time to find a better way ... for the love of your kids. And because they love you.

FEEDBACK

Rise & Shine? We asked parents if they’d ever use a squirt gun to wake their kids up for school. Here’s what they said:

“I once was so tired of trying to wake up my son, he got a cup of ice water!” — Diana O’Hara Weber

YOUR POSTS: “No phone for the day if they miss the bus. If they are younger, bedtime moves up an hour until they get the message. If you spray them, spray their feet. Works every time.” Angelia Morris

PARENT POLL: RESPONSIBLE KIDS WE ASKED 432 PARENTS IF THEIR KIDS GET UP IN THE MORNING ON THEIR OWN. THE RESULTS:

“My mom couldn’t sing. She’d come in singing a song, and we learned to get up quick!” Holly Wright Stone “Children need to be held responsible for their behavior. They need to get themselves up on time, on their own. If they don’t, they get a consequence (one that they will care about it, which will look different for each child). If that still doesn’t motivate them and it leads to someone else having to get them up, then the consequence becomes even greater. If the consequence is greater than the reward (staying in bed, etc.), then they will make choices that will reduce the need for any consequences. Consistency is the most important factor in this!” Sharlene Fletcher “I used ice cubes a couple times. Then, all I had to do was say it and they’d get up. LOL.” Lori McIntosh

44% • YES! • NOPE, NEVER. 56%

“I went to catholic boarding school where the dorm moderators would rip the blankets off you, then flip the mattress if you didn’t get up. I’ve played ‘Reveille’ on high to get my daughter up.” Cat Spears “Stick to a regular bedtime for sure. My son uses two alarms, the alarm clock and one on his phone that has fun music as the alarm tone. The music seems to be the trick this year. So far, he’s happy and gets up all on his own.” Jenifer Lapinsky “Maybe a punishment after the first try — no tablets, cell phone, etc. Kids hate their electronics being taken away.” Kristin Grubb “Mine wouldn’t get up with me telling them to, so Dad uses his booming voice every morning. I have tried taking stuff away and I’ve tried the early bed time. This works for us! LOL.” Natalie Villa

nashvilleparent.com

“Do what you gotta do. I could put my kids (ages 11 and 6) to bed at 7 p.m. and they still wouldn’t get up for school on their own. I usually resort to ‘tickle bee’ if I’m in a good mood or ‘no screens if you’re not at the table in five minutes’ if I’m in a bad one.” Katie Long “I read someone put frozen marbles in the bed. They were so cold that any time you moved they moved with you. Water guns don’t sound harsh at all in comparison. With my daughter, I sing obnoxiously.” Callie Worstall “If it makes you uncomfortable using the squirt gun, you can always find some annoying song on YouTube and play it LOUDLY in his room.” Samantha Smith “Use an air horn!” Amanda Gonzalez

MIND &BODY

How to Handle Your Child’s

BY CHAD YOUNG

NOCTURNAL ENURESIS

N

Bedwetting is seen in nearly 15 percent of 6-year-olds, and it’s twice as common in boys than girls. The key factor in parenting through it is to not blame your child.

octurnal enuresis — bedwetting — is when children old enough to control their bladder wet the bed at night when they’re asleep. Bedwetting happens twice as much in boys as it does in girls, and it when it occurs, it’s usually seen in children between the ages of 3 - 6 years. The important thing to remember is, like other childhood stages, this too shall pass.

WHY BEDWETTING HAPPENS

“Bedwetting usually happens at some time in all normal young children,” says Evan Allie, M.D., pediatric emergency medicine physician at The Children’s Hospital at TriStar Centennial. “Persistent or new bedwetting can be a sign of other problems including a urinary tract infection (UTI), constipation, diabetes or emotional problems,” she adds. Several factors play a role in why a child wets the bed at night, including genetics. “If one parent was a bedwetter, there’s a 50 percent chance the child will be as well. If both parents wet the bed, there’s a 75 percent chance,” says Abby Taylor, M.D., assistant professor of pediatric urology at Vanderbilt University Medical Center. Other factors may be a child’s poor sleep habits, overactive bladder or small bladder capacity. Some heavy sleepers experience brain-to-bladder signals for relaxation that are not yet coordinated.

14 september 2019

IS THERE A POTTYTRAINING LINK?

• Take your child to the bathroom before bedtime.

Children who have negative pottytraining experiences or even a traumatic experience of any kind can develop a resistance to achieve nighttime dryness, says Allie, although it may or may not be intentional by the child based on his developmental age and situation. If a child experiences daytime issues like urinary urgency, urinary frequency, incontinence, painful urination, UTI or constipation, Taylor says bedwetting can result. “You must address and resolve the daytime issues prior to starting treatment of the nighttime symptoms,” says Taylor.

• Improve daytime urinary and bowel habits; void every two to three hours while awake.

MANAGING IT While you may deal with moments of frustration, blame and punishment are not the answers. Stay positive and keep it light with your child. Reassure him that it is common and he’ll grow out of it. Taylor says to use positive reinforcement on dry nights. Both Taylor and Allie offer these tips:

In addition, it’s a good idea to invest in a bedwetting alarm/vibration system. These devices have sensors that detect the first drop of urine, and they sound an alarm to wake the child so he can make his way to the bathroom. Also, pick up a waterproof mattress cover. “Bedwetting alarms are the most efficacious long-term management of bedwetting, with a 60 70 percent success rate when used correctly,” says Taylor. Should your child continue bedwetting past age 6, contact your pediatrician. Chad Young is managing editor for this publication.

• Make sure your child has regular bowel movements. Constipation can trigger bedwetting. • Restrict fluids at least two hours before bedtime. • Avoid drinks with sugar or caffeine. • Never shame, use Time Out or any other punishment to “teach” dryness.

EXPECTING

By Kiera Ashford

INFANT REFLEXES BABY’S BORN WITH

T

Learn all about Baby’s natural instincts so you know what to expect with all his quirky little moves.

here you sit with Baby fast asleep in your arms. All is calm until you put him in his bed. He throws his arms out and wakes up all bug-eyed with a look of surprise. What startled him? Worry not, newbie Mom.

26

Your little guy is experiencing the Moro reflex. This is only one of several newborn reflexes, or primitive reflexes, he’s born with. According to Jonathan Spanier, M.D., a pediatrician with Tennessee Pediatrics (which provides primary care for infants, children and teens in Hendersonville, Murfreesboro, Nashville and Thompson’s Station), one’s body performs a quick reflex in response to a stimulus. It’s

an involuntary reaction, too. While most involuntary reflexes develop as you get older, you are born with others. Spanier says these primitive reflexes help Baby survive and adapt to his new environment. They, too, are involuntary and are part of his immature nervous system. “As Baby matures and his brain develops, these reflexes disappear and voluntary movements

nashvilleparent.com

replace them,” adds Spanier. Your pediatrician may asses these normal newborn reflexes to ensure Baby’s nervous system is normal. An absent or asymmetrical reflex can indicate a problem with his muscles, brain or nerves. Get to know eight common newborn reflexes with detail into what they are and how long they last.

THE MORO REFLEX When Baby presents a reflex and he gives a startling look, it’s called the Moro reflex — sometimes known as the “startle response.” Loud noises such as a doorbell or a dog bark, an accidental bump of the crib, a bright light or even his own cries are just a few things that trigger this reflex. It may even wake him from his sleep. Spanier says the Moro reflex may have been developed as a baby’s “alarm system” to alert him to unpleasant stimuli. Appearance: Baby throws his head back, straightens his arms, opens his hands and then brings his arms back in (as if in an embrace). Cries may also follow. Pediatricians may test for the Moro reflex by supporting Baby’s head and letting it drop slightly. An asymmetrical Moro reflex may indicate a brachial plexus injury or clavicle fracture. “An exaggerated Moro reflex can be seen in babies who are undergoing withdrawal from drugs used during pregnancy, such as narcotics,” adds Spanier. Duration: Present at birth, this reflex lasts until Baby’s 4 to 6 months old. Swaddle him for his first few months of life to help prevent it. If he retains the Moro reflex, it can result in over-sensitivity to light, noises, textures and other stimuli. It can also lead to problems with impulse control, sensory processing disorders, motion sickness and anxiety. It can also cause symptoms that overlap with typical ADHD signs, such as fidgeting, inability to sit still, poor focus and impaired impulse control.

THE ROOTING REFLEX You may notice that when you hold Baby, he seems to rub his face into your arm or chest. This is called the rooting reflex and

it’s a meant to help him find the breast or bottle for feeding. Appearance: Stroke the corner of Baby’s mouth, he then turns his head toward the side you touch and open his mouth. Duration: This reflex actually develops while Baby is in the womb — at around the 28th week of pregnancy. It is also present at birth and lasts until around 4 months of age.

Baby’s palm and he closes his fingers in a grasp that can be quite strong. Although this is an involuntary reflex, hold his hands to help build your bond with him. Duration: This reflex is present at birth and lasts until around 6 months of age. If Baby retains the reflex, it can result in problems with his development of fine motor skills, handwriting and self-feeding.

THE SUCK REFLEX

THE BABINSKI REFLEX

Another reflex that helps Baby with eating is the suck reflex. Appearance: Touch the roof of Baby’s mouth and see how he starts to suck your finger. Duration: It, too, develops while Baby is in the womb — at around the 32 - 36 weeks of pregnancy. Because of this, babies who are born prematurely may have difficulty sucking at first. This reflex lasts until 2 - 4 months of age, after which Baby should suck by choice and may start to refuse the bottle or breast more.

Try to tickle Baby’s foot. Notice how he has an unusual reflex to your touch on the bottom of his foot? The Babinski reflex is another normal reflex for Baby. Appearance: Stroke the sole of Baby’s foot and watch his big toe bend up toward the top of the foot as the other toes fan out. It’s actually an opposite reaction of the normal adult response in which the big toe flexes down. Duration: The infant Babinski reflex can last till about 1 year of age. It can appear again in adults who suffer from brain damage or a stroke.

THE ASYMMETRICAL TONIC NECK REFLEX

THE STEP REFLEX

This reflex is a way to prepare Baby’s body for hand-eye coordination. Appearance: Baby’s head is turned to one side with the arm on that same side straight. The opposite arm flexes, and he assumes what looks like a fencer’s position. Duration: It’s usually present at birth and resolves by 4 - 6 months of age, when he’s ready to start rolling over. If he retains this reflex, it can cause problems with the development of normal hand-eye coordination.

Sometimes referred to as the walking or dancing reflex, this one happens while you hold Baby. Appearance: Hold Baby upright and allow his feet to touch a solid surface. Watch as he takes alternating steps as if walking. An asymmetrical step reflex may indicate a muscle weakness or nerve injury. Duration: This reflex lasts till about 2 - 3 months of age. But don’t feel bad when it goes away — he will be walking again before you know it.

THE PALMAR REFLEX

THE GALANT REFLEX

You know how ticklish hands can be, so test this one out on your newborn. Appearance: The Palmar reflex occurs when you stroke

This reflex prepares Baby for crawling and walking as it helps develop a range of motion in his hips.

nashvilleparent.com

Appearance: Hold Baby face down and run your finger down one side of his back, parallel to the spine. Watch as he swings his hips toward the direction of the touch and curves his body in that direction. Duration: This reflex is present at birth and lasts until 3 - 6 months of age. If he retains the Galant reflex, it can result in fidgeting, an inability to sit still and has even been linked to bed wetting. It can also cause symptoms that overlap with typical ADHD symptoms as mentioned in the Moro reflex.

WHEN TO CONSULT YOUR PEDIATRICIAN If you notice an absence of one of these reflexes, bring it up with your pediatrician. According to Spanier, the absent reflexes can indicate a nervous system problem such as a brain injury or cerebral palsy. Also, sometimes Baby’s developing nervous system does not suppress these primitive reflexes and he may retain them. This may indicate nervous system problems, such as cerebral palsy. They can also lead to later problems with learning and development. Spanier adds that reasons Baby may retain some reflexes include a traumatic birth, head trauma, lack of “tummy time” and excessive time spent in a stroller, car seat or bouncer that restrict his movements. “It’s important that babies be given lots of opportunities to move and explore their environments,” says Spanier. If you suspect problems that relate to primitive reflexes your baby retains, an evaluation by an occupational therapist may be beneficial. SUMNER REGIONAL TEACHES “THE ART OF BREASTFEEDING”

O

ver and over again women lament the difficulties of getting started with breastfeeding. But did

you know that if you stick with it (as with most things in life) it can actually become an art form? Sumner Regional Medical Center (555 Hartsville Pike, Gallatin) offers numerous classes for expectant parents, taught by experts including “The Art of Breastfeeding,” which aims to help you achieve a great nursing experience from the start, weaving in techniques and tips. The free class is offered the fourth Tuesday of every month from 6:30 - 9 p.m. It’s recommended for expecting moms during the sixth to eighth month of pregnancy. Your significant other is welcome to attend, too. To register and to learn about Sumner’s “Path to Parenthood” (preparing for pregnancy) class or the “Labor of Love” (childbirth prep) class, call 615-3284123 or take care of it online at mysumnermedical.com.

Current research out of Vanderbilt University Medical Center (1211 Medical Center Drive, Nashville) reveals that consuming even small amounts of alcohol in pregnancy increases the risk of miscarriage by 6 percent. Lead investigator Alex Sundermann, M.D., Ph.D., at Vanderbilt’s School of Medicine, says while fetal alcohol syndrome is often associated with heavy alcohol consumption in pregancy, meta-analysis indicates modest use of alcohol has an impact, too — steer clear!

3 Things to Know About the Hospital Going In ...

A

s it gets closer to your delivery date you may wonder: What if everything doesn’t “go” the way you hope it will? Here are some tips to know before you go to the hospital:

1) It’s Wonderful to Love Your Nurse Although you’re in a vulnerable state, this is an important moment for you, so it’s important you jive with your team. If you don’t like your nurse, ask to speak to the nurse in charge and try to make a change. 2) You Won’t Be Allowed to Eat Ice chips and clear liquids only, especially if you’re having an epidural.

Having any type of anesthesia brings a risk of nausea. But when you’re in labor, are you really going to be worrying about food? We think not! 3) Yes, You Can Say “No” Especially if you’re having your first baby, you may not be aware that you can say “no” to procedures since care requires informed consent either from you or your partner. Do be aware that skilled nurses and doctors want the best for you, so before going to the hospital, make sure you make your birth wishes known.

PLAY THE LOTTERY! (for Dear Evan Hansen, that is)

T

PAC’s 2019 - 2020 Broadway Series kicks off this month with one of Broadway’s hottest shows in recent years, Dear Evan Hansen. It’s running Sept. 10 - 15, and it sold out well before this month’s flip of the calendar. If you didn’t score tickets, there’s still hope. You have a shot to get seats for $25 through a digital lottery.

HOW IT WORKS

Lucky Seat is running the lottery at luckyseat.com/dearevanhansen. The lottery will accept entries 48 hours prior to the first performance (that means you can enter starting Sunday, Sept. 8 at 7:30 p.m.). You can enter until 9 a.m. the day before each performance. Lottery seats are up for grabs for every Dear Evan Hansen show at TPAC. You must be 18 years or older with a current photo ID matching the name you use to enter. You’ll be notified via e-mail if you win and can then purchase up to two tickets at $25 each. Pick up your tickets at the TPAC box office before the performance and enjoy the show! To keep up to speed on all-things Dear Evan Hansen — including the lottery — follow the show on Facebook, Twitter and Instagram: @DearEvanHansen on all three platforms.

LIKE THE LOTTERY? THANK RENT!

Broadway’s 1996 smash musical Rent saw the ushering in of the show’s producers offering “rush” tickets. In an effort to make affordable ($20) seats available in the first two rows, the box office opened two hours prior to every performance to sell seats on a first-come, first-served basis. The rush tickets took off like crazy. Suddenly, people were camping out in front of the theater and around Times Square for a chance to get the seats. When public safety became a concern, Rent’s producers switched to a lottery system. Back then, theater-goers arrived two hours before showtime to put their names in the hat for a random drawing for those same $20 orchestra-level seats. The Rent lotteries became so popular other shows in New York started doing them, too, and they still do to this day. Hamilton lottery crowds became legendary — by the way, there’ll be a Hamilton lottery when the tour comes to TPAC (Dec. 31 - Jan. 19). Blockbuster shows out on national tours (like Hamilton, Dear Evan Hansen, Wicked, Jersey Boys, etc.) usually have a lottery in place in each city.

DIGITAL LOTTERIES EVEN THE PLAYING FIELD

If you participated in a TPAC lottery before, it was the “old-school” name-in-the-hat version (this is the first digital lottery for a show at TPAC). The digital format is easier, especially when it comes to saving time. It also makes things more fair because there are always people out there (like me!) who figure out how to “rig” the name-in-the-hat scenarios with savvy origami feats — that talent got me Wicked tickets three times when the tour was here for the first time several years ago. While I reminisce the glory days of my in-person lottery victories, I wish you the best of luck in scoring seats to Dear Evan Hansen in the digital age! — chad young
