Dr. Snodgrass, what is the main cause of cavities? There is no main reason for cavities. Cavities are caused by multi-factoral reasons. The most common cause in infancy is prolonged bottle or breastfeeding, especially at night. For children past the toddling stage, it’s usually a combination of frequency, duration, and concentration of sugar in addition to poor oral hygiene. For teenagers, it’s too many sweets and not enough attention to oral hygiene.

My child is 10 years old and still has four baby teeth left. Is it too early for braces and will she need them again in the future if we start orthodontic treatment now? Every child and orthodontic case is different. However, having four baby teeth left at 10 years of age is not abnormal. It is also not unusual to start orthodontic treatment with baby teeth still present. Under normal and ideal circumstances, I like to start female patients around 10 or 11 years of age and boys at 11 to 12 years. When orthodontic treatment is started it typically lasts for two years. If children wear their retainers as directed, no further orthodontic treatment will be needed. As I always say, it is never too late to have braces and improve your smile. Simply stated, it’s easier to correct orthodontic problems when a patient is growing. During growth spurts, teeth move more efficiently. There also seems to be the added bonus of less discomfort while in treatment.

was barely Thanksgiving when my 15-year-old boy asked, “Mom, can I bring up the Christmas boxes?” So excited, he was practically bursting. “Oh, man, it’s not even Thanksgiving, honey!” “So what?!” he exclaimed, jiggling his knees, already heading for the basement stairs. And up they came, one by one. I barely had time to throw a protective sheet down on the dining room table before the first box landed with a thud. “Careful! You don’t know what’s inside that box, something can break!” “It’s marked ‘Byers’ Carolers,’ Mom, it’s fine!” he said, whisking around and running back for more. I love telling you this because it shows that my teenager is still a kid at heart — he’d hate you to know he likes to decorate with me. Teenagers work so hard to be cool, uninterested ... and definitely not into Christmas decorating with their mom. But he loves decorating, and we’ve been doing it together since he could toddle. First, he did unbreakables only, then gradually he matured enough to handle my collection of snow globes. And here we go. Sigh. Why must we drag trees into our homes and string them with lights and all manner of dangly things from tiny hooks? Why must we bake cookies, make candy, play holiday music, knock ourselves out with shopping, wrapping, light displays, get togethers? Because we are a people of tradition. Of giving and receiving, of desiring things we cherish and trust. Every year, we each go after some idea we formed as a kid ... an idea for the best Christmas ever ... and sometimes it comes together, sometimes it doesn’t. On the inside cover of an old cookbook I wrote a paragraph about a past failed Christmas. I burnt the cookies. The tree actually fell. Things just did not come together. I wrote to remind myself of what I DON’T want to happen again. But Christmas can’t actually fail, only people can! Here’s my boy running all over the house placing treasured decorations here and there. He’s INTO IT. He’s celebrating, giving, sharing. He’s getting a jump on all of those good things and wanting me to join in, too. I can’t waste this moment, it’s too good. No more writing hapless paragraphs about what went wrong. Heart adjusted, I’m following my 15-year-old to success: Let Christmas begin!

Precious Gift F or years, one of the first things to happen right after an infant’s birth was the clamping and cutting of the umbilical cord. But at the beginning of 2017, the American College of Obstetricians and Gynecologists (ACOG) became the first medical organization to recommend delaying the precious act. Why? Because an additional 30 - 60 seconds longer can allow more blood (iron) to get to the newborn, critical for brain development. It’s an excellent thing.
Benefits of Delayed Cord Clamping “Both the American Academy of Pediatrics (AAP) and the ACOG agree that delayed umbilical cord clamping in babies is beneficial,” says Dontal Johnson, M.D., C.P.T., assistant professor of pediatrics at Meharry Medical College. Johnson says the benefits include increasing the hemoglobin levels at birth (the molecule that helps babies transport oxygen around their body) and improvement of iron stores during the first several months of life, which is important to hemoglobin. Johnson says if an infant doesn’t have enough hemoglobin, his red blood cells can shrink and lead to anemia.

Delayed cord clamping is beneficial for premature infants, too. “In preterm babies, delayed umbilical cord clamping is associated with improved transitional circulation, better establishment of red blood cell volume, decreased need for blood transfusions, decreased incidence of necrotizing enterocolitis (NEC) and decreased incidence of intraventricular hemorrhage (IVH),” says Johnson. “Preterm babies are at a higher risk of both NEC and IVH. As a pediatrician, both of these conditions cause your ‘Spidey senses’ to go a little wild, so procedures that decrease them from happening are very much welcome.”
How Long Can You Delay It? Baby’s here, and your doctor’s going to delay clamping the cord. But for how long? “Medical governing bodies differ on the specific length of time before clamping the umbilical cord,” says Johnson, “but waiting 30 - 60 seconds is generally recommended by the World Health organization, Neonatal Resuscitation Program, AAP and ACOG.” Don’t be alarmed if the delay is only a few seconds, because every second counts.
When You Can’t Delay Your pregnancy went smoothly, but anything can change during delivery. When a baby is born, the doctor immediately assesses him. If you’ve decided to delay cord clamping, he will take that into consideration as he checks Baby. “Many factors go into assessing a newly delivered baby,” says Johnson, “but two things stand out that may make your doctor think twice about performing delayed cord clamping: a baby who has decreased tone (aka limp baby) or a baby who is in respiratory distress (not crying, not showing signs of breathing or blue baby). These conditions usually prompt immediate neonatal resuscitation evaluation.” That’s when the risks outweigh delayed cord clamping, and the doctor will overule your decision for Baby’s best outcome. If you’re expecting a baby and interested in delayed cord clamping, be sure to discuss it with your doctor. New Book: Jimmy Wayne Brings Awareness to the Foster Child’s — and Dog’s — Plight
I
t’s easy turning 18 for most kids. They graduate high school, move on to college (thanks, Mom and Dad) and launch into the real world on their own (with comfortable stops at home for Thanksgiving and Christmas and plenty of communication and support throughout the year). But what if you’re a foster kid? It’s, uh, not quite the same. In fact, it’s downright hard. Embarrassing. People have preconceived ideas about you. That you’re too rough. That you’ve got “issues.” That you’re a “problem.” And where do you go from there? Enter Jimmy Wayne. Today a successful country artist, he was first a foster kid who was abandoned by his mother at a bus station at age 13. In his life, he endured numerous homes, homelessness and witnessed a violent murder. Wayne was 16 when he was taken in by Bea and Russell Costner who paved the way to college for him. But Wayne knew other kids weren’t so lucky. If they weren’t supported by loving foster parents like he finally was, they would likely join the 30,000 other foster kids who aged out of state care only to enter homelessness at age 18. He knew he had to do something. In 2010, Wayne walked from Nashville to Phoenix to help raise awareness about aging out of foster care too young. His “Project Meet Me Halfway” campaign resulted in his 2012 participation to change Tennessee legislation through the Transitioning Youth Act which raised the “age out” from 18 to 21 in Tennessee and California. Next came Wayne’s 2015 New York Times’ best-seller, Walk to Beautiful: The Power of Love and a Homeless Kid Who Found His Way (Thomas Nelson; $16.99). In the book, Wayne passionately covers the rocky road that lead to adulthood,writing, singing and speaking about his troubled growing-up years. He remains dedicated to wanting to change outcomes for foster kids everywhere.

ing and aware of a foster child’s — or dog’s — plight. “A lot of kids are in a bad situation, just like ‘foster dogs’ so to speak, which are in shelters even though they haven’t committed a crime,” Wayne told People magazine. “Foster-care children and the dogs actually have the same story: They’re in a home, or a shelter, and they need help. They need love, to be adopted and to be given a forever home,” he adds. In Ruby, Wayne (Mr. James in the book) teaches Ruby about integrity, honesty, loyalty, faith, forgiveness and trust. But Ruby mostly learns that love can rescue someone and forever change a life. Across the country today, more than 100,000 children are waiting to be adopted by a “forever family.” Wayne’s ultimate goal is to build transitional homes for teens who age out of foster care without a place to live. • Use a Natural Cake Mix
One of the ways I make things fun and keep it simple is to jump-start recipes using a natural cake mix. Those sugar cookies can be made from scratch or from my yellow mix. We can also make red velvet cupcakes from that same mix, and we add a simple homemade peppermint buttercream frosting to swirl on top. I developed my own line of natural cake mixes years ago because I wasn’t crazy about cake mixes that contained artificial colors, flavors and preservatives. It’s homemade, but it’s easy, too. Once those cookies are decorated, you can box and gift them for all ages. I have found that everyone loves decorated sugar cookies. They are personal, heartfelt and one of a kind.

hen my children were young, they loved to decorate holiday cookies. Let me put that another way — they loved to decorate the kitchen with sprinkles and sugars and batter and everything colorful and festive. So I quickly learned to drape the kitchen floor with old sheets, nest the jars of the sugar sprinkles in cake pans on the counter, put aprons on everyone and then let the decorating begin! It was sheer chaos, but also sheer joy. To this day, decorating sugar cookies is one of my girls’ favorite childhood memories. They are now 24 and 28, and last year while home for Christmas, they sat at our kitchen table — with aprons on — and the years seemed to roll back. I made the dough and cut it into shapes like I always did, and all they had to do was add their artistic touches. And eat a few baked cookies, too! The trick to making holiday baking fun is to think ahead so it appears like seamless fun. With a little forethought, planning recipes, shopping ahead and blocking the calendar for no other distractions, you can relax and let the magic happen, at any age.

People like to receive baked goods like bars and brownies that they can either eat or pop in the freezer for surprise guests in the weeks to come. One of my favorite freezable bars is a Marbled Cream Cheese Brownie (page 28) that begins with a good brownie mix. Double-batch the recipe in a 13by 9-inch pan, and you have another holiday baking time-saving strategy: Bake twice as much of a recipe so you only have one clean-up. And you can always bake ahead and freeze the goodies ’til you need them.
• Get Fancy With It Should your holidays call for a fabulous dessert, a cake worthy of the occasion, then you should take a look at my Chocolate Praline Cake recipe. Essentially it’s just a chocolate cake batter (made from a good mix) that you pour on top of an easy caramel-nut mixture, bake and unmold. Those layers are stacked right on top of each other, praline-side up, and then you pile freshly whipped cream on top. It is scrumptious! And for all of you who suffer from fear of frosting, the idea of not frosting is a real relief. Buy a pint of heavy cream and place it in your electric mixer bowl and whip on high speed, adding a tablespoon of powdered sugar, until you have stiff peaks. Chill the cream until needed and pile on top of a cake in lieu of frosting. It is very retro, minimal and yet oh-so-satisfying. And I have never met anyone who didn’t love to fork into chocolate cake and whipped cream. Holiday baking can be fun. Find your strategies and shortcuts, and to keep your sanity through the whole wonderful process, enjoy! 1 package (18.25 ounces) Cake Mix Doctor Simply Yellow mix 2 tablespoons unsweetened cocoa 1 cup whole milk 1/2 cup vegetable oil 2 large eggs 1 tablespoon liquid red food coloring, if desired
PEPPERMINT FROSTING
Easy Sugar Cookies
8 tablespoons (1 stick) butter, at room temperature 3 cups powdered sugar, sifted 2 to 3 tablespoons milk 1 teaspoon vanilla extract ¼ teaspoon peppermint extract Crushed peppermint candies, if desired, for topping cupcakes
1 package Cake Mix Doctor Simply Yellow mix 12 tablespoons butter, at room temperature 1 large egg 1 teaspoon vanilla extract
1. Place a rack in the center of the oven; preheat oven to 350 degrees. Place paper liners in two cupcake pans. Set aside. 2. For the cupcakes, place the cake mix, cocoa, milk, oil, eggs and red food coloring in a large mixing bowl and blend on low speed with an electric mixer until just combined. Increase the mixer speed to medium and blend until smooth, one minute. Scoop the batter into the liners, filling them threequarters of the way full. Place the pans in the oven.
FOR DECORATING: 1 large egg white Granulated sugar or colored sugars for decorating 1. Place the mix, butter, egg and vanilla in a large bowl and beat with an electric mixer on low speed for 30 seconds. Increase to medium and beat until the batter is smooth and stiff, about a minute. Place the bowl in the fridge to chill. 2. Place a rack in the center of the oven and preheat to 375 degrees. Spoon one-inch rounds of dough onto ungreased baking sheets. Dip the bottom of a small flat glass into flour and press the glass firmly onto the rounds of dough until they are about 1/3-inch thick. Or, roll the dough out on a lightly floured surface and cut into desired shapes. 3. Brush the dough with a little egg white, and sprinkle on granulated or color sugars. Place the pans in the oven. Bake until the cookies are golden brown, 10 - 12 minutes. Remove the pans from the oven, cool cookies on a wire rack. Store in tin. Makes about 4 dozen cookies Bake: 10 to 12 minutes
3. Bake the cupcakes 16 - 20 minutes. Remove to a rack to cool. 4. For the frosting, place the butter in a large mixing bowl and blend with an electric mixer on low until creamy. Add 2 cups of the powdered sugar, the milk and extracts, and blend until smooth. Add the remaining 1 cup sugar and blend on mediumhigh until the frosting is creamy and fluffy.
Got Kids? • Keep sprinkles, etc. in jars and in a caddy to cut down on little-finger messes! • Drape the kitchen floor with old sheets. • Put aprons on everyone. • Play Christmas carols.
5. When the cupcakes are cool, spread frosting over each one. Garnish with crushed peppermint.

Holiday Baking! Chocolate Praline Cake

PRALINE LAYER:
Parchment paper Vegetable oil spray 1 stick (8 tablespoons) butter ¼ cup heavy (whipping) cream 1 cup light brown sugar ¾ cup chopped pecans
CAKE:
1 package (18.25 ounces) Cake Mix Doctor Classic Chocolate mix 2/3 cup sour cream 1/3 cup vegetable oil 1/3 cup water 2 large eggs
TOPPING:
1 cup heavy (whipping) cream 1 tablespoon powdered sugar 1 tablespoon grated chocolate or miniature chocolate chips, if desired 1. Place a rack in the center of the oven and preheat the oven to 350 degrees. Cut two rounds of parchment paper to fit in the bottom of two 8-inch round layer pans. Spray the bottom of the pans with vegetable oil, place the parchment rounds in the pans, then spray the parchment with oil. 2. For the praline, place the butter, cream and brown sugar in a small saucepan. Cook over low heat, stirring, until the butter has melted, three minutes. Divide the mixture between the two pans. Sprinkle the pecans over the top, dividing them between the two pans.
Marbled Cream Cheese Brownie BROWNIE LAYER:
2. Stir together brownie mix, melted butter and eggs in a large bowl until well combined, about 50 strokes with a wooden spoon. Pour the batter into the pan and smooth the top. Set aside.
CREAM CHEESE LAYER:
4 ounces cream cheese, at room temperature 2 tablespoons butter, at room temperature 1/4 cup sugar 1 tablespoon all-purpose flour 1 large egg 1 teaspoon vanilla
3. Place the cream cheese, butter, sugar, flour, egg and vanilla in a medium bowl, and blend with an electric mixer on low until smooth, one minute. Dollop this cream cheese batter by the spoonful over the brownie batter, and swirl with a fork to combine the two batters in a marbled pattern. Place the pan in the oven.
1. Place a rack in the center of the oven, and preheat the oven to 350 degrees. Lightly grease and flour an 8-inch square baking pan.
4. Bake the brownies until they are just firm to the touch about 38 - 42 minutes. Let cool 30 minutes before slicing.

Place the pans in the fridge while you make the cake batter. 3. For the cake, place the cake mix, sour cream, oil, water and eggs in a large mixing bowl. Blend with an electric mixer on low speed for 30 seconds, then increase the mixer speed to medium and blend for 1½ minutes, or until the batter is smooth. Divide the batter between the two pans, pouring it over the pecans. Place the pans in the oven side by side. 4. Bake the cakes until they spring back when lightly pressed with your finger, 35 40 minutes. Run a dinner knife around the edge of each layer and invert onto a rack to cool praline-side up. Remove the parchment rounds. 5. Meanwhile, prepare the whipped cream. Place the cream in a chilled large mixing bowl and beat on high speed until cream has thickened, one minute. Stop the machine and add the sugar. Beat the cream and sugar on high speed until stiff peaks form, one to two minutes more. 6. To assemble the cake, place one cooled layer on a cake stand, praline-side up. Spread with half of the whipped cream. Place the second layer on top of the first and dollop the remaining whipped cream in the center of the cake. Garnish with grated chocolate or mini chips. Drape with plastic wrap and chill until serving time. Makes 12 to 16 servings Bake: 35 to 40 minutes
