ith the change of seasons, we also had some big changes at Spa Inc. I’m excited to join the magazine, and I look forward to spending time with the industry. Ironically, my spa adventures began in my mid-teens, when my parents introduced me to Ayurvedic treatments in the hopes that they could detox my rebellion away. (I’m predominantly Pitta, which makes me a bit “high energy” sometimes!) Through that experience, including a few lucky meetings with Deepak Chopra, I learned about maintaining balance through diet and being careful about what I put on my skin. For many years I was vigilant about self-care, including daily oil massages – even just a quick one in the shower. Then I had a baby (admittedly, more than a decade ago), and “self care” became a distant memory; it takes discipline to reincorporate “me time” when you’re a parent. Self-care isn’t just a catch phrase; it’s something we all need to focus on, especially in these busy times – and, even if you’re in the business of caring for others. It may be fate that my first issue of Spa Inc. focuses on medi-spa anti-aging treatments. As I approach one of those “milestone” birthdays, that’s been on my mind, but I can still keep people guessing about how old I really am. The last person who looked at my driver’s license literally dropped his jaw and said, “No way!” – and then wouldn’t stop staring. At my age, a reaction like that is definitely a compliment. So, those many years of watching what I eat and what I put on my skin seem to be paying off, but there’s no fooling Mother Nature. Looking and feeling good isn’t just about what others think of us; it’s about longevity and quality of life. When we go to the spa, it might lift our spirits for an afternoon, but the long-term goal is much more than that. Of course, when we invest in anti-aging treatments, we’re trying to minimize the impact of years passing, but we’re also committing to maintaining our “better self” – at least, as the mirror sees it. I hope everyone remembers, just as a magazine cover may be Photoshopped, the world of social media holds us to unreasonable standards. Be the best YOU, not somebody else. That’s where I draw the line. I used to want to change my body type, my hair colour and so on, and one day (after years of reflection), I realized that I can make the best of what I have, without rejecting who I am. How we do that is a fine balance between time, capabilities and resources. We don’t need to invest a million dollars to feel Popi Bowman like a million. Sometimes all it takes is a chemical peel or an MANAGING EDITOR expert massage.

Maple leaves may be the key to youthful skin Newly licensed by Verdure Sciences, Maplifa – an extract from red maple leaf – might help slow down the skin’s aging process, according to researchers at The University of Rhode Island. Navindra Seeram, the project's principal investigator studying the chemical composition of sugar and red maple trees for their therapeutic value, believes certain compounds called glucitol-core-containing gallotannins (GCGs) may also act as protective agents against inflammation, age spots and freckles, and could even lighten unwanted dark spots.

Scholarship Program Enables the Next Generation of Estheticians

S

hafiqa Niaz is one of four Canadian women who received scholarships last year from Epiderma Esthetics, the largest network of medical esthetics clinics in Quebec and Ontario. Niaz was born in Pakistan, a society where women are marginalized. Promised into an arranged marriage and then rejected by her husband when she became pregnant with a girl, Niaz sought emotional refuge by working in a local beauty salon. After arriving in Canada, she continued to heal with the support of other women she met while training in cosmetics care. Founded in 2000 in Quebec City, Epiderma Esthetics has grown to 24 clinics, employing 140 people and serving more than 225,000 clients. Eleven years ago, Epiderma launched its scholarship program to help exceptional students who are interested in the esthetics industry. The organization chose to invest in the scholarship because “we wanted esthetics students to know our company, our vision, our products and services, and primarily to facilitate our recruitment,” says Valérie Dutil, Epiderma’s director of human resources. Since launching, 40 scholarships have been awarded. “We hired lots of them,” says Dutil. The scholarship is valued at $3,275, including a cash prize, a one-day workshop, products and services, and the opportunity to work at Epiderma upon completing their studies. That’s exactly what happened to scholarship winner Caroline Gallant, who was

hired at the Epiderma clinic in Longueuil last July after completing her training. Technicians complete a mandatory, specialized training program that includes several weeks of paid practical and theoretical training, with intensive supervised in-clinic work and continuing education sessions that each network technician is required to take once a year. The qualifying exam was exclusively designed for Epiderma. The clinics deliver anti-wrinkle injection treatments, skin peels and microdermabrasion services, along with varicose vein, rosacea, skin tag and cellulite treatments, body sculpting, laser tattoo removal, hand rejuvenation and laser hair removal, in addition to an exclusive range of specialized cosmetics. At the root of its esthetics services is a medical team comprised of 14 physicians, ranging from physician injectors to phlebologists. The medical team trains and supervises the organization’s technicians. For scholarship winner Kristina Heebner, the company’s penchant for quality ingredients was likely a big draw. A budding chemist, she wanted to learn more about the properties and functions of the many active ingredients used in the production of esthetics products. Over 80 specially curated products are sold online by Epiderma clinics. These skincare lines are developed by international experts and are rich in active ingredients. To qualify for the Epiderma scholarship, students must be registered full-time in a vocational esthetics or electrolysis program delivered at a secondary school accredited by the Quebec or Ontario departments of education. The current call for the next round of Epiderma scholarships has an application deadline of March 15. Recipients are chosen by a selection committee composed of medical esthetic technicians, clinic managers and Epiderma’s Human Resources Director. Personalized attention to her clients makes all the difference for scholarship winner Alexandra Pruneau. It’s also a philosophy that comes alive at Epiderma, where expert advice and quality care have been the motivating factors for each of the 2.2 million treatments performed there since it opened.

Medical Science and Esthetics Converge

L

ike yin and yang, Ildi and John Arlette, MD – founders and co-owners of the Total Skincare Centre, a successful medi-spa in Calgary – complement each other perfectly. She commands the business of esthetics, he leads in the science of it. A “master injector” with a 30-year career as one of Canada’s leading dermatologists and skin cancer surgeons, Dr. Arlette is part of an elite group selected to educate the industry in the use of MD Codes, a pioneering technique first introduced by Brazilian plastic surgeon Mauricio de Maio in 2010. The highly acclaimed method is a way of looking at the skin and facial structures to understand the placement of filler products. Arlette spent time personally with Dr. de Maio in full-day sessions so he could demonstrate the method to other physicians and, as he explains, “teach people this very realistic, stepwise approach to the skin in terms of its anatomy, its structures and the changes that happen with time.” In 2006, Dr. Arlette collaborated with Dr. Martin Trotter for a groundbreaking study, published in Dermatologic Surgery, to demonstrate location of fillers within the skin. They looked at skin tissue to which hyaluronic acid filler had been administered before surgery, just before it was removed and immediately after its removal. The tissue samples were examined under a microscope to determine where the filler appeared within the skin’s layers. What the researchers found was that the deeper fillers were placed, the greater the lift. The study occurred at a time when people were becoming more aware of what could be achieved with

fillers, and it helped change industry understanding of the technique’s potential, an evolution that continues today. “Now we can much more predictably look at where we can place the filler within the skin and the underlying tissue to make a physical change that – Dr. Arlette, co-founder/owner, enhances appearance,” Dr. Total Skincare Centre Arlette explains. Both Ildi Arlette and her husband are seeing more sciencebased cosmeceuticals, in particular products that include growthfactor technology, such as stem cell formulations. Dr. Arlette says these products make a tremendous difference in terms of facial appearance and are far beyond simple antioxidants like Vitamin C. “There will be new products which have active agents we have not seen in the past,” he adds. “They are for people looking at how to blend science into the management of their skin.” One of the biggest changes in the last five years is a move away from needles toward cannulas, which deliver product deeper into the skin tissue. A cannula is a long, thin tube with a tiny, smoothtipped opening that is not sharp like a needle; it slides through tissue very smoothly but doesn’t damage blood vessels, so there is less bruising. Dr. Arlette says, “If I were treating a cheek with needles, I would be poking maybe eight times. Now I can make that entry point only once, for what would have been eight to 10 injection sites.” They admit, the landscape for esthetic clinics, medi-spas and spas has changed. “It’s getting more difficult to differentiate between a spa and a medical clinic,” says Dr. Arlette’s wife. “Spas now have lasers, injectables, silk peels – the nurse injector market has changed everything. Between 2005 and 2009, you couldn’t find a nurse injector to hire – and now the market has proliferated.” She estimates there has been about a 300 percent growth across the country in medi-spas that are opened by nurse injectors; as registered nurses, they are regulated by provincial nursing associations. Some of these spa facilities have medical directors on paper, but not necessarily on site, which Arlette says makes some facilities “less regulated” than a medi-spa owned and operated by a physician. “On the other hand,” she notes, “there are some nurse injectors running clinics that are just amazing.” She admits those smaller clinics “are giving some of these established clinics a real run for their money. “All injectors and treatment providers are not created equally,” Arlette cautions. “The industry right now is rolling the dice. It is literally not monitored, and it’s been allowed to proliferate a little bit far – until something happens.” There’s a greater risk if clinics haven’t educated their patients, something which Total Skincare considers essential. “Technically, we have less variety of services than some of our peers,” Arlette admits, “but interestingly, we continue to be one of the top 15 clinics in Canada because we are so focused on patient education and enabling people to understand the choices they have for solutions.” www. s p a inc .c a

WHAT’S UP WITH

Medi-Spas? Medical spas are changing with the times – restructuring the customer journey and bringing in new treatments to attract the younger crowd. BY VIVIENNE O’KEEFFE

W

hat’s trending and new today in the medi-spa business? As a professional spa consultant, I get asked this question a lot. The answer is: lots! U.S. spa revenue hit a record US$17.5 billion in 2017, up 4.3 percent according to International Spa Association (ISPA). The medi-spa industry by itself generates US$4 billion annually in revenue and, according to AmSpa’s 2017 State of the Medical Spa Industry Report, will continue to grow at an eight percent average annual pace from 2017 to 2022. Interestingly, according to the 2017 American Society of Plastic Surgeons (ASPS), these numbers include a one percent drop in surgical procedures, especially among the 20- to 29-yearolds of the millennial cohort. My suggestion is, why not ignite innovation in the millennials’ engagement, customer journey and spa experience? After all, they’ve surpassed baby boomers in numbers, and have enormous potential for making the medical spa future bright as they enter prime income-earning years. They tend to be curious about clinical treatments targeting older customers and cautious about ordering them, and they appreciate the nonclinical environment. Feeling social pressure, fear of aging and a desire for prevention, they currently make up some 20 percent of medi-spa patients in the U.S., and probably a similar number in Canada. Smart marketing can easily lift that number. Here’s just one example of what clever innovation in the customer journey can achieve. Alchemy 43 in Los Angeles offers focused micro medical spa treatments in a beautiful, non-clinical environment – and without compromising on professionalism. A trendy, one-year-young “dry bar” (aesthetics clinic specializing in cosmetic micro-treatments), it features a completely customerfocused website-to-front-counter experience including pink furniture, white marble countertops, free snacks and four upscale locations. Botox and other dermatological treatments are aimed squarely at the millennial, with the goal of making needles as commonplace as mascara wands.

In Canada (bearing in mind that legal regulations differ from those in the U.S.), there is a fantastic opportunity to take the experience one step further by deepening the bond of the relationship with each client – promoting sales by customer evangelization of the experience, as is already happening with Alchemy 43 medi-spas, where millennials are bringing in their parents. Alchemy 43’s innovations may be one of the trends pushing the injectables market to an expected US$5.8 billion by the end of 2024. The trend is, essentially, clinical treatments being delivered in a non-clinical environment. And while taking cosmetic procedures out of doctors’ offices is nothing new, according to the ASPS, the use of Botox alone has increased more than 800 percent since 2002. ASPS statistics also reveal that minimally invasive cosmetic procedures have nearly doubled since 2000, thanks to new and changing methods for reshaping faces and bodies. Now it’s being used to stimulate the growth of hair follicles. These quick and affordable injections take just a few minutes, can be done on your lunch break, and don’t require any downtime. 2. Thread lift: An alternative to a surgical facelift for sagging skin around the face, neck, jawline, arms, knees and buttocks, the thread lift – made famous by actress Gwyneth Paltrow – no longer requires anaesthesia, takes only 15-30 minutes and boasts a fast recovery time. Roughly six to nine months after the procedure, the sutures are absorbed through simple hydrolysis, and the results typically last up to two years. T

he state-of-the-art PicoSure laser is a revolution that is taking medical skin treatments to new heights with many benefits compared to other modalities. This very rapid laser stimulates collagen, decreases pigment and improves texture, all in one treatment. Other advantages are minimal downtime and almost all skin types can be safely treated. Furthermore, treatments are comfortable and require no anaesthetic or analgesics. Client satisfaction is extremely high as results are impressive with minimal risk. How does the PicoSure laser work? PicoSure uses a FOCUS Lens Array, which targets just above the dermal-epidermal junction, exactly to the level which is most beneficial to reduce wrinkles and decrease pigmentation with the least downtime possible. These fractionated laser beams (measured in picoseconds, which are one trillionth of a second) are faster than other lasers used in the past. Also, the PicoSure laser breaks up pigmentation with pressure waves rather than heat. Why is this fact so important? When our pigmented target is stubborn and difficult to treat (such as melasma), or when our skin type is darker (like Asian, olive-toned or darker skin types), this laser does the job with much less risk than other modalities.

What can we treat with a PicoSure laser? When targeting deep, dense, stubborn pigment such as Asian pigment, melasma, tattoos and other difficult-to-treat pigmentation, the PicoSure laser is second to none. Originally designed as a tattoo removal laser, the benefits of picosecond laser treatments were recognized and capitalized more so as an extremely effective anti-aging treatment. We also treat acne scars, surface texture, lines, wrinkles and crepey skin. What are the results seen with PicoSure lasers? Clients rave about their glowing skin, even after their first treatment. This glow and radiance is visibly seen immediately after the treatment and continues to impress in the coming days and weeks. They also enjoy a reduction of pigmentation gradually with each treatment, as well as a reduction in lines, wrinkles, acne scars and irregularities in skin tone and texture. Clients often report they need less or no makeup to hide pre-existing flaws. How many treatments are needed? We usually recommend three to four treatments, six weeks apart for most skin concerns. science of the spa What are the risks or side effects? As with most medical skin treatments, potential risks should be weighed against the benefits, and the client must be thoroughly informed. The risk of hyperpigmentation is present, although much less than with traditional lasers. The risks of blisters or burns exist as with most lasers, although it is a low risk. The treatments must be conducted by highly trained individuals in a medical skin clinic that is well versed in laser treatments. What is expected after the PicoSure treatments? Immediately after the treatment, most clients will have some redness that lasts anywhere from one hour to 24 hours. Fairskinned clients experience less redness for less time than darkerskinned clients. An ice pack is suggested for the first 24 hours on an as-needed basis. Clients usually do not need to take time off work but are instructed to avoid overheating and exercise for the first 24 to 48 hours. Occasionally clients may experience small pustules that look like tiny acne pimples. These are transient, lasting only a few days, and they clear up spontaneously. One of the most noteworthy benefits is that glowing skin is commonly reported, often immediately after the first treatment. A reduction in pigmentation, however, is more gradual, with fading over the next few weeks. The initial treatment may not reveal a huge reduction if pigmentation is moderate or severe, but most clients notice a decrease even after the first treatment. After the second treatment, pigmentation is usually much reduced and continues to improve after the third and fourth treatments. Expectations must be clearly outlined. Lines/wrinkles, crepey skin and acne scars gradually reduce with each treatment as new collagen and elastin is stimulated. This stimulation and regeneration takes time, usually six to eight weeks, and skin texture gradually improves. How does PicoSure compare to other skin treatments? PicoSure has revolutionized the medical esthetic industry because it accomplishes so much more than most other available treatments and it has many benefits, all in one treatment and with no downtime. The most common treatments that PicoSure can replace are: 1. Intense Pulse Light (IPL) This technique is otherwise known as Photorejuvenation. It has been the “bread and butter“ of many medical esthetic treatments for almost two decades. IPL treats the superficial skin, targeting redness, brown spots and stimulating collagen. It is still one our favourite “go-to” treatments, especially for lighter skin. PicoSure can treat deeper into the skin than IPL, however, so it has become our first recommendation for clients who have darker skin types. PicoSure carries much less risk than IPL for hyperpigmentation and brown spots. One of the few issues PicoSure laser does not address is redness of the skin. For clients with rosacea, we primarily recommend IPL. For clients with redness but not rosacea, they can still benefit from PicoSure treatments for pigmentation, acne scars or skin rejuvenation, followed by IPL to target redness. 26 S pa Inc. | Win te r 2 018 -19

2. Fraxel Laser For deep pitted scars, if the skin type is fair and not at high risk for hyperpigmentation, we may suggest the Fraxel laser. The client must be instructed that the Fraxel laser results in two to three days of redness and swelling that is significant enough to require time off work and activities. If the skin tone is darker or if there is any melasma present, however, we would suggest PicoSure. 3. Microneedling Most clients who choose microneedling are usually on a budget and don't mind some downtime (usually two to three days of redness and swelling) to address acne scars or skin irregularities. 4. Microdermabrasion, Chemical Peels, OxyGeneo or Other Skin Treatments These other skin treatments are very beneficial and can be used in conjunction with PicoSure, but they cannot achieve comparable results. Microdermabrasion, chemical peels and OxyGeneo are all great to exfoliate the skin, treating the superficial surface to give smoother, brighter, fresher skin. The PicoSure laser is treating the tissue at a much deeper level to achieve changes at the dermalepidermal junction. We usually recommend alternating treatments every three weeks; we may start with one OxyGeneo treatment or chemical peel to exfoliate the skin, followed in three weeks by one PicoSure treatment. A series of three to four alternating treatments is then suggested along with medical-grade skincare products for optimal results. How do you choose the best treatment for each client? Each client is unique, so a personal consultation and customized treatment plan is crucial. Clients are assessed for their skin type, skin quality and skin concerns, along with their lifestyle and ability to accommodate downtime. As well, the client's budget and their expectations for improvement must be determined. The cost for a single PicoSure treatment usually is around $800, and package prices are available. Although PicoSure is suitable for most clients, alternative treatments may need to be considered. Clients should choose their treatment provider carefully and have all benefits, risks and options discussed. Educating each client in a personal consultation will allow the client to be comfortable in choosing the safest and most appropriate treatment for their skin.

I

started in this industry 19 years ago, which seems like a lifetime but yet like yesterday. My passion for skincare and all things beauty started even before that; I can remember as early as 11 starting a proper skin routine, going to the department stores and admiring the cosmetics counters. By this age my Christmas list was filled with all things skincare and makeup. The entrepreneur in me started even younger – basically, as a child I was obsessed with selling things. Even as young as five years old, I loved the art of selling. I would line up everything I owned on my bed and I would “sell” it for real money, pretend money – it didn’t really matter. I started out with traditional toy cash registers, but this wasn’t “real” enough for me. When I was 10 years old, I finally convinced my uncle to let me play with a real register from his convenience store – and I loved it. As both these passions naturally manifested, I quickly realized when I entered the working field as a teenager, I had my own ideas and very quickly the idea of working for myself was normal – it was something I had to do. Today, of 22 years working, I have been selfemployed for over 15 years. Now I have a multi-million-dollar company that empowers an amazing team of professionals across the country. Getting here was not easy; on the hardest days, I always think to myself, if it was easy everyone would do it. You need perseverance to keep going and the belief that you can. When I see a problem, I automatically try to find a solution. That’s how most entrepreneurs’ brains work: You see something and you want to fix it. And when it comes to risk, I can lay everything I own on the line without the slightest twitch, knowing I will make this work mostly because I identify solutions. So what could I really encounter that I can’t tackle, right?! This is sounding much easier than it actually is. There is no owner’s manual to running a business. Each and every business is different and always will be, because you have people: staff, clients, accounts, patients, vendors – there are always people. We are in an industry where you can never fully take the people away. This is also why each and every day is a different and new experience. You have to adapt to people, challenges, industry changes and growth. I personally believe your company culture can make or break you. The more (or less) effective your company culture is, the more (or less) engaged your employees are – which then influences your reputation. This is something that happens every day. By formulating a Mission Statement, Vision Statement and Core Values (below), we created a very effective company culture. This also guides our hiring practices, as everyone we bring on has to align with our company core values. As my entrepreneur brain takes charge, I generally want results NOW and know that isn’t always possible (I’m still working on this one). But with a daily commitment to be the best version of you, you will continue to grow personally and professionally.

spa star for me, and comedy actually happened later. I knew I was funny, I just didn’t think I could make a living out of it.

You were studying for your master’s degree. What happened with that? I was in grad school when I was diagnosed with cancer. I was defending my thesis when I went into chemo, so I withdrew from the program. I was going to go back two years ago to get my PhD, but then the job at CHFI came up, so I’m in academic limbo. My thesis was about women and humour in visual culture. When I first started studying this – which would have been 15 years ago – women were still on the sidelines of the comedy industry. There was Tina Fey, but not a whole lot of other women. It has changed so rapidly since then. Women are becoming increasingly aware of the fact that we are capable of so much more than just “being equal.”

very weekday on Toronto’s popular CHFI morning show Darren and Mo, Maureen “Mo” Holloway makes audiences laugh. The long-time radio host won the Canadian Association of Broadcasters Gold Ribbon Award for Humour in 2006, and received the 2018 Rosalie Award for women who are “radio trailblazers.” But while the tiny creases around her bright green eyes testify to a life well-laughed, she also conquered breast cancer more than a decade ago. Where did the courage and strength come from to face and overcome cancer? Everybody says, “Oh my God, I don’t know how you coped.” And you cope just fine. People say they can’t handle certain things, and then they do. I guess I’ve always been pretty strong. I’m the eldest of four kids. I had a weird family upbringing – I’ve been on my own since I was 12. I was always different than other people, and while that was hard when I was very young, after a while it becomes kind of liberating. If there was a book about your life, what would it be called? I think it’d probably be “Laugh? I thought I’d die!” I really think humour is a fantastic stress buster and a great healer, and that’s kind of my philosophy of life. Have you always been funny? I started 32 years ago on a station called CKFM, which is now Virgin, so I’ve been doing this my entire adult life. You wouldn’t know it, but I was extremely shy as a child – I would hide in the bathroom whenever people came over. My mother put me in drama school because she thought it would be good for me – and she probably created a monster. Performing has always been an outlet

Can you share your spa experiences? I usually try to go to a spa whenever I travel, so I’ve been to quite a few. John and I went to take the waters at a spa in Schlangenbad, Germany, where my brother was getting married, and it was one of the most embarrassing things that ever happened because we thought it was a nude spa. Not that we are nudist; quite the opposite – we are not very comfortable with that. So they sent us to a room where we thought we had to take our clothes off and we did, and we walked into the thermal bath area – and everyone was wearing bathing suits, except us. In Aix-en-Provence, France, the woman who ran a spa there was as much of a healer as she was an esthetician. I went right after I finished chemo and we didn’t talk much. She applied holistic healing, aromatherapy and Reiki, which I’m not big on, but I remember leaving and thinking she had corrected something that I didn’t even know needed correcting. It was quite remarkable. What is your beauty routine? I have an esthetician I’ve been with for 10 years – she knows me better than my husband! Her specialty is microblading, but she does everything else, as well – including my facials, which I get four times a year. I’ve been getting Botox since I was 38 – gee, that’s 20 years. That’s a lot of money, isn’t it? I’m lucky; my skin is pretty good. I don’t spend a lot on skin products, but I’m diligent about taking my makeup off and using moisturizer. What makes a beautiful human? Someone who is interested rather than trying to be interesting; somebody who is open and light-hearted and enthusiastic, and wants to listen and hear and learn; somebody who is positive and doesn’t let anything get them down – someone who can find humour in life. I wish women wouldn’t take themselves so seriously. We are obsessed by how we look, and that’s fed constantly by the beauty industry. It seems so important to us to be a certain weight, that our hair looks a certain way and that we be a certain shape – and it’s a cold and humourless task that we conform to a certain ideal. There’s no doubt that looking good makes you feel good, but it can’t be a dominant force. Loosen up! I wish women felt freer to tell rude jokes and misbehave, and not be so judgmental about ourselves and others – and laugh more.
