ou won’t find chef Jon ashton screaming at contestants on any of the tV reality cooking shows.

“to me, it’s not about shouting in the kitchen,” says the affable brit.“I refuse to do a lot of shows because I don’t believe in being unkind.” Instead, the Liverpool-born chef uses his talents to promote healthy living and family togetherness as well as send a message to teens to think positively about themselves. He’s coming to town Feb. 1 for the Mytime Women’s Show at the Hampton roads Convention Center as part of his dash around the table tour. ashton credits his grandmother with turning him on to cooking after she invited him into her kitchen to make bread. after graduating from St Helens 4 | MyTime Women’s Show | January 29, 2014

College, he worked for several british restaurants and eventually began appearing on british cooking shows. but ashton says his dream was always to come to the united States. after arriving on these shores a decade ago, when “I had $2,000 in my pocket and knew no one,” he set about getting his face in front of the right people. “When I first came here, I went knocking on doors and wouldn’t take ‘no’ for an answer,” he says. after a year working in Florida television, he appeared on “the tonight Show with Jay Leno,” which led to a regular spot on NbC’s “today Show.” He’s currently a contributing editor for dash and Parade magazines and food correspondent for “the daily buzz,” a nationally syndicated show in Chicago. “I was bullied in school because I was overweight,” he says.“It affected me for a short while but I managed to bounce back.” today he talks to teens about good eating habits. “understanding food is the conduit in having students feel positive about themselves,” he says.“It’s about fighting negativity and believing in yourself, looking in the mirror and loving yourself.” despite his achievements, ashton doesn’t like to be called a “celebrity chef.” His aim, as he writes online, is to be “just someone who has a passion about food, and a goal to inspire you to cook, and make lots of amazing memories with your meals.”

More than an inconvenience.

A gynecologist talks about abnormal uterine bleeding.

Dysfunctional. Excessive. Abnormal. Irregular. Heavy. You get the picture. These are not words that inspire confidence. When you use them as adjectives to modify “uterine bleeding” the picture definitely doesn’t get any better. Despite its less than welcoming name, however, this condition, known as menorrhagia, is relatively common. In fact, it is the most common GYN disorder among reproductive women. Abnormal uterine bleeding affects 10% to 15% of women in this category with a high concentration among adolescents who have recently started menstruating and older women approaching menopause. My hope is to provide some question and answer-based information about abnormal uterine bleeding that you can use as part of your own approach to better health and wellbeing or share with other women. If there are any men still with us who haven’t excused themselves for being on the wrong side of the gender aisle, the “information sharing” aspect can apply to you, too. In all of this discussion the most important point I want to convey is that abnormal uterine bleeding can be successfully treated, managed and controlled. Because the condition is sometimes related to social embarrassment, sexual compromise and even a diminished quality of life understanding the potential for reducing or eliminating symptoms is the critical message for woman of all ages affected by abnormal bleeding.

What is it? Abnormal or dysfunctional uterine bleeding generally involves irregular (in terms of the menstrual cycle) blood flow that is often heavy and prolonged and is not associated with other medical conditions

like blood disorders, thyroid or other metabolic problems, fibroids, pregnancy, certain medications you might be taking, eating disorders and gynecological infections.

Why is it a problem?

What causes it?

One of the conditions that the condition can cause, because of the blood loss, is iron deficiency anemia. That’s something we check for and treat immediately. In some cases the flow of blood can be sufficient enough to require hospitalization for fluid management, IV hormone therapy or even a transfusion.

In some cases, the approach may be what we call watchful waiting or basically taking no direct action while the condition is closely monitored. Watchful waiting can be effective for teens whose hormone levels may even out over time and eliminate the irregular bleeding. The same approach can also make sense for women who are quickly approaching menopause, a time when their periods stop and the abnormal bleeding generally stops along with it.

In about 90% of cases, abnormal uterine bleeding is usually caused by changes in hormone levels. What normally happens during a menstrual cycle is that one of the ovaries releases an egg, the process we call ovulation. Hormonal changes, and the abnormal bleeding that often follow as a result, can occur when ovulation doesn’t take place. In around 10% of cases the abnormal bleeding takes place even when a woman ovulates. While it may be the result of changes in body chemicals, the truth is we don’t fully understand this type of bleeding.

How would you know you have it? It may seem a little unscientific, but a common measurement of symptoms is related to how many tampons and/or pads you’re going through within a specified period of time. Another symptom that occurs in some, but not all women, is severe menstrual pain. Given that, chances are fairly good that you would have a reasonable idea that something isn’t quite right. But then again, you may think that this is how it’s supposed to be. The point is, everybody is different so that’s why if you think you have abnormal bleeding it is very important to talk with your healthcare provider and include all the information related to your concern.

How is abnormal uterine bleeding diagnosed? Basically it’s through what we call a diagnosis of exclusion, meaning that we first rule out all of those other conditions that can cause uterine bleeding. That means a thorough medical history, a complete physical — including a pelvic exam — lab work and some further diagnostic tests if necessary. In my practice, I find that ultrasound is a particularly effective imaging tool that can help diagnose abnormal bleeding while excluding other causes. In some cases, generally in

Another concern for women in childbearing years who aren’t ovulating is the possibility that this condition could lead to infertility. In the long run, abnormal uterine bleeding can also increase the risk for endometrial problems including cancer. In addition, some people may experience fairly severe menstrual cramps. All that and there still the possibility that the bleeding is irregular or heavy enough to put limits on some of your lifestylerelated activities.

How is the condition treated? Simply put, a lot better than it used to be. We now use both medical (drugs and medications) and surgical treatments that rely on innovative techniques. Basically, there are two main approaches to treatment: the first and most common is to return the menstrual cycle to normal; the other approach which we use less frequently is designed to specifically reduce bleeding or stop monthly periods. Many — but not all — cases of abnormal uterine can be effectively treated by regulating bleeding patterns with hormone therapy. That includes oral contraceptives and oral progesterone as well as a hormone-releasing IUD. Nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen, Advil, Motrin and Aleve are also used to help reduce bleeding with the added benefit of relieving pain. For women that are anemic or have low iron levels we would also include iron supplements. If treatment with hormonal or other drugs isn’t effective the good news is that surgical options are varied and rely on minimally invasive procedures. With rare exception, we are able to

Like virtually all medical concerns, abnormal uterine bleeding isn’t a onesize-fits-all kind of condition and treatment will vary depending on a number of factors including the cause and severity of the bleeding, your age and overall health, your plans related to childbearing and your personal preferences. And I would add that your preferences are something we take very seriously. My hope is that this information will be of interest and value to you with regard to your own approach to health and wellness as well as the health of someone close to you. What I hope you carry away from this discussion, in keeping with what I said earlier, is that abnormal uterine bleeding isn’t something that has to be part of your life. Make sure you share any concerns you have with your primary care physician or your gynecologist and act as soon as possible. fter practicing as a vascular surgeon for three years on the Peninsula, Susan Hancock, MD has recently opened a solo practice in Oyster Point.
