ORAL CANCER IS USUALLY DETECTED ONLY AFTER THE LESION METASTASIZES TO THE LYMPH NODES, KILLING MORE THAN 300,000 PEOPLE EVERY YEAR.

As early diagnosis can save lives, this system was created to detect healthy looking abnormal tissues. The Velscope is a handheld device that emits a safe visible blue light. The light excites the cells from the surface of the epithelium till the connective tissues beneath, causing the tissues to fluoresce. Healthy tissues appear green, while abnormal tissues appear irregular and dark. The abnormal tissues it detects can be anything from a burn, bacterial infection, inflammation, premalignancy, or malignancy.

Dental clinics use it as an adjunctive aid in detecting oral abnormalities, but definitive diagnosis of dysplasia can only be reached through biopsy. The device can also be used to identify the lesion margins to assist in surgery. Its latest version is lightweight and permits the dentist to attach a digital camera to it allowing dentists to document any suspicious findings for referral or consultation. WRITTEN BY MAYAND AMER

MODIFIED LIP REPOSITIONING AÂ CASE BY DR. FADY GENEDY*

Abstract Gummy smile is the term used to describe a situation where, upon smiling, a patient shows gingival tissue that is equal to or more than 4 mm. There are many causes for a gummy smile that require very careful diagnosis and treatment planning to prevent relapse. This procedure aims to correct this problem by limiting the retraction of the lip elevator muscles which is done by shortening the vestibule.

Introduction In this month's edition, Dr. Genedy was presented with a 37 year old patient, highly concerned with aesthetics,due to her gummy smile. The patient was medically free, but had undergone a previous Gingivectomy to treat the same complaint; however, it has failed. She also had Composite Veneers to control the case but it was ineffective. According to Dr. Genedy, there are several causes for such cases, with each cause requiring its own treatment plan. His diagnosis stated that this patient suffered from excessive soft tissue between the muco-gingival junction and the depth of the vestibule that had to be removed.

After careful diagnosis and treatment planning, Dr. Genedy decided that the lines shown in the picture will mark the extent of the tissue removal. In other words, all the tissues between drawn lines and the muco-gingival junction are to be removed.

The soft tissue that was diagnosed as excessive was removed. Incision lines were placed from the upper left second premolar to the upper right second premolar, as this was the extent of the patient's smile.

In order to create a new labial vestibule, suturing was carried out layer by layer** after the removal of the targeted tissues along with the elevator muscles. To prevent relapse, muscle attachment was removed in order to avoid the re-attachment of the muscles to their old positions.

After the incision was properly secured with a Surgeon's Knot, the patient was dismissed. However, she suffered from a substantial amount of edema on the following day. This is a normal occurrence in such a case. Therefore, Dr. Genedy prescribed prophylactic antibiotic; Amoxicillin 500mg t.d.s, a non steroidal anti-inflammatory drug; Ketorolac trimethamine 10mg b.i.d* and direct ice pack massaging of the lip in order to control the bleeding. The sutures were removed a week later. The case is to be monitored for the duration of 3 months, in order to observe any relapse that may occur. However, if the procedure remains a success after that time-frame, the chance of relapse decreases to zero according, to Dr. Genedy.

Researchers in University of Alabama; Velu, Wu and colleagues, invented a molecule that stops or prevents tooth cavitation. In their study, rats on a low sucrose diet were infected with Streptococuss mutans, the major initiator of dental caries. They applied the chemical they developed, #G43, topically twice a day for four weeks which resulted in a great reduction in enamel an dentin caries. The chemical #G43 works by inhibiting the activity of S. Mutansrelated enzymes GtfB and GtfC, which are two of the three glucosyltransferase enzymes forming the glucan biofilm. The agent binds to these enzymes' binding sites, thus inhibiting them.

By inhibiting those enzymes, the S.Mutans are not able to produce the biofilm they depend on for nutrition, growth, and tooth cavitation. It was noted that the chemical does not kill S. Mutans or any other bacteria and does not affect the biofilms of other Streptococci. The research funding was granted by the National Institute of Dental and Craniofacial Research and National Natural Science Foundation of China. The chemical is proven to be effective by in vivo studies on rats and is yet to be tested on humans.

Professor Samir plays a huge role in the support system of the students of Misr International University. When asked about what her position can offer for students, her response was that she is responsible for everything that can affect a dentistry student, ranging from, and not limited to, schedules, notes, handouts and exams. The professor added that her story was similar to many of ours as students; she originally wanted to join Medical School yet her results were a half a mark off so she joined "Dental School by momentum, graduated by momentum, finding myself being hired as a staff member, doing my Masters and Ph.D. again, by momentum". Despite her huge success, she was not keen on repeating this experience again, hence she why urged her children not to attend Dental School due to the immense working hours and work load along with the neverending exams and papers she went through.

What you really need is to have some kind of time between your lectures or clinics in order to experience the "uni life", as it cannot be relived in any other place or time. The ones who do enjoy the uni life are usually those who don't care about their academics as those who don't bother to attend. However, the ones who do handle their academics seriously, don't feel the beauty of this time. It came to the point that even during your free time, you still do stuff that are dentistry-related.

Don’t kill yourself over anything! For example, don’t worry about what happns after graduation,. Live your lives! Girls forget themselves, they keep taking courses , work hard to become uni staff members and so on...but where's your life in that? Boys keep running around. go to the army…etc. But when are you going to live? Life isn’t about running around. I’m not saying don’t push yourself, but there has to be a balance. You have to live your life before time runs out. I see a lot of really successful people in dentistry and their family life is absent. Those who didn’t get married and those

Can you diagnose this month's case? This is a 41-year-old white female, presented with a red and white lesion, with central ulceration on the left lateral border of the tongue. It measured 0.5*0.8 cm at its greatest dimensions. It was described to be painful and had been present for ~1 month. The past medical history is negative for tobacco use, but is significant for diffuse Systemic Sclerosis (Scleroderma) since 2006. There was palpable cervical lymphadenopathy.
