SPECIAL SPLIT INSPECTION INSTRUCTIONS The KAB-TECH s.r.o facility does not have the test equipment needed to conduct the Manufacturer’s Test Program as defined in the Follow-Up Inspection Instructions (FUII) for Special-Use Switches, UL 1054 for the following switch families: A split inspection has been authorized between the KAB-TECH s.r.o and EAO AG facilities to conduct all the required tests at the EAO AG facility on switches manufactured the KAB-TECH s.r.o facility. The packaging and shipment of all test samples shall be handled by the manufacturer and shall be performed the same day as sample selection. Manufacturer’s Test Program: The KAB-TECH s.r.o manufacturer shall select the test samples as per the Follow-Up Inspection instructions for the Special-Use Snap Switches, UL 1054 and in accordance with the manufacturer’s quality Assurance Program. The lot of switches from which the samples are selected, shall be segregated and placed “on hold” by the manufacturer pending the successful completion of the required tests at the facility at EAO AG. The manufacturer shall maintain detailed records sample selection (quantities and Cat. Nos.), date samples are shipped, test results, any corrective action taken and copies of actual test data sheets sent from the EAO AG facility in accordance with the requirements of the FUII. These records shall be available for the UL Representative’s review. Responsibilities of the UL Representatives: UL Representatives at both the KAB-TECH s.r.o and EAO AG facilities are to assure that each manufacturer is following the Sample Selection, Test Procedure and Record Keeping Requirements as outlined in the Follow-Up Inspection Instructions for Special-Use Switches, UL 1054 and in accordance with the manufacturer’s Quality Assurance Program. In addition, the test facility IC UL Representative will verify that the samples were tagged, shipped, tested, and the test results were reported back to the KAB-TECH s.r.o facility within a three-week time. The production only facility IC UL Representative will also verify that all switch lots were placed “on hold” by the manufacturer until successful testing on the corresponding samples has been completed within the three-week time period. These records shall be available for the UL Representative’s review. Additionally, under this split inspection, the UL Representatives at both Inspection Centers will follow the guidelines outlined in “Instructions to UL Representative at the KAB-TECH s.r.o Facility” and “Instructions to the Representative at the EAO AG Facility.” If either UL Representative observes repeated nonconformance during a review of these records, the RROP shall be notified immediately.

INSTRUCTIONS TO UL REPRESENTATIVE ATKAB-TECH s.r.o FACILITY: The KAB-TECH s.r.o facility does not have Follow-Up test capability for the following switch families: (switch families or all) . Samples of switches from the families described above are to be selected by the production only facility IC UL Representative at the KAB-TECH s.r.o plant per the requirements as outlined in the Follow-Up Inspection Instructions for Special-Use Switches, UL 1054 and the Appendix pages. The lot of switches from which the samples are selected shall be placed “on hold” pending the successful completion of the required tests at the EAO AG facility. The production only facility IC UL Representative shall perform a detailed examination of the switches selected as representative of the lot. This examination shall involve a review of the device to determine that the details of construction, ratings, and markings are as described in the applicable paragraphs of the Follow-Up Inspection Instructions (UL 1054) and the Follow-Up Service Procedure description. The selected samples shall be tagged, dated, sealed, and sent to the facility marked to the attention of the UL Representative/test facility IC Inspection Center. The box shall be marked “Hold for UL Representative.” The three-part sample tag shall be completed by the production only facility IC UL Representative. The top part is to be retained by the UL Representative and the two remaining parts are to be attached the sample package. A letter or fax shall be sent to the test facility IC Inspection Center advising what samples are being forwarded to the EAO AG facility and what action (specific testing) should be taken. A copy of this letter or fax shall accompany the actual samples being forwarded to the EAO AG facility. Following completion of all required testing the test facility IC UL Representative shall report the results to the production only facility IC Inspection Center and send the production only facility IC Inspection Center the completed data sheets. The report of the results must be completed within three weeks of the date when the selected samples were tagged at KAB-TECH s.r.o facility. A phone call followed by a fax is acceptable to expedite results and release shipment. If results are acceptable, the lot of switches from which the samples were selected shall be released for shipment. In case of nonconformance, the production only facility IC Inspection Center shall be notified immediately by the test facility IC Inspection Center and / or the EAO AG facility and shipment of the lot of switches from which those samples are selected shall continue to be held from shipment. In addition, the production only facility IC UL Representative shall return to the KAB-TECH s.r.o plant to select additional samples from the held lot the KAB-TECH s.r.o facility may wish to rework the switches and ship them in the same manner as the original samples to the EAO AG facility. The lot shall remain on hold pending acceptable results of additional testing as per the requirements of the FUII. The acceptance/rejection criteria for the lot in question shall also be as defined in the FUII.

INSTRUCTIONS TO UL REPRESENTATIVE AT THE EAO AG FACILITY: Upon notification from the production only facility IC UL Representative that samples are available for testing, the test facility IC UL Representative shall schedule a visit to the EAO AG facility at the earliest opportunity. Samples sent to the EAO AG plant shall be tested in accordance with the requirements of the Follow-Up Service Procedure and Follow-Up Inspection Instructions (UL 1054) Pages. The testing must be completed and the test facility IC UL representative shall report the results to the production only facility IC UL Representative within three weeks of the date when the selected samples were tagged at KAB-TECH s.r.o facility. The test facility IC UL Representative will verify that the samples were tagged, shipped, and tested and test results were reported within the three-week period. If the results are acceptable, the test facility IC Inspection Center shall inform the production only facility IC Inspection Center in writing, and attach the data sheets. To expedite the release of an acceptable lot, it is permissible for the test facility IC Inspection Center to notify the production only facility IC Inspection Center of the results by telephone, fax, or E-mail. Any shipment release by telephone must be followed up with a memo to the production only facility IC Inspection Center, which includes the test data sheets. If the results are acceptable, but the results are reported after the three-week period, test facility IC UL Representative shall notify production only facility IC UL Representative. If it continues to occur, the test facility IC UL Representative should write a Variation Notice to the corresponding CAS Office for further action and copy the production only facility IC UL Representative. In case of a nonconformance, the test facility IC Inspection Center shall be contacted immediately by the KAB-TECH s.r.o facility. After the manufacturer has taken corrective action, additional samples will be sent to the EAO AG facility per the requirements of the FUII for testing. All additional test results shall be provided to the production only facility IC Inspection Center. ENGINEERING CONSIDERATIONS (Not for UL Representative Use): Use - The switches covered by this Report are for use only in complete equipment where the suitability of the combination is determined by Underwriters Laboratories Inc. STANDARD CONDITIONS OF ACCEPTABILITY General - The following five Conditions of Acceptability apply to all switches covered by this Report. 1. The switch terminals have been investigated for use only with copper wire or copper alloy quick-connect terminals. 2. A standard sized quick-connect tab (per Table 7.1 of UL 1054) is to be mated with the appropriate standard size quick-connect connector. The tab is provided with a detent that shall be properly matched to the connector. 3. The spacing between any terminals and a flat mounting surface has been judged in accordance with the Standard for Special-Use Switches (UL 1054). However, the spacing requirements between the connection when installed on the terminal and the mounting surface shall comply with the enduse Standard spacings. 4. For switches with integral leads, the temperature rating of the leads is 60째C minimum unless the leads are surface marked with a higher rating. 5. test.

SPECIAL CONDITIONS OF ACCEPTABILITY General - One or more of the following Conditions of Acceptability apply as indicated in the Product Covered table beginning on Page 1 of this Report under the SPCOA (Special COA's) column. 1. The nonstandard quick-connect tabs (i.e. other than noted in Table 7.1 of UL 1054) have been investigated with a specific nonstandard connector attached to wires of a specified size. 2. These are lighted switches employing a lamp. The lamp life should be evaluated when required by the end-use product Standard. 3. The switch has openings in the housing adjacent to arcing parts. The enduse application may involve environments (such as excessive dust or adjacent combustible material) that would exclude an opening in the switch housing. 4. These are diaphragm activated water level switches. Samples of the diaphragm have been subjected to aging tests for use at a specific temperature (shown within parenthesis in 째C) and have also been examined for tensile strength and elongation after exposure to detergent. However, if the switch is mounted below the level of water which indirectly actuates it and the switch has an integral metal case, the metal case is to be considered a live part. 5. These are speed control switches. The investigation was limited to the switching function of the switch. In the final application it should be determined that the speed control circuit can be used with a particular appliance without resulting in a hazardous condition such as overheating of a motor or the switch in other than the full speed position. Open and shorted components of the speed control circuit shall be evaluated for compliance with the end-use Standard. 6. The switch employs screw-type pressure wire connectors or push-in terminals. These have been evaluated for use with solid and/or solder-dipped stranded conductors of a specified size (shown within parenthesis in AWG). 7. These switches employ an integral potentiometer. The investigation was limited to the switching function of the switch. The insulating materials and the spacings of the integral potentiometer should be investigated for compliance with the end-use product Standard. 8. The switch employs auxiliary contacts located externally to the main switch contact chamber. The auxiliary contacts were not tested as part of this investigation. The suitability of the auxiliary contacts must be determined in accordance with the end-use Product Standard. A1. Cat. GENERAL: The devices covered by this, report are single or multi-pole, 1 or 2 circuit, momentary, alternate or rotary action switches. They employ selfcontained switch modules to which are added supplementary actuators such as pushbuttons, bezels, keylock mechanisms, etc. The switches may be provided with indicator lamps. ENGINEERING CONSIDERATIONS (Not for UL Representative Use): Use - The switches covered by this Report are for use only in complete equipment where the suitability of the combination is determined by Underwriters Laboratories Inc. STANDARD CONDITIONS OF ACCEPTABILITY General - The following five Conditions of Acceptability apply to all switches covered by this Report. 1. The switch terminals have been investigated for use only with copper wire or copper alloy quick-connect terminals. 2. A standard sized quick-connect tab (per Table 7.1 of UL 1054) is to be mated with the appropriate standard size quick-connect connector. The tab is provided with a detent that shall be properly matched to the connector. 3. The spacing between any terminals and a flat mounting surface has been judged in accordance with the Standard for Special-Use Switches (UL 1054). However, the spacing requirements between the connection when installed on the terminal and the mounting surface shall comply with the enduse Standard spacings. 4. For switches with integral leads, the temperature rating of the leads is 60째C minimum unless the leads are surface marked with a higher rating. 5. test.

SPECIAL CONDITIONS OF ACCEPTABILITY General - One or more of the following Conditions of Acceptability apply as indicated in the Product Covered table beginning on Page 1 of this Report under the SPCOA (Special COA's) column. 1. The nonstandard quick-connect tabs (i.e. other than noted in Table 7.1 of UL 1054) have been investigated with a specific nonstandard connector attached to wires of a specified size. 2. These are lighted switches employing a lamp. The lamp life should be evaluated when required by the end-use product Standard. 3. The switch has openings in the housing adjacent to arcing parts. The enduse application may involve environments (such as excessive dust or adjacent combustible material) that would exclude an opening in the switch housing. 4. These are diaphragm activated water level switches. Samples of the diaphragm have been subjected to aging tests for use at a specific temperature (shown within parenthesis in 째C) and have also been examined for tensile strength and elongation after exposure to detergent. However, if the switch is mounted below the level of water which indirectly actuates it and the switch has an integral metal case, the metal case is to be considered a live part. 5. These are speed control switches. The investigation was limited to the switching function of the switch. In the final application it should be determined that the speed control circuit can be used with a particular appliance without resulting in a hazardous condition such as overheating of a motor or the switch in other than the full speed position. Open and shorted components of the speed control circuit shall be evaluated for compliance with the end-use Standard. 6. The switch employs screw-type pressure wire connectors or push-in terminals. These have been evaluated for use with solid and/or solder-dipped stranded conductors of a specified size (shown within parenthesis in AWG). 7. These switches employ an integral potentiometer. The investigation was limited to the switching function of the switch. The insulating materials and the spacings of the integral potentiometer should be investigated for compliance with the end-use product Standard. 8. The switch employs auxiliary contacts located externally to the main switch contact chamber. The auxiliary contacts were not tested as part of this investigation. The suitability of the auxiliary contacts must be determined in accordance with the end-use Product Standard. A1. Cat.
