EF International Language Campuses Worldwide Liability Waiver Form & Guardian Consent for Underage Students on an Adult Course EF International Language Centers provide language training. The courses are designed for students aged 7 years or older who generally enroll on an individual basis. Students usually travel independently from their home country, with EF transfers from the airport to chosen accommodation. We require EF transfers to be booked for all students under the age of 18. Once in the course town students will travel independently to and from school, living in homestay or student residence accommodation selected and arranged by EF staff. In the homestay half board is included during the week and all meals are provided during the weekend. In residential accommodation the meal plan varies according to the residence booked, please double-check the EF publicity material for full details. Many residences are self-catering. EF recom- mends that you choose the half board option, in residence for your child. This option is available in all UK schools and many schools in other countries. If you haven’t chosen this option, you understand that your child will need to cater for themselves, which means buying all food and drink essentials, and independently cooking and cleaning up. Whether the accommodation choice is homestay or residence, EF provides recommended curfew times according to the local destination. However, you as parents and guardians should be informed that EF does not provide direct supervision when students are not in class. In order for the student to get the most out of their EF program we want to make sure he/she is mature enough to be able to adjust to a different culture and display a level of independence that is appropriate for such a program. It is preferable that the student has traveled abroad in the past. To help ensure the safety and well-being of our customers EF requests all students to carefully read and follow the regulations set out in the document termed Student Code of Conduct. This can be found at MyEF, our Online Campus. Students will receive the URL, username and password after book- ing a course. For more detailed information about your destination and chosen language center, please read the information provided on MyEF and in the Pre-Departure document provided by your sales office prior to departure. On arrival we take photographs of students to help staff easily identify them and in order to produce their EF student cards. It is also sometimes neces- sary to provide photographs for external examinations. Occasionally we put photographs of students enjoying EF activities on our EF Facebook page. By signing this document you are agreeing to allow EF staff to take and use such photographs of your son/daughter. Please also note that by signing this document you are agreeing to allow an EF staff member or host family to authorize treatments and/or medical aid in the case of an emergency. I authorize school staff and affiliates to consent to X-ray examination, anesthetic, medical or surgical diagnosis or treatment and hospital care which is deemed advisable by, and is to be rendered under the general or special supervision of, any fully licensed physician and surgeon, whether such diagnosis or treatment is rendered at the office of a physician or hospital. It is understood that this authorization is given in advance of any specific diagnosis, treatment or hospital care being required, but it given to provide authority and power on the part of the person (s) named below to give specific consent to any and all such diagnosis, treatment and hospital care which the physician may in the exercise of his/her best judgment, deem advisable. In consideration of the EF accepting the student into the program, the student, his/her heirs, executors, administrators, representatives, successors and assigns, and the Student’s parents or legal guardians, hereby release and discharge EF International Language Schools, Inc., its parents, affiliates, direc- tors, officers, faculty, employees, agents, and representatives (hereafter “Released Parties”) from any and all claims that may arise from any cause what- soever, whether resulting from acts or omissions of any persons, from the operation or condition of facilities or premises, from acts of war or terrorism, or from acts of God or nature, or risks associated with the consumption of alcoholic beverages, use of illegal drugs in any form and injury or death from causes such as traffic accidents, crime, assault and theft; responsibility for any accident, illness, injury, or any other damage or consequence arising or resulting directly or indirectly from the student’s participation in the Program; any liability, damage, or injury that may be caused by student’s negligence or willful acts committed prior to, during or after participation in the program; and any liability, damage, or injury caused by the intentional or negligent acts or omissions of any other participant in the program, or caused by any other person; except to the extent that the liability, damage, injury, loss, accident, or illness is caused by the sole negligence or willful misconduct of the EF, its officers, trustees, faculty, employees, agents, or representatives.
