EDITORIAL Volume 5 | Issue 5 | May 2010

Achieving with Achievers The healthcare sector has become one of the most glorified sectors of the Indian economy with launch of world-class facilities, cutting-edge technologies, government initiatives, indigenous manufacturing, and unprecedented growth rates. Even the global meltdown has not deterred growth of this sector as the demand for healthcare is not dependent on the customers’ discretion. Reports suggest that the sector grew at a rate of a whopping 9.3 percent in the last decade (2000 - 2009) and, going further, will grow at 23 percent per annum. This encompasses all major segments including healthcare providers in the public and private sector, health insurance providers, technology vendors, laboratory medicine, life sciences and pharma, among others. The leading contributors are hospitals, medical diagnostic and pathlabs, medical tourism and the medical insurance sector. The growing awareness of the unmatched benefits of digitisation in healthcare facilities has provided the requisite impetus to the health IT industry in the country with the vendors providing excellent software and solutions and providers opting for the best available technologies. In the medical technology space, while big giants like GE, Siemens and Philips continue to provide the best technologies in India, the indigenous medical device industry is also getting established with key players like SushrutAdler Group making a niche space for themselves in the market. Indigenous medical device manufacturing to curb the prices of medical equipment saw a new light, this year, with Trivitron investing in India’s first ever medical technology park in the country. Newly emerged but promising segments such as medical tourism and health insurance are also growing rapidly making India’s healthcare sector bigger and better. The healthcare providers space is also undergoing complete transformation with several corporate and big multi-speciality hospitals, in addition to smaller facilities along with public care centres emerging rapidly. In the coming times, about 15 major hospital projects are expected to come up in the country. Another emerging trend is the overflow of private investments in this sector with various private equity and venture capitalist entities trying to capitalise on the growing opportunities in healthcare. In fact, it is interesting to note that almost 12 percent of the USD 77 million venture capital investments in the third quarter of 2009 were made towards healthcare. Leading funding entities such as ICICI Ventures, IFC, Ashmore and Apax Partners have all plunged into this space. Even the government, in association with the private sector, is all set to spend somewhere between USD 1-2 billion with an aim to make India one of the top five global pharmaceutical innovation hubs by 2020. The success of the Indian healthcare sector can be attributed to certain key people, who have brought about a complete revolution in this industry through challenging times. This issue is dedicated to the leaders in the healthcare sector in India—their dedication and commitment. The ‘Special Feature’ on ‘Road to the Top’ reflects on the journeys of the most eminent people in the healthcare industry in India and shares their experiences and visions for the future. Hope you like the compilation!

TECH TRENDS

Getting ‘IT’ Right The intelligent use of technology by a hospital and its administrator Inderjith Davalur is an MBA and MS (University of Massachusetts, USA) in Healthcare Administration and has worked for over 20 years in the US. With an exclusive background of IT in healthcare, Mr. Davalur has served on patient privacy and electronic health record committees in the US. He is currently the CEO of Aosta Software Technologies, Coimbatore, India. This article is an account of Mr. Davalur’s introduction to a hospital that has garnered a fair level of recognition and respect and its President, who is a well known figure in the healthcare sector in India. Through this article, Mr. Davalur shares his observations made over the past few months of the hospital and its tech savvy President.

Inderjith Davalur CEO Aosta Software Technologies

42

May 2010

It was the year 1990, when Dr. Nalla G Palaniswami and his wife Dr. Davamani returned to India from the US after working as physicians, and founded Kovai Medical Center and Hospital (www.kmchonline.com). It is a 500-bed multi-disciplinary super-specialty hospital on the NH 47 corridor, minutes from the airport in Coimbatore. The hospital has more than 50 medical disciplines managed by highly qualified and trained full time medical specialists providing round the clock service. Over 1000 inpatients and outpatients are treated every day at the hospital. There are three more centers within a 100 km radius. Mr. UK Ananthapadmanabhan, President, KMCH (ukananthsvp@ gmail.com) is a familiar face, and quite well known as a good orator in the healthcare sector. He is one of those people who takes things forward very easily, even in a situation critical, though. He has lived with and breathed in administering hospitals in India most of his life. An engineer by profession,

www.ehealthonline.org

Mr. Ananthapadmanabhan is a tenacious technocrat, who could be an asset to any technological company and the proof of that is live at KMCH, which houses some of the latest in clinical diagnostic and treatment technologies and gadgets. My primary interest, however, lies in the hospital’s information system and the extent to which they have managed to utilise the technology. It has been my theory and pet peeve that most of the companies (I include the manufacturing and distribution sectors in this) all over the world spend money on software for the wrong reasons. While there are several reasons to go around for this malady, one reason that stands out in my opinion is that there is not enough education from the software industry. I belong to the software industry myself and I feel that in a general sense, we (here, I am referring to the software applications in the healthcare sector) have only recently come to recognise and promote software purchases as an investment and not as an expense.

the number of claims to be processed has increased multifold. The process of filling out claim the forms claiming and submitting them manually is fast becoming a major challenge for most mid-size and large hospitals. I also feel that we have not done enough to understand the healthcare industry, its practices, and its needs well enough to be able to proclaim ourselves as suppliers of “software solutions” to hospitals. However, my discoveries at KMCH should demonstrate that it is not all of bad news; if the vendor and the hospital work together (as they always should) a lot can be accomplished and gained on both sides. In the interest of sustaining readability, I am keeping the mundane features of most hospital information systems (HIS) in the market out of this article. What interested me were some very specific uses of technology and clever customisation that have been done at KMCH. While a good part of the credit goes to the software vendor a sizeable portion of credit goes to the President and teams within the hospital that worked tirelessly to make it all come together and work right. 1. Insurance Claims Processing: With the rapid increase in the number of patients with insurance

Manual process introduces errors and delays making automating the process a major requirement. Unlike some hospitals that use a third party software, KMCH chose the HIS BackBone’s insurance claims module

to accomplish this task. Since all the data on the patient and his/her visit is already in the system, only a minor portion of data needs to be entered and the claim submitted electronically. In cases where the insurance company or a TPA is unprepared to receive the claim electronically, BackBone can fax the claim electronically which will appear as a regular hard copy on the fax machine of the TPA. This versatility has increased the efficiency and reduced the errors and turnaround

time for KMCH in insurance claims processing. 2. Paperless Patient Ordering System: KMCH has a specified list of default suppliers for certain items and also their information for Reorder Quantity to be supplied. Suppliers can logon to BackBone and monitor the stock position of the items they supply. A text message will be sent to the suppliers when the stock goes below Reorder Level. The pharmacy,

lab requests, supplies, surgical issues etc., are all done within BackBone thereby completely eliminating paperwork. This also improves accuracy and reduces errors. Several built-in safety nets block most of the common errors in the ordering process and reduces waste and losses. It also improves patients’ experiences and reduces administrative work. 3. Text Messaging: while SMS usage in business is nothing unique, its use at KMCH is

purely aimed at improving patient time utilisation. Text messages are used in communicating vital information such as lab results, insurance pre-authorisation, Job Order Status, Collections, and Patient Statistics etc. For the management, text messages carry information on census, collections, and other alerts

May 2010

43

that would otherwise take longer times. 4. Information Kiosk: A very simple to use touch screen kiosk in the hospital’s lobby has reduced the number of patient visits to the information desk. The kiosk provides information on patients’ details, bill details, and certain test results and for general information and nonpatients, it also provides information on ATM locations, call taxi phone numbers, airline and train schedules. A very interesting feature offered by the kiosk is a patient self-registration

service that speeds up the process for the patient and the hospital staff. 5. Supplier Portal: Based on the success of the paperless concept, there is also a mechanism to generate purchase orders directly through the system for wholesalers and retailers. This allows the system to bring those transactions directly into the financial system and generate GRNs. 6. Personnel: With BackBone in all KMCH satellite centers, employee attendance, leave, permission can be monitored by the personnel department from anywhere. With

44

May 2010

all information available centrally, personnel department can generate salaries on the fly. 7. PRI: BackBone is interfaced with Ericsson PRI Exchange. When a patient gets admitted in the Private Room, the telephone facility with the required services (ISD, STD, Local) is are allotted which in turn will be integrated with the PRI Exchange. Any calls made during the patient stay are posted in the patient bill. 8. Patient Movement Tracker: Request for wheel chairs, stretcher, etc are raised in BackBone to the Central Transport System (CTS) eliminating the normal procedure of using telephones. 9. Equipment Integration: Two-way integration of Lab equipment such as Cobas 6000 helps reduce manual intervention and automate the entire process. 10. CSSD: BackBone helps KMCH manage the CSSD with reports for instance, that analyse the cycle time. The system captures each of the

www.ehealthonline.org

stages of processing, preparation, sterilization etc. 11. Report Builder: With BackBone report Builder, KMCH generates reports with the Backbone Implementation Coordinators. Report Builder can generate data and graphical reports on the fly. The uniqueness of this feature is that someone that cannot

even spell SQL or know what it means can build a report in minutes. Why should any of these features matter to the average or large hospital? Given the competition in the marketplace, every hospital has to find ways to reduce costs, increase efficiency and most important of all, enhance the experience for the patient. It is an accepted fact that an electronic system (HIS/HMS) for managing the hospital operations is a must and considered an

investment rather than an expense. How well a hospital chooses an HIS and how well it gets implemented play a key role in its success and in my opinion, KMCH is a textbook of cases of success. In fact, it is in pursuit of perfection. As for the Chairman, Dr. NG Palaniswami and Mr. UK Ananthapadmanabhan, they continue to push the envelope on all fronts of healthcare technology.

What are the hardware/system requirements for this software to work? What are the various applications that it is compatible with? The software needs PIV with 1.8 Ghz (or equivalent), 1 GB RAM, O/s: Windows 2000, XP, Vista & Win-7. It supports MS office suite and the medical edition works on most windows based application including electronic medical records (EMR), electronic health records (EHR), and picture archiving and communication system (PACS).

What are the software’s major advantages, especially in the healthcare domain? Nuance’s speech recognition software is one of the most accurate speech recognition softwares for PC applications. It enables healthcare professionals to complete clinical notes instantly and at a fraction of the cost of manual transcription. Its extremely fast and easy to use and is amazingly accurate—up to

www.ehealthonline.org

30 percent more accurate than the prior release. Moreover, it can be used within Windows-based EMR solution or any Microsoft Office application and does not require any complicated HL7 interface. The software is packaged with medical speciality vocabularies covering 80 sub-specialities and allows the user to create other custom vocabularies to maximise recognition accuracy. Key features of the software include: Creating and editing clinical • reports Sending email, instant messages • and surfing the web, all by voice Dragon voice shortcuts to • perform tasks faster than ever Dictating into a handheld • recorder for later transcription Fully network compatible • Combining commands to • perform complex tasks on the PC Third party correction features • EMR support • Faster turn-around of reports, • cuts dependency, enhances productivity and saves cost.

How do you see the market in India? Being a cost-sensitive market, do you think this software has enough potential to grow here? As the healthcare industry is evolving in India, there is a lack of support staff including efficient stenographers or administrative staff. In such a scenario, there is tremendous scope for growth as it helps eliminate the high cost and long turnaround times associated with manual transcription of patient notes. As the demand for service grows, efficient and smart technologies will only make the organisation more productive and efficient, resulting in greater returns.

NEWS REVIEW

SoftLink International Conducts Angio Review Session at CSI PTCA Registry 2010 Annual PTCA Registry 2010 was conducted in Bhubneshwar, where about 300 Cardiologist from all over India Participated, debated and discussed their Clinical experiences, Peculiar cases, complications etc. The ‘angiography review session’ is a “highlight’ of the scientific meeting and a challenge in itself to manage. Usually these sessions ‘drag’ the schedule of the conference and invariably run into delays as people bring in CDs in various formats, Films, Laptop etc. and as an organizer it is very difficult to juggle all these and still keep conference on time.

SoftLink International with its rich expertise to design products in

medical imaging has developed a DICOM Imaging suite to cater the requirement of such conference. A networked solution was installed with a Server and couple of workstation at the Conference Venue. All the angio CDs, received from over 100 centers in India were uploaded, sorted and categorized under clinical sessions like, “Chronic Total Occlusion”, “Angioplasty during MI” etc. This categorization and uploading of angio CDs on a unified DICOM platform helped manage the show very efficiently. As a result of deployment of HCP DICOM Net, a cardiac imaging network, the conference and the angiography session turned out to be a ‘resounding success’!

Siemens Introduces a Cockpit for Intervention Rooms Siemens Healthcare presents the Artis zee Cockpit, a new workplace designed to significantly ease the tasks performed in the control room of radiology and cardiology interventional suites. The new Artis zee Cockpit can be used in combination with all angiography systems of the Artis zee family and is available on the market with immediate effect. Several screens side by side, each with its own mouse and keyboard. Until now, this has been the norm in the control room of interventional suites, where medical technicians support doctors during procedures and have to rush back and forth between the screens. The new workplace from Siemens gives a much clearer overview, since the Artis zee Cockpit’s 30-inch medical-grade monitor can display the content of up to six individual monitors. Images generated by the imaging system, video signals from the registration system and 3D images can now be

viewed and processed using just the mouse and keyboard of the Artis zee Cockpit. This means that medical technicians no longer have to switch between workplaces, for example, when taking hemodynamic measurements, retrieving reference images from the database, evaluating intracardiac ECG data or reconstructing 3D images for the intervention. Artis zee is the product

family name of Siemens systems used for interventional radiology and cardiology as well as for surgery and electrophysiology. These systems are available in biplane, multi-axial, ceiling-mounted, floor-mounted, and multi-functional configurations, which means that they can be easily incorporated into any intervention room layout.

Toshiba Aquilion ONE Scans Entire Organ in Single Rotation A new computed tomography (CT) scanner that provides physicians with precise images of internal organs with a single rotation of the gantry has been developed by Toshiba. The Toshiba Aquilion ONE 320detector row CT system with pediatric imaging capabilities can image an entire organ in a single rotation or over multiple rotations, showing real-time dynamic movement and resulting in lower doses of radiation. The scanner, which is in operation at Stony Brook University

Medical Center (SBUMC) in the US, safeguards children and teenagers from exposure to radiation, which is equivalent to 200 X-rays through

typical CT scanners. Unlike other CT scanners, the new non-invasive scanner will allow physicians to not only see a 3D depiction of an organ, but also

how the organ and blood flow are dynamically functioning or moving within and around the organ. The Aquilion ONE can reduce healthcare costs by performing functional studies, while reducing the amount of contrast material and radiation dose. The system often leads to a definitive diagnosis, and prevents some patients from further noninvasive and invasive testing such as nuclear stress test, cardiac catheterisation or coronary stenting. The pharmacist also can consult via video with patients.
