Excursion Checklist Permission form to go on excursion Participants need to bring: • Water bottle • Hat • Enclosed shoes • Jacket/jumper • Appropriate clothes, i.e. warm clothes if cold, protection from sun burn • Medication, e.g. asthma relievers/preventer OHS equipment needed: • Appropriate first aid kit • Resuscitation mask • Emergency contact numbers list • Students guardian emergency contact numbers • Medical information • Mobile phone • Sunscreen • Water Travel • Hire bus and driver • Driver phone number • Transport risk to be managed by provider Group management • No splitting of group • Head count/name check each time on bus • Staff member to be in lead and at tail where appropriate • Monitor weather condition • Monitor student safety Other OHS risks to be addressed as appropriate. Refer organisational guidelines.
