All About Enrollment Benefit programs give you important financial protection when you need it most. You may not think about your benefits every day – you expect them to be there when you need them. Spend a few minutes and review the benefit programs that UST Global offers to make the choices that are right for you and your family.
Employer-Provided Benefits As an eligible employee, UST Global automatically provides you with several benefits. UST Global pays the full cost of life insurance, short and long term disability coverage, and an employee assistance plan. You are not required to make any contributions for these benefits.
Optional Benefits In addition to the employer-provided benefits, eligible employees may enroll in optional benefits including medical, dental, vision, flexible spending accounts, and retirement plans. You contribute toward the cost of the optional benefits that you elect.
When Coverage Begins For most benefits, coverage begins on your date of hire. The exception is the retirement plan, which begins on the first of the month following 31 days of full-time U.S. employment.
IMPORTANT: New hires/transfers: You must enroll during your first 31 days of employment with UST Global. If you do not, you will be automatically enrolled in the Premier Savings Plan, a High Deductible Health Plan (HDHP) with employee only level coverage and you will be waived for dental and vision coverages. Your next opportunity to enroll in benefits will be during the annual open enrollment period, typically held in November of each year for the upcoming plan year. Current employees: Each year you have an opportunity to review your benefit elections during the annual open enrollment period, typically in November, and make changes for the upcoming plan year.
When Coverage Ends For most benefits, coverage will end on the last day of the month in which: •
Your regular work schedule is reduced to fewer than 30 hours per week;
•
Your employment with UST Global ends due to resignation, termination, or death;
•
Or you stop paying your share of the coverage.

All About Enrollment Who Can Enroll You are eligible for benefits if you are a regular, full-time employee. If you elect coverage, your dependents are also eligible for medical, dental, vision, and life insurance coverage. Eligible dependents include: •
Your legal spouse

Your domestic partner (same sex or opposite sex)
•

If you choose to cover your domestic partner, you will pay the same cost and receive the same coverage as you would for a spouse.

Covering a domestic partner will result in additional tax liability (imputed income). If your relationship ends, you will need to notify the US Benefits Help Desk.

Natural children, adopted children, foster children, step children, your domestic partner’s children, or any child for whom you have legal custody

Up to age 26

Regardless of age, if fully disabled and unmarried, provided s/he became fully disabled either prior to age 19 or between the ages of 19-26 if the child was covered by the plan when the disability occurred
How to Enroll Self Registration for First Time Users
Signing In
If this is your first time using ADP services, follow the self-registration process below. Otherwise, use your existing credentials. ➢ Go to https://my.adp.com. ➢ Click Register Now. ➢ Enter the registration code USTECH-adp and click Go. ➢ Enter your name and other requested information and click Confirm. -If the prompt indicates that your record was found, click Register Now. -If the prompt indicates your record could not be found, contact your administrator or help desk. ➢ On Register for Services page, enter your contact information. ➢ View or create (if permitted) a user ID. ➢ Create a password. ➢ Select and answer security questions. ➢ If prompted, read the terms and conditions and select the I Agree check box. ➢ Click Register (or Register Now). ➢ Activate your contact devices by following the instructions in the two emails you will receive. ➢ You can now log in to your ADP service.

How to Change your Coverage Mid-Year The IRS provides strict regulations about changes to pre-tax elections during the plan year. If you experience a qualified IRS family status change mid-year, you are permitted to make a change within 31 days of the event. If the change request is not completed within 31 days of the event, you will not be able to change your elections until the annual open enrollment period. Below are some of the more commonly known qualified family status changes: •
Marriage, divorce, annulment, or permanent separation from a domestic partner

Birth of a child
•
Placement of a foster child or child for adoption with you
•
Obtaining legal guardianship of a child
•
Change in employment status that affects benefits eligibility
•
Return from an unpaid leave of absence
•
Gain or loss of coverage
The change you request must be consistent with the qualifying event. All mid-year changes require documentation, which must also be provided within 31 days of the event. Please contact the EONE BAT Team if you have questions.

Our Plans PPO 250 is our richest plan design, which maximizes coverage at the point of service, so if you and your family would like to pay the least out of pocket at the doctor’s office, then this plan is the right one for you. PPO 500 offers the same rich benefit design as the PPO 250, but requires a little more out of pocket at the time of service. It comes with lower premiums than the Plus Plan. PPO HSA 2000 requires the most at the time of service, but comes with the lowest premiums. Not only does this plan have budget friendly premiums, but it also has the lowest total out of pocket costs (out of pocket maximum + annual premiums). This plan is the only one that can be paired with our Health Savings Account. Note: It is recommended that you use the PlanFit tool on the benefits portal to help you choose the right plan for you and your family.

Flexible Spending Account A Flexible Spending Account (FSA) is funded with money you contribute on a pre-tax basis, which lowers your taxable income. These funds can be used to pay for qualified out of pocket health care costs for you and your eligible dependents, dependent day care charges, or work related transportation expenses.
IMPORTANT: Save your receipts, even when you use your debit card. The IRS requires that you retain actual receipts to document that an expense is FSA eligible.
PayFlex Health Care FSA Your Health Care FSA comes with a debit card to make using your funds easy. All money you elect to contribute is accessible immediately. If you don’t have your debit card on you, you can submit reimbursement claims on the PayFlex website or phone app. The money you contribute can be used to cover out of pocket costs such as: •
Medical expenses - copays, deductibles, and coinsurance
•
Prescription drug costs and over-the-counter drugs with a written prescription
•
Dental expenses - deductibles and coinsurance
•
Vision expenses - prescription glasses, contact lenses, and copays
All FSA expenses must be qualified expenses as Section 213(d) of the Internal Revenue Code defines.
2019 Health Care FSA annual contribution limit: $2,650 PayFlex Dependent Care FSA Your Dependent Care FSA works on a reimbursement basis by submitting your claims on the PayFlex website or phone app; there is no debit card for this. Unlike the Health Care FSA, you can only access the money that is currently in your account. The money you contribute can be used toward care for a child under age 13, a physically or mentally disabled parent or child, or elder care for tax-qualified dependents. Annual contribution limits: •

Highly Compensated Employees (HCEs): $2,400
PayFlex Commuter Benefits The Commuter Benefits allow you to pay for certain work related transportation expenses via pre-tax deductions. Unlike the Health Care FSA and the Dependent Care FSA, the Commuter Benefits are pre-paid on a monthly basis. Elections that are completed on or before the 15th of the month will initiate payroll deductions in the following month. These have a monthly contribution limit of $260 each. •
Parking benefit is used for parking expenses at or near your worksite.

Dental & Vision Insurance Aetna Dental Insurance Aetna Dental Preferred Provider Organization (PPO) Network UST Global offers you the choice of two dental plans: the Basic Plan and the Enhanced Plan. Company Paid Benefits Life & Accidental Death & Dismemberment (AD&D) UST Global provides each employee with basic life insurance and AD&D through Principal in the amount of 2x your basic annual salary up to $750,000. You can purchase additional coverage through the supplemental life insurance.
Disability UST Global provides each employee with Short and Long Term Disability coverage through Principal. This benefit provides an income in the event you become disabled due to an injury or illness that is not work related. Short Term Disability (STD) pays 60% of your earnings up to $1,400 weekly for up to 25 weeks. It includes a one week waiting period and rolls over into LTD if you are still disabled after 25 weeks. Long Term Disability (LTD) pays 60% of your earnings up to $6,000 monthly for up to 36 months, or the date you reach Social Security Normal Retirement Age, whichever is sooner.

Voluntary Benefits Supplemental Life Insurance In addition to the Basic Life Insurance provided to you by UST Global, you may elect an additional 1, 2, or 3 times your annual salary through the supplemental life insurance plan. You can also purchase coverage for your spouse in increments of $5,000 up to $100,000, but the coverage cannot exceed 100% of what you elect for yourself. Children can also be covered at $5,000 or $10,000 and respectively cost $0.50 and $1.00 per month. Rates for you and your spouse are available when you are going through your enrollment in MyADP.

Critical Illness/Cancer Helps protect your finances from the expense of a serious health problem. Includes a Wellness Benefit that pays $50 per calendar year for yourself and your covered spouse if a covered health screening test is performed. The benefit for child coverage is $25 per child, with an annual maximum of $100 for all children. Rates are based on your age and level of coverage you select; they are available when you are going through your enrollment in MyADP.

Retirement Plans New Hires New Hires are eligible to begin contributing to the retirement plans on the 1st of the month following 30 days of your hire date. You will be auto-enrolled at 3% unless you opt out.
Rehires Rehires are eligible to begin contributing to the retirement plans effective the date of rehire. If you haven’t already elected a deferral contribution amount, you will be auto-enrolled at 3% on the 1st of the month following 30 days of your hire date. You can opt out as well.
Type of Retirement Plans Available UST offers two types of retirement plans: the traditional 401K and the Roth 401K. The traditional 401K contributions are made on a pre-tax basis, where it comes directly out of your paycheck before taxes are taken out, which lowers your taxable income. When you begin to take distributions at retirement, you will pay normal income tax on the distributions based on your tax bracket at the time of distribution. The Roth 401K contributions are made after an employer withholds taxes. Because you’ve already paid taxes on these funds, you will not owe taxes on the distributions. To know which is best for you, you should determine when you’ll be at the highest tax bracket, now or at retirement.
