Speakers ALEXANDER HUTTON QC, KEYNOTE SPEAKER Alexander’s practice is focused in two fields: clinical negligence, for claimants and defendants, and all aspects of costs law. In both these fields, Alexander is recommended in the highest band of leading silks in the directories and acts in connection with a range of clinical negligence cases, with special focus on those of the most serious nature and highest value. Alexander is instructed in cases involving all aspects of medicine and medical law, with particular emphasis on maximum value injury cases and those which involve complex medical and legal issues. He has always had a mixed practice of claimant and defendant work in both clinical negligence and costs.
PROFESSOR DOMINIC REGAN, MASTER OF CEREMONIES Prof Dominic Regan is visiting Professor at City Law School, London, and head of KnowHow at forensic accountants, Frenkel Topping. He worked with Sir Rupert Jackson on aspects of Civil Procedure Reform from 2011 until the Lord Justice retired in 2017. He has advised Government on costs in Employment Tribunal cases and the House of Commons on legal aspects of non-disclosure agreements. Each year he delivers litigation training to thousands of lawyers and other professionals who engage with civil claims. ‘The Times‘ legal commentator recently described him as a “Costs guru”. He is also wine critic for ‘Counsel’ magazine with Sean Jones QC.
CLARE STAPLETON Clare Stapleton is a Medicolegal Consultant at the London Office of Medical Protection. Before joining MPS she was a Consultant in Anaesthesia and Intensive Care at Wexham Park Hospital, Frimley Health for 16 years. She was the patient safety lead clinician and also led the mortality review process and set up the Medical Examiner service at Frimley Health. At Medical Protection she assists members by providing medicolegal advice and supports and advises doctors who are involved in Serious Incidents, inquests, disciplinary and regulatory investigations.
MR RONALD MILLER Mr Miller has 30 years of consultant urological experience and is widely published in diverse urological disciplines. He has extensive medico-legal experience and regularly deals with cauda equina cases. He routinely attends court in the UK and Ireland. He works privately at The Hospital of St John and St Elizabeth. Previously he was head of department at the Whittington Hospital and Hon. Consultant Urologist and UCLH, Royal Free and the Institute of Urology.

Speakers CLODAGH BRADLEY QC Clodagh Bradley QC of 1 Crown Office Row specialises in clinical negligence, medical inquests and healthcare regulatory law. She acts on behalf of Claimants and Defendants in clinical negligence claims and her cases typically involve high value cases which have complex medical and legal issues, often involving brain or spinal cord injury and other lifechanging or fatal injuries in a wide range of medical contexts. She is a skilled advocate in the court room and a persuasive negotiator in settlement meetings. Her client handling skills have been among her many qualities praised in the directories over the years. Legal 500 2022 describe her as “a superb strategic mind and very effective advocate, who is excellent with clients and very collegiate to work with.”
MISS LORIN LAKASING Lorin in a Consultant Obstetrician & Fetal Medicine Specialist with 30 years of clinical experience. She works in both the NHS and private sector and has extensive operational experience in the challenges of service delivery. She has also worked in Australia, South Africa and Denmark. Lorin has prepared reports for instructing solicitors from medical defence organisations, legal firms and coroner’s courts for over 15 years. She is an educational supervisor for the Royal College of Obstetricians & Gynaecologists. Her special interest areas include maternal auto-immune disease, late gestation fetal loss and termination of pregnancy, placental failure, fetal abnormality and stillbirth.
DOIREANN O’MAHONY Doireann’s practice includes all areas of clinical negligence, with a particular focus on birth injuries. These include those sustained by mothers including third and fourth degree perineal tear injuries, and those sustained by babies including brain damage leading to cerebral palsy and developmental delay. She is based in Dublin where she has developed a busy practice since her Call to the Bar of Ireland in 2012, acting principally for claimants in clinical disputes. She took the Call to the Bar of England and Wales in 2018 and is a member of the Middle Temple. Doireann has represented a significant number of bereaved families at Inquests into hospital deaths and has vast expertise both in fatal claims and negligentlyinflicted psychiatric injury or so-called ‘nervous shock’ claims. As of July 2021, Doireann is also an accredited mediator. She is author of the award-winning legal textbook ‘Medical Negligence and Childbirth’, published by Bloomsbury Professional, the second edition of which was released in January 2021, as well as numerous journal articles. Doireann is known for her compassionate and empathetic approach to others, making her the choice barrister for victims of medical error.

Speakers LINDA MILLBAND I have been qualified as a solicitor for more than 30 years’ with experience managing complex, high-value clinical negligence cases. I joined Thompsons Solicitors LLP in 2000 and became Head of Clinical Negligence at Thompsons Solicitors in 2012. During my time at the firm, I have specialised in cerebral palsy cases and group actions. I led the litigation which secured a multi-million pound compensation fund for victims of jailed breast surgeon Ian Paterson, Spire Healthcare and Heart of England Foundation Trust. The case raised issues of non-delegable duty of care and settled for £37 million on behalf of almost 700 victims. It has had far-reaching consequences for actions against private hospitals and on the management of such institutions. I am also working on a second group of Paterson cases following recalls of patients by Spire and the NHS. I am also currently leading a group of cases involving rectal and vaginal mesh.
SIMON HAMMOND Simon Hammond joined the NHS Resolution Senior Management Team on 7 January 2019 as the Director of Claims Management. Simon has a wide variety of experience in handling claims having worked for the insurance industry for over 25 years. Simon held head of fraud roles at national law firms, before joining his previous employer, BLM, in 2009. He was promoted to associate solicitor in 2010 and to partner in 2011. Simon has experience of handling all aspects of claims, from policy interpretation, indemnity issues, litigation and prosecution matters. He sat on the Personal Injury Sub Working Group of the Treasury’s Insurance Fraud Task Force, and has also presented at national conferences and edited a chapter of a leading insurance text book. More recently Simon has chaired national conferences on the subject of Clinical Negligence and has given evidence to the Health and Social Care Select Committee on the subject of NHS litigation reform.
DR DENISE CHAFFER Dr Denise Chaffer is a Registered Nurse and Midwife and currently holds the post of Director of Safety and Learning for NHS Resolution (formally known as NHSLA). She holds a PhD in healthcare leadership, a Master’s degree in Management and Social Care, a BA (Hons) Education, plus a Higher Education Teaching qualification. She is a professional Clinical Nursing leader with many years’ of board executive experience, has been an Executive Director of Nursing in two Acute Trusts, as well as a Director in a PCT commissioning organisation and was previously Director of Nursing for North West London Area Team at NHS England as well as being lead for patient safety across the London Region. She has significant experience of working at international, national and regional level within acute and community settings, nursing, midwifery, education, and on major change and reconfiguration initiatives. She published a book in 2016; ‘Effective Leadership, A Cure for the NHS?’ which draws on the personal experiences of a range of senior healthcare leaders.

Speakers BRIAN WESTBURY Brian started professional life as a General Dental Practitioner but soon entered dentolegal spheres. He became LDC secretary to four LDCs and in 1999 was Chair of LDC National Conference. He was GDSC representative for Essex and served on the Statutes and Regulations Committee. He Chaired the Dentists’ Health Support Trust for 10 years. Brian joined Dental Protection in 1995 and stayed with them for 25 years as Senior Dentolegal Adviser with responsibility for NHS and GDS matters. He ran their very successful High Risk Assistance programme. Brian was on the Board at the Royal College of Surgeons that became FGDP and last year became a founder fellow of the new CGDent. Brian was in the first tranche of DLAs to join the FFLM as a Fellow. Brian is qualified in Dentistry, Psychology, and Law. He is an Archers Addict and keeps chickens (Scott & Bailey) and a 100 year old tortoise (Monster). He is currently the Academic Dean of the Faculty of Forensic and Legal Medicine at the Royal College of Physicians.
TIM WALLIS Tim’s career path - from Alternative Dispute Resolution to Online Dispute Resolution -> Litigation -> ADR -> Mediation -> ODR -> Technology -> Arbitration -> Mediation. Tim is an specialist mediator and ADR/ODR practitioner who has been involved in dispute resolution of personal injury, clinical negligence and commercial claims for 30 years+. Tim is also a director of: • Trust Mediation, a specialist ADR provider for personal injury and clinical negligence claims (high and low value claims) • Trust Arbitration, a specialist ODR provider which has an online arbitration joint venture with insurtech company Nuvalaw (high volume lower value claims) and • Claims Portal, an online platform for processing a high volume of road traffic personal injury claims (high volume lower value claims). Tim writes on mediation and contributes the ADR section in Vol 2 of the White Book. He was a founder member of the Civil Justice Council and is currently a member of its Judicial ADR Liaison Committee. As a mediator Tim is particularly interested in multi-party and “unsettleable” claims.
PANKAJ MADAN Pankaj specialises in Catastrophic and Life-Changing Injury with specialist expertise in Catastrophic and Subtle Brain Injury, severe limb injury, high-value Pain and Complex Regional Pain Syndrome (CRPS) claims and Clinical Negligence. He is from a medical family and has an excellent understanding of medical issues. In fact he is the first legal member to sit on the Pain Section Committee at the Royal Society of Medicine. He is renowned for his meticulous eye for detail and sensitive, sympathetic client approach, The Legal 500 has said the following – ‘A first rate lawyer with IT and technology skills second to none. Speakers MR COLIN HOLBURN Colin Holburn has been a consultant in Emergency Medicine since 1992. He qualified from Edinburgh University in 1981. He has been a Fellow of the Royal College of Emergency Medicine and of the Royal College of Surgeons of Edinburgh. He has been actively involved in training in Emergency Medicine and served on the Council of the Royal College for 3 years. He has been an expert witness since 1992 now mainly undertaking Clinical Negligence work as well as providing Expert evidence for inquests. He is instructed both by claimants and defendants and as a joint expert.
FLORA MCCABE Flora is a 14 year qualified solicitor. She read History at Corpus Christi College, Cambridge University and went on to study the Postgraduate Diploma in Law, followed by the Legal Practice Course. She trained and qualified as a lawyer at global firm Eversheds Sutherland and worked there for a further 2 years after qualifying. She then moved to Mills and Reeve where she developed a real passion for healthcare work - all on the defence side - ranging from defending individual consultants to large health corporates and Trusts. She joined Lockton 5 years ago in her role as Head of Advocacy and Risk Management for Healthcare, where she can now play an active part in our clients’ risk management strategies. Her work includes reviewing healthcare client’s policies and procedures; offering training based around case studies on topics such as consent, candour, record keeping, avoiding complaints and regulatory referrals and learning from mistakes. In terms of handling claims, she helps clients to nip complaints in the bud, helps a variety of clinicians to negotiate the stormy waters of being involved in litigation, supporting them at all times of day and night, offers strategic advice and assistance to clients’ in-house legal teams; keeps the pressure on insurers and external lawyers to do the right thing for our clients as fast as possible and generally acts as a complete advocate at times of crisis. She does a lot of work with clients’ governance teams, training them on their notification obligations, as well as triaging calls and notifications to ensure they are telling insurers when required. She also offers lots of hand holding and support to individuals who require representation at disciplinary or regulatory hearings and drafts GMC and GDC as well as Ombudsman responses and is currently writing a book with a consultant ophthalmologist on how clinicians should handle complaints and claims.
ANGUS PIPER Angus specialises in Clinical Negligence and high value personal injury claims, principally instructed on behalf of Defendants. He has more than 20 years experience in the Clinical Negligence field, and 30 years in personal injury. He acts for NHS Trusts nationwide and on behalf of the MDU, the DDU, and other insurance organisations. He is a Senior Junior who frequently acts against Silks and is used to dealing with claims worth £millions. As well as being a fearless advocate, Angus has a wealth of tactical acumen and is used to settling claims on the best possible terms for his clients at Mediations and RTSMs. He has been ranked for many years in the Legal 500 as a leading junior.
