The Cornwall Community Foundation are delivering development work on behalf of People’s Health Trust in support of their Active Communities funding programme within Cornwall. The People Health Trust focus on investing in great local ideas which: - Are designed and run by local people - Create stronger connections between people - Help people to help make their community or neighbourhood even better The Active Communities programme concentrate on two areas of funding: Active Communities targets funds at the neighbourhoods which are the most affected by health inequalities. Theses are called ‘fundable neighbourhoods’. Examples of funding: - Computer group for older people - Group of community drivers to support isolated people - Young people setting up their own local group - Community organisers group on an estate To find out if your neighbourhood is listed please visit www.peopleshealthtrust.org.uk once on the website: - Click on the top right hand corner, highlighted in red, titled Apply for Funding - Scroll to the bottom of the screen & click on the map of Cornwall - You will be taken to the Active Communities criteria page, please read and scroll to the bottom of the page, click continue - This will take you to an interactive map, Zoom into the map (using the +/- slider to the left) to check which streets in your area are fundable neighbourhoods. Only areas highlighted in white are fundable. It doesn't matter if a very small part of a street is outside the fundable area as long as 90% of the beneficiaries reside in the fundable areas, you can still apply. Communities of Interest targets groups of local people who have things in common and wish to come together to address something that is important to them. It is important to note only communities of interest which experience social and economic disadvantage will be considered (you do not have to be listed as a fundable neighbourhood to apply). Examples of funding: - Young people from Town A living with mental ill-health wishing to address access to work issues or discrimination - Isolated older people from the north of Village B who wish to establish more frequent and affordable transport in their area - Parents of children with autism from County C coming together to establish a support group to highlight local services and share information - A traveller community based in and around City D which aims to address economic disadvantage through encouraging small businesses Page 1 of 2

Who can apply? You are a not for profit, voluntary or community group or social enterprise (you don't need to be a registered charity but your activities must be considered charitable) You have a governing document (Constitution, Set of Rules or equivalent) that show how your group is run and managed You must have been in operation for at least three months You are based in Cornwall You must have a management committee with at least three unrelated people over the age of 18 years Your income is less than £ 350,000- this can be averaged over two financial years Your unrestricted reserves (savings) must be less than six months of your yearly expenditure (after deducting the amount you are thinking of requesting from the Trust) Your project must be completed within one to two years of the grant Projects that are small and local for example a few streets or a small village Sorry, but this fund cannot help: Profit making organisations Projects that are strictly health related and focus on certain diseases (e.g. cancer, diabetes, stroke), or which aim to change behaviour (e.g. giving up smoking, weight loss etc.) CIC limited by Shares or Limited Liability Partnerships Statutory/public sector organisations such as health authorities, schools, hospitals, parish and town councils Grant making organisations or bodies who fundraise or distribute grants on behalf of other organisations Regional offices of national bodies if they are not independent i.e. you must have your own local accounts and management committee Groups whose beneficiaries are not people Projects that help only one individual Political or religious projects Activities already funded by another organisation Projects that are part of a larger project, the Trusts minimum contribution must be 50% Retrospective funding (things that have already happened) Loan repayments Capital items only (e.g. equipment), running costs must be included Capital items that are more than £15,000 in total Contingency or unspecified costs How much can you apply for and when can you apply? Grants range from £5,000.00 to £25,000.00 a year. Project must lasting between 12 months and two years. This fund is open to applications now until the 10th April 2015, 11am.
