Medical History Form ESU Health Services strongly recommends that you submit the Report of Medical History form, which includes your immunization history, prior to your entrance to the university. Many academic areas require this information during your course of study including education, the health sciences (such as nursing, psychology, speech-language pathology, athletic training, and exercise science), as do many internships and academic placements. We recommend that you gather this information and submit it well before classes begin to avoid postponements in your class schedule. Students wishing to participate in intercollegiate athletics are also required to complete the physical examination section within six months of team tryouts.

The Report of Medical History form can be downloaded from http://www4.esu.edu/students/health.cfm or the myESU portal. If you have questions, please contact ESU Health Services at 570-422-3553. Pennsylvania law requires all students residing in universityowned housing to have received the meningitis vaccination or be informed of the risks and benefits of the vaccine. This is to be completed when you submit your ESU housing application.

Smoking, Alcohol, and Drugs All forms of tobacco, including nicotine producing products (i.e., electronic cigarettes), are prohibited in all interior work, living, and academic areas of the university, including offices, classrooms, laboratories, lounges, libraries, student rooms, etc. Tobacco products are prohibited in the designated “No Smoking/ Smoke Free” zones between the University Center and the Keystone Room, between Stroud Hall and the University Center and on the walkway below these two buildings. Tobacco use is permitted in any exterior open area which is not within (or closer than) 25 feet from the nearest facility entrance. Ashtrays are placed at the permitted locations. Smoking areas are designated around facility entrances, and smokers are asked to smoke OUTSIDE of these designated areas. Additionally, seven smoking shelters are strategically located throughout campus. Smoking and chewing tobacco are not permitted in Eiler-Martin Stadium or in any state/university-owned vehicles. Anyone found smoking within a designated non-smoking area will receive a warning notice and be asked to extinguish their material and move outside the non-smoking area. Those failing to comply may be subject to disciplinary action. ESU students regardless of age are prohibited from engaging in the use, consumption, possession, sale, distribution or abuse of alcoholic beverages on campus or on property and facilities supervised by the university or during university-sponsored events and activities. Students found in violation of this policy will face disciplinary action and may also be cited for violating any and all applicable law, ordinance or regulation. This policy also applies to parents.
