injury
risk of complications during pregnancy
Naomi's preeclampsia has not yet resulted in an injury for her or her fetuses, but it is a serious risk that can be addressed by a nurse. While Naomi is experiencing daily headaches, an acute headache diagnosis is less urgent to address compared to the risk of complications during pregnancy; in addition, addressing the risk of complications during pregnancy first through preeclampsia management education will ultimately reduce headaches and discomfort.

edema in lower extremities
To minimize risk for Naomi and her baby, it is important to identify signs and symptoms indicating that her gestational hypertension is causing damage. These signs and symptoms include sustained hypertension, lower extremity edema, and protein in the urine. Nausea, frequency of urination, and weight gain are not concerning symptoms that would indicate preeclampsia or a "risk of complications during pregnancy" diagnosis.

Address a patient's insufficient knowledge as soon as possible (in this current visit), and give the patient the correct information yourself, as opposed to encouraging them to conduct their own research, which can lead to less evidenced-based learning.

Instruct patient to drink 8 glasses of water a day
While nothing alleviates preeclampsia symptoms quite like the delivery of the baby, a patient can manage the condition through careful lifestyle changes. Increasing lean protein intake and increasing fluid intake can all help address high blood pressure. Additionally, having the patient monitor her own blood pressure at home can help her know when it's time to call her physician.

by increasing water intake, monitering BP, and altering the diet, the pt. will have the causes of managing her preeclampsia
Making changes to diet, hydration, activity level, and blood pressure monitoring are all healthy and effective ways to manage preeclampsia.
Patients should receive detaile instructions; vague suggestion like "work on your diet" or "monitor your blood pressure" (without any idea of how or ho often) do not lead to optimum patient outcomes.

Explain the rationale behind your nursing diagnosis, as well as your goal(s) for Ms. Adebayo and the interventions and data collections you will use to help her achieve this goal(s).
ege
Ms. Adebayo, as you know, preeclampsia can affect the health of you and your babies, placing you and them at risk for injury. Fortunately, we're at a stage where we can make concrete changes that can drastically reduce that risk. Some contributing factors to preeclampsia include your current sodium intake, hydration habits, strenuous exercise, lack of at-home blood pressure monitoring, and the history in your family of high blood pressure. We're going to talk more about how to address these factors and better manage your preeclampsia.
Showing a patient that you understand their needs can he build rapport and ensure that y have your information right. Additionally, explaining the details of your interventions an data collections will help Ms. Adebayo understand the plan, and give her an opportunity to raise concerns.
Before beginning her postpartum education, ask for Ms. Adebayo’s consent to the Care Plan.
do you consent to this care plan
Ms. Adebayo, if you don’t have any other questions, we can start our discussion now. Does this plan I just talked about sound good to you?
A patient must consent to all interventions in their Care Plan Disagreements are opportuniti to provide further patient education and to consider alternative options.
