Anger Management Yesterday I got offended. For those who know me well, I am no easy person to aggravate. In fact, when push comes to shove, I normally end up getting shoved. But I digress. My wife bought a new-fangled weighing machine that was supposed to tell me more than just the threedigit figure that I call my weight. It had readings for body fat, BMI, a place to enter your height, your age and a host of other critical information that were supposed to lead me to the same conclusion – that I was overweight. The fact that I am overweight is no shocking revelation to me. I’m used to blaming my heavy bones, water retention, last night’s beer, et cetera. So when a red light started flashing two seconds after I got onto the scale for the first time, I thought it would be the same revelation told to me in a more dramatic way. I was wrong. This time, the flashing words on the tiny digital scale screamed “OVERFAT!!” O V E R – F A T. Can you believe it? What was that supposed to mean anyway? Seriously, I never considered myself thin, but calling me OVER fat

was pushing the limits! As I flipped over the scale, a quick look at the country of manufacture quickly showed me where the problem really was – the translator of course. This issue, we look at the wide range of medical and health services that are available for persons like me. Factoring out those who are born with supermodel-thin genes, shedding some weight would most probably have been on all our agendas at some point. Today, spas and therapy centres offer much more than just a relaxing massage, slimming has become the cornerstone of many therapies. Find out more about them on page 18. My ‘extensive’ research into weight-loss, however, also tells me that there is no quick and easy way to a desirable waistline. Dave Nuku, celebrity trainer from The Biggest Loser Asia, shares with us all about the ‘hard’ way to reach the same desired outcome (pg. 28). Whether you decide to choose the ‘hard’ way or the ‘easy’ way, we hope that you will gain some insights into ensuring that it is the healthy way to achieve weight-loss.

Letters of the Month “Share with us your healthiest, personal local food recipe. Desserts and snacks after dinner can be unhealthy and fattening. In order to keep my children healthy and happy, I came up with a healthy yet tasty snack. All you need is a small tub of plain yoghurt, honey (amount depends on how sweet you want it to be), diced fruits (kiwi, strawberries etc) and a handful of chocolate sprinkles. Mix them altogether and viola! You get your tasty and healthy snack.

I am diabetic. Therefore it is twice as important that I abstain from anything sweet and sugary. However, I refuse to believe that sugar-less food are bland and boring- so I came up with my own recipes. I love the coconut fruit and when I read that coconuts are highly nutritious, rich in fiber, minerals and vitamins, I decided to create my own simple coconut rice recipe. Extremely rich in flavour and aroma, all I have to do is combine my rice, coconut milk and salt in a large saucepan over high heat and boil. Try it, you’ll like it!

The Nicorette Quitters Run The world’s first and only run to promote smoking cessation, The Nicorette Quitters Run held on the 18th of September saw 863 participants. Held at the East Coast Park, the non-competitive 7.5 km run was open to the general public. Registration fees of $10 per participant were donated to the Singapore Cancer Society. “Yoga keeps me stress-free!” Constance Song tells us how she looks within herself for inspiration and why everyone should take up yoga too By Jason Lim

onstance Song is not the kind of girl that you would walk up and introduce yourself to in a social setting. If you are a guy, you would definitely love to. But then again, you probably wouldn’t. She carries all the trademarks of a successful, self-assured lady who relies on nothing and no one save her own talent and beauty. Such is her confidence and bearing that it is possible that many would-be suitors have seen their macho bravado wilt at the very thought of introducing themselves uninvited. Definitely no stranger to the flashbulbs and torrents of instructions from the cameraman, the svelte actress showed no hesitation as she morphed into a series of poses for our cover shoot. Perhaps her hours spent pouting to the lens of the camera in her early days in showbiz might have contributed to her persona somewhat; it seems to have left her with a look that is permanently camera-ready. When the work was over, we sat down to get to know the real Contance Song. As the award-winning Mediacorp actress starts to share with me her passion and dedication as an artiste, a very genuine, simple, and sincere side of her comes across clearly. Here was a woman, who harbours a simple goal and purpose – to do better in every subsequent role she receives. EHB: Tell us a bit about your career as an actress? What prompted you to go into acting and when did you start? Constance: It was pure curiosity. I was studying back then and doing modeling parttime. Acting was a natural progression for me after modeling. It’s been a great 12 years in the showbiz industry for me! EHB: Congratulations on your big win earlier this year at the Star Awards. Was this win especially sweet because you waited quite awhile for it? Constance: Thanks! It’s really good to win! In fact, maybe having to wait awhile for the award was really good! It made me want to try harder and harder each year to be better in my roles and I’m very happy that my effort has paid off. EHB: Tell us about your role in Ultimatum that enabled you to win this award. Constance: Initially when I was given this role, I was very pressurized by it. The pressure came from a feeling of inadequacy. I constantly asked myself, “How am I going to deliver this role properly?” The role required me to be a lady who is in her mid40s. This character was to be very powerful and confident; she had to be very smart, very cunning, very selfish and power-hungry. Furthermore, she had a son who was about 20 years old. The tough part about this character was that she was very bad in a very subtle way. Not outright evil, but very subtle and with a lot of hidden motives. I found that this was a very challenging role as I have never acted in this way before. The stress from playing this character even caused me to have insomnia! However, looking on the bright side of things, I found that this stress was really instrumental in pushing me to the next level and I didn’t expect that I accomplished this role successfully and even win an award for it. EHB: When you were nominated, did you guess or expect to win? Constance: In previous years, I had been

nominated to win before, however I didn’t win. So this year, I did not dare to presume. Especially since this time I was also under such immense pressure for this role, I honestly thought that my chances of winning were not really high. But it was a very happy surprise! EHB: What type of roles do you fit best into? Constance: I like to play my own age! I’ve been always acting quite matured roles till now, for example, as an elderly widow. It’s a really big challenge for me and I think I have already conquered that. So now I would like to do something nearer to my age. That would be nice. EHB: I see, and what exactly would that age be? Constance: …. EHB: Never mind, we know that acting is a very stressful and tiring job. How do you maintain your health and release stress? Constance: I do a lot of yoga! I go for yoga practically every day if I’m not filming. When I am filming I try to go in the mornings before the filming starts. Now I am doing yoga consistently almost 4-5 times a week. EHB: What types of yoga do you do? Constance: I do hatha, vinyasa, hot yoga, ashtanga, basically all types of yoga. That’s how I relax. I’ve been practicing for almost a year at the Omshiva at Turf City and Absolute Yoga. EHB: Who introduced you to yoga? Why did you pick it up? Constance: I picked up because I wanted to lose weight! [laughs] When I first started with Hot Yoga, I would sweat so much during each session and I began to lose weight. Also, I used to have bad knee-cap problems and my joints were giving me problems as well. The orthopaedic and

sports doctor that I consulted told me that my joint problems were due to my muscles not being strong enough. This was the reason why I was experiencing so much pain at my joint area. The doctor advised me to build up my leg muscles. In the past, I could not even stand on one leg for long because it would cause me a lot of pain. As my muscles started to strengthen through yoga, my joints are no longer painful and I feel much stronger. Flexibility is also another area that I wanted to build up. I am such an inflexible person! When my yoga teacher first saw me, she said that in yoga terminology, my body was considered like “fire”- meaning that it is very inflexible! That means I have to work extra hard compared to other people. Yoga is very motivating, if you notice, those people who practice yoga for a long time they all look very young! That has become my main motivation! [laughs] EHB: How difficult was it for you to start learning yoga? Constance: People see yoga as being so easy, but let me tell you, it is actually so difficult! I even see all the men in class struggling! The first time I tried yoga many years back, I did not know how to breathe and I just did it without following the proper steps. At the end of the class, I was lying straight on the floor feeling very giddy. I started throwing up and was covered with cold sweat. Ever since I was so scared of yoga and I stopped for a long period. Then a few years back, I did a health program with Channel U and I was reintroduced back to yoga. That was when I picked up again. I really encourage people to try and do yoga. I find that yoga is easier to start compared to running or going to the gym. There is yoga for beginners and it is easier for someone to go and start. You will feel so calm, so relaxed, as you listen to music and believe me, you will feel so good after that. EHB: Any beauty or fitness tips to share with our readers? besity is a growing problem in South East Asia, and Singapore is no exception. How then, do we deal with the obese teen? Why is obesity an issue? How do we prevent it? And how to we address the obesity itself in a rational way?

Teen obesity is not a simple issue. Obese children are three times more likely than their peers to develop hypertension, and they are at risk of cardiovascular disease (e.g. atherosclerosis or the blocking of blood vessels), high cholesterol levels and impaired glucose tolerance and diabetes. Obese children are also more at risk for worsening of asthma and obstructive sleep apnoea (having a double or triple chin makes it more likely that your air passages are blocked when you are fast asleep). The severely obese also develop bone and joint problems due to their excessive weight. Obese children also tend to develop puberty early – hence they may end up shorter in adulthood than their normal weight peers. Obese teens also tend to have a self image problem and may have issues with self-esteem. Overeating may also result when an already overweight child is feeling bad about not being able to excel in sports and then drowns his or her sorrows by avoiding sport, watching TV or playing computer games, and indulging in eating as a displacement activity.

Acknowledgement is the First Step The first thing to do is to acknowledge the issue. Many a time, a child is seen to be ‘just plump’ or he or she has ‘puppy fat’. Many doctors do not want to deal with obesity because it is a difficult subject, and they quite rightly surmise that parents may be sensitive about the doctor bringing up what is not seen as a problem. If you notice your child is getting bigger, there are objective ways to tell if he or she is fat. There are height and weight for age charts, Body Mass Index (BMI) for age charts, and Ideal Body Weight for Height charts available on the internet (www.hpb.gov.sg, www.cdc.gov). The Health Promotion Board has recently published BMI for age charts for Singapore children. Do remember that the ideal BMI


changes with age in children (the shape of the curve is like a gentle Nike swoosh), so it is not possible to use adult BMI charts for young people below the age of 18 yrs.

Prescribing Help to the Child In Singapore, children get measured for height and weight at various stages of their schooling. Once a child is identified as being overweight or obese, action is taken to counsel the child and the family, and the child is asked to exercise more and control aspects of his or her food intake. At this time, it would be necessary to take a step back to also analyse the pattern of obesity – some of the questions include: • Is this a question of excessive intake or insufficient calorie expenditure? • Are there aggravating factors? i.e. who else in the family is obese? • What does the child eat, when does the child eat and who decides what is on the menu? • How about exercise? Is there any? • Is there a heavy schedule of school, tuition and non-sporting CCA activities? • Is there lack of sleep? (lack of sleep is also associated with obesity) Subsequently, a dietitian may need to be consulted for an assessment about the diet, but often it is obvious even to the lay person that the child is eating too much. A doctor may need to be consulted to look for and treat complications such as hypertension, hyperlipidemia (high cholesterol) and perhaps even prediabetes or diabetes, or even obstructive sleep apnoea.

Causes of Obesity Causes of obesity include: • Poor eating habits • Overeating on a regular basis or binging (eating a lot on certain occasions) • Lack of exercise • Family history of obesity (partly because of genetic reasons and also because eating and activity patterns are learnt) • Medical illnesses (endocrine, neurological problems) • Medications

(steroids, some psychiatric medications) • Stressful life events or changes (separations, divorce, moves, deaths, abuse) • Family and peer problems • Low self-esteem • Depression or other emotional problems

Instituting a Plan Working with a Team A plan for weight loss interventions should be instituted immediately. This may include judicious changes in food intake and increase in activities which promote caloric expenditure and muscle building. Barriers to change should be identified. For example, if the person in charge of feeding the child is not convinced that there is a problem, or the parents have no time to bring the child for an exercise programme, then it is unlikely that weight loss will be achieved. The key is to institute sustainable lifestyle change and not expect quick fixes. You may need to have your child work with a team of professionals including a paediatric endocrinologist, an adolescent physician, a dietitian and an exercise physiologist or a physical trainer. Other doctors such as the respiratory paediatrician, ENT specialist or psychiatrist may need to be involved too. Making Wise Food Choices Sometimes the interventions are quite obvious in terms of food choices e.g. each can of soft drink not consumed means a saving of 120-130 kilocalories. However it must be remembered that gradual, consistent changes are more effective than suddenly deciding to avoid all fried food, all ice creams etc – because behavior changes must be sustainable to be effective. Overcoming the effects of Travel Sickness

M

otion sickness hits us when we least expect it! One moment, you are sitting there on the boat chatting with friends and sipping your soft drink, next thing you know, your stomach is heaving and your head is spinning faster than a top! Many of us have experienced these horrifically uncomfortable effects of travel sickness – some more than others. What causes this drastic change in our bodies and why are some people more prone to it than others? What Causes Travel Sickness When we move intentionally (e.g. walking), our brain tells our bodies to expect travel in a specific direction and our bodies accept the motion naturally. When there is unintentional motion (e.g. a rocking boat or a bumpy car ride), our brain tells us that we are sitting at a resting position, but our ears are struggling to balance us due to the actual motion that we are experiencing. This clash of signals to the brain is hypothesized as the reason for travel sickness. That is why reading in a moving car often exacerbates travel sickness as our eyes are focused on a still object, thus confusing the brain even further as the ears continue to struggle to maintain balance.

Dealing with Travel Sickness If you know that you are prone to travel sickness, take steps to prevent it early. Here are some tips you can consider:  Choosing a seat – Place yourself in a direction where your eyes will see the same motion that is being felt by the body and inner ears. • Cars – The best place to sit is in the front seat so that you can look out at the distant road. • Boats – The deck allows for cool sea breeze and an unlimited view of the horizon, both of which are good preventions of travel sickness. • Aircraft – Window seats help. Focus on what is going on outside the window. Also try to choose a seat near to the wings of the plane; travel is minimized closer to the wings

 Reading is a big NO for people who are prone to travel

sickness. Keep your attention on what is going on outside. Do not sit in a seat that is facing backward from the direction that you are travelling.

 Food makes a difference too! Avoid spicy or greasy foods that may cause indigestion or tummy upsets (immediately before and during your travel).

For many, shedding unwanted weight has been an endless struggle, like a dark tunnel with no light. Yes, losing weight has always been big on the books for many of us. To lose those pounds, we have relied on drastic measures in hopes of seeing equally drastic results - we have fallen prey to crazy diets and harmful weight-loss pills; we have laid our hands on anything that promise weight-loss and literally starved ourselves to death. Yet much to our dismay, such weight-loss methods are mostly temporary and end up failing us time and again. This avid sportsman and sports enthusiast is also the regional fitness manager of Fitness First Asia. Ezyhealth talks to him to find out more

about his secrets to staying healthy and in shape. Ezyhealth & Beauty: For the sake of the few who have not watched an episode of The Biggest Loser Asia, please tell us what your involvement in this show is. Dave Nuku: I am the Blue Team trainer for The Biggest Loser Asia and I play an active role in the auditions and selection of candidates.


EHB: Tell us what made you decide to become a fitness trainer. Was there anything that inspired you to do so? Dave: My passion for sports began when I was in school during one PE (Physical Education) class. I decided to pursue it and I graduated in 1997 with a bachelor’s degree in Sports Science in Victoria, New Zealand. At the age of 20, I went on to train clients and I really liked it. That was when I decided to continue with fitness training. EHB: What is it that you enjoy about your job? Dave: My job brings a new challenge every day. Fitness is personal and I enjoy streamlining the personal method that people adopt for themselves to achieve best results. At The Biggest Loser Asia, I enjoy getting to meet different people. More than anything else, I get to share the satisfaction and success with my participants whenever they conquer their weight issues – it is a joy to watch them overcome their obstacles and transform as they beat their weight problems. EHB: Tell us a little about your strategy in helping the Blue Team in Asia’s Biggest Loser. Dave: I operate with two sets of rules: First, never feel sorry for yourself and never give excuses. Second, be humble even when you succeed, never get complacent. Most of the participants I’ve met are people who have been hampered by their weight issues for a long time. As a result, they carry the mindset of a victim. The key is to try and change that negative mindset into a victorious one, one which sees the attractive, healthy body as a possibility. It is important to make the change a gradual one because most of the participants are used to lifestyles which do not include exercising. For the new season, I will be concentrating on more intense programmes which will include a few new techniques and equipment. I also believe in taking a step at a time. I believe in the power of one – losing 1 kilo at a time, taking 1 step at

a time, 1 mile at a time. It is advisable not to focus on moving a mountain, for instance, losing 40 kg at a go or running 10 km for a start. Focus on the losing the first kilo, or walking the first kilometer.

EHB: In general, do you think Singaporeans are a fit bunch? Dave: Singaporeans are generally quite conscious of their health. On top of that, the Health Promotion Board and Singapore Sports Council have played a big part in promoting healthy lifestyles. However, it varies from person to person. Some work out regularly whilst others don’t. EHB: What are some of the different problems your clients go to you with? Dave: There are a lot of reasons that may motivate people to seek out a fitness trainer. Some of the most common reasons are obesity, whereas some just want to attain a more toned body or improve their stamina and sports performance. Aside from that, health reasons are also a few of the reasons why people seek a fitness trainer’s advice. For many, we create a fitness programme that lasts for a period of time.

My strategy for my team members will be for them to train up to 6 hours a day – their workouts will include warm ups, cardio training that will be made up of exercises that work various parts of the body. I will focus more on complex stretching and not just stick to isolated exercises such as simple crunches. EHB: As the Regional Fitness Manager of Fitness First Asia, tell us where you think the fitness industry in Asia today is heading. Dave: I would say the fitness industry in Asia has a positive outlook as people are starting to become more and more health conscious. The fact that the Asians are becoming more and more influential enables them to invest in fitness programmes and such. The awareness of the consequences an unhealthy lifestyle can bring also plays a part in the positive outlook Asians have toward fitness. EHB: Share with us the approach you take towards training your personal clients. Dave: The belief that they are able to attain a fitter body is very important when we set out on a fitness routine. I don’t encourage clients to seek advice only when they have lost all hope. They need to realize that they play a huge part in working towards that level of fitness that they want to achieve. They need to stop making excuses for themselves each time they fall short and take responsibility for the change that they desire in life. As soon as they are able to take up that responsibility, I am able to step in and design an optimum fitness routine. I am more of an enabler and influencer rather than the engine for change. I also believe that we should not confuse results with sustainability. Some people achieve weight loss but are not able to sustain these results, they put back on the weight they’ve lost.

EHB: Share with us how someone in their 30s, with no pre-existing medical conditions and who has not exercised for a long time, can achieve fitness. Dave: There are a few components of fitness, the key ones being: cardiovascular, strength and flexibility. Ensure that you get a well-balanced training plan that touches on all these aspects. It is also important that you warm up starting with a walk or slow jog for a few minutes. For cardio training, try to walk or jog slowly for about 1 km. Strength training, such as stomach crunches and push ups to build core strength play a crucial part in a person’s fitness regime. Women need not to be afraid of building body mass like body builders as they are not genetically programmed to build such muscle mass. In fact, body builders need a special diet to be able to gain that much muscle mass. Lastly, stretching and flexibility exercises like yoga help.

EHB: Any hobbies outside work? Dave: I enjoy books by James Clavell, listen to music by John Legend, Kings of Leon and Paolo Nutini, cook and spend my free time watching movies. I also enjoy running marathons, boxing and Muay Thai.

EHB: Let us in on a few easy tips to lose weight and keep fit. Dave: First, all things are difficult before they are easy, so keep at it. Healthy habits take time to sink in. Second, be realistic, transformation takes time, so don’t expect miracles overnight. Set realistic, achievable goals and celebrate the wins no matter how big or small. Last but not the least, never give up. Everyone has fit and unfit days. Don’t beat yourself up if you fall off the wagon. Get back up, dust yourself off and keep moving forward. EHB: As a fitness trainer, how do you maintain your own fitness? Dave: I run and train in the gym daily, a few hours each day. In the morning, I take a run. In the afternoon /evening, I do strength and circuit training .

There are also a bunch of people who come to me with ‘grand plans’. For example, losing 10 kg in 2 days – this is neither possible nor healthy. As such, it is our responsibility as fitness trainers to be honest. We have to let these people know that it is unhealthy and unrealistic to try to lose so much weight within a short time. In general, people these days do not suffer from the lack of knowledge. Most of them suffer from the lack of motivation.

avid Heber, M.D., Ph.D., is a world-renowned expert in nutrition, leading the battle against the global obesity epidemic. He was in town recently and Ezyhealth managed to interview him on what he sees as an obesity epidemic and that is spreading across Asia. An accomplished professor, Dr Heber is also the founding director of the Center for Human Nutrition at UCLA. For the past 30 years, he has been one of the country’s leading anti-obesity and nutrition expert with a focus on cancer prevention. He also serves as chairman of Herbalife’s Nutrition Advisory Board made up of leading health and nutrition experts, and the company’s Scientific Advisory Board comprised of world-renowned scientists whose work furthers the field of nutrition science. As a speaker, his expertise is much sought-after by audiences around the globe and he has appeared on national television and in newspapers, including CNN, NBC, Washington Post and Los Angeles Times. Ezyhealth & Beauty: You mentioned that Asia is the focal point of the international obesity epidemic. Why is that so? Dr Heber: There is really an obesity epidemic centered here in Asia especially in China, India and the entire Southeast Asia. The major factors contributing to Asia being the focus of the obesity epidemic are the change in lifestyle and an increase in prosperity leading to more restaurants especially fast food restaurants opening in Asia. As the income of people in Asia increases, they tend to buy more food and better tasting food. This has led many fast food companies coming over to Asia from America. The consumption of fast food has exploded in recent years. No meals are planned, everything is instant gratification. Fast food tastes good and people don’t care about the nutritional content. calories in fries and hamburgers and coke. For instance, the average fast food meal of French fries, hamburger and coke contains 1300 calories which is what a normal woman needs for a whole day. So if you eat that frequently, you can just imagine the amount of fats being stored by your body.

EHB: Are there any figures to illustrate the extent of the obesity epidemic? Dr Heber: Diabetes, heart disease, asthma, joint disease, cancer are all associated with obesity. It’s forecast for a 120% increase in diabetes in the next 10 years. Even though Asians by built do not get really round and fat, they tend to store more fats in the center of the body. Right now in China, 10% of all obese people have Type 2 diabetes compared to 11% of obese people in the US suffering from the same condition. The population in Asia is so large that the numbers forecasted for the next 10 years really center in Asia, especially China, India and Southeast Asia. We are starting to see teenagers in Asia having adult-type diabetes which is a phenomenon. EHB: How does Asia’s obesity epidemic affect the rest of the world? Dr Heber: Well, the prosperity of the world depends on human capital. Asia, having the largest population centers in the world, definitely has an impact on the rest of the world. The obesity epidemic is causing people to die younger and suffer from diseases younger which costs a lot of money. The costs of healthcare would be overwhelming. And the human capital costs are staggering. If you look at it over the next few decades, that could represent trillions of dollars that could have been put to better use. To combat this, we need more education on nutrition and encouraging people to have more home-cooked meals.


EHB: What are the solutions to this epidemic with regards to integrating social, economic and nutritional biological solutions? Dr Heber: The best way to combat the obesity epidemic is prevention. The way to do it is to start with a healthy breakfast everyday. A quick and simple solution would be for a healthy and nutritious protein shake in the morning that provides the basic fiber and minerals and vitamins. The traditional Asian breakfast of noodles or rice should be substituted as it is very high in calories and carbohydrates. Also, by eating smaller portions everyday, there is more efficient absorption of nutrients. Your body will process small portions effectively. Having a huge lunch or dinner will lead to fat storage in your body. Education is needed starting from the grassroots level especially in schools. Families need to start eating together again by taking control of their diet by buying groceries from the market, cooking in a wok – basically, back to the traditional Asian diet of vegetables, rice, and fish protein. We need to get away from the westernized diet. Because western food is produced for profit with a lot of refined carbohydrates, too much fats, low protein, and practically zero vitamins and minerals.

EHB: What is the effect of having a proper breakfast? Dr Heber: By having a proper breakfast based on protein, you will have enough energy for the whole morning. You will feel energetic. The idea is to have a healthy breakfast that gives you energy for the whole morning rather than a carbohydrate-rich breakfast that gives you enough energy for two hours and leaves you craving for more food. A very healthy breakfast is the most important meal of the whole day. EHB: How do you propose dealing with any expected resistance? Dr Heber: There are always naysayers. The reason that there would be some resistance is because fast food is instant gratification. Like any addiction, people are now addicted to fast foods and high-sugar foods. I call these trigger foods. People always go to these foods when they are stressed out. Sleeping enough and exercising regularly would lower stress and hence the need for trigger foods. This will help you in the long run. EHB: Has the role of meal replacements and protein in the diet been overplayed? Dr Heber: Meal replacements and protein in the diet is not a fad but something that is gaining ground after people start discovering that it is a great way to start the day. There are 3 ways to successful weight loss: (1) physical monitoring of your diet, (2) adequate exercise, and (3) meal replacements that contain more protein than carbohydrates. Meal replacements are a great tool as they help to organize the diet.

Myths about Traditional Chinese Medicine (TCM) Seeking help for weight-loss, chronic disease or weakness in the body? Read these tips carefully to have a clearer expectation of how TCM can help you. By Physician Wu Xue Rong, St Gregory

raditional Chinese Medicine is an often misunderstood and misquoted science. It’s benefits have stood the test of time since the ancient Chinese experimented and understood the healing power of herbs and natural remedies. Today, it is still important for anyone who is seeking the healing benefits of TCM, to be able to separate the proven facts from the “old wives fables”. This issue, we look at 6 myths that need to be understood from a TCM Physician’s perspective:

Myth #1:

Weight-loss methods of TCM are suitable for everyone Using TCM methods to lose weight has the dual benefits of promoting good health without side effects, but it is not for everyone. Weight loss using TCM is done via tui na massage and specially customised and prescribed herbs or medicine, or acupuncture. Such treatment methods are not advisable for patients with pre-existing conditions like high blood pressure, heart disease or stiffened blood vessels. On the flip side, TCM weight loss methods are particularly useful for women who suffer from hormonal imbalance and find it difficult to see results even after exercise. Acupuncture on the other hand helps by reducing fats, detoxifying and cutting one’s appetite. Do note that there is no one-size-fits-all, as TCM weight loss methods and medication are prescribed based on individuals’ body conditions. It will take three to six months to achieve desired results.

Myth #2:

TCM is unscientific The diagnostic methods of TCM mostly lack intuitive and quantitative data, and thus often give the impression of being “unscientific.” However, Chinese medicine is a medical subject that differs greatly from Western medicine; it has been misunderstood because no scientific research has been conducted on it in the past. TCM pays great attention to adaptation of people, time-frame and circumstances, as well as dialectical theory from a macro point of view, to achieve the purpose of preventing and curing diseases and sicknesses. With the progress of times, a lot of theoretical knowledge in TCM is continuously being verified by modern technology.


Myth #3:

TCM can only be applied to treat chronic disease In our daily life, people tend to see TCM as a means to treat chronic diseases or as the adjunct or secondoption treatment when Western medicine fails. What is unknown to many is that the core belief of TCM is precisely to put prevention first before treatment in medical work. The ancient books of TCM promote the idea that “the higher grade doctors focus on the prevention of diseases”, they encourage us to preserve and care for our health, as an ounce of prevention is worth a pound of cure. At the same time, the ancient books have also recorded many precedents for treatment of emergency cases. Hence it can be seen that once we have grasped the distinguishing features of Chinese medicine, it is not impossible to carry out emergency treatments, and sometimes the effect may take place faster than Western medicines.

Myth #4:

Chinese medicine has no side effects Although most Chinese medicine are made from natural herbal plants, the saying goes “as long as it is a medicine, it contains some components of poison”, which means that some seemingly natural herbs also contain harmful toxins, or non-toxic pharmacological contents that are not directly related to the treatment of the diseases - these need to be taken into consideration during prescriptions. Furthermore, Chinese medicine also includes mineral or chemical deposits, which can cause the body to react adversely when not applied appropriately.

It is always right to take tonics when the body is weak It is not wrong to appropriately take tonics when one’s body is weak, but we need to take note that in the consideration of TCM, a weak body can be categorised differently such as problems in hormonal balance,

Myth #5:

Only males suffer from kidney deficiency People invariably think of men when the topic of kidney deficiency is brought up. Does it mean that women will never suffer from kidney deficiency? The answer is no. Various reasons such as excessive sexual intercourse, inherent problems and the after-effects of serious or chronic diseases will all lead to the symptoms of kidney deficiency among women. Symptoms usually include: insomnia with many dreams, memory loss, abnormal menstruation, decreased libido, frigidity and infertility.

Another condition is when “a weak body takes in no nutrition”, which means that a human body that is overly weak would not be able to absorb the nutrition of the tonics. How a lazy teenager became a yoga champion in 3 years By Jason Lim

eenage obesity is a health problem that exposes a child to a host of other diseases and health abnormalities from an early age. Most concerned parents are in a dilemma regarding using the right methods and incentives to help their children out of obesity. Definitely, it is a common hope that diseases can be solved through natural means rather than reverting to the first available pharmacological source. That being said, a lifestyle overhaul is never easy to achieve especially for academically-bound students in their teenage years. In a bid to source out a healthy example of a child who has effectively revamped her sedentary lifestyle into one of health and fitness, Ezyhealth came across a 13-year old award winning yogini named Madeline Lu.

Life before Yoga Although Madeline was never obese, her lifestyle up to the age of 9-years was simply static and sedentary. Not unlike most children that age, after school hours involved lazing around at home and chatting on the internet for hours. Being slightly overweight at that age did not really bother her, but the lifestyle contributed to her being a sickly child. Fevers, coughs, and the common flu were a part of her life; causing her to miss a lot of school days. Madeline never could reach her full potential in studies with the number of days she spent recovering from bouts of sickness. And needless to say, her annual National Physical Fitness Award (NAPFA) met with the poorest results.

A Life-Changing Lesson When she reached the age of 9, Madeline’s mother took up yoga. It was not long before Madeline was joining her mom at Bikram Yoga City Hall. “That’s because mom couldn’t leave me at home alone!” she quipped. By the end of the first few sessions, Madeline knew that this was something that would become an integral part of her life. Practicing every weekend as a pair, both mother and daughter soon came to appreciate and look forward to the bonding time shared through their love for yoga. “Yoga helps me to balance my mind and body,” said Madeline. When asked if she took a liking to yoga because she found it to be ‘easy’, she replied, “No. In fact, initially when I started, all I wanted was to be able to finish the class! It was that tough!” Madeline has come a long way from just trying to finish a yoga class. In February 2010, she swept aside an international gathering of young yogis to win the 1st place at an International Yoga Championship organised by Bikram Yoga.

At the age of 58, such family times are perfect ways for him to bond with his toddling grandchildren and get updated about his children’s progress at work and school. Recently, Mr Ong began realising that as the family was enjoying the chatter or the drama unfolding on television, he found himself struggling to hear and understand the dialogues. Occasionally, a grandchild would call him from across the room, but he did not respond till someone closer to him informed him. Conversations were becoming strained as the family members found themselves having to speak louder and louder just to be heard by him. Mr Ong’s experience with hearing difficulties is not new, in fact, many ageing Singaporeans are experiencing similar problems. These hearing difficulties can be very frustrating not only for the patient themselves, but also for their family and loved ones.

depending on genetics, age, and other factors. Dr Chan Kwai Onn, Consultant Ear Nose & Throat Surgeon, runs a private practice at K.O. Chan ENT Sinus & Sleep Centre in Novena Medical Centre. He tells us that for age-related hearing loss, the patients he sees are normally around 60 years of age. By then, patients would have already experienced degenerative hearing loss for several years. It commonly occurs in both ears, but the loss of hearing is so gradual that people with presbyacusis may not realize that their hearing ability is deteriorating. Sadly, even when the hearing disability is quite pronounced people may be slow to seek help. “It is the social stigma of being labeled as ‘hearing disabled’ that prevents many people from seeking help earlier,” says Mr Helder Araújo, an Audiologist who is also the Regional Manager for Bay Audio. Unfortunately for someone like Mr Ong, age-related hearing loss has no known cure, i.e., the condition progresses with age and hearing loss is permanent.

They are also more discreet, comfortable and enable clearer speech understanding in noisy environments. “Hearing instruments amplify all the sounds that comes through it, including background noise,” Dr Chan explained. “This can cause a bit of discomfort to new wearers as they have been used to a very quiet environment up till that point. Instruments that reduce the background noise are especially helpful to these patients.” “When prescribing a hearing instrument, we must be careful to explain and educate the patient about its features as well as its limitations,” continued Dr Chan. “There is no ‘one-size-fits-all’

solution for hearing instruments. They must be carefully tuned to meet the needs of the individual wearer.” Since hearing loss can happen in both ears, companies manufacturing hearing instruments have created dual, or “binaural” hearing instruments to treat hearing loss. “80% of people who suffer from hearing loss actually suffer from binaural (both ears) hearing loss,” said Mr Araújo. “New technology in binaural hearing instruments feature ear-to-ear technology. This means that the instruments in each ear actually ‘talk’ to each other and give a better, all-round hearing experience for the wearer.”

Screen Your Hearing Mr Araújo further encouraged everyone to take hearing tests just as often as you would take a blood test or any other health screenings. As a general guideline, hearing tests should be done bi-annually. Tests to determine the level of hearing loss such as the Pure Tone Audiogram are available at shops and outlets such as Bay Audio. ypertension, otherwise commonly known as high blood pressure, is a very common condition that affects millions of people all around the world. It is a condition where the force of blood against the artery walls is so high that until it risks causing further health complications progressively.

Defining High Blood Pressure Most people do not know they have high blood pressure because it does not exhibit much initial symptoms. Blood pressure is recorded as two numbers, for example: 120/80mm Hg. The larger number represents the systolic blood pressure, which is the pressure when the heart pumps during a heartbeat. The smaller number represents the diastolic blood pressure, which is the pressure in between heartbeats when the heart is filling with blood.

Causes and Risk Factors Causes of high blood pressure can be difficult to determine. For most people, there are no identifiable causes of high blood pressure and it develops gradually over many years. This type is called essential hypertension. However, underlying conditions such as kidney problems or certain medicines may cause a patient to develop high blood pressure too. This is known as secondary hypertension. There are several risk factors that may contribute to high blood pressure. As a person grows older, the risk of developing high blood pressure increases too. Conditions such as dyslipidaemia (abnormal cholesterol or triglycerides level in the blood) and diabetes may enhance the damage caused by high blood pressure and increase the chances of developing these complications.

Controlling Hypertension Effectively Blood pressure control is a lifelong challenge. Most treatments will involve lifestyle modifications first. Dietary changes, lifestyle changes and weight loss will be initiated before eventually adding medication if needed. Always target a healthy weight range for your height and body type, and try to maintain it. Lowering of blood pressure can be achieved even by

losing a little weight every month Always go for healthier food choices whenever possible. Emphasise on fruits, vegetables, whole-grains and low-fat dairy foods. Sufficient potassium intake may help control your blood pressure as well. Most importantly, avoid food containing high amounts of fat such as deep-fried ones. Get involved in frequent physical activities such as sports at least half an hour everyday. Regular daily physical activities are proven to lower your blood pressure. A person with high blood pressure should cut down on alcohol intake and drink in moderation. Avoid smoking as tobacco will worsen the condition of the arteries. Home monitoring of blood pressure is an effective way of controlling your blood pressure. It provides a more convenient way of keeping tabs on your blood pressure, and indicating if the medication taken is working.

Early Detection with Health Screenings As high blood pressure is always regarded as a silent killer, it can be difficult to identify due to the lack of initial symptoms. amily physicians often treat more than one member of a family. In fact, it is exactly because of that reason that they are known as “family” doctors. Often, we determine our family doctor simply by how much we like or “click” with the physician. Dr Tan Tze Lee is a family physician who practices at The Edinburgh Clinic in Choa Chu Kang town – and it is hard not to like him! Cheerful and approachable, he was all set to help me with my list of questions from the moment we sat down to chat. The reason we met Dr Tan that day was because he is not just a family physician but is also the Vice President of the Chronic Obstructive Pulmonary Disease Association, Singapore (COPDAS). A disease that warrants much more awareness than it currently commands, Chronic Obstructive Pulmonary Disease, or COPD in short,

is a growing problem in affluent Singapore. As its name suggests, COPD is a disorder that is characterized by a gradual loss of lung function. A sufferer of COPD will feel effects such as breathlessness and chronic cough. This debilitating disease not only affects a person’s health, it is severely disabling to a person’s quality of life as well. This year, World COPD Day falls on 17 November and the COPD Association, Singapore is all set to bring more awareness and focus to this disease through a public forum. As one of the keynote speakers at the upcoming public form, Love Our Lungs, Dr Tan Tze Lee was eager and ready to tell us more about the disease as well as what we can expect from the upcoming forum.

Dr Tan: Usually for smokers, the severity of COPD has to be quite great before they notice that something is wrong. Once they are symptomatic, at least half of their lung function will have been affected already. We have to understand is that the process of smoking cessation does not consist of just telling the smoker to stop. As it is an addiction, the smoking habit is often something the smoker cannot easily quit. It is hard, though not impossible, to overcome the addiction by themselves. We as health professionals should be in a position to help support them through this difficult time as they strive to stop smoking. And we are. There are many sources of help available, like smoking cessation clinics, nicotine replacement therapies and drugs. Across the island, smoking cessation clinics are run by the Health Promotion Board (HPB), polyclinics, pharmacies, and GP clinics. In addition to these counselling supports that are available, there are also medicines that are available to help smokers overcome the need and craving for smoking, like varenicline and other nicotine replacement therapies (patches, gum etc). Smoking rates have been increasing in recent years, unfortunately, and this sobering fact should make us sit up and analyse why more people are smoking and how we can convince them that the choice they are making is bad for them. Why do people take on the risk of smoking? Sadly, it is because they “enjoy” the risk! We need to let them know that there are better ways to relieve stress and enjoy free time. Instead of just going cold turkey, once the smoker has decided “Yes! I want to quit!” there are many avenues of help available to them.

EHB: We understand that smoking is the No. 1 cause of COPD worldwide. However, it is not easy to tell a smoker to quit immediately. As a physician, what kind of advice can you give to smokers who are hoping to kick this damaging habit?

EHB: What are the most effective treatments currently in the market for treating COPD? Dr Tan: Certain medications have been shown to positively affect the long-term quality of life as well as survivability. The medications that come to mind

EHB: Approximately how often does a doctor like you in a family practice see COPD patients? Dr Tan: For COPD, there is a need to be aware that these conditions do exist out in our community. I make it a point to screen a person for COPD, based on criteria such as whether they are smokers for more than 10 years, or the presence of recurrent cough with phlegm. Even for patients who consult at my practice for any other medical problems, the chances of detecting early COPD is actually quite high. Maybe out of every two smokers, one will fulfill test criteria for COPD. The most important thing is to have public awareness so that people will not be thinking of COPD as a distant disease that is not likely to affect them. COPD is in fact a disease that is very close to heart and can indeed affect them. Smoking in Singapore is probably the biggest contributing factor for COPD, and people must realise that any exposure to smoke can put them at high risk of developing COPD. EHB: How prevalent is COPD here in Singapore and what is the mortality rate like? Dr Tan: 3.5% of Singaporeans ≥ 30 years of age are estimated to have moderate to severe disease and, as the population ages, the burden of COPD is likely to increase.* In Singapore, COPD accounts for 4.6% of overall deaths and 5.8% of those persons ≥ 55 years of age. COPD ranks sixth among the common causes of death in Singaporeans {male to female 5:1} **

would be a inhaler medication called tiotropium that has been shown to improve survival as well as quality of life. This means that people who are suffering from COPD can become more active, and go back to doing more of the things they could do before. EHB: COPD is a great burden for patients to carry. Their quality of life is evidently severely affected by the disease as well as the need to continuously take medication on a daily basis. Are there any methods of devices that are available to assist doctors in patient’s compliance? Dr Tan: People with COPD are like people with a problem that cannot be changed. Once you give them these new medications, though their lung functions may improve slightly, they can never go back to enjoying completely normal lung function. EHB: Does that mean that when you have COPD there is nothing that you can do? Dr Tan: NO! There are lots of things you can do. First and foremost is to stop smoking! That will immediately stop the deterioration in lung function. Exercise and lung rehabilitation is another. The ideal medicine is one that is easy to take and need only to be taken once a day. If you asked me to take a medication a few times a day which is also difficult to use, I might forget to take it or decide I just don't want to use it anymore. As medications become more sophisticated, easy to use and only taken once a day, we know that it will improve compliance across the board, whether it is a pill, inhaler or device. One such medication is the Respimat® Soft Mist Inhaler. EHB: We hear that there is a COPD Association in Singapore! Tell us a bit about this association and how does it hope to help the general public? Dr Tan: The COPD Association of Singapore was formed in 2004 by Dr Ong Kian Chung and some of his volunteers. It was formed with the aim to improve

medical express the awareness of the condition of COPD for patients, their caregivers and the public. The reason for raising awareness is so that the public, patient and caregivers are in the position to understand the disease better, manage it better and know how to prevent or limit its impact on the general public. As we know, smoking plays a big part in it; without better knowledge amongst smokers, many smokers will carry on in their smoking habit without knowing their risk of COPD. EHB: Tell us about the upcoming love our lungs public forum LOVE OUR LUNGS on 13 November 2010. Dr Tan: I think you will find it to be a very exciting time! We had an excellent forum last year where we presented about common lung problems and the feedback we got was encouraging. Those who attended found it most interesting and also found that their knowledge improved greatly. This year we are reprising it with this second installment of the forum. We will be talking more specifically on the common issues like asthma and COPD as well as other topics related to our lung health. If you are a smoker, I’d definitely encourage you to attend. Through this forum, smokers will be able to learn about how smoking can actually affect their lungs and quality of life. If they make the decision to quit smoking, we’ll be on hand to give them the advice and support to help them on their journey towards giving up their cigarettes.

COPD Patient’s Story EHB:Can you tell us an example of a patient who came to see you for COPD and how her treatment helped her? Dr Tan: Madam H came to see me in Dec 2009 as she had been getting more and more breathless over the past few months. She had been smoking since her teenage years, and had recently cut down to 10 sticks a day as she was getting more breathless when walking to the market. Previously she could walk slowly the distance of 2 ½ bus-stops without stopping, but now had to stop twice to catch her breath. She had heard from her daughter about a breathing test and asked if we were able to do it for her. We obliged, and found that she had moderate stage COPD. She was advised to stop smoking and was also started on tiotropium inhaler. On the next visit in January 2010, she reported that she was feeling much better and not so breathless. Her trips to the market were much easier now although she felt she could not quite give up smoking yet. She was counselled again about quitting smoking and she said she would try to cut down further. Over the months, her condition continued to improve and her quality of life was very good when assessed after 4 months. Her smoking has reduced though not completely.

Understanding Chronic Obstructive Pulmonary Disease (COPD) Chronic Obstructive Pulmonary Disease (COPD) is a fast growing national health burden worldwide and in Singapore. Dr Ling Hui, Senior Medical Affairs Manager for Boehringer Ingelheim Singapore tells us that latest WHO figures estimate that 210 million people are currently suffering from COPD and more than 3 million people died from the disease in 2005 –more than breast cancer and diabetes combined. “In Singapore,” she said, “there are an estimated 64,000 cases of moderate to severe COPD which represents 3.5% of population aged 30 years and older. As the population ages, the burden of COPD is likely to increase. COPD is the 6th leading cause of death and the 9th leading cause of hospitalization in Singapore. COPD mortality rate in Singapore is higher than countries like the United States, Canada and Australia.”

What is COPD? COPD is a term used to describe two pulmonary conditions – chronic bronchitis and emphysema. Basically, when the airways are inflamed, it becomes constricted. This causes the difficulty in inhaling and exhaling of air. Patients suffering from COPD face a persistent blockage of their airways in the lungs that interferes with normal breathing. As the disease progresses, patients find it increasingly difficult to breath. This decrease in lung function manifests in shortness of breath and difficulty breathing when a patient is carrying out normal day-to-day tasks. Walking short distances or climbing a flight of stairs becomes an increasingly difficult task for a patient as the disease progresses.

What Causes COPD? Most COPD is caused by long-term exposure to cigarette smoke. However, people can develop COPD due to breathing in second-hand smoke, fumes of certain chemicals, exposure to industrial pollution and other forms of smoke inhalation.


Dealing with COPD – Drugs and Devices Since smoking is still a leading cause of COPD, smoking cessation is still a key factor in the management of COPD. Patients usually show a positive improvement in lung function shortly after quitting smoking. However, it is a well-known fact that quitting smoking is no easy task! Dr Lim Kai Toh, Senior Product Physician from Pfizer Singapore says that data from national smoking cessation programs and clinical trials show that not many smokers are successful kicking the addictive habit. “The most effective methods of helping smokers to quit smoking are a combination of pharmacotherapy (use of medication) with advice and behavioral support, rather than depending on an individual’s will power alone,” adviced Dr Toh. One medication that can assist in this process is varenicline. “A patient will not get the same ‘kick’ out of the cigarette if they happen to smoke while they are on varenicline,” explains Dr Toh “This reduces their dependence on smoking and nicotine.” The objective of pharmacotherapy for COPD is to improve symptoms especially complaints off breathlessness. Medications called bronchodilators are the mainstay of COPD therapy. Common bronchodilator drugs include tiotropium and ipratropium bromide. Dr Hui from Boehringer Ingelheim tells us, “Tiotropium belongs to the group of long-acting broncodilators which are recommended as first-line maintenance treatment option for patients with moderate to severe COPD (GOLD stages II-IV) in international guidelines developed by Global Initiative for Obstructive Lung Disease (GOLD). Early diagnosis and treatment of COPD can positively impact its clinical course and change the way patients live with their disease. Tiotropium has demonstrated efficacy and safety in an extensive clinical development programme, which included approximately 20,000 patients.” Being such a debilitative disease, COPD patients can expect to rely on the drug for symptom alleviation on a daily basis. Having to use a drug consistently on a daily basis is never easy, and pharmaceutical companies have been innovatively helping patients in this area. “Tiotropium is widely available in the HandiHaler® device and Respimat® – a unique and technologically advanced new generation inhaler,” Dr Hui tells us. Both devices, which are available in Singapore, are designed to ease a patient’s task of transporting and using an inhaler throughout the day. Although treatment does not reverse the damage to the lungs, it can go a long way in making a patient feel better and enjoy a better quality of life.

After adjustments for age, race and pre-pregnancy body mass index, the mean systolic blood pressure was found to be 3.72 mm Hg higher in short sleepers and 4.21 mm Hg higher in long sleepers. Similar results also were found for diastolic blood pressure. The study also found an association between sleep duration and preeclampsia, a condition that involves pregnancy-induced hypertension along with excess protein in the urine. The risk of developing preeclampsia was almost 10 times higher in very short sleepers (<5 hours) during early pregnancy. Because blood pressure is known to dip by an average of 10 to 20 percent during sleep, the authors propose that short sleep durations

Nose drops to treat brain cancer A new form of the anticancer drug methotrexate, designed to be given as nose drops rather than injected, seems to be a more effective treatment for brain cancer. Brain cancer is difficult to treat, partly because the so-called bloodbrain barrier (a protective layer of cells surrounding the brain) prevents medication in the blood from entering the brain. But new evidence indicates that some drugs administered through the nose, either as nose drops or nasal spray, can bypass this barrier and travel directly into the brain. The scientists tested methotrexate nose drops on laboratory rats with brain cancer. U

nderstanding the Jaw Bone

In this article, I would like to write about the bone in the human jaw and its relationship to your oral health. Bones that surround your teeth and hold them in place are called alveolar bone. This bone has the unique feature that allows your teeth to be held firmly by periodontal ligaments or tiny fibers so that your teeth can function well and look good. Neglecting your gum health by not removing plaque and debris especially around the tooth would eventually result in your teeth getting wobbly, painful and sometimes gum boils may result. If gum disease has progressed too far, these teeth would need to be extracted or they will eventually fall off. Whether you lose your teeth due to dental decay, gum disease or accidents, the alveolar bone which used to surround these teeth will simply “melt” away with time. As long as there are no teeth, this special alveolar bone will gradually disappear. You may not realize it but once a tooth is extracted, if the socket is left alone to heal up on its own, within one month it would have lost at least fifty percent of its volume. This leads to a thinning of the bony area.

To avoid this, I usually place some artificial crystal (PerioGlas) into the socket to minimize the loss of bone. The body’s bone cells utilize these crystals to form new natural bone. It is a simple procedure and would cost a patient just a few dollars more, but the benefit far outweighs the cost. However, just leaving the area alone or placing a denture over it will still result in resorption or “melting” away of your bone with time as there is no direct stimulation to the bone. In full denture cases, a two-year period is all you will need to know that you have bone loss as your dentures will no longer be fitting and will start to give you mouth ulcers – the result of rubbing and pressure on the gums. reservation of your Bones

Hence, in my view, the need to preserve what little bone you have is very important. If you decide on dental implants to have a new set of fixed teeth then you would have to consider between minis or the more painful conventional implants. Conventional implants have their drawbacks. Here are a few – being larger in diameter (3.0mm or more) before placing them in, you may have to rebuild your bone. Bone has to be harvested from other parts of your body (usually rib or hip) to increase the volume in your jaw. Some may use treated human bone from dead bodies (cadaveric bone), animal bone or crystals. In all these approaches it may take 3 or 6 months or more for healing to take place. For the upper jaw you may encounter an additional procedure. Due to low sinus you may have to do a surgical procedure known as sinus lift where bone substitute will be introduced to build bone height and volume – again it could be avoided with minis. After waiting usually 3 to 6 months or more the dentist or specialist will need to dig out bone and place the larger diameter conventional implant and again wait for 3-6 months or more for the area to heal. Only on healing he will be able to build fixed teeth. Because of the complex procedure it may be traumatic, painful and has more opportunities for failure.

inding help from Mini Dental Implants

Certain minis, on the other hand, have been approved by USA Food and Drug Administration (FDA) for long term use and have recorded high success rate. The FDA’s approval, in my view, is important as these implants have to undergo vigorous review of its safety and effectiveness before it can be marketed. Furthermore it takes at least 12 to 15 years to see approval – or in other words it has to stay fixed for at least that period of time. Minis with its kinder approach, no digging or complex surgical procedure will allow you to have your teeth at a much lower cost, less pain and with very little abuse to your gums and the little bone you have. In my view, the better obvious solution to your missing teeth is mini dental implants restored with fixed teeth.

Cancer A patient’s survival story

here’s probably no better opportunity to raise awareness about cancer than when a celebrity falls prey to it. When multi-awarded Hollywood producer and actor Michael Douglas announced in September that he had stage IV throat cancer, the upshot was for cancers of the head and neck to share the limelight. We asked Dr Ang Peng Tiam, Medical Director and Senior Consultant at the Parkway Cancer Centre to shed some insights on this severe form of cancer. EHB: What constitutes head and neck cancer? Dr Ang: Cancers of the head and neck comprise a very varied lot as it depends on the exact site as well as the different histologies. Most cases of head and neck cancer are referred to as the squamous cell carcinoma, which may occur in the oral cavity, the buccal mucosa, the tonsil, the oral pharynx, as well as the esophagus. Others cases would include salivary gland tumours and nasopharyngeal cancer, which of course are very different in terms of management and prognosis. EHB: What are the reasons why a lot of patients are already in an advanced stage of cancer when they first consult the doctor?

Dr Ang: There are many reasons why patients present at an advanced stage of their disease. Cancer in its early stages often has no symptoms. Or the patient may note a small lump and think nothing of it. They would assume that it is an ulcer related to some trauma and not imagine that it is actually a cancer. As such, by the time they seek medical attention, the disease could have already advanced. In other cases, there is denial or superstition that prevents them from consulting a doctor. Many try alternative medicines or symptomatic treatments hoping that it will go away. Cigarette smoking is a definite cause of lung cancer as well as cancer at the neck region. We must continue to preach the message of avoiding smoking. For non-smokers, we recommend that they do not take it up and for those who are currently smoking, we encourage them to participate in smoking cessation programmes and stop smoking. EHB: How curable is this disease in its various forms? Dr Ang: I consider head and neck cancer as one of the more treatable malignancies. Unlike in the past where prognosis is poor, the current use of multi-modality treatment combines the best of surgery, chemotherapy and

radiation. A large number of these patients can have good control of the disease as well as high cure rates. EHB: What are the treatment options? How effective are they? Dr Ang: The standard treatment of squamous cell carcinoma of the head and neck has been surgery. However as this is a very visible and complex part of the body, surgery can often be associated with significant morbidity and mortality. For instance, in the cancer of the voice box, a patient would have to consider a total laryngectomy and the loss of voice. Increasingly, patients are opting to have chemotherapy and radiation instead of surgery. In my practice, many patients have been able to avoid loss of organ function through appropriate use of chemotherapy and radiation. I know of many patients who have benefitted from this approach. However, it is important to qualify this statement with the fact that not all patients undergoing chemotherapy and radiation will be able to avoid loss of organ function because of possible eventual need for surgery. In those who failed, they can take comfort in the fact that at least they have been able to preserve organ function for whatever duration of time while the disease has been kept at bay.


EHB: A recent published study showed that combining targeted therapy with radiotherapy has prolonged the long-term survival of patients (more than five years) by almost 50%. Could you please share with us your insights? Dr Ang: There is no question that the use of targeted agent has helped to enhance the response to treatment. This has been associated with improvement in survival. The addition of targeted agent like Cetuximab has helped to improve responses, improve cure rate and optimise treatment results for patients with head and neck cancer.

A Patient’s Story For Dan Steele, perhaps one of the cruelest things about battling cancer is not just the destruction it wreaks on his body, but also its impact on the life of his loved ones. When he was told that the lump on his neck was actually stage 3 cancer, his first thought was his kids, aged 3 and 6; he resolved to beat the Big C for their sake. He went through several rounds of chemotherapy, and despite all the side-effects his body had to go through, Dan continued to be positive and proactive in his treatment. His wife, Jessica, remained stalwart through it all. She sends newsletters to friends and relatives, updating them of Dan’s progress, giving them a peek into their lives as they deal with Dan’s condition. Here’s a snippet: “These are not the happiest of times but we certainly try to laugh as much as possible. Dan is now doing chemo on Mondays and radiation every day of the week. His throat is hurting and he is already struggling to eat. He can’t taste things well and all the side effects from the previous sessions have not gone away, so it is like more and more stuff to take on. But he has been amazing through all of this. I continue to tell friends that the only possible way to be able to successfully undergo the kind of treatment he is undergoing is to start with a very fit body because his body has taken such a beating with already 13 rounds of chemo and about 15 days of radiation so far and more to come. He is still going to the office but I continue to remind him of what the doctor says about listening to his body when it is asking for rest, and I think he is already quite in tune with it. Our next immediate milestone should be Sep 17 following which the doctors will do another major scan and determine where we go from there.” Dr Ang Peng Tiam graduated from the National University of Singapore (NUS) and completed his Fellowship in Medical Oncology at MD Anderson Cancer Centre, Houston, Texas and Stanford University Medical Centre. Dr Ang was founding Head of the Department of Medical Oncology at Singapore General Hospital from 1991 to 1997. He held the concurrent post of Director of the Oncology Centre and Clinical Associate Professor at the Faculty of Medicine, NUS, and served as a president of the Singapore Society of Oncology. He is a Visiting Consultant at National Cancer Centre and is currently the Vice Chairman of Singapore Cancer Society, Chairman of the Medical Advisory Committee of Breast Cancer Foundation.

What’s the Next Big Thing in LASIK? An Introduction to Advanced Surface Ablation By Dr Tony Ho and Martin Kwan

A Brief Background of LASIK LASIK is currently one of the most commonly performed eye procedure in the world. Over 30 million people worldwide have had LASIK to date and about 700,000 LASIKs are performed annually. But even as LASIK gains wider acceptance as a means to eliminate spectacle and contact lens wear, there is a movement amongst eye surgeons and patients towards Advanced Surface Ablation, in short - ASA. ASA is essentially a more advanced, improved version of Photo-Refractive Keratectomy or PRK, which is the original laser procedure that predates LASIK. Introduced to the public in the early 90s, PRK was the first ever laser procedure to correct a person’s eye power. However, PRK has certain drawbacks. Patients suffer from uncomfortable, teary and red eyes that will last for three days while the cornea skin recovers. Complaints of blurry vision and the slow vision stabilization of between three to six months were also not uncommon. A significant number of patients develop cornea scarring, which at that time had no effective preventive treatment. Hence, PRK never really took off. Then, eye surgeons found that by cutting and lifting up a cornea flap and then using laser to reshape the cornea tissue underneath the flap, patients can expect to have comfortable eyes and clear vision as soon as the next day. Vision was also rapidly stable with LASIK and there was hardly any cornea scarring. LASIK took the world by storm, and became the procedure of choice from the mid 1990s.

Recent Advancements In recent years, eye surgeons have begun to re-look at the original surface-based flap-free PRK procedure, giving it a modern twist. Advanced Surface Ablation (ASA) is the modern term for excimer laser treatment that is performed on the surface of the cornea. Epi-LASIK is a form of Advanced Surface Ablation. Although Epi-LASIK and other ASA procedures are derived from the older PRK, they are radically different in many ways as advancements in medicine and technology has brought about newer techniques in surgery and patient management. 58 ezyhealth&beauty . nov 2010

In Epi-LASIK, surgeons use a device called ‘Epi-keratome’ that employs a blunt blade oscillating eleven thousand times a second to shake loose the cornea skin with greater precision and ease as compared to PRK. In other Advanced Surface Ablations procedures such as Advanced PRK and LASEK, eye surgeons now employ a new technique utilizing ethyl-alcohol to presoften the epithelium cells prior to removal. These less traumatic methods of cornea skin removal allows for faster recovery and reduced risk of cornea scarring. The newer generation ‘flying-spot’ excimer lasers are now used to reshape

the cornea to correct the eye power. These new lasers create a much smoother treated surface, as compared to the old ‘broad beam’ lasers. Treatments are also done faster and with less tissue removed per dioptre of power treated. This in turn leads to faster tissue healing and visual recovery. On top of that, there are sophisticated machines that are able to map out the contours of the cornea, as well as the optical imperfections of the eye prior to surgery. All these measurements, broadly termed as ‘wavefront measurements’ are used to provide enhanced precision in the cornea reshaping process. (continued on pg 60)

Reasons Why Advanced Surface Ablation is the Choice There are many reasons why increasing numbers of eye surgeons and patients are now turning to Epi-LASIK and other forms of Advanced Surface Ablations: Dry Eyes Almost all LASIK patients will complain of dry eyes lasting weeks to months after surgery. They will become quite dependent on artificial tears eye drops to relief their complaint. This is because by cutting the cornea flap, tips of the thousands of nerve endings are severed. These nerve endings send signals to the brain for more tears to be produced by the glands. By not cutting the cornea nerve tips, Advanced Surface Ablation procedures including Epi-LASIK do not cause or worsen dry eyes. Cornea Flap Complications The creation of the cornea tissue flap during LASIK is also fraught with risks. The fact is flap-related complications remain the most common cause of serious LASIK complications. A host of flap-related complications can occur during surgery such as a flap that is completely cut off, partially cut flap, de-centered flaps and flaps with a hole in them. After surgery, the cornea flap may not adhere itself well and the flap can be displaced by rubbing of the eyes or an accidental finger poke. Infection and inflammation of the flap can also happen, causing cornea scarring. ASA procedures including Epi-LASIK simply avoid all these cornea cut flap complications totally. Cornea Thinning Over the years, surgeons have been trying to dodge a potentially devastating cornea thinning complication called ‘Keractectasia’. This condition arises when the cornea end up so thin or weak after surgery that it begins to protrude and warp causing poor and distorted vision. The fact is that both Advanced Surface Ablation and LASIK weaken the cornea, as there is a need to reshape and thin the cornea for the correction of the eye power. But LASIK is the worse culprit as cutting a cornea flap will weaken the cornea structure more. Therefore, ASA procedures including Epi-LASIK reduce the risk of Keractectasia.

Hope for “Unsuitable LASIK” candidates A significant number of people who wish to have LASIK get rejected because of inborn thin corneas or high myopia and astigmatism. As Epi-LASIK and other ASA procedures do not thin out the cornea as much, these people may now have a renewed chance of having their vision corrected. Finally, patients who participate in contact sports such as soccer, rugby, martial arts and diving, as well as military personnel who are in vocations such as navy divers, commandos and airforce pilots can now have their vision corrected with Advanced Surface Ablation without worry of cornea flap-related complications. This is because unlike LASIK, there is no risk of cornea flap dislocating in EpiLASIK and other ASA procedures. In summary, modern Advanced Surface Ablation procedures including Epi-LASIK are safe, reliable, causes less dry eyes and cornea thinning. The comfort and downtimes are also comparable to LASIK. More patients are beginning to choose Epi-LASIK and other ASA procedures when they are provided with greater knowledge of the benefits of these ‘no cornea cut’, ‘no cornea flap’, purely ‘surface based’ laser vision corrective procedures.

Case 1

M

s L saw me last year; she was only 42 years old. She had been suffering from rectal bleeding for a week that was increasingly getting worse. She also complained of pain and swelling around the anus. She had seen her GP, who subsequently referred her to me for painful piles. During the consultation, I discovered that she had been having difficulty with her bowels for the preceding 2 months and she just put it down to simple constipation and ignored her symptoms. She finally decided to seek medical attention only when the pain from her “piles” became worse. When I examined her, I found that not only did Ms L have piles; she also had a low rectal cancer. This was confirmed on colonoscopy with biopsy of the tumour. She was very shocked when she learnt about her diagnosis and wished that she had seen a doctor sooner for her symptoms. The main reason for her resistance was because she was too embarrassed to discuss about her bowels and thought that her “constipation” and “piles” were not that serious.

Case 2

M

rs C, 51 year old, came for her colonoscopy just after the Colorectal Cancer Awareness Month this year. Her bowel habit was normal and she had no symptoms at all, she recently underwent general health screening and even had a negative stool test. She had read about colorectal cancer and polyps, and wanted to be sure that she

had no polyps. She was advised that colonoscopy is the best way to detect polyps and polyps could be removed at the same time during colonoscopy if they are present. Her colonoscopy showed six polyps in her colon and one of them was large (1cm). Fortunately, we were able to remove them at colonoscopy, and histology has confirmed that they

have not turned malignant. Hence, with screening colonoscopy, Mrs C has prevented her polyps from developing into colorectal cancer. She will require follow up surveillance colonoscopy to make sure she does not develop further polyps in the future.

The Facts Colorectal cancer is the most prevalent cancer in Singapore and the incidence is increasing with about 1500 new patients diagnosed each year. It is more common among the Chinese,with similar incidences in women and men, except for rectal cancer which is more common in men. Colorectal cancer is the 2nd most common cancer in women in Singapore after breast cancer. No one knows the exact cause for colorectal cancer. We know that family history plays a role but only 15% of cases have a strong genetic basis; the majority of patients >75% have no family history of colorectal cancer. There is some evidence that tobacco smoking, alcohol, too much animal fat and lack of fibre in diet, lack of exercise and obesity can increase the risk for developing cancer. The symptoms from colorectal cancer may be similar to the common benign conditions but women, particularly, feel too embarrassed or are afraid to seek help and advice for their symptoms. The symptoms for colorectal cancer may include: • Blood in the stools • Change in bowel habits – constipation or diarrhoea • Unexplained anaemia • Unexplained abdominal pain • Abdominal mass • Unexplained weight loss • Persons aged 50 and above are at increased risk. However, colorectal cancer may have no symptoms especially in the early stages; symptoms and pain may only develop when the cancer has developed into a late stage. Some of these cancers are slow-growing but some are very aggressive and spread quickly. Generally, younger patients tend to have more aggressive forms.

The good news about Colorectal Cancer is that it can be cured if diagnosed early. The 5-year survival rates differ, depending on the stage of cancer upon presentation:

In 1973, only 1 in 3 patients with early stage colorectal cancer survived after 5 years. Today, with improvements in surgery, adjuvant radiotherapy and chemotherapy, the survival rate has more than doubled. However, >60% of our colorectal cancers is still picked up too late, often already at more advanced stages, thus leading to poor survival rates.

The best news about colorectal cancer is that it is potentially preventable. This is because colorectal cancer usually takes time to develop and it usually starts from a polyp stage before developing into a cancer. Polyps are benign lumps on the inner wall of the colon and rectum. They can look like a small grape attached to the colon by a stalk. Some types of polyps (called adenomatous polyps) increases in size and with over time, they will transform into cancer. If such polyps are detected, they should be removed to prevent the development of cancer. However, polyps rarely produce any symptoms unless they become very large and most polyps are found only detected screening. Sadly, screening rates are still quite low. Among those aged 50 and 70, only 1 in 9 men and 1 in 13 women have undergone a colonoscopy at least once. Even for the less invasive stool test, only 1 in 4 has been screened. The best known way to prevent colorectal cancer at this point in


time is screening. The least invasive test is a stool test – the faecal occult blood test (FOBT) but this is a cancer detection test not a cancer prevention test. The gold standard test for colorectal cancer screening is colonoscopy. Screening should begin around age 45 to detect polyps before the significant increase in risk of colorectal cancer at age 50.

ost make jokes about them. Many shy away from talking about it. In some parts of our region, talking about it is almost akin to committing a crime. I’m talking about the topic of sex, in particular suboptimal erections and sexual satisfaction. What in the world are suboptimal erections and why should we care about them? Before we talk about why it’s a condition men should pay attention to, let’s first understand exactly what it is.

What are suboptimal erections? It is common knowledge that the penis erects in response to sexual stimulus for the purpose of sexual intercourse. In the process of erection, the penis gets larger and hardens. A suboptimal erection describes a penis that is hard enough for penetration, but not completely hard. It is measured against the Erection Hardness Scale (EHS), and suboptimal erections fall under EHS grade 3.

Grades of EHS (Erection Hardness Scale):

Grade 1: Severe erectile dysfunction. The penis is larger but not hard. Grade 2: Moderate erectile dysfunction. The penis is hard but not hard enough for penetration. Grade 3: Suboptimal erection. The penis is hard enough for penetration but it is not completely hard. Grade 4: Optimal erection. The penis is completely hard and rigid. As a rough guide, EHS4 is the hardness of a cucumber and EHS3 that of a banana. As can be seen from the Erection Hardness Scale, suboptimal erections are not the same as erectile dysfunction.

More common that you think Nonetheless, as the recent Asia Pacific Sexual Health and Overall Wellness (APSHOW) survey commissioned by Pfizer shows, suboptimal erections are no laughing matter. Not only are they more common than most would think, they also affect a couple’s satisfaction with sex and thus their overall wellbeing and happiness with life. According to the APSHOW survey, one in four men in Asia Pacific and almost one out of two Singaporean men suffer from suboptimal erections during sexual activity. In addition, lessthan-optimal erection hardness was found to be equally common in men below the age of 40 and older men, debunking the myth that these problems afflict only men of a certain age. It was also found that a quarter of women in Asia Pacific believe that their partners’ erections are suboptimal during sex and an overwhelming 88% of these women are less than completely satisfied with their partners’ erection hardness. In Singapore, the situation is only slightly better, with 62% saying that they are not satisfied with their partners’ erection hardness. In addition, women with partners who have suboptimal erections also feel less attracted to them, with only 25% of Singaporean women surveyed stating that they were satisfied with every aspect of their relationship. Consequently, the findings of the survey also showed that women whose partners experience suboptimal erections tend to have sex less often, and are less satisfied with their sex life, relationship and overall outlook on life. And that’s not all. The results of the study also revealed that women in the Asia Pacific are worried about erection hardness issues for a variety of reasons, with about half of those surveyed saying they are hesitant to try again because of the lack of erection hardness and almost half worrying that it’s their fault. 76 ezyhealth&beauty . nov 2010

Clearly, suboptimal erections have a significant impact on not just an individual’s happiness and wellbeing, but his partner’s too. So what can men and women who are faced with this condition do to improve it?

Causes Before we find out how suboptimal erections can be treated or managed, we should first understand how men could develop the condition. Either physical or physiological factors or both can cause suboptimal erections. Most men develop the condition as a result of both. Like many health conditions, lifestyle is a huge factor. The leading causes of Singaporean men getting suboptimal erections are typically stress, high blood pressure, diabetes, doing very little or no exercise and obesity, systemic heart disease and prostrate problems. These causes are also commonly present in men with erectile dysfunction. Men with metabolic syndrome, which is a combination of factors including being overweight or obese, diabetes, hypertension and high cholesterol, are more likely to have suboptimal erections.

Treatment options In view of this, it makes sense for a sufferer of suboptimal erections to make some lifestyle modifications. Women can also help in this regard, by encouraging their partners to stop smoking, limit alcohol consumption, exercise regularly, maintain a healthy diet, get enough sleep and manage their stress levels.

These are all steps to maintaining a healthy lifestyle, and may go a long way in improving erection hardness levels. Men who believe they are suffering from suboptimal erections should give the abovementioned lifestyle changes a try for three to six months, but if there is no or limited improvement, it may be time to consider seeking help from a doctor. In terms of medical treatment, less-than-optimal erections, which include suboptimal erections, can be managed through the use of medication known as PDE5-inhibitors, such as Viagra. Such medication will improve erection hardness from EHS3 or a suboptimal erection, where the penis is hard enough for penetration but not completely hard, to EHS4 or an optimal erection, where the penis is completely hard and fully rigid. Be it lifestyle modifications or medical treatment, the best thing that sufferers of suboptimal erections and their partners can do is to speak up, communicate with each other and take steps to resolve the problem. In Asian society, men who are afflicted typically feel too embarrassed to talk about their condition. However, being silent will have repercussions that affect not just their self-confidence, wellbeing and satisfaction with life, but their partners’ as well.
