ow many times have we stopped midway through an endeavour and asked ourselves, “Why am I even doing this?” I have. Many times over. Take for example, the time when my enthusiasm got the better of me and I attempted a standard distance triathlon for the first time. The flag-

Why off timing at 11am ensured that the hardest part of the event, the 10km run, would be done under the withering glare of the noonday sun. 1.5km of swimming, 40km of cycling and 5km of running later, the question started to carry a weight of it’s own on my exhausted physique. It is times like these that draws us back to the fundamental question of what motivates us. Earlier idealistic motivating factors had long since fled along with my energy and sadly, I found myself relying on that big, well-deserved meal to drag my unresponsive mass across the finish line. Our cover personality, Felicia Chin, shares with us how the question often pops into her mind as well as she pursues her degree after spending the last 6 years in showbiz (pg.14). Come 2011, she has set her sights on completing her maiden triathlon and needless to say, she may have an opportunity to discover more in-depth answers to the question at that point too. Yes, the fact is that we need motivation for virtually every aspect of our lives; whether it be for work, family, chores, hobbies, or exercise.

To find that spark each day to complete your tasks, speak to certain individuals, rekindle your romance or simply to will your slumbering body to get off the bed, becomes a great hurdle if the motivation, enthusiasm and the reason for doing it has been lost. When it comes to taking care of our health and bodies, it is no different. This issue, we look at the largest organ in our body, our skin, and how we can add knowledge to our motivation and in so doing, arrive at the best complexion possible (pg.20). Age, sunlight, stress and a variety of other factors gang up on our skin in an effort to rob us of youth, beauty and vibrancy. And whilst the allure of youth is a good motivating factor for us to go for that next facial treatment, we should also take a moment to consider again, what really spurs us on each new day. The American poet, Samuel Ullman, who is best remembered for his poem Youth, once said, “Years may wrinkle the skin, but to give up enthusiasm wrinkles the soul.”

I try to eat one serving of fruit or vegetable at every meal. I would add a banana to my cereal at breakfast or bring along a fresh apple for my lunch snack. Trail mix cereals with dried fruits and nuts is now my favorite high fiber snack. Aside from that, I always include legumes in my soups and salads. I also eat lots of vegetables, especially broccoli during dinner. Sometimes I make avocado salads as well. When I’m in a rush, I grab a cup of plain yogurt and mix it with whatever fruits I have in the fridge. Lina Jupri I used to suffer from bouts of constipation and have been advised by my family doctor to consume foods rich in fiber content. Therefore, I make it a point to have a glass of freshly-made orange juice at breakfast every morning. For lunch and dinner, I opt for more vegetables and fruit instead of meat. Uri K Ever since my mother fell ill, my siblings and I have made it a point to eat healthy, live healthy. Apart from exercise, we have started replacing our usual unhealthy snacks of salted chips with fruits. Lately, we bought a blender and started making our all natural fruit-and-vegetable juices. Event Highlights (continued from pg 6) Sunrise over the Singapore Skyline Singapore Bay Run 2010 (Army Half Marathon) Sunday, 12 September 2010 Singapore’s skyline has been rapidly evolving over the past few years with the addition of the iconic Singapore Flyer, Marina Bay Sands, and the all-new Double Helix Bridge. With this array of new icons to reshape the skyline, the Singapore Bay Run 2010 became a brand-new run for everyone. The extremely well-timed flag-off ensured that the bulk of the participants were well-positioned at the crest of Benjamin Sheares Bridge as beams of purple sunlight began filling the dark sky. Almost like a planned welcome for the brand new route, the sunrise gave a beautiful array of colours that matched cacophony of city lights that were blazing the night sky moments earlier.

As this run is also co-named as the Army Half Marathon, the route was lined with various units and divisions that pushed the boundaries of creativity with their cheerleading antics and costumes. The atmosphere of festivities and fun were bolstered by energetic and colourfully dressed belly-dancers who were strategically positioned after the 15km mark to motivate weary runners to the finish line with song and dance. At the rate that Singapore’s city skyline continues to evolve, this is a run not to be missed in your calendar of sports events!

4-in-1 The SAFRA Quadthlon 2010 Sunday, 12 September 2010 The rainy skies failed to dampen the crowd of participants who gathered on the morning of 10 October for the annual SAFRA Quadthlon. Held at East Coast Park for the 3rd year in a row, this event is the only event on Singapore’s sports calendar to feature four sporting disciplines – swimming, inline skating, cycling and running. Unlike regular triathlons – which feature swimming, cycling and running – the SAFRA Quadthlon incorporates a 12km skating route as the second leg of the event. Local skating enthusiasts who participate in triathlon have always looked forward to this event to “stretch” their skating legs and compete with other enthusiasts. Race organizers, SAFRA, flagged off the race exactly on

time as the weather began to clear up just half and hour before the flag off. After a 500m swim in the open sea, competitors raced to start their 12km skate which consisted of three 4km laps. Thereafter, a 20km cycling route along the East Coast Park service road signaled an end to leg 3 of the race. The final 6km run was routed into 3 loops of 2km each, allowing competitors to keep their motivation and morale high as they ran alongside each other. Singapore Keeps Fit! Singapore Wellness ’10 at Jurong Point In a bid to promote healthy living, Singapore Wellness ’10 was organized and held at Jurong Point Shopping Centre from 22 to 24 October. Gracing the event was Guest-of-Honour Mdm Ho Geok Choo, Member of Parliament for West Coast and Adviser to Boon Lay Grassroots Organisations. Following a keynote speech, a 10th year Yearbook by the National Healthcare Group was presented to Mdm Ho. The 3-day event encompassed pap smear tests at subsidised rates, a healthy food session by Chef Eric Low from the Singapore Chef Association, a yoga and fitness demonstration, educational talks by doctors and health screenings. Ambassador of Singapore Wellness Koh Chieng Mun and the Soroptimist International of Singapore (Garden City) were invited on-stage for a photo-taking session. To top the event off, a group of nine talented lady drummers in colourful sequined costumes showcased their drumming talents with enthusiastic drumming performances. An AVA mobile bus exhibits displaying how AVA protects & ensures safety of food and health of people. Participants were also invited to have their BMI measured and given goodie bags during the event. These sports will definitely help as she prepares to fulfill her new year’s resolution in 2011. “My new year resolution is to take part in a Triathlon next year!” she declares. “I did my first mini triathlon a year ago at the OSIM triathlon. I felt very good and it was a very great experience, I think everyone should try it! You really feel a sense of accomplishment! Surprisingly I still had a bit of energy after the triathlon. It was a very good introduction.” This year, she intends to try a slightly longer distance which we are convinced, will make full use of her blood type potential.

he desire to remain youthful is human nature. With the rising affluence in our society, there is more attention paid to the way one looks. An increasing number of men and women are embracing aesthetic treatments in an attempt to enhance their appearances. These treatments have become widely available and accessible in recent years. There are constantly new and supposedly better treatment options and machines. The main treatments available are: Chemical peel Botulinum toxin Fillers Intense pulsed light Pigment laser Titan Laser hair removal

Chemical peels slough off the superficial layer of dead skin revealing smoother, younger and more radiant skin. It helps to prevent clogged pores and stimulate skin cell growth. There are also moisturizing effects. Many different types of chemical peels are available and they can be broadly divided into superficial, medium and deep peels. Superficial peels are most frequently used on Asians as deeper peels are likely to cause post-inflammatory hyperpigmentation. Chemical peels are used in the treatment of acne, scars, fine wrinkles, uneven pigmentation and for facial rejuvenation. It is a procedure with practically no down time. Some patients may experience redness and skin peeling.


Botulinum toxin is a protein produced by a bacterium called Clostridium botulinum. It blocks the nerve receptor of the muscle which in turn, blocks the signal from the nerve that causes muscle contraction. It is a simple non-surgical procedure that weakens and relaxes the muscles that cause wrinkles. It is mainly used to treat dynamic wrinkles like the glabellar frown lines, forehead horizontal lines, crow’s feet, and bunny lines. The effect usually lasts for about three to six months. It is increasingly used for facial contouring like reducing square jaws that result from hypertrophy of the masseter muscles. Botulinum toxin can also be used to treat excessive sweating of the palms and armpits. The side effects are minimal if the procedure is done properly and they are reversible.

Pigment laser (Q switched Nd YAG) is an effective means of treating pigmentation of the skin. It targets the melanosome which contains melanin. It can range from one to several sessions. It can also be used to remove tattoos. The response of tattoo removal depends on the type of ink used, the duration of the tattoo and how it was applied. Titan is a device that is used for skin tightening. It utilizes infrared to heat the dermis, stimulating new collagen formation. This leads to tightening of the skin, giving rise to a more youthful complexion with less visible lines and pores. It is a relatively safe procedure and patients of all skin types are suitable. Areas that can be treated include the jawline, chin, abdomen and upper arms. Some redness and mild swelling of the skin may be experienced immediately after treatment but this usually subsides after a few hours.

Soft tissue fillers are substances that are injected beneath the surface of the skin to fill facial lines and creases. There are two main types of fillers – temporary fillers and semi-permanent fillers. Temporary fillers like hyaluronic acid lasts for about six months while semi-permanent fillers like poly-L-lactic acid can last up to two years. They can be used to improve creases and furrows of ageing, deep facial wrinkles, sunken cheeks, and some depressed scars. For most fillers, the effects can be seen immediately. The treated area may be a little red and swollen afterwards, but they usually wear off after a few days.

Many people are bothered by ‘problems’ that may be insignificant to others. For instance, a patient suffering from acne problems may be bothered by a small mole on her cheek rather than the acne. How much is 1 unit of Botox®? This question is frequently asked to compare the prices of Botox treatment at different clinics. Botox comes in 100 units per bottle. In its dry powder form, it must be reconstituted with saline water before it can be used. The amount of saline water used will determine the number of units per 0.1ml administered. Unless you buy the whole new bottle of Botox and reconstitute it yourself, there is no way to know how many units there are in a given syringe. As such, there is no way to verify the actual amount of Botox you are paying for. In this case, you will have to trust the clinic to inform you of the exact number of units of Botox used. A clinic could easily overstate the amount of Botox used to make the cost“per unit of Botox” cheaper than it should be and the patient would be none the wiser. 100 units of Botox used do not necessarily mean 100 units gets administered to where it is supposed to. A patient who pays for “per unit of Botox” pays for all the Botox that has leaked from the syringe or left in the syringe. They also pay for inefficient injection techniques where some of the Botox is put outside the intended muscle. An alternative to charging per unit of Botox would be to charge per area treated. Critics, however claim that each individual needs different amounts of Botox so there should not be a flat rate for an injected area. Proponents of the “charge per area” system get around this criticism by assessing the condition of the patients first before deciding whether the price would deviate from the standard charge. The “charge per area” system can be further tweaked to “charge per result”. Patients are assessed and told the results to expect and the price of the treatment. year and a half ago, Mary Kay Cosmetics quietly stepped into Singapore to make a name for itself in the cosmetics industry here. However, you will not find their product on any retail shelf anywhere in Singapore, nor will you see sales girls dressed in Mary Kay uniforms approaching you as you walk into the cosmetics department. This unique cosmetics company, which originated in the USA, defines itself by only using direct selling as it’s method of distribution. Founded by the late Mary Kay Ash in the 1960s, Mary Kay Cosmetics has grown to be a major player in the cosmetics industry in 40 countries. She advocated “praising people to success” and her slogan “God first, family second, career third” expressed her insistence that the women in her company keep their lives in balance.

We speak to the President of Mary Kay Asia Pacific, Mr KK Chua, to understand more about his quiet cosmetics giant. Mr KK Chua, a Singapore citizen himself, was in town recently to speak at the Global Enterprise Summit and he is very happy with the progress Mary Kay has made in Singapore. “I don’t want to be the biggest,” he tells me, “I want to be the best! My strategy is to build a strong foundation and train our people well, and I’m sure that the business will grow on its own from there. We’ve been here in Singapore for about a year and a half and have about 1000 beauty consultants. We go for a steady growth and not an explosive growth.” Mr Chua’s bold statements are not without a substantial track record; with similar strategies, Mr Chua led the Mary Kay team that went into China many years ago. Today China is the biggest market for Mary Kay products in Asia and more than 500,000 women act as beauty consultants

Another 200,000 beauty consultants is equivalent to having 200,000 more shelves, think about that. That is a very powerful medium of distribution!”

The evolution of cosmetics When asked regarding the future progression of cosmetics, he replied, “I think skin care is basically skin care – the basic steps of washing your face, applying moisturiser, using a toner or a mask, will not change. What has changed is the technology behind the products that are available today. The ingredients are also different today, cosmetics are moving away from the chemical base and more into natural, herbal and mineral ingredients. “Moving ahead, I believe nano-technology will make greater impact on the cosmetic world and the other big thing that is coming up is DNA and stem-cells. Both are very interesting concepts to replicate cells and reproduce new cells. What Mary Kay is currently very involved in is the usage of botanical extracts in our products. The Chinese has 3000 years of experience in herbs and there is a lot more to be discovered and learnt from them.” Despite traveling almost 280 days a year all around the world, Mr Chua remains focused on his plans for Singapore. “Singapore is a very sophisticated market,” he tells me. “Consumers here require a lot of information. It is no longer a matter of feeling but more importantly, they want to understand the reason why a product works. That is why we take a lot of effort to train our beauty consultants even better.”

How It Works and How You Win By Peter Lim

Root causes Our research into recovery from cancer has met with many challenges, discoveries and disappointments. It requires a discerning and keen mind to understand cancers that have plagued many people who long for quality of life and health. Many cases that come to us are already in the final stages, with weakened immune system to have a winning fight. In good conscience, no one should regard cancer care as exclusive, but collectively we work to discover and achieve a strategic and holistic answer to benefit mankind. How Cancer Works Cancer is the uncontrolled growth of abnormal cells in our body. It is primarily due to emotional instability, nutritional deficiency, environmental toxicity and immobility over a period of time. When immune system is suppressed and compromised, the body gradually fails to respond effectively to pathogenic invasions, leading to toxicity, diseases, tumours and cancer. Most diseases begin with uncontrolled emotional stress, anxiety and insufficient quality of rest and sleep. These lead to weakened cells, organs and immune system that are susceptible to pathogenic attacks. Nutritional deficiency from poor and unhealthy diet and balance leads to under nourished cells, premature aging and death. Toxicity from environment, water, food, air, stress, chemicals, poor metabolism, bowel irregularities, chemical treatments and lack of water consumed, results in toxic built up and organ failures to cause body energy imbalance. Immobility due to the lack of exercise and failure to clear toxins and from lymphatic system, lead to toxins trapped in the cells. How Therapies Have Limitations The conventional ways are usually surgery,

chemotherapy and radiotherapy with a quick but temporary fix that usually do not address the root causes. Most cases experience cancer relapse with much gravity in a weakened body. To illustrate this, I have recent cases that could be an eye opener. A lady had endometriosis in her early adult life. She was operated five times over a period of five years. These weakened her uterus. Ten years later, uterus cancer was operated and spread. Within two years, she had chemo and radiotherapy, removing some lung tumours which further spread to the lungs, liver and the limb. Had she known the root cause of pathogenic attack, she could have been spared. A man had his right breast and arm pit-lymph nodes removed in 2001 with a stage-zero tumour. These affected his bladder meridian point. Bladder cancer was operated seven years later. Chemo treatment was over $200K within two years, and for life. It now spreads to the lungs, liver and bones, with blood disorders. A lady in her late sixties found she had 4th stage lung cancer and went through chemo. She went to Mexico for natural treatment and later found her cancer spread to the bones, lungs, liver and colon. Her energy level was only 55 per cent when she came. But after a month, it went up to 85 similar to healthy people. It was same for the second month and her weight was maintained. Organic food, juicing, coffee enema, water, herbs, supplements, and energy/ exercise therapies have limitations too. Due to much toxicity, most natural therapies take a longer time to heal than the body allows. How To Win Cancer requires a timely, strategic and holistic approach. It demands knowledge,

vision, discipline and fighting will to address cancer challenges to win it. Cancer is said to take twelve years to mature with the first seven years too minute to be seen by machines. From the eighth year, it becomes clearer and shows up at different times and places. Surgery alone may not be a complete answer. The root causes must be found before any treatment to prevent a repeat. They will consistently have some sort of blocked

reactions. I also administer antibiotics to clear the infection and bacteria. However, these treatments cannot restore the normal functions of the sinuses and the production of normal mucus. It is therefore important for me to help the recovery along by enabling the mucus in the sinuses to return to its normal consistency. In order to do that, I use Flumicil,” said Dr Leong. “Flumicil, is used as a mucolytic agent to reduce the thickness of mucous secretions and return it back to the normal salivaconsistency.” With these treatments, Jennie no longer suffers from restless sleep and drowsy days. What are Sinuses for? Our sinuses are small spaces of air within our skull. We have a pair in the forehead, a pair in the cheek area, and many other small ones between the eyes. These are not “dead” spaces of air; one of their key functions is to continuously produce mucus. The consistency of “healthy” mucus is similar to saliva and in a healthy person, mucus flows smoothly to the back to the nose and it swallowed. However, in a sick person, mucus is overproduced, causing it to overflow to the front and resulting in symptoms of a runny nose. Besides the overproduction, the consistency of the mucus becomes thick and yellow or green. “The difficult part is that these

nose sensation. This congestion can affect people over time by disrupting sleep and making them feel very tired during the day. Children will have difficulty concentrating at school or have poor memory. Some kids have even been misdiagnosed as having ADHD, but actually they are just very tired kids which makes them ‘cranky’ and hard to control.”

Recognising the pattern “The key is recognizing the pattern,” says Dr Leong. Thereafter, steps can be taken to treat the chronic sinusitis. For Jennie, her path to recovery began with a thorough history of her medical background. Thereafter, an endoscopy will be done to see if there were any signs of infection, swelling, pus or abnormal mucus.

Triglycerides No more fats please! By Ng Yun, Patient Care Pharmacist

T

riglycerides (TG) are the chemical form in which most fats exist in food and in our bodies. They are derived from fats eaten in food or made in the body from other energy sources such as carbohydrates. The calories which are not used from a meal are converted to TG and transported to the fat tissues for storage. High TG may contribute to the blockage of the blood vessels (atherosclerosis) and this increases the risk of stroke, heart attack and other heart diseases. High TG may also be associated with the presence of other conditions such as obesity, high blood pressure or diabetes, and these conditions elevate the risk of heart diseases.

Understand the values TG values are expressed as either milligrams per decilitre (mg/dL) or millimoles per litre (mmol/L). Below is the interpretation of the readings (according to American Cholesterol Education Program Expert Panel’s guideline on fasting blood TG): • Normal: Less than 150 mg/dL or less than 1.70 mmol/L • Borderline high: 150-199 mg/dL (1.70-2.25 mmol/L) • High: 200-499 mg/dL (2.26-5.63 mmol/L) Testing for TG is usually done with a cholesterol test, also known as a lipid profile test. Total cholesterol (TC), lowdensity lipoprotein cholesterol (LDL-C) and

high-density cholesterol (HDL-C) are the other three parameters which are obtained from the test. There are “good” and “bad” cholesterols: HDL-C is the “good” cholesterol while LDL-C is the “bad” one. High HDL-C value with low LDL-C and TC values are desirable.

Lifestyle modifications Increased TG is likely due to a sedentary lifestyle and diet that is rich in calories or fats. This translates to more calories input and less calories output which results in an accumulation of calories in our bodies. Hence, the most effective method of TG reduction is by starting lifestyle modifications. This includes eating well, such as eating less fatty and oily foods, and exercising more regularly (about 30 minutes for five times weekly). If a person is a smoker, quitting smoking helps plunge the risk of heart diseases.

Medication and supplements to lower TG When lifestyle modifications are not adequate to lower TG, your doctor may recommend medication to further decrease your TG. These medication may include: Fibrates. The fibrates decrease TG by increasing elimination of TG. They may cause mild stomach irritation and in rare cases, liver problem. Statins. The statins mainly inhibit the production of cholesterol. It reduces triglyceride to a lesser extent. These

medicines may cause muscle aches and in rare cases, liver problem. Niacin. Niacin increases the elimination of TG, decreases the production of TG and increases HDL-C. It may cause gastric irritation and flushing. In addition to medication, your doctor may recommend you to take the following supplements to lower your TG: Omega-3. The benefits of omega-3, which is found in fish oil (animal source) or flaxseed oil (plant source), are numerous and they include lowering of TG, improvement of heart function and reduction of dry eye symptoms. Red yeast rice extract. Red yeast rice contains the naturally occurring form of lovastatin, a type of statin as discussed earlier, and the extract may be effective in lowering cholesterol values. Nonetheless, because it is a statin, do check with a pharmacist if it will be appropriate for you if you have any concurrent medical condition or medication. High levels of triglycerides may lead to fatal heart diseases but it can be managed through diet, exercise, medication and supplement. Regular screening with a lipid profile test is recommended for early detection of high TG levels. You can take care of your health!

ursing is a career that is extremely noble in purpose yet not without difficulty in execution. Changing blood soaked bandages, helping bed-bound patients to ease themselves and cleaning up after sick individuals might not fit the job description of many ambitious young people today. Yet these tasks are just the tip of the iceberg when considering the daily tasks of nurses and caregivers. No one, except the patient receiving care from the gentle nurse, can fully appreciate the depth of self-sacrificing help and patience that a nurse needs to have.

Establishing homebased medical care Foreseeing the burden that an ageing population would potentially have on the medical infrastructure of Singapore, one of our founding fathers, Dr Toh Chin Chye, established the initiative of home nursing care back in 1976. Christened the Home Nursing Foundation (HNF), the voluntary welfare organization began operations more than 30 years ago in their headquarters at 26 Dunearn Rd. Today, they are located at Toa Payoh and the team of 30 nurses has

made more than 1.1 million home visits since its conception in 1976. HNF’s team of nurses spend their days making an average of 7 house calls to people suffering from a variety of ailments including those who are bed-bound, suffering from chronic ailments, the elderly and frail, and those who require specific skilled nursing care and procedures. Without the benefit of dedicated transport, these nurses move from home to home via public transport and toting a huge kit bag of bandages, syringes and other nursing paraphernalia for the benefit of the patients.

House calls on foot I spoke to Nurse Chua Soo Kheng, 54, who covers the Woodlands region by herself. “The weather makes no difference to me,” she said, “I will still make it to our patient’s house to see them.” However, the perspiration that trickled off her face on that scorching day told me just how much a difference it actually made for her. The efforts made by these nurses may go largely unnoticed by the general public, but to the patients, they are angels in a purple uniform, sent to provide them some relief. “Good!” exclaimed Mr Amri Bin Mohamed Samat, a patient of HNF, when asked how he felt about the nursing care that Nurse Chua provided. Without Nurse Chua, Mr Amri would possibly be back in the hospital as severe bed sores continually plague him after his mid-body amputation many years back that left him without a body from the waist downwards. Today, he moves around on a motorized wheelchair and is exceptionally grateful for the help that HNF gives him to keep him feeling clean and

healthy despite his condition. Unable to make a living after his serious motorcycle accident, Mr Amri is an example of a patient who is fully subsidized by HNF.

Subsidized and Affordable “25% of HNF’s home visits are totally free-of-charge,” says Mr Lawrence Chen, Senior Manager from HNF. “Our Social Work Assistants visit patients to conduct a MEANS test to assess their ability to pay for home nursing care, this test will determine the amounts of subsidy they receive.” Currently, more than 70% of HNF’s visits are subsidized at different levels from 100% to 25% of the total home visit cost. As the burden of an ageing population becomes increasingly heavy on our caregivers, the role of home-based care will increase in demand and necessity. With the amount of subsidies that HNF continues to give to needy patients, HNF continues to rely on donations from the public to support its cause. Their work and mission is not just noble and heroic, it is hours of sweat and toil to see that those that need help the most get it. hat were the last 3 movements you made before you read this question? Think about it. The most recent action was likely when you flipped this page. How about before that? You were probably sitting there on the couch, bored to the point of insanity and then – ah ha! – Ezyhealth & Beauty happened to be lying within arms reach. You commandeered yourself in that zenlike state to reach over and pick it up and guess what? Now it is making you think! Well, at least we’d like to think so. The not-so-obvious point of the introduction was simply to say that we have a habit of taking our movements for granted. We walk, run, jump, fall, curse, kneel, sit and stand (oftentimes in that order) without even a second thought. Unfortunately, the first thing that turns our attention back to our movement is usually an injury. Pain, that lovely feeling that reminds us we are still mortal, is a great way to force us to take a closer look at our movement. You’ve felt it: the sharp pain in the knee, that ache in the lower back or that pull just behind the shoulder blades. Mostly a result of a pull or stress injury due to an action that was ill-timed, over-

enthusiastic or totally unnecessary (the couch potato in me somehow finds that it usually happens to be the latter).

To run or not to run? Statistically, the most common reason for movement related injury is still running. Dr Tan Swee Kheng, the founder of Fifth Ray (fifthray.com), hopes to change that perception completely, “I don’t think running is the cause of these injuries. I think its because people don’t run correctly.” Poor posture, totally erroneous running gait and too much too soon would accurately sum up the three main reasons why many people retire from running before they’ve even started. As a firm believer in the benefits of proper movement, Dr Tan helps runners who come from all walks of life to achieve their fitness goals in the most painless and scientifically sound way possible. Schooled in the minimallypublicized field of Kinesiology, Dr Tan holds the distinction of being one of the only “Movement Specialists” in Singapore. Kinesiology is, afterall, the science behind the movement of the body. I asked Dr Tan how she determined if someone was ‘running properly’, she replied, “I go by the philosophy of mid-

foot running. Essentially, you land on the ball of your feet and the knees are bent when you land to dissipate the impact. You use your muscles to cushion the impact of the landing. Although you will take smaller steps, your cadence will be a little bit faster and this helps you with your speed as well. I am also looking at their posture to ensure that their shoulders are square and their back is straight.”

Practicing what she preaches Not content simply to help others, Dr Tan practices what she preaches in various endurance sports events. Having completed a full Ironman triathlon (3.8km swim, 180km cycle, 42km run) and the Ultramarathon (84km) just last year, there is not much that can faze this feisty doctor when it comes to endurance. Dr Tan also spent much of her young adulthood as a member of the Singapore national squad for women’s hockey and competed in four SEA Games. When Dr Tan first started Fifth Ray, her clientele consisted mostly of women. Today, she teaches larger groups that are comprised of both genders. But the majority of her runners today are still women who hope to get themselves

physically fit, lose some weight, and achieve some fitness targets for themselves. “There are a lot of ladies who are training with me and are consistently running full or half marathons today,” says Dr Tan. “But when they first started with me, some of them could not even run 400 meters! That means they could not even go around the track once. I had to train them one lap at a time and increase the distance very gradually.” Since bees will always be attracted by flowers, I asked Dr Tan about her decision to open the group up to include men as well. “The men are faster and more competitive so they help to drive the women as well!” she answered. The group runs twice a week together at Clementi Stadium and their training routines consists of speed and interval training, pacing, and circuit training. They meet on weekends to do a 15-20km run at East Coast Park, NUS, or the Botanical Gardens. December is the month for the Singapore Marathon and needless to say, many of the 50,000 runners will be experiencing a certain degree of pain after the epic race. “Not running properly or running for a long time in the wrong posture can cause injuries,” advised Dr Tan. “The other reason might be insufficient preparation for the race. If you are thinking about doing marathon, my advice is to start early! At least 6-8 months is necessary to condition yourself, particularly if you don’t have a good base to begin with.” “It’s a long distance,” she says matter-

of-factly, “you got to give the distance the due respect!”

Moving it! Besides her running group, Dr Tan is also very involved in training children from a young age in the areas of movement and control. Fifth Ray has a large variety of courses and programmes for children and it takes up the bulk of her non-exercising moments. A regular guest speaker on 91.3FM and a much sought-after person by the media, Dr Tan continues to keep herself busy spreading the message of proper movement and it’s impact on your life as a whole. S

ports injuries are increasingly common as more and more people engage in sporting activities and active lifestyles. Sports injuries do not happen only in competitive sports or extreme sports such as ultra marathons; they also affect individuals who engage in recreational sports as well as in everyday activities like brisk walking, sepak takraw etc. As a matter of fact, many of my patients are everyday working people who are active sportsmen and sportswomen, but not professional athletes. Sports injuries can be broadly classified into 2 big groups: traumatic injuries and overuse injuries. Traumatic injuries are fairly straightforward in their origin; there was a history of contact, and an injury occured. Traumatic injuries are very common in contact sports such as soccer, rugby, etc. The cause of overuse injuries, on the other hand, is less obvious. In overuse injuries, there is no history of contact or trauma and it usually starts insidiously. The pain and swelling come slowly and gradually, usually starting as after-exercise ache, to pain during exercise and finally to pain throughout the day.

What causes overuse injuries?

Our approach to definitive treatment of sports injuries is based on 4 basic principles: • Accurate diagnosis • Identification of root causes • Individualised treatment • Safe return to sports Accurate diagnosis The first principle in the management of sports injuries is to get an accurate diagnosis. Simple as it may sound, it is often overlooked. Many of us are guilty of self-treatment without a proper diagnosis, only to seek attention later and realise it is not a muscle pull as we had thought. An accurate precise diagnosis is the key foundation to the management of any sports injuries. It forms the roadmap to the formula for recovery and treatment plans.

Overuse injuries are a result of exercise-induced micro-damage accumulating past the point where the body is able to repair, hence manifesting as clinical injuries. Our body is a wonderful machine that is capable of treating itself of minor injuries like strains and sprains. However, when the pace of injury exceeds the pace of repair, clinical injury develops and symptoms of pain and swelling begin to appear. One of the most common causes of injury is toofast-too-soon syndrome. This usually happens when an athlete starts on a new exercise programme and expects unrealistic progress, advancing faster than the body’s ability to repair. A rule of thumb in increasing exercise load is about 10 percent increase per week in the exercise regime. I like to draw an analogy between progress in exercise and the snakes and ladders board game. All of us like to make leaps and bounds in our exercise progress, just like climbing the ladder in the board game. However, be ever mindful of the ugly snake waiting quietly at the corner, like an injury which will undo your progress and return you to square one. 58 ezyhealth&beauty | December 2010

The management of sports injuries can be approached from 2 angles: acute management and definitive treatment. Acute on the field management includes moving the casualty to a safe place and resuscitating, if necessary. Start RICE therapy as soon as possible, i.e., Rest, Ice, Compression and Elevation (RICE). The injured body part must be allowed to rest to facilitate recovery. The body’s intrinsic natural response to injury is pain and it is primarily protective. No-pain-no-gain does not apply here! Ice is useful to encourage blood vessels to contract and therefore reduces the amount of bleeding and hence, swelling. Swellings around the joint have been shown to be inhibitive to muscle contraction, another inbuilt protective reflex. Compression in the form of bandages is useful to limit the degree of swelling and hence, pain. Finally, elevate the injured part to minimize swelling. Most sports injuries are mild and self-limiting and it is not practical to see a doctor for every minor sprain and ache. The rule of thumb is to see a doctor if the symptoms of an injury persist or get worse after 3 days.

Definitive Treatment

Identification of root causes Having diagnosed the problem, the next and probably the most difficult step is to identify the root cause. Sports injuries are notoriously difficult


to eradicate because they keep recurring. One of the main reasons is that the root cause has not been eradicated. The root cause could be in the equipment, the technique, the training or other biomechanical factors. An example would be tennis elbow, which is a very common condition affecting many athletes, such as tennis players and golfers. In tennis elbow, the outside part of the elbow is painful and it restricts turning of the wrists, shaking hands etc. The diagnosis and treatment is straightforward, but the problem is that it tends to recur. Many factors contribute to the cause of tennis elbow, e.g. inappropriate racquet string tension, inappropriate grip size, faulty stroke technique, excessive training, etc. All of these can contribute to tennis elbow and it is important to sieve out the ‘noise’ to get to the root cause. As such, it is very difficult to find the root cause because there are so many possibilities and patience, together with a firm understanding of the nature and requirements of the sport, is important. Individualised treatment The next principle is to individualize the treatment to each patient depending on his needs. In the management of sports injuries, the requirements between a professional athlete and an amateur vary greatly, and it would be inappropriate to have a treatment regime that is similar for both. Although the ultimate goal would be similar in both cases, that is full recovery and return to sports, the approach will be different. For a start, the professional athlete is fitter than the amateur, and such factors will impact rehabilitation. Treatment goals are clearly identified and a team approach is adopted. Sports medicine is one specialty where a team comprising of coach, trainer, team physician, sports therapist and sports orthopedic surgeon works together to draft out a recovery plan for the patient. Safe return to sports Finally, the last tenet of sports injury management is a gradual and safe return to sports. At JL Sports, it is our objective to return our injured athletes to sports and we want to do so safely. Remember the snakes and ladders board game? This step starts with realistic goals-setting and following those goals. A common misconception is to pick up where you left off, not realising that the body has not been exercising for at least a few weeks and may not be able to handle the stress. The heart may be unfit, the muscles may be weaker, the joints may be stiffer and the agility poorer. For example, if you had been running 10 km before your knee injury, it would be unrealistic to return to 10 km at the first run after recovering. I would suggest dropping to at least 50% of where you left off and build up gradually. Pace yourself and build up the mileage and stay injury-free. Dr Tan Jee Lim is an Orthopaedics Sports Surgeon. Dental implants like in most surgical procedures have undergone a natural progression from conventional implants which are invasive and painful to the use of mini implants for a more refined, less invasive, patient friendly and almost painless procedure.

Well-studied and researched by specialists About 50 years ago when the larger diameter conventional dental implants were being used by general dentists, many dental specialists questioned its use without valid scientific reasons. Proposed by a dentist from New York an expert in conventional implants some 25 years ago, the use of smaller diameter minidentals with its gentler and less invasive procedure with the use of dental implants saw a paradigm shift. More than 12 years of success The USA Food and Drug Administration (FDA) subsequently gave approval for its use in patients for long term purposes for both stabilization of dentures and fixed crowns and bridges. FDA’s approval is specific to a product line (i.e. the brand) and it usually takes an average of 12 - 15 years before such an approval is given. In other words, minis have at least a 12-year record. Today, some minis placed in the mouth more than 20 years ago are still functioning well and have stood the test of time. Like most developments, improvements continue to appear making minis a much better choice.

A personal note As a dental surgeon, I’ve been placing minis for more than 7 years and I have

placed more than 3000 minis with a very high degree of success. These little wonders have become life-savers for many patients. From patients with controlled diabetes, to medically compromised patients, and elderly in their late eighties and nineties, minis have given them the opportunity to have a better quality of life. To be able to eat well is an important factor for health. To look well definitely has its psychological impact as well. Minis have, in my view, served patients and will continue to serve patients well in the future. Today, minis are more affordable. Choose minis rather than the more invasive, complex and painful larger diameter conventional implants. Minis are painless, less invasive and a boon to patient care. Try to keep all your teeth, but if you have missing teeth you should seek out minis as a first and obvious choice.

Understanding the use of scans and radiology for detecting medical problems

N

uclear medicine is the specialty of medicine which uses unsealed radioactive materials for diagnosis and treatment. Positron emission tomography, referred to more commonly by its acronym of ‘PET’, is a type of nuclear medicine imaging which uses small amounts of radiation called PET radiotracers to look inside the body. Radiotracers are given to the patient, usually through a vein. The PET camera then looks at the distribution of the radiotracer in the patient and takes a CT image as a part of the PET study. This is commonly called a PET/CT study. PET imaging uses different radiotracers to help map the functions of the body in a noninvasive manner. Depending on the type of radiotracer different functions of the body can be seen. PET imaging is performed by trained nuclear medicine physicians.

Finding Cancer With Imaging The most common form of PET imaging uses the radiotracer called FDG (F18 fluorodeoxyglucose) to look at the use of

glucose (sugar) in the body. By using FDG PET, these cancers can be detected because they take up more FDG than surrounding normal tissue and appear ‘brighter’ on a FDG PET scan. FDG PET is useful for locating suspected cancers cells. This can help find out the type of cancer a person has so that the appropriate treatment can be done. For example, surgery, chemotherapy or radiotherapy. FDG PET is also used to show how far the cancer has spread, if the cancer has gotten better with treatment or if the cancer has come back after treatment has been stopped. This is very helpful in changing treatments that do not work, continuing treatments that are effective and restarting treatment if the cancer comes back. Some of the cancers that FDG PET is commonly used for include lung cancer, breast cancer, esophageal cancer, stomach cancer, colon cancer and lymphoma. While FDG PET is very useful in detecting cancers, it cannot detect all cancers. For example FDG PET is not good for detecting some types of liver cancers, thyroid cancers and cancers in the prostate. These cancers may not be seen for a number of reasons, such as not taking up enough FDG or not holding on to the FDG. As a result, they do not have more FDG than normal tissues and so are not seen. Similarly very small cancers may not be seen if they do not take up enough FDG.


Seizures And Dementia Some organs in the body will normally take up more FDG than other normal tissues. These include the brain and the heart. However, some organs like the kidneys and the bladder remove FDG from the body. This makes it difficult to see cancers in these organs. Though FDG is most commonly used to look at cancers it is also used to look at the glucose used in the brain and the heart. FDG imaging of the brain is useful for people with seizures. It may show the side of the brain which is active during seizures. FDG is also used in imaging dementias. Certain patterns of FDG uptake in the brain are typical for different types of dementia. This can help the clinician pick the treatment most helpful to his patient.

Looking At Our Hearts Several radiotracers are used for PET imaging of the heart. Rubidium (Rubidium 82) is a radiotracer used for imaging the heart with PET. Rubidium is injected through a vein and taken up by the heart. When there is narrowing of the blood vessel or decreased blood flow to a part of the heart, less rubidium will be taken up by that part of the heart. When this is imaged by a PET camera the areas of the heart that receive less blood can be detected as they have less rubidium compared to the normal heart. This is called a ‘myocardial perfusion scan’. It has two sets of images; one when they are resting (called rest images) and

again when they are exercising (called stress images). Exercising increases the need for blood by the heart. Often diseased vessels can provide enough blood when the heart is at rest but not during exercise. Patients who cannot exercise are given medicines that help increase blood flow similar to exercising. By looking at the ‘rest’ and ‘stress’ images hidden heart disease can be revealed. A heart attack causes damage to the heart by interrupting the blood supply. When this happens, FDG PET helps look at the heart for tissue that has been damaged so much that it is not functioning but is still alive. These damaged areas will often show as having decreased or no uptake of rubidium is used but will take up FDG showing that they are alive. This can help doctors see which part of the heart is likely to recover if blood supply is returned, for example through surgery or coronary angiography with stenting. This is a brief introduction to PET imaging. There are many other radiotracers and uses for PET imaging. For example bone disease can be detected using F18 sodium fluoride (NaF) which reveals the increased activity in bone when it is damaged and repairing itself. New radiotracers are being developed worldwide and gradually more are being used in daily medical practice to help patients detect disease in a non-invasive way. riends add a wonderful dimension to our lives: they help us celebrate the good times and make it through tough times! Several hundreds of studies attest to the health benefits of friendship. People with plentiful social support have stronger immune systems and are less likely to suffer from depression, anxiety and other types of mental illness; they have been associated with lower levels of stress, increased longevity, and greater levels of happiness, success in family and at work. Having friends can also mean differences in an individual’s physical and biochemical reaction to environment stressors while mitigating the physiological consequences of unhealthy stress and anxiety and depression. Psychology Today (1999) quotes research that proves that individuals in general and women in particular, underutilize the ‘friendship factor’ as a powerful healing force for physical and mental illness. According to the article, a group of chronically depressed women experienced substantial improvement in mental health when they were prescribed personal ‘befrienders’ who visited, went on outings with and became confidants of the participants over a 12 month period. The most successful group of women in the study experienced a deep sense of renewal in their personal and social lives that enabled them to move forward in more healthy ways through their journey of life. There is also solid scientific evidence that friendship can extend life. Because friendships are grounded in communication, they provide a strong sense of personal support. Individuals who feel supported by their closed friends tend to live longer, stay healthier, are more confident and have a more positive outlook when faced with adversities.

People with strong social networks are generally shown to: ✔ Have stronger, more resilient immune systems. ✔ Improve their mental health. ✔ Boost their chances of surviving life-threatening illness. ✔ Live longer and more successfully than people without social support.

Here are some key communication tips that can help you to build relationships with people that are truly supportive and sustaining: Stay in Focus: Sometimes it’s tempting to bring up past seemingly related conflicts when dealing with current ones. Unfortunately, this often clouds the issue and makes finding mutual understanding and a solution to the current issue less likely, and makes the whole discussion more taxing and even confusing. Try not to bring up past hurts or other topics. Stay focused on the present, your feelings, understanding one another and finding a solution. Listen Carefully to Your Friends: People often think they are listening, but they are actually thinking about what they are going to say next when the other person stops talking. Truly effective communication goes both ways. When dealing with friends, it’s important to give as well as to receive support. While it might be difficult, try listening to what your friends are saying. Here are some basic habits to remember when you listen:

Apply Assertiveness and Honesty: People often think of assertiveness as ‘standing up for yourself’ and ‘not letting people push you around’ – basically the alternative to passivity. The answers to these questions may help you begin to develop the connection with your own ‘insides’ (Your innerself/ intuition), which will help you strengthen your relationships, help you with the next step...

6 Myths about the penis, debunked

here is a dire lack of sexual education in Singapore; not to mention timely, accurate and positive sexual information. There are a lot of misconceptions about the male sexual anatomy that could possibly be preventing both men and women from feeling the pleasure they deserve. It’s time to set the record straight. Here are six of the most common myths, debunked:

The penis is dirty. There is a common misconception amongst women of the penis being unclean, smelly or downright dirty. It goes without saying that a penis, circumcised or not, will be dirty if it is not washed. Yet, in reality, the penis is an external organ, unlike the female vagina which is an internal structure, and hence more likely to be ‘clean’, all things considered. The penis should be washed in the bath or shower just like any other part of the body such as your armpits or your rectum. Just roll back the foreskin (if uncircumcised), wash the glans or head of the penis with soap and rinse it off with warm water.

Myth # : Penis size is proportional to other body parts. There is a myth based on the premise that you can tell how big someone’s penis is by measuring their feet, hands, or nose. There is no scientific proof of a correlation between body part size and penis size. It is true that there are certain genes that control the development of the limbs which also control the development of the penis in the embryonic stage. Yet, when it comes to fully-developed males, there is no absolute relation between the size of the penis and the size of the limbs or other body parts.

Myth #4: A ‘real man’ can last all night long. This myth would have you believe that if a man is not capable of maintaining a cucumber-hard erection and performing all night, he is an incompetent lover. Truth: Men typically reach orgasm five to ten minutes after the start of penile-vaginal intercourse, taking into account their desires and those of their partners. A good erection is dependent on how good the blood supply is to your penis. Hence, men who have any kind of vascular problems have high chances of their erection being affected. Smoking affects the blood vessels making them less pliable and less compliant. Avoid smoking, fatty foods and cholesterol-rich foods. Exercising and eating healthy are good things. Whatever affects the vascular system affects your erection. (continued on pg 68)

Myth # : Your penis is your most powerful sex organ. This is patently false. Your mind is your most powerful sex organ, and your skin is your largest one! The brain programs our sexual function, our reproductive behaviour and our sex drive. Hence, it has the potential to be our most powerful tool for accessing mind-blowing sex. This applies for both men and women. We can use our minds to fantasize about anything – even the impossible or seemingly unattainable. Fantasies are healthy and normal as long as you can separate them from reality and accept (and sometimes appreciate) that you may not be able to fulfil them. For many, fantasies should remain that way they are – fantasies – as reality creates potential for letdown.

Myth #1: Bigger is better. This has to be the most well-known myth, especially since guys tease or insult each other about being small in size. The penis is a symbol of male identity, sexuality, and masculinity, which is why size is often fussed over among men. The notion that a larger penis equates to someone who is more masculine has, in turn, led men to think or feel that bigger is better. The penises of Singaporean men average 3.5 to 5.9 inches in length. Most men fall somewhere in-between. Size has little to no relation to sexual performance. If he asks you if what you think of his penis, know that he is actually asking for reassurance. Encourage him to appreciate his body for what it is – healthy, functioning and perfectly normal. There you have it: six of the most common myths debunked. Take a moment, think of the penis as not an incomprehensible alien but as a magical wand promising endless hours of pleasure and ecstasy.

n April 1999, two American high school seniors Eric Harris and Dylan Klebold shot 13 people and injured 21 others with illegally purchased guns and crudely homemade bombs that if had detonated properly, could have resulted in hundreds of casualties. Later, the duo went on to commit suicide in the school’s library. Known as the Columbine High School massacre, this killing spree was recorded as the fourthdeadliest school massacre in the United States history. As the horror of the killings unraveled, investigations showed that the pair had spent a large amount of time on violent internet sites and online games, such as Doom and Wolfenstein 3D. These blood-and-gore games were believed to have influenced the pair’s decision to kill.

BUZZ OFF! Protecting yourself against this buzzing brute By Celene Ting

W

e’ve heard of genetically-modified (GM) food, but what about GM mosquitoes? If you haven’t, you just might in a notso-far-away future. Pest management specialists and industry researchers who convened at the recent Pest Summit held in Bali concur that ‘new approaches’ aimed at reducing the population of the ‘urban mosquito’ (more commonly known as the Aedes aegypti or the Aedes mosquito) may have to be looked into sooner rather than later. These unwelcomed yet familiar visitors to our homes are also undergoing shorter

life cycles and experiencing higher ecological adaptability even in the harshest environments as a result of climate change. In other words, the traditional chemicalbased household insecticides that we were used to in the past may indeed be a thing of the past. Industry observers said that dengue control programmes across Southeast Asia are shifting from the straightforward application of chemical-based insecticides towards a more integrated vector control approach including the public, private and, increasingly, the people sector as well using a consistent and integrated combination of biological control agents, pyrethroidbased insecticides, source reduction and environmental management. But to what extent will these measures be sustainable in the long run as today’s myriad of mosquito species continue to evolve in terms of their variety, complexity and tolerance towards current pest control solutions, and as the challenge of coping with the healthcare consequences of their growth and development intensify further amidst the uneven yet rapid urbanization of the region? According to Dr Lee Han Lim from the Institute for Medical Research in Malaysia, he observed that even with the strict enforcement of larval control through

inspections and fines in Singapore, the country continues to report a considerable number of dengue cases every year. So it seems, even in our first-world citystate, many of the current vector control measures being implemented within the rest of the developing region have proven unsustainable due to systemic factors such as a lack of community participation and persistence of long-lasting and effective public-health vector control programmes. So will genetic control using modern molecular technologies (which could prove successful regardless of the level of community participation) be able to control the spread of such vector-borne diseases? Such methods will also include the sex ratio distortion, genetic manipulation and replacement of the mosquito population with one of a less harmful nature. However, experts say that GM mosquitoes need to be very efficient in spreading themselves and competing with natural vector populations. In addition, acceptance of the strategy by the community may meet with considerable resistance arising from a whole range of ethical and social issues in such a Mantrying-to-play-God scenario. But individuals tend to harbour the convenient presumption that ‘it’s someone else’s job, not mine’ to rid pests from within (continued on pg 76)

Compared to other common chemical-based insecticides such as the widely-used temephos, there has been no known resistance recorded by mosquito larvae against this biological larviocide which is safe for use by humans in close proximity with live animals and within aquatic environments. Johnny Wong, General Manager of Best Chemical Co. (S) Pte Ltd which is the exclusive distributor for the Bti briquette (also known as Bti ‘donut’) in Singapore, said, “While much has been said and reported about the Aedes mosquito and the global carnage

caused by dengue crises worldwide, more needs to be done to educate households on what they can do to contribute to effective and sustainable vector control in Singapore.” Many industry experts including Wong also agree with Professor Brian Forschler, professor of entomology at the University of Georgia who highlighted that pest management should be perceived as a continuous process involving all sectors of society rather than as an ‘isolated event’. Indeed, controlling the rise of dengue is not just the responsibility of either the public or the private sector per se. Rather, a more integrated, ‘knowledgebased approach’ involving the third ‘P’ (that is, the ‘people’ sector) through public education and engagement on

What is Bacillus thuringiensis israelensis (Bti) This is a naturally-occurring soil bacterium registered for control of mosquito larvae. The Bti prevents a mosquito larva from developing into the pupa and, subsequently, into the adult stages. Since 1982, it has been used successfully worldwide as a biological pest control agent to combat mosquitoes and blackflies. The Bti was first registered by the Environmental Protection Agency (EPA) as an insecticide in 1983.

How Bti works The Bti is a type of microbial larvicide that kills mosquito larvae from the inside out. During the spore-forming stage of its life-cycle, the Bti bacterium produces a protein crystal which is toxic only to mosquito and blackfly larvae. Once ingested, the active Bti agent attaches itself to the gut lining of the larvae which kills them within an interval of one to twenty-four hours. In addition, the sustained release of the Bti agent into the water for up to thirty days or more helps to ensure continuous kill.

Who uses Bti Both households and pest controllers can use the Bti to prevent mosquitoes from breeding in and out of residential and commercial compounds. Especially in less accessible areas where it becomes inefficient or impractical to check for stagnant water on a regular basis, the Bti when used in accordance with instructions on product labels can serve as an indispensable mosquito control mechanism for anyone in the family.

How to use Bti Bti is applied directly to the water where mosquito and blackfly larvae are found. The bacteria are suspended in the water where the larvae will ingest it. Apply to standing water in places such as unused swimming pools, clogged roof gutters, old automobile tires, construction sites, flowerpot plates, vases, drains, ponds, water gardens and/or birdbaths.
