“Perfect Fit for Army Medicine” YONGSAN, Korea – Over a year ago, on 14 February 2013, the Brian Allgood Army Community Hospital received the highest level recognition for its Patient-Centered Medical Home (PCMH) from the National Committee for Quality Assurance. This achievement set the standard for military medical healthcare on the Korean peninsula. The Patient-Centered Medical Home is a team-based model emphasizing care coordination and communication to transform primary care into 'the way healthcare should be.' Research shows that medical homes lead to higher quality and lower costs, and improve patients' and providers' reported experiences of care. PCMH began outside the military and its quality standards are recognized in the civilian healthcare industry. As part of the hospital’s continuous effort to provide the 8,500 beneficiaries in our medical community with the best possible care, PCMH is currently available in the Family Medicine and Pediatrics clinics. PCMH allows providers and patients to take a more proactive approach to health care. The medical home reduces the percentage of specialty referrals and results in the team providing the needed care in one v isit. Patient Centered Medical Home allows for better coordination of care, improved communication among team members and beneficiaries, with an emphasis on preventive care, chronic care management, and empowered, active patient participation. MAJ Pamela DiPatrizio, Clinical Nurse Officer-in-Charge, stated that "the Patient Centered Medical Home is a one-stop shop where patients can get all of their medical needs taken care of at one place in one appointment." She continued, "It is completely different from the previous models for military health care. I truly believe that the PCMH model is a perfect fit for Army Medicine." Moreover, PCMH identifies practices that embolden partnerships between individual patients and their healthcare team, offering expanded hours, nurse-triage advice line, and secure messaging via RelayHealth. Instead of treating patient care as a collection of sev eral episodic office visits, each patient care is managed by a Primary Care Manager and team that provides health care and coordinates treatments across the health care system. "What excites me about PCMH is the number of improvements that ensures our most v alued customers--our patients--receive the benefits that they deserve and have come to expect. Will keep the dog’s leash in-hand while in the park. Stay with dog at all times and monitor behavior. Immediately leash and remove a problem dog (aggression, mounting, etc.) from the park. Quickly stop your dog from digging and fill any holes with dirt. Quickly clean up after your dog and dispose of the waste. (Don’t even think about leaving it for others to clean up!) Honor other patrons’ requests regarding their dogs. Keep park gates closed at all times. Turn off running water Are legally and financially responsible for their dog’s behavior.
