In the past it was a choice of shopping at an area business in town or making a trip out of the area to secure services or make purchases. With the internet the choices and decisions have become seemingly endless and sometimes overwhelming. Some of these decisions are easy, while others require careful consideration as they may have a significant impact on our health, quality of life, wallet and more. What determines a good purchase or business partnership? Is it price, quality, ease of purchase, all of these or something else? Should we shop online or at an area business or store? How do we know if we are getting a good deal and if it is the best choice for our situation? Our How To Guide will provide you with helpful suggestions and points to consider before making a decision. It can also damage vehicles and be downright dangerous, with cracks and potholes creating trip hazards for visitors to your property. If you’ve been putting off a paving project because you don’t know how to go about getting it started or are worried about costs, a paving professional may be able to set your mind at ease. Keep the following in mind and call local paving customers to learn more about the price and process of updating your drive or walkway.

Choose a paving service PROCESS
Understanding the process of laying pavement will help you understand what you need and which company is best for you. There are many types of pavements, including popular options such as asphalt or cement. Be sure you know the strengths and weaknesses of each material. The construction of these materials is also important. The preparation work, dimensions, depth and mixture of materials will impact the longevity of the surface, as well as the ongoing maintenance required. Tell your paving company how the

surface will be used. If you are paving a commercial lot, for instance, provide them with an estimated traffic count with a note about the heaviest trucks that might use it. The best paving companies will have deep roots in your community. Climates and types of soil vary around the country and a company with a long track record understands your area. They know what challenges various pavements face throughout the year.
OTHER WORK Previous work can tell you a lot about a paving company. Request referrals from past customers. Look for work that is similar to what you need. Whenever possible, ask the property owner about their experiences during
construction. Did the company finish the job on time and on budget? Were their expectations met? It is a preferable to look for older work the company has done. If pavement installed years ago has held up against the test of time, that is the sign of a quality job.
INSURANCE For your protection, verify that the paving company is properly insured. Many homeowner and business insurance policies don’t cover accidents that occur by contractors working on your property. Decide if anterior hip replacement is for you
D
irect anterior hip replacement is a minimally invasive muscle sparing hip replacement surgery. This new innovation in hip replacement surgery is unlike traditional posterior (back) and lateral (side) approaches which require cutting of muscles to reach the hip joint. Anterior (front) hip approach is a muscle sparing approach.
WHAT ARE THE BENEFITS OF ANTERIOR APPROACH HIP REPLACEMENT? Hip replacement done through this approach does not require cutting any muscle, it potentially provides quicker recovery, less pain and early discharge from the hospital. This procedure is done under X-ray control which allows the surgeon to restore the leg length more accurately and also allows for accurate placement of implants which potentially provide longevity to the implants. This approach also has a lower dislocation rate compared to the posterior approach.
HOW DOES ANTERIOR HIP REPLACEMENT IMPROVE PATIENT RECOVERY? Conventional lateral or posterior surgery typically requires strict precautions for the patient. Most patients must limit hip motion for six to eight weeks after surgery. They must limit flexing of the hip to no more than 60 to 90 degrees which complicates
normal activities like sitting in a chair, on a toilet seat, putting on shoes or getting into a car. Simply climbing stairs may also be more difficult during recovery. The Anterior Hip Replacement allows patients to immediately bend their hip freely and bear full weight when comfortable, resulting in a more rapid return to normal function. After surgery, patients are instructed to use their hip normally without cumbersome restrictions. In supervised therapy, patients go up and down stairs before their hospital release.
WHO IS A CANDIDATE FOR ANTERIOR HIP REPLACEMENT? Many people suffering with arthritis, hip pain, stiffness and limited hip movement can now choose this minimally invasive surgery when hip replacement is the chosen treatment of the patient and their doctor. Although the direct anterior approach may make sense for some patients, only your surgeon can help you decide what is best for you. Dr. Rupal Patel, Orthopedic Surgeon at CMH Orthopedic Service specializes in hip and knee joint replacement surgery and is a member of the Medical College of Virginia Physicians, the faculty practice group for Virginia Commonwealth University Medical Center. He is also an assistant professor in the department of orthopedic surgery at Medical College of Virginia.

LOCATION Real estate agents tell you that location is everything, and they mean it. When you purchase a house, you are investing in a community. Checking out the community and neighborhood is your first step. Most people want a convenient location near shops, jobs, parks and schools. As a result, the most desirable neighborhoods are usually the most expensive.

Whether you’ve lost electricity due to the weather, a downed power line or some other occurrence, a generator will help you get back to business quickly by allowing you to produce enough electricity for your own needs on-site.

If you have a relationship with a good auto repair shop, you won’t sweat it when these things come up. If you haven’t had a regular mechanic is recent years, now is the time to find one. Here are a few points to consider: • Less than 40 percent of new customers in auto shops are walk-ins. That means word-of-mouth is still king in the industry. Find a shop that
gets rave reviews from family and friends, and you’ll be in good hands. • The Better Business Bureau can inform you about the repair shop’s performance and reliability. • If you travel often, consider a shop that offers a national warranty. hen you need to move, travel or simply find more space in your house, self-storage can be a good deal. Storage centers are increasingly popular solutions for safely stashing your worldly possessions on a temporary basis. There are some important aspects to consider before selecting a storage facility.
LOCATION Location is a key factor when selecting a storage company. Your ideal facility should be close to your home or office for ease of access.
SECURITY Your belongings should be safe for the duration of the rental. Visit the facility in person to verify its security protocol.
Choose a storage center
The overall security of the facility will be stronger when it is in a safer part of town. The best facilities have three security layers. 1. The building has a security fence that is not easy to climb. It should be topped with barbed wire to discourage attempts to infiltrate the perimeter. 2. Heavy, secure doors seal the center. epending on where you live, chances are you have a few centers in your area to choose from. Whether you have the luxury of time to prearrange your rehab stay before an elective surgery or whether you send out the ‘scouts’ from your family to look into a few places for you, it can be confusing and daunting. We recommend going yourself or asking a friend or family member to go set foot in all the centers you’re considering so you get a true feel for the place. Any center will be more than happy to show you around so you can see how things work and what to expect. When you head in to learn more about a center, here are some questions we recommend getting answers to. 1. Is my insurance accepted here and
what steps do I need to take ahead of time to ensure coverage? 2. Does the staff specialize in any particular areas and what is the staff to patient ratio? 3. What types of therapy do you offer? 4. What types of treatment and team members will be active with my recovery? 5. What can be expected about the transition from hospital to rehab? 6. What items should be brought with me and what is provided? 7. Does the center offer outpatient therapy or will they help set up in-home services if needed after the rehab stay is complete? 8. Are care planning meetings conducted on a regular basis and are family members invited to participate? If you have other questions don’t be
shy about getting answers that will help you make the best decision for you or your loved one. Most of all, take note of your gut feelings about a center. You’ll notice how you felt when you set foot in the doors. Were you warmly greeted? Did the staff smile in the halls? Did people seem content? Were common areas clean and free of odors? Once you’ve decided on a center, we recommend verifying the best way to contact patients via telephone and where friends or family can send mail. Additionally, make sure appropriate emergency contact information has been communicated and ask for the names and contact info of integral team members at the center, from the social worker to the billing office. Once you’ve made your decision you can focus on healing and recovering.

While complicated, intricate plans may be best left to the pros, a simple deck is very feasible for many do-it-yourselfers. Here’s what’s involved.
SAFETY AND REGULATIONS Before you start any project, it’s important that you know how to do it safely and follow any applicable rules for construction projects in your area. Make sure you know and follow the safety instructions for the tools you’ll be using, including using protective equipment like safety glasses and gloves when needed. You should also check with your city or county’s building department to make sure you’ve got the right permits and know the requirements for your deck. Many municipalities regulate the footing depth, materials and location of your deck, including setback distances from utilities or property lines.
THE PLAN Once you know the applicable rules and have an idea about the deck you want to install, you can start making more concrete plans for turning that dream into reality. f an injury or illness threatens the safety and well-being of a loved one, it isn’t difficult to make the decision to seek emergency help. But it can sometimes be difficult to assess the medical urgency of a problem, especially in moments of stress when quick thinking is required. Knowing where to go to seek the right treatment can be difficult when the clock’s ticking. That’s why it’s important to know ahead of time the difference between the type of care provided by emergency rooms (ER) and urgent care centers. Knowing in advance can help you save time, money and potentially even a life. The National Institutes of Health estimated in 2012 that more than onefourth of visits to the emergency room could instead have been addressed by the medical staff at urgent care center. Patients who choose urgent care centers over the emergency room, when appropriate, help lower the overall
costs incurred within the healthcare system by approximately $4.4 billion each year. What’s the goal of urgent care, and when is it the right choice? Urgent care addresses the type of emergent situation that lies somewhere between waiting until Monday to see your primary doctor, and rushing straight to the emergency room. Acute care centers began opening across the country in the late 1980s, leading to a dramatic drop in the number of annual emergency room visits. In short, urgent care fills a gap. The American Journal of Medicine reports more than 9,000 urgent care centers currently operate in the United States. Urgent care centers’ walk-in convenience and affordable services help treat more than 70 million patients annually. Most urgent care centers are open 24 hours a day, seven days a week — including holidays. That’s a valuable service if you sprain an ankle on Christmas Day. They are fully staffed to ensure that patients are treated quickly. One of the greatest benefits of urgent care is its affordability. The cost of urgent care is usually one-third that of an ER, and most facilities accept insurance. ERs, though, are required by federal law to provide a free medical exam to anyone who can’t afford to pay. Doctors in acute care must be competent in all areas of medicine. Their training and experience is extensive, and

the quality of care is about what you’d expect in an ER. One critical aspect of urgent caregivers’ jobs is knowing when to direct patients to the ER. Urgent care is appropriate for high fevers, cuts requiring stitches, mild allergic reactions, insect or snake bites, minor fractures that do not break the skin, minor back or neck injuries, heatstroke, frostbite and other problems that are not quite emergencies. ERs are better equipped for problems requiring immediate life-saving measures, the attention of a specialist, surgery or hospitalization. Their diagnostic capabilities are superior. Urgent care centers offer X-rays, urinalysis and other simple tools, but they cannot provide involved lab testing or advanced imaging like MRI. If possible, bring all bottles of medications that you are currently prescribed with you when you visit the emergency room. Bringing your own medications to the emergency room will greatly lower the chance of medication mixups following hospital admission. The American Heart Association urges you to call 911 if you exhibit signs of heart attack or stroke. It lists the following as possible warning signs of heart
attack: pain or pressure in the chest and upper body, shortness of breath, clammy skin, jaw pain and nausea. The acronym FACT is used by the AHA to educate people about stroke: facial drooping, arm weakness, speech difficulty and time that symptoms appeared. The last is crucial because clot-destroying drugs are ineffective if too much time has passed. Additional symptoms of stroke include leg numbness, confusion, blurred vision and dizziness. Other valid reasons to seek emergency care include serious breathing difficulties, loss of consciousness, chemical poisoning, heavy bleeding, severe burns, traumatic head injuries, gunshot wounds and sexual assault. Problems that call for psychiatric intervention, such as suicidal thoughts or drug overdose, are also best handled in the ER. Any illness or injury can be life-threatening if left untreated. The distinction between deeply cutting a finger and wounding a major artery is an important one. Educating yourself beforehand will help you choose the best treatment when you have little time to weigh your options.
