St. Luke’s Home Adult Day Health Care Program
St. Luke’s Home’s Adult Day Health Care Program is a medical program that offers a wide range of services and activities, including therapy, nursing care, case management, personal care and therapeutic recreation, that promote wellness and help registrants remain at home. Registrants’ needs are assessed and met through an individualized plan of care that is developed and implemented by an interdisciplinary team of medical professionals, including the registrant’s personal community physician. The Adult Day Health Care Program is located in the Center for Rehabilitation and Continuing Care Services on the St. Luke’s Campus. The spacious, state of the art facility features a rehabilitation gym, therapeutic recreation area, fine dining, bathing facility and medical treatment room, as well as a quiet room.
Registrants arrive at the facility through a private entrance and are greeted by members of our staff at the reception area. Conveniently located near the entrance is a locker room for registrants to keep their personal items during the day. Transportation is provided to and from the program for those who live within a 10 mile radius.
We organize many recreational and social activities in our speciallydesigned activity room to keep our registrants busy and active. Activities range from pet therapy, bowling, crafts, word games and trivia to group exercise, entertainment, outings and shopping trips.
Registrants are provided nutritionally-balanced, warm lunches, served in a private dining room. A continental breakfast and afternoon snacks are also available.
For those interested in quiet activities and relaxation, we offer areas for TV viewing and conversation. A library of large-print books is also available.
We accommodate each registrant’s individual needs and schedules. For those in need of rest, we offer a private nap room for short periods of use.
Registrants are given nursing, case management, clinical management, medical, diagnostic, social and rehabilitative services on a regular basis. Services are provided according to the needs of the registrant, and range from monitoring, observing and maintaining the individual’s health status to in-depth interventions and utilization of resources.
Our bathing facility includes a shower and state of the art Parker tub which are easily accessible to accommodate registrants requiring assistance with personal hygiene.
Our registered nurse (RN) conducts a comprehensive assessment to identify an individual’s healthcare needs, and the type and intensity of services required. The individual will be admitted if it is the professional opinion of the RN, the individual’s personal physician, the other interdisciplinary team members, the registrant and the caregiver that his or her assessed needs can be satisfactorily met, either entirely or in part, by the services we provide.
