Hold Harmless Agreement To the Parent: All individuals between the ages of 12 and 18 are invited, however parents will assume full responsibility for the conduct and safety of their children both on library property and off-site. As part of the pre-registration process, all parents will be required to sign and return this parental permission slip in person or to call Nadia Ingram, Librarian, at (941) 861-1130, by 1:00 p.m. on January 18th, to confirm their child’s registration and that the parent signed this permission slip.

Description of Event: Selby Public Library Winter Scavenger Hunt On Saturday, January 18th, from 2 – 4 p.m., Selby Public Library is holding a scavenger hunt that will take place inside the library, and outside, within an approximate three-block radius surrounding the library. This program requires pre-registration and this release, signed by a parent. Participants will be assigned to a team of two or more and asked to provide photos of themselves at certain landmarks surrounding the library. These landmarks are: Starbucks, Whole Foods, The Sarasota Opera House, Patellini’s, and Five Points Park. Library staff will remain in the Library and will not be accompanying the teens on their “hunt” to the above stated establishments. At 3:30 p.m., teens will be required to return to the library with their pictures, and then will enjoy snacks, a slideshow of their pictures, and an award ceremony for the winners. Parent to Complete and Sign: In consideration for accepting this entry in the Sarasota County Selby Public Library Winter Scavenger Hunt, I ___________________________________________(Responsible Party) hereby, for myself, my heirs, executors and assigns, do waiver and release any and all claims and right for claims for damages I may have against Sarasota County Libraries, or other agencies or individuals used to conduct the event and other unnamed, for any and all injuries suffered by my child as a participant in the Sarasota County Selby Public Library Fall Scavenger Hunt. If my child should suffer any injuries or illness, I authorize the official of the tournament to use their discretion to have him/her transported to a medical facility, and I take full responsibility for such action. In addition, I hereby authorize the use of any photographs, motion pictures, videos, or other materials related for publicity, promotional or news purposes by the various media as authorized by the official of the Sarasota County Selby Public Library Winter Scavenger Hunt. ____________________________________ (Signature of Responsible Party)
