In our profession, the New Year means third-party plan changes: patients with new insurance plans and new member IDs, group numbers, copays, etc. It seems the stress level in the pharmacy always rises. In the Sunshine State, it also means we start hearing some rumblings in Tallahassee. The

Along with our Governmental Affairs Committee, chaired by Shannon Wedekind, we have a strong team to make sure our voice is heard. Our team also includes another vital member: YOU! With YOUR help, we can achieve our legislative goals. We need your attendance at the Flor-

ne of the things that associations do is share with their members all of the policy issues going on around them. The common messages for our association include things that the Board of Pharmacy, Agency for Health Care Administration, Florida Legislature, Food and Drug Administration and other governmental agencies are doing that our stakeholders learn from our advocacy work. It may be possible that the Florida Pharmacy Association has been doing a decent job of keeping our members informed through our monthly journal Florida Pharmacy Today and also our electronic newsletter Stat News. Over the past 18 months, we have been quietly building a series of tools through our website (Pharmview.com) to help our members interconnect with each other. There is a real purpose to developing this new feature. For example, we will send to you information on emerging policy development and give you an idea as to what that policy may do to the profession or business of pharmacy. There is generally a lot of dialogue from within the leadership of the FPA to either prepare a response to the proposed policy or to craft a targeted action plan to push out to the membership. What we did not have until recently was a way for the members to bounce their thoughts and ideas on these issues among our peers. Well the wait is over and the tools are now active and appear to be working fine. Where are these tools? The easiest way to find one is to look on the right hand side of Pharmview.com and search for the balloon marked “Latest News.” Select one of the articles of interest to you and browse through the

information. Remember to log in with your FPA username and password. Some of the news items are open to the general public while others are specifically targeted for our membership only. At the bottom of each article you should be able to see a comment box where you can make some notes or provide feedback on the information that you were reading. Remember though that your comments will be viewable by all membership visiting that section of Pharm-

If you are not on an FPA committee or council, you will still be able to view blogs that are posted, though you will not be able to comment on them. Still, you can have direct input to committee or council members by simply selecting a member of that group directory and use the messaging tools that are in your membership profile to communicate directly with that member. Are you looking for a specific member? You can find them easily through our “Community Search” tool in the upper left hand corner of Pharmview. com. You can refine your search by navigating to your member profile under “My Profile” on the right hand side of Pharmview.com and selecting “Messages.” On that page you will see “Message a Member” listed under “Message Options.” Select that and you will see options for either a “Basic Search” or an “Advanced Search.” For example, you can use the “Advance Search” tool to find all FPA members who live in Miami, Orlando or Tampa. You can further refine your search by selecting a member type. This would allow you to search for members who are students or graduates of one of Florida’s pharmacy schools, or those members who

have indicated in their profile a specialty area of pharmacy practice. The possibilities for interconnectivity with colleagues of similar interests are only limited by your imagination. Antibiotic Resistance Threats in the US Antibiotics are powerful tools for fighting illness and disease, but overuse of antibiotics has helped create bacteria that are outliving the drugs used to treat them. Source: Centers for Disease Control and Prevention Antibiotic resistance is a quickly growing, extremely dangerous problem. World health leaders have described antibiotic-resistant bacteria as “nightmare bacteria” that “pose a catastrophic threat” to people in every country in the world. Each year in the United States, at least 2 million people become infected with bacteria that are resistant to antibiotics, and at least 23,000 people die each year as a direct result of these infections. Many more people die from other conditions that were complicated by an antibiotic-resistant infection. In addition, almost 250,000 people who are hospitalized or require hospitalization get Clostridium difficile , an infection usually related to antibiotic use, each year. C. difficile causes deadly diarrhea and kills at least 14,000 people each year. Many C. difficile infections and drug-resistant infections can be prevented. How Bacteria Become Resistant When bacteria are exposed to antibiotics, they start learning how to outsmart the drugs. This process occurs in bacteria found in humans, animals and the environment. Resistant bacteria can multiply and spread easily and quickly, causing severe infections. They can also share genetic information with other bacteria, making the other bacteria resistant as well. Each time bacteria learn to outsmart an antibiotic, treatment options are more limited, and these infections pose a greater risk to human health. Infections Can Happen to Anyone, Anywhere Anyone can become infected with antibiotic-resistant bacteria anywhere and anytime. Most infections occur

in the community, like skin infections with MRSA and sexually transmitted diseases. However, most deaths related to antibiotic resistance occur from drug-resistant infections picked up in healthcare settings, such as hospitals and nursing homes. Protect Yourself Against DrugResistant Infections There are many ways you can help prevent the creation and spread of resistance. First, when you are sick, do not demand antibiotics from your doctor or take antibiotics that were not prescribed to you directly for your specific illness. When taking antibiotics, do not skip doses, and make sure to follow the directions about dose and duration from your doctor. Second, like all diseases, common safety and hygiene methods can prevent disease and spread. Make sure to: ■■ Get updated and regular vaccinations against drug-resistant bacteria ■■ Wash your hands before eating and after using the restroom to avoid putting drug-resistant bacteria into your body ■■ Wash your hands after handling uncooked food to prevent ingesting drug-resistant bacteria that can live on food ■■ Cook meat and poultry thoroughly to kill bacteria, including potential drug-resistant bacteria Protect Your Patients from Drug-Resistant Infections There are many ways to help provide the best care to your patients while protecting them against antibiotic-resistant infections. ■■ Follow all necessary infection control recommendations, including hand hygiene, standard precautions and contact precautions.

Diagnose and treat resistant infections quickly and efficiently. Treatment options change often because resistance is complex. Make sure to follow the latest recommendations to ensure you are prescribing appropriately. Only prescribe antibiotics when likely to benefit the patient, and be sure to prescribe the right dose and duration. Be sure to clearly label dose, duration, and indication for treatment, and include appropriate laboratory diagnostic tests when placing antibiotic orders. This will help other clinicians caring for the patient to change or stop therapy when appropriate. Take an antibiotic time-out, reassessing therapy after 48-72 hours. Once additional information is available — including microbiology, radiographic and clinical information — a decision can be made on whether to continue the same therapy. When transferring patients, ensure the other facilities are notified of any infection or known colonization. Keep tabs on resistance patterns in your facility and in the area around your facility. Finally, encourage prevention methods with your patients. Make sure they understand how to protect themselves with vaccines, treatment and infection control practices such as hand washing and safe food handling.

2014 Florida Pharmacy Scholarships Available

lorida Pharmacy Foundation scholarships are now available online. The application process is simple and scholarships will be awarded to P2, P3 and P4 students in all six pharmacy schools in the state. Just go to http:// www.flpharmfound.org/ and fill out the application by February 15, 2014. It is exciting to be able to offer 10 different scholarships between $1,000 and $1,500. Six of the scholarships are sponsored by Walgreens which generously made one scholarship available to each pharmacy school. Three of the scholarships are sponsored by individuals: Gary Koesten, Jim and Patsey Powers, and Bill and Cyndi Mincy. One scholarship is sponsored by a local pharmacy organization: the Volusia/Flagler County Pharmacy Association. We are deeply grateful to our individual, local-chapter and corporate sponsors, and wish to thank them for helping the Florida Pharmacy Foundation achieve its mission to support research, education and other activities that improve public health and the quality of life through the profession of pharmacy. 2013 Florida Pharmacy Scholarship Winners Many of the same generous sponsors also funded scholarships in 2013. ■■ The Walgreens Scholarship, in the amount of $1,500, was awarded to Vongai J. Majekwana. Vongai is a student at Nova Southeastern University. He is looking forward to practicing patient-centered care as a pharmacist. ■■ The James A. Mincy Independent Pharmacist Scholarship, in the amount of $1,000, was awarded to Gina Gayed. Gina is a student at Florida A&M University. She was inspired to study pharmacy by her sister who also attended FAMU. ■■ The H. Gary G. and Carolyn S. Koesten Family Scholarship, in the amount of $1,000, was awarded to Xavier Thompson. Xavier is a student at Nova Southeastern University. This year’s convention theme is UNITE PHARMACY! The profession of pharmacy is transforming and has grown from being dispensers of pills and capsules, to dispensers of knowledge and services. Pharmacists today do Medication Therapy Management, Chronic Disease Management, Diabetes Education, Prevention and Wellness Services, and other patient-education services. As a result, there has been better patient care! Continuing education courses will be offered for consultant and general licenses, as well as pharmacy technician registration. The convention will address current and emerging topics that will keep you abreast of the constant change in the practice of pharmacy. The following educational courses offered will

Originally from Clearwater, Florida, Dr. Buffington did his under graduate work in biochemistry at the University of South Florida in Tampa, Florida. He received his Doctor of Pharmacy (PharmD) and Masters of Business Administration (MBA) degrees from Mercer University in Atlanta, Georgia. He also completed his Clinical Pharmacy Practice Residency and Clinical Pharmacology Fellowship at Emory University Hospital in Atlanta, Georgia. Dr. Buffington is nationally recognized for the development of a private “Clinical Pharmacology” specialty practice model that provides Medication Therapy Management (MTM) consult services, a clinical research trial center, a national drug information center, and forensic pharmacology service. The practice also services as an active training site for pharmacists, physicians and nurses and is affiliated with several different colleges of pharmacy and medicine. His practice focuses on patients who take chronic or high-risk medication therapy and provides a common resource point between patients and their various prescribers to optimize therapy and reduce risks of adverse effects and drug interactions. He has served on many different state and national committees, task forces, and organization panels in both medicine and pharmacy organizations. He currently serves on the national collaborative called the Pharmacists Services Technical Advisory Coalition (PSTAC) and the represents all pharmacists on the American Medical Association (AMA) Current Procedural Terminology (CPT) Editorial Panel and helped to develop the national billing codes that describe pharmacists’ services.

Specific courses being offered are Reducing Medication Errors, HIV/AIDS, and the always-popular offering of Florida Legislative Update. Students and Technicians will benefit from interacting with practicing pharmacists and attending student and technician focused continuing education programs. The technician track offers several hours of continuing education on a variety of topics, including the required courses for Florida registration and PTCB renewal: Medication Errors, Pharmacy Law and HIV/AIDS for initial renewal. The student track consists of several hours of fun and exciting continuing education, such as The Career Forum and the NASPA/NMA Game Show. Students will also have an opportunity to participate in the Patient Counseling Competition and showcase their Poster Presentations.

Our traditional Wednesday program will focus on Diabetes Management. Diabetes mellitus is a chronic disease associated with abnormally high levels of blood glucose. Pharmacists play a vital role in the management and patient education of diabetes. The program will focus on the most current information evolving in the world of diabetes. The diabetes program will offer 6 hours of live continuing education credit. Register for this outstanding program and walk away with valuable knowledge for you and your diabetic patients. Drowsy Driving: Asleep at the Wheel Source: Centers for Disease Control and Prevention

Prevent Drowsy Driving, Before Taking The Wheel ■■ Get enough sleep! According to the National Institutes of Health, adults need seven or eight hours of sleep a day, while adolescents need nine or 10 hours. ■■ If you have a sleep disorder, make sure to seek treatment. ■■ Refrain from drinking alcohol or taking sedating medications before driving.

Pharmacists and pharmacy technicians work long hours, work late shifts and are vulnerable to dozing off while driving home. While falling asleep at the wheel is clearly dangerous, being sleepy affects your ability to drive safely even if you don’t fall asleep. Drowsiness: ■■ Makes drivers less attentive ■■ Slows reaction time ■■ Affects a driver’s ability to make decisions The Scope of the Problem Although it may be difficult to attribute a fatal vehicle crash to drowsy driving, the National Highway Traffic Safety Administration estimates that 2.5 percent of fatal crashes and 2 percent of injury crashes involve drowsy driving. These estimates are probably conservative, though, and up to 5,000 or 6,000 fatal crashes each year may be caused by drowsy drivers.

Who’s More Likely to Drive Drowsy? ■■ Commercial drivers ■■ Shift workers (work the night shift or long shifts) ■■ Drivers with untreated sleep disorders such as sleep apnea ■■ Drivers who use sedating medications ■■ Drivers who do not get adequate sleep How Often Do Americans Fall Asleep While Driving? Among nearly 150,000 adults aged at least 18 years or older in 19 states and the District of Columbia, 4.2 percent reported that they had fallen asleep while driving at least once in the previous 30 days. Individuals who snored or usually slept six or fewer hours per day were more likely to report this behavior.

Drowsy Driving and Alcohol Cognitive impairment after approximately 18 hours awake is similar to that of someone with a blood alcohol content (BAC) of 0.05 percent. After about 24 hours awake, impairment is equivalent to a BAC of 0.10 percent, higher than the legal limit in all states. In addition, lower levels of alcohol (below the legal limit) amplify the effects of inadequate sleep. The Warning Signs Of Drowsy Driving: ■■ Yawning or blinking frequently ■■ Difficulty remembering the past few miles driven ■■ Missing your exit ■■ Drifting from your lane ■■ Hitting a rumble strip If you experience any of these warning signs, pull over to rest or change drivers. Simply turning up the radio or opening the window are not effective ways to keep you alert.

General Overview of the Florida Statewide Medicaid Managed Care (SMMC) Managed Medicaid Assistance (MMA) Program By John Lockwood, FPA General Counsel I n 2011, t he Florida Legislature created the Statewide Medicaid Managed C a r e (S M MC) p r o g r a m . Th e SMMC program is managed by the Florida Agenc y for Hea lt h Care Administration (AHCA) and includes the Managed Medical Assistance (MMA) program. The MMA program is an expansion of the original Medicaid pilot program, limited to five counties, and allows certain health care organizations to manage how plan participants receive health care services. In addition, the MMA program is unrelated to the Affordable Care Act. Beginning in 2012, AHCA solicited competitive bids from insurers for the statewide MMA program. On Sep-

tember 23, 2013, AHCA announced the award of multiple contracts to HMO’s, PSN’s and other health plan providers. These plan providers are specific to 11 regions throughout the state. It is anticipated that AHCA will finalize contracts with these plan providers, and individuals are tentatively scheduled to begin enrollment in April 2014. Once fully implemented, it is estimated that 2.9 million Medicaid recipients will enroll in the program. Individuals who must enroll in a health plan will be notified as such by letter, have 30 days to select a plan and then have another 90 days to change their selection if they so choose. If mandatory participants do not make a selection within 30 days, AHCA will choose a plan for them. Plan selections for mandatory enrollees are then set for a 12-month period. Medicaid recipients who have the option to enroll will also be informed by letter and can drop in

or out of the program at any time. All program enrollees can change their Primary Care Provider at anytime, so long as the provider is within their chosen plan. It is possible that some of the MMA program’s health plans will have closed provider networks. Thus, it is possible that certain individuals may be unable to continue using their current health care providers, including pharmacists, if those providers are not within the chosen plan. The Florida Pharmacy Association is currently reviewing what steps may be taken to reduce the potential negative impact upon pharmacists. It is imperative that Medicaid recipients are educated as to their rights to select a health plan providing the greatest flexibility and allows continued use of their existing health care providers.

What do you wish everybody knew about pharmacy?

I appreciate the networking opportunities I have through the FPA and the assistance I get from FPA staff whenever I have pharmacy-related questions.

I am a believer that all should know that we are highly educated, ethical individuals who care about our patients. They also should take into account how accessible we are.

What are some thoughts you have on pharmacy as a profession right now?

Employers seem to be heading in the wrong direction with their “attitude� toward the pharmacist. Declining reimbursement, increased volume and additional administrative tasks that pharmacists perform these days could have an impact on quality of patient care. With the pending changes fostered by the Affordable Care Act, it is critical that there be an open and frank dialogue between pharmacy managers and administrators and front line pharmacist caregivers. As for the profession, we are at a turning point. We need more recognition and partnership in the health care arena.

What are you most excited about regarding the future of pharmacy?

I am excited about pharmacists potentially being recognized as health care providers. To me, this is recognition of our status as colleagues and equals among the other health care provider groups.

As a teenager I worked for an independent pharmacy where I discovered that I loved helping and serving others.

What do you think your patients would say about you and your practice? My patients would comment that I give great service and I am very knowledgeable about healthcare.
