A

The Time for Urgency is Now!

s I write this, I am on a plane heading home from the American Pharmacists Association (APhA) convention in Los Angeles. At every turn during this convention, the topic of discussion for pharmacy practitioners was gaining recognition for pharmacists as health care providers. Obtaining “provider status” is in fact the highest legislative priority for APhA and is likewise our legislative priority in Florida. At the House of Delegates, where you, the members of the Florida Pharmacy Association, were represented by nine delegates from FPA leadership, the items for discussion again dealt with pharmacist-provided patient care services and empowering the pharmacist to take on new roles on the health care team. Some of these roles include managing chronic diseases, filling the ever-growing primary care gap, involvement in wellness and disease prevention initiatives and the like. We’ve been talking about this in Florida for a number of years. We seem to have gotten this in Florida a few years ahead of the pack. As you know, this year’s Florida Pharmacy Association theme is “Join the Evolution Revolution … Transform the Practice of Pharmacy.” Revolution implies a sense of urgency, and evolution implies change. In fact, I believe it is urgent that we change NOW. Folks, I know that change is difficult and scary. It is for all of us. I also know that pharmacists throughout history have changed, adapted and stepped up to solve the medication-related needs of society. We have done this since the beginning of time. We are here again at one of those

historical junctures in our professional evolution, and we have to decide NOW if this will be pharmacy’s “Ice Age” or “Golden Era.” Will we flourish and thrive or go the way of the dinosaur? The landscape is changing and we MUST change our practice or change will occur around us and we will be left out. We must do different things or do things differently, but one thing is certain, change is at our door step and

To do these things that we must do, we must communicate to our legislators (who are usually the last to know) what our patients keep asking us to do, to help tend to their health care needs. Don’t feel that the change has to be drastic. It may be simpler than you think. We as pharmacists do many of these things now but have been terrible at documenting the care we pro-

vide daily. Don’t feel that you have to undertake this change alone; we can change together. Use the resources that the FPA, your association, has and makes available to you to facilitate that change that MUST occur. I will also share with you that we can no longer ask others to do what we must do for ourselves. Then again, if you are reading this article, I am preaching to the choir. However, I am going to ask a little more of you. I am going to ask that YOU give a little more. A little more of your talent, your time and your money and that you reach out to ONE non-member and “convert them.” What we are embarking on is a monumental task and CANNOT be accomplished without each of us taking an active role. I know it hurts, we would probably all rather be at the beach, but we can no longer ask others to do what we must do for ourselves. MARCH 2013

We have students who had midterm exams (remember those, the tough ones?), who could have gone home to be with their families, and we had “broke” students (their student loans keep piling up), who could have put in a few hours to help pay for those loans, but instead they CHOSE to be a part of Legislative Days in Tallahassee. So I’m going to ask you, point blank, to “bleed” a little. We need YOU to take responsibility for YOURSELF and give a little of your talent, a little of your time and a little of your money. As I said earlier, we are asking a lot of the legislators this year, and this takes all of the above from YOU. I ask that each of you support the FPA Political Action Committee (PACCE) by donating $25. If $25 is too much, we’ll take $10 (heck- we’ll take anything). If $25 is too little, we’ll take $100. $100 is better than $25 and $25 is better than $10. But something is better than nothing.

What is interesting is that there is a desire to change public policy on pharmacy services such as dealing with issues related to how PBMs influence pharmacy services and whether a pharmacist could be recognized as a health care provider. We have been monitoring efforts by Florida’s optometrists to modify the types of services they can deliver to patients. A bill is currently moving (very nicely I might add) in the Legislature that would expand their prescriptive authority and improve patient access. The common denominator that we see is their support of their political action committee. Last year, the optometrists’ political action committee raised nearly $200,000. This does not include the direct contributions that their stakeholders made to candidates. In 2012, FPA members voluntarily provided just under $31,000 in support to our PACCE. This means that our profession contributed about $1.14 per pharmacist to support the campaigns MARCH 2013 |

Actually, the support for the PACCE in 2012 came from about 270 FPA members rather than 27,000 pharmacist licensees, which means that the profession is relying on 1% of us to help shape public policy decisions affecting our industry. If you see Merritt Island’s Mark Hobbs and Fort Meyer’s Reggie Mathai at an FPA meeting, thank them for their generous support of the Florida Pharmacy PACCE. They are true believers of the movement and want to revolutionize the profession of pharmacy.

Our profession must evolve to survive, and it will take a revolution to make that happen. Each of us as FPA members know a friend or colleague that is not a member of our association. If you have been reading our Stat News messages and looking at our news feed on Pharmview. com you know that there is no shortage of issues. Introduction As student pharmacists are nearing completion of their program, they are excited to embark on the next chapter of their lives as pharmacists. However, the daunting task of applying for pharmacist licensure awaits. The purpose of this paper is to review the process for applying for licensure in Florida for individuals graduating from a College of Pharmacy within this state to facilitate a smooth transition from student to practitioner. The process consists of 2 distinct steps that ultimately lead to receipt of the Authorization to Test (ATT). The author assists rotation students with navigating the application process at the College of Pharmacy. Step 1: Registering for the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE) Pharmacists must pass both the NAPLEX and MPJE to become licensed in Florida. The first step in the application process is making online payments for these examinations through the National Association of Boards of Pharmacy (NABP).1 The cost of the NAPLEX and MPJE are $485 and $200 respectively.1 Students should make their online payments via credit card prior to graduation to expedite the process. The NAPLEX/MPJE registration bulletin is a very helpful resource available for free through the NABP website.2 Step 2: Completing the Florida Board of Pharmacy Application The application is available through the Florida Board of Pharmacy and was recently updated July 2012. The application can be located

at the following link: http://www. doh.state.fl.us/mqa/pharmacy/ap_ ph-exam.pdf .3 Also, the application should be submitted approximately one week prior to graduation. Students should begin completing the application approximately four weeks prior to graduation to ensure that the information is submitted in a timely manner. The Florida Board of Pharmacy does not begin reviewing the application until after the graduation date and the Form A is received. The completed application must be submitted via U.S. postal mail to the following address: Florida Board of Pharmacy P.O. Box 6320, Tallahassee, FL 32314-6320. Additionally, page 7 of the application includes a helpful checklist that should be reviewed to ensure that all of the required information is submitted. With the application, the following should be submitted: photocopy of the Social Security Card, a 2 x 2 photo, check made payable to the Florida Department of Health for $295, and a 2 hour medication errors course completion certificate. Item #1 the Social Security Form (page 1) and Item #2 Pharmacist Examination Application (pages 2-6) must be completed. Students complete Part I of Item #3 Certificate of Pharmacy Education Form A (page 7), and the College of Pharmacy Dean completes Part II. The College of Pharmacy directly sends Form A to the Florida Board of Pharmacy upon the students’ graduation. Form A will not be accepted if it is submitted by the student pharmacist. Items 4 and 5 are only required for pharmacists applying from outof-state.3 Once the Florida Board of Pharmacy determines that an individual is eligible to take the NAPLEX/MPJE, they notify NABP. 2 Next, an ATT letter is sent via email with instructions to schedule a testing appointment. Testing appoint-


ments cannot be scheduled until individuals receive their ATT.2 NAPLEX Score Transfer As a new pharmacist, deciding where to live can be an exciting, yet challenging task. The NAPLEX Score Transfer program simplifies and consolidates the process of applying for licensure in multiple states.2 In order to use the NAPLEX Score Transfer program, candidates must register for the NAPLEX using the online application. During this process, a primary state or jurisdiction of licensure must be chosen. Once fully registered, there are three opportunities to make a score transfer request. This includes the initial online registration, the time prior to sitting for the exam through online registration and, finally, up to 90 days after taking the exam through online registration and clicking on the Transfer Score After Exam tab. Additionally, the score transfer fee is $75 per jurisdiction.2 In order for the score to be successfully transferred, candidates must complete the examination requirements from the primary jurisdiction for licensure.2 If all the requirements of the primary jurisdiction are not completed, a score transfer may not be allowed. Also, it is important to note that only the NAPLEX score is transferred. Individuals must complete all requirements of the score transfer jurisdiction including the state’s MPJE. The NAPLEX Score Transfer program provides an easy means of planning for the future and keeps all career opportunities open. There are certain states that do not accept license transfers from Florida. Therefore, individuals wishing to become licensed in those states in the future can utilize the score transfer state to transfer their license. Additionally, all 50 states, the District of Columbia, Guam, Puerto Rico, and the Virgin Islands currently participate in the

Florida Pharmacy Foundation Announces the Alton Tower Philanthropy Award The Board of Trustees, at a recent meeting, voted to honor Foundation Board member Al Tower with an award in his name -- the Florida Pharmacy Foundation Alton Tower Philanthropy Award. A cash award of $500 will be presented to the winner’s choice of charity/organization and a plaque will be presented to the winner annually at the Florida Pharmacy Association’s awards night, during convention. The word “philanthropy” means to give of oneself, which describes the life of Al Tower. Al has given so much to the profession of pharmacy and has reAL TOWER ceived the R.Q. Richards Award, James H. Beal Pharmacist of the Year Award, Sidney Simkowitz Pharmacist Involvement Award, Jean Lamberti Mentor Award, IPA Corrons Inspiration & Motivation Award, U.B. Willis Gregory Award, Abilities Guild in Community Serving Award and the Bowl of Hygeia Award. He has held leadership roles in many organizations including the Pinellas County Pharmacy Association, Florida Pharmacy Association, Christian Pharmacists Fellowship International and the American Pharmacists Association. Al is very active in his community, serving on the boards of many healthcare organizations such as the American Cancer Society, Future Scientists of America, and Florida Tobacco-Free Coalition. He also has been a career speaker in public schools. As the Chair of the Florida Pharmacy Foundation Fundraising Committee, Al set a goal for the Trustees to raise $300,000 for scholarships. The plan was that the interest earned on the endowment would fund new scholarships for pharmacy students. Thanks to Al, and all of you who have participated in the Foundation’s fundraising activities, the Foundation has reached half of its goal. FPA Officer and Director Nominations Although we have just finished the election for a president-elect and directors for the odd numbered regions to be installed at the 2013 annual meeting, it is time to start thinking about nominees for the 2014 election since the nomination deadline is September 1 of this year (9/1/13). As the form below indicates, this year we will need candidates for president-elect, treasurer, and directors for the even numbered regions. Please note that you may nominate yourself. CALL FOR FPA OFFICER AND DIRECTOR NOMINATIONS for 2014 Elections The FPA By-Laws specify that any subdivision or any member in good standing may nominate one person for the office of President-Elect and one person for the office of Treasurer. A President-Elect shall be elected every year and shall assume the duties of the President on the last day of the annual meeting of the year following election as President-Elect. The treasurer shall serve a two year term and may succeed to one consecutive term of office in that capacity. Nominees must be Florida registered pharmacists in good standing with the Florida Pharmacy Association and the Florida Board of Pharmacy. Nominees for president-elect should have a good understanding of how the Association functions and should be current on the issues impacting pharmacy. Nominees for treasurer should have good analytical skills and experience and ability in financial management and budget preparation. There are nine regional Board Directors who shall serve two year terms. Nominees must be a Florida registered pharmacist in good standing with the Florida Pharmacy Association and the Florida Board of Pharmacy. Additionally, Board Directors must be a member of at least one the FPA Unit Associations within their region. Board Directors terms are staggered such that even numbered regions shall be elected in even numbered years and odd numbered regions shall be elected in odd numbered years. All newly elected Board of Directors Regional Directors shall take office on the last day of the annual meeting, and shall continue in office until the last day of annual meeting of the second ensuing year.

What do you value most about your FPA membership? I value being part of a proactive pharmacy association that has worked diligently to shape our profession.

What are some thoughts you have on pharmacy as a profession right now? Pharmacy is a profession in a state of flux. With computerized physician order entry, centralized order entry and centralized fill, we have the ability to truly practice our profession. We need to embrace MTM, disease state management and expand our patient counseling opportunities.

What are you most excited about regarding the future of pharmacy? I am excited about the development of our scope of practice. Via our scope of practice, we can petition for our position as a health care provider. Acknowledgement by all for the impact we make in patient care.

What do you wish everybody knew about pharmacy? The clinical aspects of our profession. Many of our patients do not realize how we safeguard them by ensuring appropriate doses and appropriate medications are prescribed—also ensuring interaction-free medication use.

What got you interested in pharmacy in the first place? I always had a desire to be part of health care administration. In the late ‘70s, it was becoming evident that physicians were struggling with the pharmacology, kinetics and pharmacodynamics of the medications they were prescribing, providing me the opportunity to fill this void.

What do you think your patients would say about you and your practice? They would say I am always available to explain and answer their questions. They would say I am a dedicated professional who goes out of his way to safeguard patients— a trusted healthcare professional who strives to provide the most clinically focused hospital pharmacy practice.
