WHAT IS OPEN ENROLLMENT? Open Enrollment is your annual opportunity to enroll, continue, or change your medical, dental, flexible spending, and short-term disability benefits. Changes to other benefits, including voluntary plans and retirement programs, can be made now, but may also be changed at any time during the year. WHY SHOULD I PARTICIPATE IN OPEN ENROLLMENT? Only you can determine which ’Canes Total Rewards benefits best meet your needs. By participating in Open Enrollment, you can be sure that you and your family are enrolled in the right benefits for next year. HOW DO I ELECT MY 2015 ’CANES TOTAL REWARDS BENEFITS? 1. During Open Enrollment, login to myum.miami.edu. 2. Click the “Employee” tab at the top of the page. 3. Click “Benefits.” 4. Verify Personal, Address and Emergency/Evacuation Information. 5. In the center of the page under Benefits, click “Open Enrollment Summary.” 6. Under Benefit Plans, elect your benefits and verify smokers’ and spousal surcharge information by clicking the active links. 7. Click “Submit” to save any changes. 8. Print a confirmation page and keep for your records.

What if I don’t participate in Open Enrollment? If you are currently enrolled in a UM/Aetna medical plan and do not make a medical plan election and update your smokers’ and spousal surcharges for 2015 during Open Enrollment, you and your covered dependents will be placed in the Aetna HRA medical plan and smokers’ and spousal surcharges will apply.

WHAT’S STAYING THE SAME?
Minimal increases to health care premiums.
Same three medical plans. Aetna Select 1, Aetna Select 2, and the Aetna Health Reimbursement Account (HRA).
Aetna Select 1 and 2 out-of-pocket maximums will now include prescription copays to help reduce overall costs for participants. Generic Choice incentive program for UM/Aetna members. Save money and avoid higher out-of-pocket costs by choosing generic medications. See page 12 for details. Rx Step Therapy program for UM/Aetna members. Save money by trying less expensive, but equally effective medications, before trying higher-priced alternatives. See page 12 for details. Minimal increase to some prescription drug copays. Generic drug copays will remain at $10. The copay for tier 2, brand preferred drugs, will be $45. The copay for tier 3, non-preferred brand drugs, will be $75. See page 12 for details. Behavioral health benefits managed by Concordia Behavioral Health. Current benefits, copays, provider networks, and behavioral health services are not affected by this transition. Pre-authorization may be required. See page 16 for details. Smokers’ surcharge change. For adult smokers covered under the UM/Aetna medical plan, the smokers’ surcharge will increase to $100 per smoker, per month. Spousal surcharge change. For those who cover a spouse/same-sex domestic partner who has access to his/her own employer-provided medical plan, the medical plan’s spousal surcharge will increase to $250 per month. Maximum elections for dependent care flexible spending accounts will remain at $5,000/family/year, with a cap of $2,500 for employees earning $115,000 or more. See page 13 for details. HealthyNow mobile app allows you to manage your Well ’Canes Incentives Program account from your mobile device. See page 19 for details. The Women’s Hospital at Jackson Memorial welcomes “Little Miracles Maternity Suites,” a newly renovated maternity floor featuring 20 private suites with leading-edge equipment in a spa-like atmosphere. A UM physician, UM facility, or Jackson Health System facility. You have the option of using an out-of-network provider, but your charges will be subject to balance billing. The first $600 of eligible medical and Rx expenses are covered by the HRA fund with the exception of Concordia copays. Preventive care services will not be deducted from the $600 HRA fund. Inpatient hospital services include semiprivate room, ancillary service, physician fees/visits, and inpatient surgery. Emergency room copay is waived if admitted to the hospital. Coverage for high-end imaging (MRI, PET, and CT scans) is available exclusively at UHealth. Coverage is available outside of UHealth under certain circumstances: Imaging for children ages 13 and under; imaging performed outside of Miami-Dade/Broward counties, within an emergency room or during an inpatient hospital stay, concurrent with radiation therapy for daily placement, when open or standing MRI is required.
Out-of-Area HRA Plan: If your home address is outside of Miami-Dade/Broward counties, the Out-of-Area HRA option will appear on myUM as an option for you. UNDERSTANDING THE AETNA HRA OPTION The Aetna Health Reimbursement Account (HRA) is a high-deductible, low-premium medical plan that provides cost-effective benefits for you and your family. To help off-set your total deductible, the University provides you with an annual HRA fund of $600 per person, up to $1,800 per family. When medical services are received or prescription drugs are filled, amounts are deducted automatically from your UM-provided HRA fund first. Once the fund is used up, you will make out-of-pocket payments until you meet your individual deductible. If you meet your deductible during the plan year, you pay copays or coinsurance for subsequent services and prescription drugs. If you end up with unused HRA funds, they will roll over to the following year.

PRESCRIPTION DRUGS Each UM/Aetna medical plan covers the same prescription drugs. Costs are determined by the four-tier structure below. If the cost of the prescription is less than the copay, you pay the lesser amount. In the HRA plan, copays apply after you have met your deductible. Drugs may move up or down a tier in 2015. NEW! Rx STEP THERAPY The UM/Aetna medical plan covers thousands of medications. Some of these medications have equally effective, but much less expensive, alternatives. The Rx Step Therapy program gives you options regarding these medical conditions. Directions Soak the cracked wheat for three hours in water and lemon juice to cover. Wash, dry, and chop parsley and mint finely. Chop mint separately (preferably by hand, not food processor). FREE FINANCIAL SECURITY BENEFITS The following free financial security benefits can help you protect yourself and your family in the case of accident, illness, or disability:
Life Insurance equaling two times your base salary, rounded to the nearest $1,000, to a maximum of $100,000. You are covered immediately upon employment. Accidental Death & Dismemberment (AD&D) equaling one time your base salary, rounded to the nearest $1,000, to a maximum of $100,000. Long-Term Disability (LTD). If you become disabled, there is a sixmonth elimination period before your LTD benefits begin to pay. You will be eligible for a monthly benefit equal to 66 2/3 percent of your base annual salary up to a maximum of $10,000. SERVICE AWARDS The University of Miami annually recognizes faculty and staff who celebrate service milestones and those who have proudly retired from the University.
The Long Service Award Program provides special recognition to faculty and staff for their years of service to the University. More than 800 employees who have reached a service milestone are honored each year and recognized with a certificate and a gift based on milestone year. Service levels are recognized in 5-year increments, starting with 10 years of service. The Retirement Recognition Award recognizes faculty and staff for their retirement from the University of Miami with a gift to mark the University’s appreciation for their service and special contributions.
