PRIMARY CARE INITIATIVE PROGRAM Frequently Asked Questions Reason for the Program: People with mental illness die up to 25 years too soon due to preventable health conditions.

Who benefits? All persons with mental illness who currently have or are at risk for heart disease, diabetes & obesity.

What services are provided? Blood sugar and blood pressure monitoring Diabetes education Exercise education, training and support Health screening and education Nutrition education and meal planning Peer Support Relaxation training Smoking cessation education and support Culturally sensitive and individually adapted

When and where are services are provided & by whom? Access services Monday-Friday at Strawhun Center and soon to be at Stark Center. Nurse Care Managers, Case Managers, RN’s, Peer Support Specialists, Registered Dietician, Diabetic Educators, & Primary Care Providers.

Regional Primary Care Initiative now offers a nationally recognized program to bring needed primary medical services to our clients, right in our Merrillville location and coming soon to our East Chicago location. General medical care at the Regional office includes assistance for clients in:. Medication management, healthy eating, stress management, healthy activities, stopping smoking, help with negotiating the medical system.
