TRAINING CLINIC GUIDELINES • Student-Athletes MUST sign in each visit to the training room • Do NOT wear cleated shoes in training room • No FOOD or SODA POP in training room • Student-Athletes must sign for any medications taken • All scissors STAY IN training room • After practices and events, athletes should shower, then report for ice and treatments • Rehab equipment MUST be signed out • Training room office and storage area are off limits to student-athletes • Rinse out coolers and leave lids off • When traveling, please return team medical kits and coolers immediately so they can be restocked before next event • Rinse out coolers and leave lids off

Guide For Student-athletes


ATHLETIC INJURIES • All injuries must be reported to the coach and athletic trainer immediately! Student-athletes must have their current primary insurance card available and all insurance information must be on file at the Campus Wellness and Health Center and Athletic Training Clinic. • The athletic trainer will complete the “Injury Report” form that states the date, description of the injury, practice or game, and treatment. This is kept in the student-athlete’s athletic medical file. • The student-athlete (not the doctor or hospital) must submit a fully completed secondary intercollegiate athletic claim form within three days of an accident. Only one form is needed for each athletic injury. This form is to be filled out by the student-athlete with assistance from the athletic trainer within 72 hours after injury. • The intercollegiate athletic insurance claim form must be signed by an authorized athletic administrator. The form will then be sent to the Claims Administrator. • The student-athlete will be give a secondary insurance care to present along with their primary insurance card for all appointments related to the athletic injury. Athletes must show their GC student ID card. • Student-athletes may need to submit EOB’s and itemized bills to athletic administrator if requested. • Please keep a copy of all medical related records.

MEDICAL PROTOCOL Each year, each student-athlete must have an athletic pre-participation exam (PPE) form on file in the Wellness and athletic training centers and an insurance form on file prior to participation in intercollegiate athletics. Student-athletes are encouraged to get their PPE done within their network health care provider on the GC PPE form. Returning athletes should schedule in June for August appointments with Goshen Family Physicians clinic. All first year and transfers must have on file with the GC Wellness and Health clinic the following forms: Confidential health form with immunization record, Proof of insurance coverage or Student Health Insurance enrollment form. ALL Student-athletes MUST have: PreAthletics participation form, Student-athlete information form, insurance information, HIPAA authorization form, medical policy acknowledgement form, asthma action sheet, and training room information sheet. These forms must be completed before the first day of practice and will affect your eligibility for competition. Returning Student-athletes are required to have a yearly athletic physical. For more information please contact your specific coach. Student-athletes must carry a Primary Insurance plan to attend Goshen College. The Athletic Department carries a Secondary Insurance policy for its student-athletes which covers services their primary coverage does not. This policy covers injuries sustained during sanctioned practices or intercollegiate athletic participation only.

GC SPORTS MEDICINE Program Goal: to provide a link between the athletic program and the medical community for the implementation of injury prevention, emergency care, and rehabilitation procedures. No one healthcare professional has sufficient knowledge to handle all needs that arise: it takes a team approach. At Goshen College this team includes a state licensed, NATABOC certified athletic trainer, team physician, a network of orthopedic surgeons, family practitioners, sports psychologist, and a student athletic training staff. Goshen College is fortunate as to be located across the street from Goshen General Hospital and is located close to several medical professional practices.

INSURANCE INSTRUCTIONS FOR INJURIES To avoid processing delays, please follow all instructions: 1. Carry your insurance card (info) at all times. 2. You MUST REPORT your injury to your coach and athletic trainer within 72 hours. 3. Pick-up athletic-injury claim form from Athletic Trainer or coach if traveling.

ASTHMA ACTION PLAN All student-athletes who use inhalers for asthma must complete an Asthma Action Plan form in the athletic training clinic. Please complete this form before your first workout when possible or ASAP.

4. Follow All Instructions On Back Of Claim Form. 5. Have Assistant Administrator sign and process form. 6. Follow-up: All bills( EOB’s) must be submitted within 90 days of the date of SERVICE.
