M.D. FACS VASCULAR SURGEON What are Varicose Veins?
Varicose Veins are prominent bluish bulges frequently observed in the lower extremities. These visible unsightly veins are nonfunctional and are no longer contributing to the circulation of the leg. The bulging of these veins results from stagnant blood often due to larger underlying diseased veins with faulty valves, allowing the blood to back up instead of returning to the heart. This problem can be diagnosed with simple painless sonographic studies. Not only are varicose veins a major cosmetic concern, but as they enlarge, they can produce symptoms such as pain, itching, heaviness, bleeding, ulceration and associated swelling. Early diagnosis and treatment can prevent this.
Why should a person with Varicose Veins (large or small) seek treatment?
A person with Varicose Veins may have cosmetic or medical (symptomatic) concerns. I recommend the patient at least have a simple evaluation to clarify the source and extent of the problem as well as treatment options and prognosis. Bulging veins often produce pain, itching, and heaviness. If there is an underlying abnormal feeding vein, a patient may develop discoloration, a rash, and skin sores. Localized clot can form in these veins with redness and tenderness. If diagnosed and treated appropriately in early stages, potential irreversible skin changes can be avoided.
What are the treatment options for spider and varicose veins? The first step is to determine the underlying source of the visible veins with a proper physical examination and sonographic survey of the deep and superficial venous circulation. If visible veins are treated without first eliminating the source, the results will be poor. Options to treat poorly functioning nonvisible superficial veins causing varicose veins include Endovenous Laser Ablation, Radiofrequency Closure, Ultrasound Guided Sclerotherapy and at times various surgical techniques for more complicated situations. "Surface" varicosities are best addressed with either injections (Sclerotherapy) or Ambulatory Phlebectomy (removing large bulging veins through tiny punctures) Advanced sclerotherapy techniques seem to work better than laser treatments for spider veins.
I have had gradually worsening Varicose Veins over the past few years. Is it necessary for me to have medical treatment for my Varicose Veins? Varicose Veins are not a life or limb threatening problem; therefore, treatment is not "absolutely" required. I explain to my patients the potential associated problems such as discomfort, itching, rashes, discoloration, swelling, skin sores, and bleeding. If a patient elects not to have a procedure, I advise that they diligently wear compression hose and call me if they should develop one of the above mentioned complications. I recommend at least annual routine follow up.

Mark Homra, M.D. AJ Duboy, M.D. Quick bio of the physicians with the McIver Men’s Health Clinic: Dr. Homra is a Kentucky native who obtained his undergraduate degree from Vanderbilt University and graduated from the University of Louisville Medical School, where he completed his Urology residency. He has lived in Jacksonville since 1997 and has been Chief of Urology, as well as serving as the President of the Medical Staff at St. Vincent’s Southside. He is board certified by the American Board of Urology. Dr. Duboy is a Florida native who comes to us from Lakeland, FL. He obtained his undergraduate degree from the Wake Forest University and his medical degree from the University of Florida College of Medicine. Dr. Duboy completed his Urologic training at the University of South Florida College of Medicine in Tampa, Fl where he was fortunate enough to train under Dr. Rafael Carrion who is internationally recognized as a leader in sexual medicine and men’s health. Dr. Duboy is currently a member of the Sexual Medicine Society of North America. What is Erectile Dysfunction? Erectile dysfunction is also referred to as impotence. This is a condition where a man has difficulty achieving OR maintaining and erection. It affects at least 30 million men to some degree. It’s most common in men 40 years and older but experts believe statistics regarding this condition are not completely accurate because many men do not report it or seek help. Unfortunately, erectile dysfunction can lead to relationship problems, as well as depression and distress in men. ED can also be a symptom of larger health problems, particularly cardiovascular disease and diabetes, that would need evaluation and treatment from a physician. Other factors that can increase the risk of ED include: smoking tobacco, excessive alcohol consumption, obesity and a sedentary lifestyle. DIAGNOSING ERECTILE DYSFUNCTION Your doctor needs to examine your sexual, medical and psychosocial history to determine possible sources of the dysfunction. In addition, a physical exam and thorough review of the patient’s medications would need to be performed. A prostate exam and possible laboratory work up may also be warranted. TREATMENT OPTIONS There are multiple treatment options available for erectile dysfunction. These range from oral medications; one of which has an affordable generic and is available to be dispensed from our office. Other treatment options range from minimally invasive therapy to surgical management. WHY IS IT IMPORTANT TO HAVE A UROLOGIST EVALUATE AND TREAT ERECTILE DYSFUNCTION Evaluation and treatment of erectile dysfunction can be, often times, covered by insurance. Only urologists can offer the full range of treatments. Treatment management can be tailored to each patient’s specific need. We are not a “one size fits all” practice. McIver Clinic has been innovators in urologic care since 1921. We also offer robotic surgery, treatment for prostate, kidney and bladder cancer, as well as stone disease management and urinary tract infections. We also have a board certified Urogynecologist that is fellowship trained. He provides treatment to our male and female patients that suffer from incontinence, pelvic pain and other genitourinary conditions.
