I am thrilled to introduce the third edition of the Aging Life Care Association’s (ALCA) Mid-Atlantic Resource Directory. The 2019 edition is unique because it contains an expanded directory of elder law attorneys across the Mid-Atlantic Region! We value our partnership with members of the National Association of Elder Law Attorneys (NAELA). The elder law attorney/aging life care manager relationship is a long-standing one and in part, their partnership with ALCA is responsible for the growth of our profession. The Chapter is grateful to Steve Gurney and The Positive Aging Sourcebook for their support in producing this guide. The Mid-Atlantic Chapter of ALCA encompasses Virginia, Maryland, D.C., Delaware, Pennsylvania West Virgina and is the 4th largest of 9 regional chapters of ALCA. The guide is an important resource both to our members, the families we serve, and those with whom we have close professional relationships. Our members are nurses, social workers, gerontologists, therapists, most of whom are certified care managers at the two highest levels of membership: Advanced Professional and Professional. An Aging Life Care Manager, formerly known as a geriatric care manager, acts as a guide and advocate for families who are caring for older relatives or disabled adults. Our profession offers a client-centered approach to caring for older adults or others facing health challenges. We have expertise in eight core areas which are essential to the

well-being of our clients’ lives. The core areas include Legal, Crisis Intervention, Health, Financial, Housing, Family, Local Resources and Advocacy. In order to serve our clients in these core areas, we coordinate care with skilled and non-skilled home care companies, daily money managers, senior move managers, senior communities and hospital discharge planners, to name a few. Please enjoy the expanded 2019 MidAtlantic Chapter Resource Guide in advance of our November chapter conference, titled “The Power of I”. We invite you to participate in our conference as an attendee, exhibitor or sponsor. For more information, go to our website www.midatlanticalca. org and then follow the link for the conference. Pay For Home Care
Understanding Costs Can Help With Decision Making As the decision to remain at home is becoming an increasingly popular option, it is important to understand the costs of care and your payment options while researching different home healthcare companies. Costs for companion care are covered primarily by private payments. Home healthcare must also be paid for with private funds, unless it is prescribed by a physician and provided by a Medicarecertified agency. Long-term care insurance policies may cover companion care and home health care. If you qualify for Medicaid, a government program for low-income individuals, it may subsidize some of your homecare needs. Since laws vary from state to state, you
will need to contact your local Area Agency on Aging for more information concerning government benefits. Skilled nursing care will be paid for by Medicare as long as it is determined to be “intermittent skilled care,” which includes occupational, respiratory, speech and physical therapy. To be covered, care must be ordered by a doctor. Individuals who meet income requirements may be eligible to receive skilled nursing services through Medicaid. Major medical health insurance will also cover a limited number of skilled home visits. Costs for these services vary greatly depending upon coverage by Medicare, Medicaid and private insurance. Contact your local Office on Aging or
State Department of Health to find out if there are funds from social services block grants available to cover home care costs.
Medicare and Home Care Know the coverage options
When it comes to understanding Medicare coverage, many individuals can find themselves confused by a complex web of information. This especially applies to those who want to know if they are eligible for Medicare coverage when entering a nursing home or receiving home health care. Below is some helpful information on whether you can use your home health care benefits if you have Medicare and what services and products it will cover. First, you can use your home health care benefits if all of the following applies to your situation. Your doctor must prescribe that you need medical care at home and create a care plan. The care you need has to be intermittent and in the form of skilled nursing care, physical therapy, oc-
cupational therapy or speech therapy. The home health agency must be Medicarecertified and you must be homebound, or unable to leave the home unassisted. Once you have established those requirements, it is important to understand what Medicare will actually cover, which includes the following: • Home health aide services on a parttime or intermittent basis, as long as there is a skilled need • Medical social services • Certain medical supplies • Durable medical equipment (wheelchair, walker, bedside commode, etc.) Given the services and products that Medicare does cover, it is also valuable to take note of what is not covered. Care that
is required 24 hours a day will not be covered. Other services not covered include personal care provided by home health aides such as bathing and dressing, along with homemaker services like cleaning and laundry. Also not included are homedelivered meals. If you require care that goes beyond intermittent visits for a limited amount of time, or if your needs are not covered by Medicare, an important option to consider is private duty home care. For more information on Medicare coverage, visit medicare.gov.

How to Find an Aging Life Care Manager™ Near You All Aging Life Care Managers™ have similar skill sets, but the care managers best equipped to help you are the ones in your area. Aging Life Care Managers are experts on the resources available in their various geographic regions. We have done our best to group the Mid-Atlantic Aging Life Care Association (ALCA) members into geographic regions based on the states served (Pennsylvania, Delaware, Maryland, Virginia, West Virginia and DC), but many of our members serve large areas that include several jurisdictions. For instance, some care managers in Pennsylvania also serve border areas over the state line in Delaware, and vice versa.
Regions served by the Mid-Atlantic chapter
To make sure that an ALCA member serves your exact area, you can: 1. Check their website to verify service area. 2. Go to aginglifecare.org and click on the FIND AN AGING LIFE CARE EXPERT button. For best results, particularly in more rural areas, enter your zip code and use as wide a search radius as possible, 25 miles or more. 3. Call the Aging Life Care Professional nearest your area. If they aren’t able to help you, they will direct you to another ALCA member who can!
WHY CHOOSE AN AGING LIFE CARE MANAGER? ALCA promotes the highest standards of practice. Membership in ALCA is open only to qualified individuals with specialized degrees and experience in human services, including social work, psychology, gerontology or nursing. Those who are at the Advanced Professional level of membership hold one of four ALCA-approved certifications. Only members of ALCA can call themselves Aging Life Care Managers. Aging Life Care Managers have diverse experiences, credentials and backgrounds. Many are licensed in their state in specific fields (such as nursing or social work). Aging Life Care Managers may call themselves professionals, consultants, advisors or by other titles. All care managers do not specialize in all areas of aging life care. When looking for a care manager, 4
find out his/her areas of expertise. You will want to hire someone who regularly handles clients with needs similar to yours. Aging Life Care Managers who primarily work with older adults bring more to their practice than an expertise in geriatrics. They bring a deep knowledge of aging issues that allows them and their staff to overcome the myths related to aging and focus on the problems at hand. At the same time, they have tremendous experience working with resources in your community. They are fully aware of real life problems, health and otherwise, that emerge as individuals age and the tools that are available to address those issues. They are also connected with a community of social workers, nurses, psychologists, elder law attorneys, advocates, and other elder care professionals who may be of assistance to you.

Getting Comfortable with the D-Word Death is not a dirty word n
By Sharon Dickol, BS, CMC, owner of AgeWise Solutions
Experiencing the loss of a loved one is never easy. As an Aging Life Care Manager, I often participate in the care of others who are nearing the end of their lives. Most times, our clients have the love and comfort of family surrounding them. For some, there are just a few close friends. And for a small number of clients, there is only the caring support of an AgeWise team member. Regardless of the circumstances, we strive to make these final days comfortable and dignified for the dying individual, their loved ones and those who care for them. We avoid the D-word, death, as if talking about it will somehow make
it happen all the sooner. Planning for one’s final days—and those that come after—isn’t morbid. It’s a smart move for anyone determined to die on their own terms. And, it is one of the greatest gifts a person can leave to their family and friends. What can you do to make sure you plan properly? Here are some important steps you can take: Begin to Plan While You’re Healthy Illness or injury can occur at any age, so speak with an attorney to draw up Advance Directives for Healthcare and Powers of Attorney. These legal documents
ensure your wishes will be followed and that people you trust will make decisions on your behalf if you’re unable to speak for yourself. In most states, you can prepare these important documents without an attorney. Be sure to follow the guidelines in your state. Visit caringinfo.org for more information. Share your wishes with those nearest to you. Talk about your preferences with individuals most likely to be involved in your end-of-life care. The Five Wishes document is a great tool to begin these Continued on page 47

What is Elder or Special Needs Law? Legal issues that affect people as they age and people with special needs are growing in number. Our laws and regulations are becoming more complex, and each state has different laws. Actions taken with regard to a single matter may have unintended legal outcomes. It is important for attorneys working with seniors, people with special needs, and their families to have a broad understanding of the laws that may have an impact on a given situation to avoid future problems. Elder and Special Needs Law encompasses many different fields of law. Some of these include: • Preservation/transfer of assets seeking to avoid spousal impoverishment when one spouse enters a nursing home • Medicaid • Medicare claims and appeals
22
• Social Security and disability claims and appeals • Supplemental and long-term health insurance issues • Tax planning • Disability planning, including use of durable powers of attorney, living trusts, “living wills” for financial management and health care decisions, and other means of delegating management and decision-making to another in case of incompetency or incapacity • Access to health care in a managed care environment • Conservatorships and guardianships • Estate planning, including planning for the management of one’s estate during life and its disposition on death through the use of trusts, wills and other planning documents
www.midatlanticalca.org • Mid-Atlantic Chapter • Aging Life Care Association
• Probate and administration of estates • Administration and management of trusts • Long-term care placements in nursing homes and life-care communities • Nursing home issues, including questions of patients’ rights and nursing home quality • Elder abuse and fraud recovery cases • Housing issues, including discrimination and home equity conversions • Age discrimination in employment • Retirement, including public and private retirement benefits, survivor benefits and pension benefits • Health law • Mental health law. QUESTIONS TO ASK WHEN LOOKING FOR AN ELDER LAW ATTORNEY It is important for consumers to ask questions. Some of these include: • What are the primary services provided by your agency/ business? • How many Aging Life Care Managers are in your agency/ business? • Is there a fee for the initial consultation and, if so, how much? • What are your professional credentials? • Are you licensed in your profession? • How long have you been providing aging life care™ or care management services? • Are you available for emergencies? • Does your company also provide home care services? • How do you communicate information? • What are your fees? (These should be provided to the consumer/ responsible party in writing prior to services starting.) • Can you provide me with references? The answers to your questions will assist you in determining whether a particular Aging Life Care Manager or agency/business has the qualifications important to you for a successful relationship. If you have a specific issue that requires immediate attention, be sure to inform the Aging Life Care Manager of this during the initial conversation.

Elder Law Certifications
When researching elder law attorneys, it is important to understand their special qualifications and certifications. There are several different certifications that distinguish elder law attorneys from other professionals. These certifications demonstrate that a particular attorney has the training, experience, and ability to help seniors resolve disputes and prepare for the future. A few of the certifications available to elder law attorneys are listed and described below. Certified Elder Law Attorney (CELA) Certified Elder Law Attorneys (CELAs) are attorneys who are certified to practice elder and special needs law by The National Elder Law Foundation (NELF). The CELA designation is accredited by the American Bar Association. This
prestigious designation indicates that an attorney has passed a rigorous examination, demonstrated experience with elder law matters, taken continuing education courses in elder law and provided references. Currently, there are 500 CELAs across the country. National Academy of Elder Law Attorneys Fellow For more than 30 years, the National Academy of Elder Law Attorneys (NAELA) has been the leading professional organization for attorneys specializing in elder law and special needs planning. NAELA Fellows are attorneys who have been members of the Academy for at least three years, whose careers concentrate on elder law and/or special needs planning, and who have distinguished themselves both by making exceptional contributions
to meeting the needs of older Americans and by demonstrating commitment to the Academy. Fellows are recognized by their peers as exceptional lawyers and leaders. Council of Advanced Practitioners An invitation-only group, the Council of Advanced Practitioners (CAP) includes distinguished members of NAELA. NAELA established the CAP in 2005. To be invited to join the CAP, an attorney is required to be a Member of NAELA for 10 consecutive years, receive the highest rating available for legal skills and ethical conduct, and be a NAELA Fellow or Certified Elder Law Attorney. The CAP recognizes NAELA members who are innovators in the field of elder law. For more information, visit naela.org or nelf.org.

Selecting a Senior Housing Option Retirement Communities Offer Something for Everyone Offering everything from lowmaintenance homes to luxury apartments, retirement communities in the Mid-Atlantic Region are as distinctive as their residents. Local retirees have a seemingly endless array of options to choose from when selecting their new home. However, before they can select the appropriate community, retirees and their loved ones should be sure to fully understand all the options. Learning the basic terminology used in the retirement living industry will help consumers begin to consider and compare options. The chart after this article features a list of communities that offer a variety of retirement living options. Life Plan Communities The wide range of retirement living options begins with life plan communities, also known as continuing care retirement communities. These communities generally feature care options ranging from independent living to long-term nursing care on one campus. Life plan communities usually appeal to active, healthy older adults. Residents can transfer to higher levels of care in the community as needed. Life plan communities also offer a wide array of amenities and activities. In addition to monthly fees, life plan communities usually require payment of a one-time fee called an entrance fee. The entrance fee amount depends on the level of care selected at the time of entry, type of housing unit, and the agreement type. Some agreements allow for all or part of the entrance fee to be refunded if the resident moves or passes away.
Independent Living Communities For seniors who want to leave the burdens of home maintenance behind, Independent Living Communities may be the ideal option. In addition to property upkeep, these communities also generally offer 24-hour security and activities to keep residents engaged. Independent Living Communities are similar to any apartment, condominium or single-family development, except that they provide special services including security and activities. These communities typically offer a full range of activities such as shopping trips, outings to cultural events and organized gatherings that promote socialization. Many communities also feature tennis courts, swimming pools, activity rooms and other amenities to keep residents active and engaged. Assisted Living Communities Individuals who need assistance with the activities of daily living—including eating, dressing, walking, transferring and toileting—may want to consider an Assisted Living Community. In addition to daily assistance, these communities also offer activity programs to fit the interests of almost any resident. As opposed to Independent Living Communities, health care services are available at Assisted Living Communities. Residents of assisted living generally need some assistance with at least one of the activities of daily living. They may also need transportation assistance and help with housekeeping and laundry, all of which are typical offerings. Additional amenities can include private units, stateof-the-art facilities, beautifully decorated common areas, barber shops and beauty parlors, pharmacies, physical therapy services, recreation rooms, libraries, gar-
dening areas, fitness centers, and many others. Nursing and Rehabilitation Centers Finally, whether for a short-term rehabilitation or a long-term medical stay, Nursing and Rehabilitation Centers are a vital senior living option. These centers are equipped to help residents heal and/ or maintain their best quality of life and often feature a full schedule of activities. At a Nursing and Rehabilitation Center, care is administered by professionals under the direction of a physician. Many facilities also offer sub-acute, respite, rehabilitation and other short-term care. Rehabilitation services can be especially helpful for individuals who are recovering from surgery or an illness. Choosing a Community After selecting the type of community that best fits their wants, needs and budget, retirees should start calling individual communities to request additional information. Communities will also likely have detailed websites, as well as staff members who will be happy to answer questions about the property. When researching potential communities, retirees should be sure to ask about admission and discharge criteria. Prospective residents should fully understand the entrance and monthly fees, as well as any additional fees that may be incurred. Finally, potential residents should ask about amenities, activities and resident involvement. Once they have narrowed their search down to a few communities, potential residents should plan on-site visits with friends or relatives. Only by visiting a community in person can individuals get a true feel for the lifestyle residents enjoy.

Five Myths About Hospice and End-Of-Life-Care n
Joy Sexton, Executive Director of JSSA Hospice and Susy Elder Murphy, BA, CMC, Owner of Debra Levy Eldercare Associates
1. The patient must have a written referral from their doctor before they can be assessed to see if they are appropriate for hospice care. NOT TRUE. A patient or their family/ medical POA, can ask for an assessment by hospice at any time. This is an opportunity to learn what supports hospice can provide, and to answer any questions that the patient and family may have. If the patient decides that they would like to elect to use their Medicare Part A benefit for hospice, THEN the physician will need to write a referral to hospice and a hospice nurse will be
sent out to do a comprehensive assessment, explain the hospice benefits to the patient and their family, and the patient or their Medical Power of Attorney will sign the admission paperwork.
a new curative treatment, or to resume dialysis. You can also be re-assessed by hospice and re-admitted when your condition declines again.
2. Once you elect to be on hospice, you can never change your mind.
3. Hospice provides 24 hour care for you when you are diagnosed with a terminal illness and have 6 months to live (or less) if the disease runs its normal course.
NOT TRUE. At any time, a hospice patient can revoke their hospice status. There could be several reasons for this, such as falling and breaking a hip, and choosing to have surgery to repair or replace it, as well as rehab services to regain function. You may also wish to try

Memory Care Knowing when it is time
As Alzheimer’s disease or dementia progresses, there often comes a time when the patient will need more care than can be provided at home. For instance, Alzheimer’s patients frequently need 24-hour supervision to ensure their safety. Individuals with late-stage Alzheimer’s or dementia may even need intensive care around the clock. While it may be difficult for family caregivers to turn to a residential care facility to care for their loved it, it is often the best choice for both the patient and the caregiver. The questions below may be helpful when determining if a move to residential care is a good option: • Is the person with dementia becoming unsafe in their current home?
• Is the health of the person with dementia or my health as a caregiver at risk? • Are the person’s care needs beyond my physical abilities? • Am I becoming a stressed, irritable and impatient caregiver? • Am I neglecting work responsibilities, my family and myself? • Would the structure and social interaction at a care facility benefit the person with dementia? Even if a family has planned for a move, making the transition to a residential care facility can be a stressful experience. Caregivers may feel guilty and wonder if they are doing the right thing. These feelings are common. Try to remain focused on the fact that this decision is ultimately about making sure the

Myths About Hospice... Continued from page 37
nurse 24/7, the hospice benefit does not provide 24/7 custodial care. Support is provided to the patient in their home, which may be a private home, an apartment in an assisted living facility or a room in a skilled nursing facility and can include several hours per week of homecare. If an acute level of support is required, all hospices have access to a residential facility where patients can receive respite care or a higher level of pain management. This could be their own facility, or a relationship with a local skilled nursing facility giving them access to beds there. 4. Once you are admitted to hospice, you can remain on hospice until you die, even if you live longer than 6 months.
NOT TRUE. After the initial 90-day admission period, hospice staff are required to assess each patient every 60 days to determine if they are still hospice eligible. During this time, the patient must continue to experience “measurable” decline in order to remain hospice eligible. The hospice providers’ records are regularly audited and they can have payments retracted if they continued to provide hospice services past the time that the patient is considered eligible. 5. Electing hospice means giving up and hastening death by withholding food and/or medication. NOT TRUE. A hospice nurse will review all medications that each patient is taking, but it is up to the patient, and/ or their family, which medications they want to remain on. The focus of hospice care is on comfort, so anything that can make the patient more comfortable is
done. While on hospice, patients continue to be offered favorite foods/beverages, although the texture may need to be adjusted to help with safe swallowing and to avoid aspiration (for example, pureeing food or thickening fluids). In the last days and hours of life, it is normal for patients to limit their intake as their bodily functions begin to slow down and prepare for end of life.
“You have no weak links, rather, a caring team that works together to make everyone’s life a lot better.” — SUSIE STRITAR, Daughter of Miss Theoda — My precious Mom, Miss Theoda, lived at Kensington Park until she was 91 years old. I spent lots of time there over the years, so I know the place and the people well. There are not enough beautiful words to express how good they all were for Mom—and for me. Whenever I visited, the staff’s consistent delivery of superior care and service was obvious. Mom was secure, comfortable, and best of all, happy. When she got sick, her move from Independent Living to Assisted Living was seamless. The staff’s shift toward clinical support was instant, yet their Hopkins-quality professionalism was infused with thoughtfulness and compassion, and sometimes, entertainment. I’ll never forget Jessie singing Over the Rainbow or Mom’s happy hour birthday serenade. I treasure knowing that while Mom lived at Kensington Park, she felt heard, respected and loved. So did I. Kensington Park, you shine, brightly. Thank you.

Remain at Home
Receive Assistance While Enjoying the Comforts of Home
Home care can include a wide variety of healthcare and supportive services—from professional nursing and home health aide services to physical, occupational, respiratory and speech therapies. Options in Home Care An incredibly diverse field, home care includes companion care, home healthcare or skilled nursing. COMPANION CARE is provided by caregivers or companions, who serve as surrogate family members, performing many of the tasks that family members would typically complete. Assistance from companion caregivers can include meal preparation, medication reminders, laundry, light housekeeping, shopping, trans-
portation and assistance with exercising. The overall goals of companion care providers are to ensure that the home environment remains safe and that the individual has a companion to spend time with when family is not available. HOME HEALTHCARE is for those who are no longer able to perform all of the functions of daily living by themselves but who do not require skilled medical services. A home health aide can help with activities of daily living (ADLs), which include bathing, dressing, transferring, eating and toileting. Additional services generally include meal preparation, mobility exercises, housekeeping, laundry, medication reminders and transportation. SKILLED NURSING CARE is pre-
scribed by a physician and is administered by a registered nurse. The services provided by skilled nurses include: administering injections, administering medications, wound care, IV monitoring, blood tests, catheter care, respiratory therapy, physical therapy, feeding tube administration and more. Finding a Provider After determining the type and amount of services needed along with payment options, consumers should begin interviewing potential providers. This is a highly important decision, since finding a qualified, reliable and compassionate individual is essential to the health and happiness of patients and their families.

Planning for the Care You Need at Home n
by Phyllis Mensh Brostoff, CISW, CMC, Aging Life Care AssociationTM Member, Fellow of the Leadership Academy
If you took a poll of older adults and asked each person if they wanted to remain in their own home until the end of their life, most people would say, “of course.” That is a wonderful goal, which often takes a lot of sound planning to be successful. Anything can get in the way—accidents, illness, lack of energy, declining physical abilities—of achieving your desired independence. It is hard to give up even a little autonomy, but asking for assistance can help keep you where you want to be—in your own home.
What if You Become Homebound? If you are recovering from an acute illness, you may be eligible for home health services that are paid for by the Medicare program. An Aging Life Care Manager™ can help you access licensed home health services by facilitating a conversation with your physician about your need for skilled nursing services, physical therapy, occupational therapy, and/or speech therapy in your home. This is an important conversation to have with your doctor and discharge planner if you are in the hospital or a nursing facility and need to

decide on and plan for services in your home once you are discharged. However, Medicare will only pay for these services if you are “homebound.” Homebound is defined as only leaving your home for “medical treatment or short, infrequent absences for nonmedical reasons, like attending religious services.” You may also receive help from a home health aide under the supervision of a professional nurse or therapist. A physical or occupational therapist can evaluate your home and recommend equipment

(such as guard bars in the bathroom) that will improve your safety. Accessing services These services must be obtained from a state licensed home health agency certified to provide Medicare-reimbursed services. They may be freestanding agencies, or they may be part of a large, hospital-based organization. Licensed home health services are usually timelimited, ending when you have met the goals of treatment and the need for the skilled care has ended. If you have enrolled in a Medicare Advantage program, you may also have access to additional services such as home visits after a hospitalization. Other Home Care Services Many other care services are pro-
vided in the home but are not licensed or paid for by Medicare. An Aging Life Care Manager can help you understand what they can offer, access reputable services, and monitor them on an on-going basis. Non-Medical Aides, Companions and Homemakers There are many types of paid care giving services available—often called non-medical aides, companions and homemakers—who many people hire privately. If you do, you or your family should check at least two work-related references, develop a specific set of tasks and expectations for the caregiver, arrange to pay all appropriate taxes, and have the required insurance coverage. Caregiver services can also be arranged through an agency. While this
may cost more per hour than hiring an aide yourself, hiring an agency assures you of coverage when the aide is sick, on vacation or quits. The agency also carries liability insurance and is responsible for all employment taxes and employee bonding. An Aging Life Care Manager can help you find a reputable agency and get important questions answered, such as: do they check police and driver license records; do they provide supervision; how do they handle cancellations; and how long have they been in business? Your family can come together and construct a plan for supporting you in your wishes to remain in your home. If it becomes clear that you all need help in doing that, hiring an Aging Life Care Manager is a wise investment.

Are You Lonely? n
by Mary Ann Buckley, LCSW-C, C-ASWCM
As a Care Manager and Social Worker with more than 30 years of experience, I have conducted a myriad of assessments and have asked many questions over the years. Yet, it wasn’t until I read John T. Cacioppo’s and William Patrick’s book Loneliness: Human Nature and the Need for Social Connection that I realized I needed to add this critical question to my assessments. Are you lonely? I decided to plunge into the topic of loneliness in older adults and was intrigued by studies both in the United States and in other countries. The research is clear that older adults are experiencing loneliness and the negative impact that it can have on their medical and mental health at pandemic proportions.
One study on loneliness found that time spent with a friend, neighbor or peer was more enjoyable than time spent with family. Sadly, the value of meaningful interpersonal connection is often minimized in our society. The consequence of loneliness is striking. Older adults who report being lonely are more likely to go into a nursing home, have more difficulties with the activities of daily living, and experience more rapid progression of Alzheimer’s. Undoubtedly, these studies on loneliness can assist us in creating solutions, programs, services and environments to alleviate loneliness in older adults. Inspired by the work of Cacioppo and Patrick along with these studies, I created a presentation on the topic: One Really is the Loneliest Number!
The presentation covers three Social Work CEUs and focuses on how to identify, assess and alleviate the negative impacts of loneliness in older adults. Loneliness is preventable. Prevention requires not only asking the question but also devising a plan to alleviate the loneliness. As Care Managers, we are uniquely qualified to create care plans and solutions to reduce the impact of loneliness. These care plans are individualized to meet the unique needs and circumstances of each client. As care managers, we work with our clients to begin the process of transforming a situation or circumstance that negatively impacts their life by helping to envision what life would be like by accomplishing a change. Imagine a life without loneliness.
Pay For Home Care Understanding Costs Can Help With Decision Making As the decision to remain at home is becoming an increasingly popular option, it is important to understand the costs of care and your payment options while researching different home healthcare companies. Costs for companion care are covered primarily by private payments. Home healthcare must also be paid for with private funds, unless it is prescribed by a physician and provided by a Medicare-certified agency. Long-term care insurance policies may cover companion care and home health care. If you qualify for Medicaid, a government program for low-income individuals, it may subsidize some of your homecare needs. Since laws vary from state to state, you will need to contact your local Area Agency on Aging for more information concerning government benefits. Skilled nursing care will be paid for by Medicare as long as it is determined to be “intermittent skilled care,” which includes occupational, respiratory, speech 52
and physical therapy. Contact your local Office on Aging or State Department of Health to find out if there are funds from social services block grants available to cover home care costs.

5 Things an Aging Life Care Professional™ Did Today by Heidi L. Garvis, BS, CMC® (Certified Care Manager), Aging Life Care Professional™ Here’s what an Aging Life Care™ Professional (aka Geriatric Care Manager) did for you today: 1. Listened. She* listened to your concerns and began to assess your shortterm and long-term needs with regard to caring for a parent. She heard that your loved one wants to age in place, but the house, the finances and her ability to care for herself has diminished. 2. Asked questions. She asked about important aspects of your life and the life of an aging loved one. How has quality of life diminished? What worries you the most? What happens when you talk about your concerns to your loved one? What motivates him/her? Who has Power of Attorney? What is the medical history? What is the financial picture? 3. Created a mutually agreeable plan of care. The Aging Life Care Professional assessed the risks and needs and created a dynamic plan of care to
solve problems and monitor outcomes. She helped you select a home care company. She did a home safety evaluation and coordinated changes. She tracked medical appointments and made sure medication management was in place. She advised you on senior living communities if desired. She made sure your loved one had purposeful and enjoyable things to do each day. 4. Gathered information and created an emergency file. She put all the pieces of information together (doctors, medications, medical history, do not resuscitate (DNR) order, family contacts, legal contacts and other documents) and told you that she would be there for you and your loved one 24/7 when they go to the doctor or hospital. She was there when you went on vacation, so you could enjoy a break. 5. Became the neutral party. She helped you and your loved one meet common goals. And most importantly, she helped you be the daughter,** instead of a harried, potentially short-tempered caregiver. * Or He **Or Son

Professional Assistance May Be the Solution—No Matter What the Problem Is
When it comes to life’s biggest decisions, most challenging transitions, and largest opportunities, we generally turn to professionals for help. From planning a move to planning for the financial future, there are professionals who can help with every step of life’s journey. With an increasing number of credentialed professionals specifically trained to meet a diverse array of needs, there is generally a professional for that—no matter what that is. Many of these professionals work directly with Aging Life Care Managers to help clients achieve their best quality of life and to deliver the best possible results for families. In fact, in many instances, care managers will recommend professionals who become a vital part of a client’s care team. No matter what stage of life you find yourself in or what your age, there is never any reason to face life’s changes and challenges alone. Enlist the help of a professional and get the help you need. Some of the more prominent categories of professionals are detailed below. The tables on the following pages list dozens of options that are available in these and other categories. For specific information on available services, please contact the individual providers. Aging in Place Solutions Need to update your home to meet your changing needs? Want some help running errands? Have medical needs that require special equipment? There are individuals and businesses available to meet all your needs and allow you to remain safely in your home. all accustomed to turning to technology for solutions. Help managing the challenges that come with aging or disability is no different. There are many technologies that can help you with all types of needs—from staying in contact with family to hearing better. Technology professionals can help clients find specific technologies that can improve their living environments and their lifestyles. Downsizing/Moving Solutions Moving is one of life’s most monumental tasks. Add in the fact that many seniors have been in their homes for two, three, or more decades, and moving becomes even more daunting. Whether you need to declutter and downsize in preparation for a move to a retirement community, or just have your home deep cleaned to provide a safer living environment, there are professionals available to help. When it comes time to sell the family home, there are also professionals available to prepare the home and ensure that you get top market value. Medical Solutions Sometimes medical needs require the help of a specialist or a business that
caters to specific conditions or illnesses. Professionals can help you with a wide range of needs—from vision services to hospice care to mental health care and everything in between. Planning Solutions Planning ahead for financial and legal needs is important at every stage of life, but never more so than during retirement. Financial and legal needs frequently become more complex with age. You may need help with tasks ranging from paying bills and managing paperwork to estate planning and managing Medicaid. Professionals are available who specialize in each of these areas and can make meeting your financial and legal needs easy. Solutions for Professionals From Aging Life Care Managers to Elder Law Attorneys to assisted living and homecare providers, all professionals who work with senior clients and their families need specialized training to ensure that they have the most up-todate information and education. Professionals who train their peers help to ensure that all seniors get the best possible assistance with all their needs.
