Tips to Prepare for a Visit to Virginia Hospital Center A well-planned visit to the Hospital helps you focus your energy and attention on your healthcare needs and questions. Whether the visit is for an appointment or an emergency, there are a few ways to keep it from being chaotic. By preparing a few simple things, you will be able to focus on the role you play in your own healthcare or the healthcare of a family member or friend.

Preparing for an Appointment or a Visit to the Hospital: Prepare what you can before you arrive. • Always carry your photo ID and insurance card(s). • If the visit is to see a physician, plan ahead of time and see what paperwork needs to be completed for the appointment. • If you are visiting a family member staying in the Hospital, know the visiting hours, where to park and if you need any legal documents should you have medical power of attorney. • If the visit entails an outpatient service (i.e. radiology, laboratory or therapy visit), make sure you have your physician’s order with you. • Have your medical records up to date and readily accessible to share with your clinician.

Know where you are going. •W hen you make your appointment, ask if there is a particular building or zone you should look for. • Inquire about the parking garage that is closest to your destination. Building signs and landmarks are helpful to know before you get into your car. • Bring exact change ($5) or a credit card for parking, if you’ll be driving to the Hospital.

Community Members: Giving Back While Staying Active

or Ray Husson and his wife, Irene, the key to a happy and healthy retirement lifestyle has been getting out into the community and giving back. Through the Virginia Hospital Center Senior Health Department, Ray and Irene have found the help they needed—and have been able to help countless others. Irene became involved with Virginia Hospital Center when she retired. After nearly 30 years with the Department of Justice, Irene found the Senior Health Department’s Ballston Common Mall Walkers following a frightening incident. “I was walking along Lee Highway one day, and I was shot by a bunch of young kids,” Irene recalled. “It wasn’t serious. We’re not sure if it was an air gun or a BB gun or what, but I was OK, just very bruised. That led me to what has become my sanctuary.” Irene has now been a member of the wellness program for 28 years. “I’ve developed a lot of friendships, and they have been a great support. The group has meant so much to me— emotionally and physically.” Due to renovations at Ballston Common Mall, the program recently transitioned to Walk-Fit at the Fashion Centre at Pentagon City. Irene continues to be an active walking member and enjoys benefits from exercise and socialization. In addition, Irene regularly attends the monthly Healthy Aging Lecture Series offered by the Senior Health Department and has found her primary care doctor, several specialists and additional senior service providers through this community resource. Ray became an active member of Virginia Hospital Center under different circumstances. At the age of 75, Ray had no plans to retire. He was

still working as an Occupational Safety and Health Management Consultant. Unfortunately, as was the case for many workers, Ray was forced into retirement, as his long-time government client let him go due to the market crash. Despite his best efforts, there was no work to be found.

“I’ve developed a lot of friendships, and they have been a great support. The group has meant so much to me—emotionally and physically.” — Irene Husson, member of the Virginia Hospital Center Mall Walkers “He was becoming desperate in his transition from full-time work to retirement,” Irene said. “One day I saw him in the living room looking out the window and counting the cars passing by. I knew he had to get involved.” Ray needed to get out of the house, and Irene encouraged him to

start volunteering. Irene turned to the Hospital for help. Ray became an Auxiliary member and he has never looked back. “This has really filled the void for me. I meet a lot of people here,” he said. Ray now volunteers as an Auxiliary wheelchair escort and works for the Senior Health Department to perform in-home service calls for their Philips Lifeline subscribers. His service calls and wheelchair rides are more like friendly visits, and he easily connects with subscribers and patients. “I’m as old as they are, and I get so much out of visiting with them,” he said. “I have fun with them, and I listen to them, and we laugh a little together.” With the help of Virginia Hospital Center’s Senior Health Department, Ray and Irene Husson have found resources, gained friends and made some real connections. The Hussons are certainly giving back to their community—and they are getting a lot out of it as well.

Supporting Our Community Virginia Hospital Center Foundation Lifeline Assistance Fund Recognizing the value and need for the Philips Lifeline service for vulnerable seniors in our area, Virginia Hospital Center Foundation established the Lifeline Assistance Fund in 2009. Through this fund, we are able to lower the monthly cost by 50% and assist at-risk community members who otherwise may not be able to afford the service, providing the opportunity for seniors to remain in their home with confidence and safety. Our goal is to continuously enroll low-income individuals into the Lifeline Assistance Fund each year, and to support each subscriber for as long as he or she is able to use the Lifeline service. Virginia Hospital Center Safe Transition for Hospital Patients Virginia Hospital Center is the only hospital in the DC Metro area to offer the Philips Lifeline Safe Transition for Hospital Patients (STHP) program. The transition from Hospital to home is a critical time for many patients, especially during the first few weeks following discharge. Early intervention minimizes potential complications from a fall or other medical crises. Through the STHP program, “at-risk” patients being discharged home from Virginia Hospital Center have the ability to get the Philips Lifeline Medical Alert System at no cost for their first two months home. Staff from the Senior Health Department coordinate the installation within two days of discharge in order to ensure the patient has a safe recovery and can maintain their independence. Senior Exercise & Wellness Virginia Hospital Center’s Community Wellness program offers a diverse menu of fitness and healthy lifestyle classes and assessments. The programs share a common goal: to improve and maintain the health of the community.

Walk-Fit at Fashion Centre at Pentagon City Virginia Hospital Center and Fashion Centre at Pentagon City partner to host the intergenerational Walk-Fit group. The mall-walking program originated 28 years ago and continues to be free and open to the community. Walkers arrive at the mall Tuesday and Thursday mornings and complete laps along the top three levels of the mall. A cool down and stretch is led by Senior Health Department staff at the end of the session.

Support groups bring together people facing similar issues, whether illness or major life changes. Virginia Hospital Center believes that support groups can help you cope better and feel less isolated.

Alzheimer’s & Dementia Caregiver Support Group

Receive emotional support for grieving the loss of a loved one from cancer or any condition. Meet other people who may be experiencing similar loss of their friends and families. Special Programs Healthy Aging Lecture Series: Knowing current healthcare trends and options for services in the area can help individuals age in their homes safely. This lecture series is a free, monthly program where specialists and professionals are invited to present the latest information on a variety of aging issues. Call 703.558.6859 for upcoming lectures. Memory Screening: The Senior Health Department sponsors free, confidential memory screenings to promote proper detection of memory problems and provide education about successful aging. The one-on-one, noninvasive screening takes only five to ten minutes and is administered by a qualified healthcare professional. Call 703.558.6859 for the next available screening date. Fall-Risk Assessments: A comprehensive screening measures one’s risk for falling via a medication review, blood pressure check, vision screening and six physical therapy tests to assess balance. A physical therapist will review results and provide a personal evaluation and recommendations on how to reduce risks, as well as offer education for successful independent living. Total Joint Replacement Class: Being prepared is the first step toward a successful surgical outcome, so be sure to attend our in-depth Total Joint Replacement Class. Learn what to ask your physician and nurses, and what to expect before, during and after your surgery. It is best to take this class four to six weeks prior to your surgery. Call 703.558.6621 to register for the class. Diabetes Education: The Virginia Hospital Center Diabetes Education Program is an American Diabetes Association Recognized Program. We cover comprehensive content on diabetes self-management for Type 1, Type 2 and gestational diabetes. Weight Loss Surgery Seminars For individuals who are interested in learning more about weight loss surgery, informational seminars on surgical options are free and routinely held every month. The seminars allow you to meet the surgeons, learn about the various weight loss surgeries, such as sleeve gastrectomy, gastric bypass, lap band and duodenal switch, as well as the non-surgical intragastric balloon, and understand the risks and benefits, the alternative therapies, and the expectations before and after these procedures. Support Services

Case Management Our Case Managers are here for discharge planning to help you and your family with the transition from the Hospital to your home. Discharge planning begins when you are admitted to the Hospital. Case Managers can assist you with arrangements for home health services, equipment, transportation and alternative living plans (such as finding a skilled nursing facility). Social workers also can provide emotional support and community resource information during your Hospital stay. Auxiliary A proud 80-year tradition, the VHC Auxiliary has always been an integral part of the Hospital, as its original purpose was to build a Hospital for the County. Auxiliary members have given millions of hours in service to the Hospital community. They also help with fundraising, enabling the Hospital to continually improve both medical care and patient comfort. If you need any assistance (wheelchair, escort, or navigating) upon arrival to the Hospital, you may find our Auxiliary members in their pink and red coats. Medications and Pharmacy Questions to Ask You should be able to answer the following questions before taking any new medication. Although each medication comes with instructions, your doctor and pharmacist should be available to answer any or all of the following questions in more depth and in language that is easy to understand. What is the name of the medication and what is it supposed to do? You should know the names of all the medications–both prescription and nonprescription–you take, so you can inform each doctor you see. It is also important to know what each medication looks like. When and how do I take it? You need to know how often to take your medication, if the medication is best taken on an empty stomach or with food, and if you should take it at the same time each day. For how long should I take it? Your prescription order indicates the length of time you should take the 12

medication and whether refills are available. Skipping doses or stopping medication to save money or because you “feel better” can result in health problems requiring more expensive treatment in the future. Should I avoid alcohol, any other medications, food and/or activities? Certain foods or alcohol may also interact with your medications. Some drugs can cause drowsiness and may affect activities such as driving. Should I expect any side effects? All medications can cause side effects, but they are not necessarily serious. Your pharmacist will inform you of the most common side effects. If you experience any unexplained effects, contact your doctor or pharmacist. What if I forget to take my medications? Be sure you know the answer to this question when you receive the prescription. The decision to take a missed dose depends on the drug.

Don’t panic and don’t take a double dose unless you are specifically directed to do so by your doctor. Is there a generic version of the medication? Not all medications have generic counterparts. If a generic version is available, the Food and Drug Administration (FDA) has judged it to be equivalent to the brand name product, and it can save you up to half the cost. How should I store my medications? Proper storage ensures a medication’s effectiveness. The bathroom medicine cabinet is not an ideal storage place. Heat and humidity can adversely affect your medication. Most medications require a cool and dry storage location, and some need refrigeration. Does this medication replace anything else I was taking? Make sure you understand if this medication is replacing another medication.

Palliative Care is special healthcare that can ease pain, other symptoms and side effects throughout all stages of illness and/ or treatment. The VHC interdisciplinary team addresses the psychosocial needs to help you and your family cope with your illness, as well as emotional, practical and spiritual concerns. Chaplain Services The Chaplaincy Team is the spiritual arm of Virginia Hospital Center’s patient healthcare support. We believe that spirituality, in the broadest sense, is an integral part of a holistic approach to health care, where the physical, psychosocial and spiritual needs of the patient are addressed. Spiritual, emotional and companion care for patients and families is available 24 hours a day, seven days a week by the Chaplain Team, either on duty or on-call. Advance Directive Planning ahead for medical decisions can make difficult situations more manageable for patients and their families. Advance planning is the only way to ensure that a patient’s wishes are followed in the event of a medical emergency or long-term illness. Under a Virginia law called the Healthcare Decisions Act, adults may sign a document that makes their choices about treatment known to their doctor and family in advance. This document is known as an Advance Directive. Virginia Hospital Center supports the aims of both the Virginia Healthcare Decisions Act and the Patient Self Determination Act. Understanding Medicare Medicare’s Alphabet Soup: Know the Options and the Coverage Medicare is the federal health insurance program for people who are 65 or older, certain younger people with disabilities, and people with End-Stage Renal Disease. Medicare provides important benefits for most Americans who are age 65 and older. When enrolling in Medicare, changing plans or determining benefit coverage, consumers need to fully understand the many different types of Medicare plans. The process of selecting the best plan begins with learning more about all the options. Medicare Part A, or Hospital Insurance, covers inpatient hospital stays, care in a skilled nursing facility, hospice care and some home health care. Medicare Part B, or Medical Insurance, covers certain doctors’ services, outpatient care, medical supplies and preventive services. Medicare Advantage Plans, also known as Medicare Part C, are health plans offered by private companies that contract with Medicare to provide subscribers with all their Part A and Part B benefits. Medicare Advantage Plans include Health Maintenance Organizations, Preferred Provider Organizations, Private Fee-for-Service Plans, Special Needs Plans, and Medicare Medical Savings Account Plans. Most Medicare services are covered through the plan and aren’t paid for under Original Medicare. Most Medicare Advantage Plans offer prescription drug coverage. Medicare Part D, or prescription drug coverage, adds coverage for medications to Original Medicare, some Medicare Cost Plans, some Medicare Private-Fee-for-Service Plans, and Medicare Medical Savings Account Plans. Because of the many services and programs they offer, AAAs are one of the single most important retirement resources. The Neighborhood You Dream About The Village Movement is Prompting Communities to Think About Creative Solutions for Aging in Place Imagine you are living in a community where you know your neighbors. Imagine that your patio needs weeding, and you prefer not to do it. In this neighborhood, a simple phone call can have someone over to take care of your weeding. Perhaps you need some home repairs, or you are returning home from the hospital after surgery and need some assistance. In this neighborhood, you can get assistance from trusted volunteers and providers with one phone call. This type of neighborhood is becoming a reality, and the Washington, D.C. region and the state of Maryland are the launching pad for several. How it All Started Nearly 90 percent of Baby Boomers say they want to stay in their homes and close to family and friends as long as possible, according to a survey by AARP. The challenge these individuals face, however, is that if they do nothing to prepare for future needs, they may not be able to continue living in their homes as long as they would like. Residents of Beacon Hill, a downtown Boston neighborhood, responded to the challenge of staying in their community by joining their neighbors to take control over where and how they will live in the years to come. They formed a non-profit, Beacon Hill Village, which serves those aged 50+ in central Boston, to take advantage of social, cultural and wellness activities without leaving their homes. The various services provided are designed to respond to the members’ specific needs and wants and include concierge services, comprehensive home care, home repair, house cleaning, grocery shopping, transportation and other services.

Advance Directive A written document that tells what a person wants or does not want if, in the future, he/she can’t make his/her wishes known about healthcare treatment.

This form is a combined Durable Power of Attorney for Health Care and Living Will for use in Virginia.

With this form, you can:

ontinuing Care Retirement Communities (CCRCs) offer long-term contracts that guarantee lifelong shelter and access to specified health care services. A staff of medical professionals is on hand to provide different levels of care, from independent living to assisted living and nursing care. A member can move up or down this continuum of care for the rest of their life. Most CCRCs establish requirements for incoming residents based on age, financial assets, income level and physical health. Residents are typically expected to move in while they are still independent to get to know the community. In return for guaranteed lifelong shelter, amenities and health care, residents usually pay a lump-sum entrance

fee and regular monthly payments. There are three basic types of CCRC contracts. An extensive contract covers shelter, residential services, and amenities, plus unlimited long-term nursing care without an increase in monthly payments (except for increases related to operating costs and inflation adjustments). An extensive contract spreads the risk of catastrophic health care costs among all residents, so that no single person faces financial ruin. Entrance fees and monthly costs under this contract are typically higher than those under modified or fee-forservice contracts. ssisted Living communities offer housing and a variety of supportive services delivered by qualified staff members. As opposed to independent living communities, health care services are available at assisted living communities. The Assisted Living Federation of America (ALFA) notes that assisted living is “a unique transition from independent living without care” that “helps seniors enjoy an improved quality of life with independence and care combined.” Community amenities vary widely but can include private units, state-ofthe-art facilities, beautifully decorated common areas, barber shops and beauty parlors, pharmacies, physical therapy services, recreation rooms, li-

braries, gardening areas, and fitness centers. Who Needs It? Residents of assisted living usually include anyone who needs some help with the activities of daily life. These include eating, bathing, dressing, transferring, toileting, medication management and walking. Most communities offer three meals a day served in a common dining area. Many also offer some type of assistance with transportation. Housekeeping and laundry services are standard offerings. Staff members are available to meet resident needs 24 hours a day. Most communities are equipped with security systems, and emergency call systems are often available in all residences. For residents with more

acute health care needs, such as Alzheimer’s or other forms of dementia, special units and/or specially trained staff members are often available. Choosing a Community First and foremost, consider the needs and desires of the potential resident. Narrow the search by reviewing a list of all the local communities. Request a brochure from your top choices, as well as other specific information such as the resident agreement or contract, a list of services and amenities, detailed prices, staffing information, admission and discharge criteria, and community rules. Visit a facility more than once to get a complete feel of the lifestyle at the community. Talk with staff and residents and tour the entire facility.

hile some individuals are able to continue running their households with little assistance, there are those who may have debilitating physical or emotional conditions that require care beyond what family or professional home care aides can provide. What is Nursing Care? At a nursing home, care is administered by professionals under the direction of a physician. Many facilities also offer sub-acute, respite, rehabilitation and other short-term care. Rehabilitation services can be especially helpful for individuals who are recovering from surgery, an illness or any other life-changing event. These homes serve as permanent

residences for individuals who are too sick or frail to live at home, or as temporary facilities during a recovery period after a surgery or illness. Who Pays for Nursing Care? Medicare, Medicaid, private insurance and personal assets are all used under various circumstances to pay for services in a nursing care facility. If a facility is not certified by Medicare and Medicaid, the care will have to be paid for entirely with personal funds. In general, Medicare pays for skilled nursing care following hospitalization for the same illness or condition at 100 percent of the cost for days 1 – 20. Some long-term care insurance pays for nursing home care; insurance payments are usually a fixed amount for a specified number of qualified days.

The basic daily rate is the standard charge the nursing home bills to all residents, which covers the fundamental services every resident receives, including rent for the room, housekeeping, meals and general nursing care. What to Look for in Nursing Care Before choosing a home, contact the Long-Term Care Ombudsman Program of your Area Agency on Aging. Ombudsman programs promote the highest quality of life and care for residents of nursing facilities and can help families and staff with inquiries and complaints. Visit your prospective communities at different times of day. Talk with residents. Be persistent about getting your questions answered.

Glossary of Terms Activities of Daily Living (ADLs): Basic tasks of everyday life that include dressing, bathing, eating, transferring (i.e., from bed to chair) and toileting.

Do Not Resuscitate Order (DNR): An order written by a doctor to fulfill a patient’s expressed medical care wishes during a medical emergency.

Adult Day Center: Also called Adult Day Services, Adult Day Care or Adult Day Health Centers, these facilities and programs provide regular daytime care to senior adults for socialization; recreation; help with personal care; safety; and in some cases, health and rehabilitation-related services.

Durable Power of Attorney: A legal document executed as part of a person’s estate planning. In it, the person names an “attorney-in-fact” or “agent” to act on his or her behalf in business and/or health care matters.

Adult Protective Services: A public agency that investigates reports of abuse and neglect of vulnerable adults, usually works with local law enforcement. Immediate dangerous situations should be directed to 911 or local police. Aging Life Care Professional (formerly known as Geriatric Care Manager): A professional who performs an assessment of a person’s mental, physical, environmental, and financial conditions to create a care plan to assist in arranging housing, medical, social, and other services. Dementia: Disorders of the brain, including Alzheimer’s disease and other illnesses that result in a decline in memory and other intellectual functions. Discharge Planning: A service provided through Hospitals and other health care providers to help place a convalescing patient in an appropriate care setting, or to arrange appropriate services at home. Durable Power of Attorney for Health Care: Also called a “Health Care Appointment” or a “Health Care Proxy.” This is a legal document that lets you give someone else the power to make health care decisions for you if a time comes that you can’t speak for yourself. Family Caregiver: Anyone who provides unpaid assistance to another person who is ill, disabled, or needs help with daily activities. Health Care Directive: Also called a “Living Will” or “Advance Directive.” This is a document that lets you say what kinds of care you would want and not want if you were nearing the end of your life. Usually deals with lifesustaining measures. Health Insurance Portability and Accountability Act (HIPAA): A federal law that gives you rights over your health information and sets rules and limits on who can look at and receive your health information. Understanding Home-Based Care in Virginia

n Virginia, home care organizations, certified home health agencies, and licensed and certified hospice programs are reviewed by the Virginia Department of Health Office of Licensure and Certification. There are several types of agencies in Virginia that provide these services. For individuals needing home-based care, it is important to first determine what type(s) of help you need to narrow your search of agencies and find one that will meet your specific needs. The following information from the Virginia Department of Health Office of Licensure and Certification provides a look at the different agencies and their state regulations. Home Health Agencies Home health agencies are public or private agencies, organizations or

parts of agencies or organizations that meet the requirements for participation in Medicare under 42 CFR 440.70 (d). This indicates these federally-certified agencies provide skilled nursing services and at least one other therapeutic service, e.g. physical, speech or occupational therapy; medical social services; or home health aide services, and they also meet the capitalization requirements under 42 CFR 489.28. An agency may choose to be exempt from state licensure through certification in one of four ways: • The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Certification; • Community Health Accreditation Program (CHAP) Certification; • Medicaid Personal Care Certification; • Medicaid/Medicare Certification.

When trying to find a home health agency, ask for their license number. Be specific in asking for the license number issued by the Virginia Department of Health, and not the number for their license to operate a business. Home Care Organization The code of Virginia defines a home care organization (HCO) as “a public or private organization, whether operated for profit or not for profit, that provides, at the residence of a patient or individual in the Commonwealth of Virginia, one or more of the following services: 1) home health services, including services provided by or under direct supervision of any health care professional under a medical plan of care in a patient’s residence on a visit or hourly basis to patients who Continued on page 53

have or are at risk of injury, illness, or a disabling condition and require short-term or long-term interventions; 2) personal care services, including assistance in personal care to include activities of daily living provided in an individual’s residence on a visit or hourly basis to individuals who have or are at risk of illness, injury, or disabling condition; or 3) pharmaceutical services, including services provided in a patient’s residence, which include the dispensing and administration of a drug or drugs, and parenteral nutritional support, associated patient instruction, and such other services as identified by the Board of Health by regulation.” Companion Services In Virginia, companion organizations can provide only chore, companion or homemaker services, in addition to any of the following, without the aforementioned licenses. Com-

panion services can include providing non-routine home maintenance, along with assisting individuals unable to care for themselves with transportation, meal preparation, shopping, light housekeeping, companionship and household management. Also included is assistance with bathing areas the client cannot reach, fastening client’s clothing, combing hair, brushing dentures, shaving with an electric razor, and providing stabilization to a client while walking. Homemaker services, however, do

not include feeding, bed baths, transferring, lifting, putting on braces or other supports, or cutting nails and shaving with a blade. Such services would need to be provided by a licensed HCO or HHA in Virginia. Questions to Ask When Hiring Paid Caregivers If facing this decision, the following questions will help in making a safe and educated decision: 1. If the hired caregiver becomes ill or otherwise unavailable, what alternate arrangements can be made?

5. Are criminal history background checks, state abuse registry checks, and prior work reference checks completed?

2. Are social security, federal and state taxes, and unemployment insurance paid so the family is not legally responsible?

6. I s there bonding and insurance in place that would cover any injury to the client, damage, or theft? When an agency says they are “bonded,” family members should be sure that this covers injury or damages related to the caregiver(s) working in the home.

3. Is there verification that the paid caregivers can legally work in the U.S.? 4. I f the hired caregiver is injured, who is responsible? Remember that many homeowners’ insurance policies exclude such injuries via “domestic employee exclusions.”

7. Assuming family is not available to supervise paid caregivers at all times, will there be documentation that substantiates the completion of services?

If the adult caregiver does not have the time or resources available to answer these important questions, the services of a professional home care agency or adult day care center may be the right solution.

Personal Emergency Response System (PERS): A portable electronic device with a call button that a person can use to summon help in an emergency.

Meals on Wheels: Community-based meal service that delivers meals to the homes of senior adults at a modest charge. National Family Caregiver Support Program: A federally funded program available in local communities that provides a range of supports to assist family caregivers who care for their loved ones at home. Services may include caregiver education, training, information, counseling, links to community services and respite. Ombudsman: The ombudsman program provides advocacy and trouble-shooting support for Skilled Nursing and Assisted Living residents. Open access to the ombudsman is a protected resident right.

Power of Attorney: A legal document that gives another person legal authority to act on one’s behalf. Respite Care: Temporary care for a person, provided by a home care agency or other provider, in order to give the person’s regular caregiver rest and personal time. Respite care can be in the home, at an adult day center, in a longterm care facility or hospital. Social Security: A benefit earned by eligible workers that provides guaranteed inflation-adjusted monthly income for life. A person with the required number of quarters in Social Security covered work who is either disabled or age 62 or over, as well as certain family members, are eligible.
