DEAR DR. ROACH: I’ve been hearing about tianeptine being sold at gas stations. Is it really dangerous? -- S.F. ANSWER: Like so many substances, tianeptine can be a useful drug when prescribed and taken correctly (it’s used in several countries for depression), but it is indeed dangerous when used recreationally. Its use has been banned in several (unfortunately not all) states across the U.S. It is sold (yes, especially at gas stations) as a dietary supplement. Because it acts on the same receptor as morphine, when used at high doses far beyond the medically approved dose, it has potential for abuse and even overdose, with a handful of deaths ascribed to its use.

I recommend in the strongest possible terms against its recreational use and hope it is completely banned soon.

DEAR DR. ROACH: I have severe pain in my right shoulder. I have seen two orthopedists and received X-rays, MRI scans and a cortisone injection, which didn’t work. Both surgeons recommended a shoulder replacement. I am taking three adult aspirins each morning (I am allergic to NSAIDs), which makes the pain go away. Is this safe? I am 83 and otherwise healthy. -- J.B.

ANSWER: Aspirin is a type of non-steroidal antiinflammatory drug and has the toxicities common to all the drugs in that group, including an increased risk for bleeding and potential damage to the stomach and kidneys. But it has additional toxicities beyond NSAIDs like ibuprofen, especially at higher levels. I am very cautious about prescribing aspirin and generally prefer other anti-inflammatory medicines. However, people have long taken aspirin -- sometimes at much higher doses -- usually without problems. The fact that you are taking it only once a day reduces the toxicity, but I have to say I am surprised the benefit is lasting all day.

The risk of toxicity is higher at older ages. At 83, you are at a significantly higher risk (seven times the risk) than younger people. Most of the data on exactly what your risk is comes from people taking low-dose aspirin (81 mg, not the 975 mg you are taking), so I can’t give you an evidence-based answer. But, for men in their 80s taking low-dose aspirin, about 1.5% will have a serious bleeding event per year. Your risk will be significantly higher due to the much-larger dose you are taking. However, one has to consider both the benefits of the aspirin treatment (the pain being gone is the best possible outcome) and the toxicity of other possible treatments, such as surgery (which is very significant).

There are many classes of NSAIDs, with very different chemical properties, and it is likely there is another type of NSAID you could safely take, with a lower risk than the dose of aspirin you are currently on. On the other hand, you can’t get any better effectiveness, at least in terms of pain, and the absolute risk is still fairly small, so I’m not sure I recommend you change. I am always very cautious about recommending surgery to someone in their 80s when there is an effective medication available to treat the issue.

Why the new name?

Since 1948 the Genesee Valley Penny Saver has been serving communities by bringing readers and local organizations and businesses together. Your faithful readership has allowed us to grow from our Avon, New York roots to becoming a trusted resource for residents in the greater Western New York and Finger Lakes region. Over the years people have come to know us simply as “ e Penny Saver.”

Funeral Planning: Don’t Be Rushed

One life event we don’t want to think about having to do for a loved one is planning a funeral. But sometimes we must, and it can be stressful and loaded with the potential to be scammed in some way.

A small amount of help is coming. e Federal Trade Commission wants online funeral pricing. And it makes sense. When you need to organize a funeral, emotions are running high. You won’t be at your best when it comes to thinking about pricing and services.

Back in 1984, the FTC created the Funeral Rule, but it only requires funeral homes to show lists of the prices of caskets and any outer burial containers, such as a grave liner. And it was only required that a potential customer be given those prices in person, certainly not by phone. To make it worse, funeral homes only had to show pictures or the item itself a er the prices were disclosed.

By then, of course, you were at the funeral home, unhappy and sad and stressed. e last thing you wanted to do is treat it like a car purchase and shop around. So you signed the paperwork on the spot, possibly paid too much and maybe didn’t get the items and services you wanted.

ere are a few ways to ensure that your vulnerability at that time isn’t exploited.

Before you leave the house to organize a funeral, go online to check local funeral home websites. ey might be among the very few to actually post pricing and services.

Take someone with you.

Get the price list (in writing) of what the funeral home o ers.

Before you sign any paperwork, contact the Funeral Consumers Alliance (funerals.org) and get their advice. ere’s a group in every state. On their website, click on “Consumers” and scroll down to “Don’t Know Where To Begin? Start Here.”

Kindness!

While leaving my SUV to enter Tops in Avon on March 3rd, a young man was coming out with a large bouquet of flowers. I said, “Oh, flowers for me? You shouldn’t have.” We both laughed and went on our ways. When I came out, my daughter (who had waited in the car) had a bouquet of flowers on her lap. She said the man came over, said he was a Christian, and God wanted him to give the flowers to me and he would go back in and get more for his wife. My daughter said thank you and my mom will cry, which I did. What he did not know was that our family had a very difficult week. I will think of his kindness each time I look at these flowers and when the time is right will do the same for someone else. They say bad people are taking over our world, but I continue to believe that the good ones are all over; we just don’t hear as much about them. Many thanks to this kind man.

My rant is to open the eyes of the masses to the technically handicapped. I am one of them. At 68 years of age I fall in the gap of no major training of computers, smart phones, etc. I am also on a fixed income and can’t afford all the gadgets nor the price of the Internet. When I need help for things that have to be done online. I have to hire someone to do so. Every shop, restaurant, government, and medical office has some form, deal or discount that is only online. Having no cell phone, computer to access these deals, or forms, etc. Life for myself with technology is extremely frustrating! I only ask that businesses and the government, etc. stop discriminating against my handicap as they do for the physically handicapped, waiting just a while longer until demanding things be only assessed online until every generation is technically taught at a young age. Yes, I have taken classes available at libraries. I also do not want a government issued phone. Do others agree?

omas Reed “Tom” Mahoney of Canandaigua, formerly of Albion, NY, passed away March 4, 2023 of complications following spinal surgery. He was born July 27, 1943 in Albion to the late omas E. and Marjorie B. (Reed) Mahoney.

Tom was predeceased by his aunt and uncle, Mary and Sam Martillotta, and his cousin,Susan Martillotta.He is survived by his loving and devoted wife of 52 years, Jennifer “Jen” Mahoney; sister, Marnie of Canandaigua; cousin, Sam (Paula) Martillotta of Chardon, OH and their children, Alexandra, Samuel Jr., Audrey and John (Olivia) Martillotta; and Susan’s children, Samantha Martillotta and Daniel (Colleen) Cohen.

Tom earned his B.A.in Speech,Radio &TV at Ohio Wesleyan University and his master’s degree in Journalism at the Newhouse School of Syracuse University. A lifelong broadcast journalist, Tom worked as a news anchor and reporter at several television stations in Western New York and Honolulu. In 2000 he retired from his position at the Voice of America in Washington, D.C. where he worked for 17 years, rst as a news anchor and later as host of several interview and public a airs shows.

After his retirement,Tom and his wife moved to Auckland, New Zealand where Jen, in Tom’s words, “could spend some quality time with her New Zealand family.” From their base in the South Paci c, they traveled extensively throughout Australasia and Europe and returned to the United States every year to visit Tom’s family.

Tom’s greatest love was spending time with his wife and extended family, to which he was dedicated with a passion.He loved his friends and nothing pleased Tom more than quality time spent in one-on-one conversations, often over a sumptuous meal. He was caring, kind and sincere in his interest in everyone he encountered. Tom was known for his “Tomisms,” puns, wordplay and humor, and was an avid reader and news-follower.

Tom’s family will receive friends at the First Presbyterian Church of Albion, 29 East State Street, Albion, NY 14411 on March 18th from 10 a.m. to 11:00 a.m. followed by a service to celebrate his life. ere will be a short co ee reception in the church dining room after the ceremony. In lieu of owers, memorial contributions in Tom’s name may be made to the First Presbyterian Church of Albion at the above address.

BREAKING DOWN THE LEPRECHAUN MYTH

Now widely depicted as tiny, bearded and mischievous men clad in coats and hats, leprechauns have been traced to ancient Ireland. The precise etymology of the word “leprechaun” is unknown, though many scholars believe the word we use today is derived from the old Irish “Lú Chorpain,” which means “small body.” Some scholars point to the 8th century word “luchorpán,” meaning “sprite” or “pygmy,” as the origins of the word leprechaun. Another word, “lubrican,” which first appeared in the English language in a 1604 play written by Thomas Middleton and Thomas Dekker, also has been linked to the modern word leprechaun.

HOFFMAN SCULPTURE RECEIVES WARM WELCOME FROM THE FLOWER CITY AT DRYDEN THEATRE

Last year when the George Eastman Museum unveiled the bronze, life-sized sculpture of Fairport native and famed actor Philip Seymour Ho man in May, its future in Rochester was unclear. e sculpture was cra ed by David Annand and commissioned by lm producer James Declan Tobin with funding provided by the Sumner Roy Kates Charitable Trust.

Originally, the sculpture was to reside at e Eastman Museum for a certain amount of time, then make its journey to New York City. Ho man was a graduate of NYU Tisch School of the Arts and was a well-known inhabitant of New York’s West Village. Many of Ho man’s movies were lmed in New York City as well, so Tobin initially felt New York was the appropriate location to memorialize the actor’s spirit. A er further discussions between James Declan Tobin and Bruce Barnes, Director at Eastman Museum, however, the Museum board recently voted unanimously to retain the sculpture in Rochester permanently.

e sculpture has since received an overwhelming response from the community and is now considered a point of pride for all Rochesterians. It was an easy decision to make according to Ho man’s mother, Marilyn O’Connor, who says, “We had a wonderful response from the community. e response was so positive, the question was, why is it not staying here? It belongs here, he’s from here. A lot of people hug him and take sel es with the sculpture. I walk by and say, ‘Hi Phil” on my way to the Dryden.”

Although creating a sculpture wasn’t the rst idea Tobin had in mind when it came to keeping the legacy of Ho man alive. He established two scholarships in Ho man’s name at his alma mater, NYU. However, a er viewing artist David Annand’s work, it was only a matter of time before Tobin conceptualized a bust of Ho man, began the six-yearlong project. O’Connor adds, “David Annand made a bust, then he entered it into a European competition and won and everyone was thrilled, so it became about how would we add the body to this. It was sort of one step at a time, then came the unveiling and David called it the high point of his career. It was his rst sculpture in the United States.”

Ho man, who passed in 2014, was an Academy Award winner for his compelling work in Capote (2005) and acted in over 50 lms throughout his career. Ho man is arguably one of the most celebrated actors of his time.

With the sculpture project, Ho man was essentially brought home to where his roots began, and where he rst considered acting as a profession. It’s a poignant and emotional tribute for Ho man’s family and all who enjoyed his artistry. Tobin considers Ho man one of the single greatest actors of his generation, which O’Connor feels attered to hear. She says, “It’s a wonderful compliment and it’s a good thought, but I think Phil would have just laughed because he knew and respected other actors. I don’t think he would have accepted the mantle so easily.”

e Eastman Museum is known worldwide as the world’s oldest photography museum, housing collections in elds of photography and cinema as well as cinematic technology. ey are world-renowned leaders and experts in lm preservation. While Eastman may not be the originally intended site, few argue there’s a more appropriate location for the sculpture and for bringing Ho man home at last.

Barnes says the Eastman Museum board is grateful for the opportunity to keep the sculpture permanently in Rochester, NY. He says, “ e George Eastman Museum is thrilled to be able to honor Philip Seymour Ho man and his contributions to cinema by having this wonderful sculpture of him near the entrance to our Dryden eatre.”

is year in honor of Ho man’s legacy, the Dryden eatre will host twenty-four of Ho man’s lms as part of a year-long series. e series, which kicked o in January, will feature many of Ho man’s most memorable lms, a touching tribute for the decorated actor who called the Flower City home.
