A TRADITION ASSOCIATED WITH CHANUKAH CELEBRATIONS
Playing dreidel: There are various stories about the origins of the dreidel, but historians say the first mention in Jewish writings did not occur until the 18th century. The four sides of the dreidel are marked with different letters, which in German are G “ganz” (all), H “halb” (half), N “nischt” (nothing) and S “schict” (put). This dictates whether the player who spun the dreidel gets all, half or nothing in the collective coin pot, or has to put in his own. Yiddish is quite similar to German, and the letters on the dreidel also correspond to the first letters of the Yiddish phrase “nes gadol haya sham,” which translates to “a great miracle happened here,” a reference to the Chanukah miracle.
Retail sales during the 2022 holiday season inched ever closer to crossing the trillion dollar mark. Though total retail sales during the November-December holiday season fell short of National Retail Federation forecasts, the group still reported that sales totaled $936.3 billion in that period in 2022. Though a host of variables, including inflation, affect how much consumers spend during a given holiday season, it’s not outside the realm of possibility that 2023 could be the year shoppers cross the trillion dollar threshold. That would require an increase of a little less than $64 billion in spending compared to 2022. Such an increase is not unheard of, even in a single year, as NRF calculations based on data from the U.S. Census Bureau indicate holiday season spending between 2020 and 2021 increased by just under $106 billion.

As women enter perimenopause and begin the transition that will ultimately lead to menopause, many doctors prescribe hormone pills to help regulate menstruation cycles and ease certain symptoms. Compared to regular birth control pills that contain 30 to 50 micrograms of estrogen,a doctor may suggest a low-dose pill that contains between 10 and 35 micrograms of estrogen. These levels can be increased as needed.WebMD says that,in addition to preventing pregnancy, which can still occur in perimenopause, low dose pills often regulate heavy or irregular menstrual periods and mayofferprotectionfromovariananduterinecancers.Thepillsalso may prevent bone loss, which leads to osteoporosis. It’s important to note that contraceptives come with an elevated risk for breast cancer and blood clots, particularly for those with a history of heart disease and breast cancer. So doctors should be consulted about the pros and cons and women should initiate an open and honest dialogue with their physicians to avoid any complications.

Plastic pollution: Singleuse plastics are turning up in greater numbers in the world’s oceans. Greenpeace says 90 percent of global plastic isn’t recycled. Plastic can be consumed by marine life, and certain marine animals are getting tangled in plastic, resulting in injury and death.

We Will Be CLOSED Thursday, Nov. 28th and Friday, Nov. 29th for the y Thanksgiving Holiday
By Keith Roach, M.D.
DEAR DR. ROACH: I’m an active, 5-foot2-inch, 125-pound, 72-year-old Caucasian womanwithoutahistoryofknownosteoporotic risk factors, other than demographic ones. I exercise daily, eat calcium through food, take 2,000 units of vitamin D, and follow a Mediterranean-style diet. My last dual-energy X-ray absorptiometry (DXA) scan showed a T-score in my spine of -3.3 and a T-score in my hip of -2.7. My Z-scores were 0, and my FRAX score showed a 15.4% overall risk of fracture, with a 4.4% risk of a hip fracture in the next 10 years. Overall, my bone density went down 6.5% for my spine and 5% for my hip, compared to three years earlier.
My primary care physician has recommended starting bisphosphonates. My preference would be to postpone medication for at least a year, whileIworkwithadieticianandaphysical therapist to refine my exercise program and try to build bone more effectively. But I also don’t want to take foolish chances with a spine or hip fracture. At my request, I received a referral to a specialist, but I cannot see her for four months.
How accurate are the results of a DXA scan? I have scoliosis from my childhood; could it affect the accuracy of the test results? Would any other imaging, blood or urine tests be appropriate to provide additional information or indicate if there are other causes of bone loss, besides age and gender? -- A.M.
ANSWER: The most important number here is the FRAX score, which combines your clinical risk (age, gender, height and weight, ethnicity, steroid use, smoking, previous fractures, alcohol use, etc.) and your bone density results from your DXA scan to provide an estimate of the overall fracture risk. It also provides an estimate of the most dangerous type of fractures: hip fractures. It’s freely available at frax.shef.ac.uk/FRAX/tool.aspx.
Your results are above the recommended threshold for medication, which is either a total-fractureriskof20%orahip-fractureriskof 3%.Among the different options, most experts would choose a bisphosphonate drug like alendronate, based on decades of experience and strong data. However, a very low T-score (worse than -3) makes some experts choose a different type of agent -- a PTH analogue like teriparatide. Your scores have dropped a lot quickly, so I definitely recommend treatment.
I do understand why you are hesitant. These drugs can cause more harm than good when they are not used correctly, and many of my patients are concerned after reading others’ experiences. But when used properly, generally for three to five years, they are very good at preventing fractures.
Scoliosis can affect the accuracy of DXA tests, but the tests usually underestimate the severity of osteoporosis in people with scoliosis. Furthermore, the fact that both your hip and spine have osteoporosis makes the diagnosis pretty certain. A Z-score of 0 means your bone density is at a level that is expected for your age and sex. This means that a secondary cause is unlikely; however, the T-score still indicates osteoporosis. Four months may seem like a long time, but it’s entirely reasonable to wait on deciding the optimal medication until you see the expert. She is likely to do additional testing and give you more personalized information. Finally, if you can improve your diet and exercise from the standpoint of osteoporosis, this will help you whether you decide to take medication or not. Getting enough protein and calcium from your diet will help, as will avoiding excess alcohol.
Dr. Roach regrets that he is unable to answer individual questions, but will incorporate them in the column whenever possible. Readers may email questions to ToYourGoodHealth@med.cornell.edu.

RENOVATIONS THAT HELP SELL HOMES
Minor bathroom remodel: Remove dated wall coverings, replace fixtures, consider reglazing or replacing an old tub, and swap old shower doors for fast fixes in a bathroom. If there’s more room in the budget, replacing a tub, tile surround, floor, toilet, sink, vanity, and fixtures can cost roughly $10,500, says HGTV.You’ll recoup an average of $10,700 at resale, making a minor bathroom remodel a potentially worthy investment.
Pupil function: The eye doctor will briefly shine a handheld light into each eye and then move the light away to watch how the pupils constrict and dilate.
RENOVATIONS THAT PROVIDE A STRONG RETURN ON INVESTMENT
Minor kitchen remodel: According to the “2024 U.S. Houzz & Home Study” from Houzz Research, kitchens were the most popular rooms to renovate in 2023. There’s no denying the appeal of a newly renovated kitchen, and homeowners considering such a project may be happy to learn that a minor kitchen remodel that costs an average of around $27,000 provides a 96 percent ROI. Bankrate. com notes that minor kitchen remodels typically keep the current kitchen design, size and layout intact, but these projects may involve painting walls, refreshing backsplash, replacing lighting and plumbing fixtures, and changing cabinet hardware and facades.

A cataract is a clouding of the naturally transparent lens of the eye that can occur as one ages. Natural proteins in the lens clump together and can cloud the eye. The National Eye Institute says most cataracts develop with age, as more than half of all Americans age 80 or older either have cataracts or have had surgery to remove them. But cataracts can occur for other reasons, such as after an eye injury or after surgery for another vision problem. The American Academy of Opthalmology says cataracts can make it seem like a person is looking through a foggy or dusty car windshield. Everything is hazy, blurry or less colorful. Additional vision changes associated with cataracts include extra light sensitivity; seeing bright colors as faded or yellowed; seeing double or a ghosted image; and difficulty seeing at night. Using brighter lights at home or getting a new eyeglasses prescription can help manage cataracts in their early stages. An eye doctor may suggest surgery if cataracts are getting in the way of daily activities. Surgery removes the clouded lens and replaces it with a new, artificial lens.

Keep Your Pets Safe During The Holidays
DEAR PAW’S CORNER: Last anksgiving, we had the scare of our life. Our little dog “Mamie” jumped onto the counter and ate a huge amount of the stuffing that I had just scooped out of the turkey! She got sick almost immediately and threw up, but because she kept drooling a erward I rushed her to an emergency vet. Mamie spent a few days at the vet getting supportive care because the ingredients in the stuffing affected her so badly. She’s completely recovered now, but please warn your readers to keep pets away from the kitchen and dining room during holiday festivities. -- Paul and Diane G., Norwood, Massachusetts
DEAR PAUL & DIANE: I am so relieved to hear that Mamie is OK! As you noted, many of the delicious, richfoodsweeatduring anksgivingand Christmas contain ingredients that are poisonous to dogs and cats. Onions are one culprit, but garlic, avocados, grapes (and raisins), macadamia nuts and chocolate can also cause serious illness and death if ingested. Foods or candies sweetened with xylitol can be deadly to pets. Alcohol and caffeine can also sicken them.
While preparing the feast, keep pets out of the kitchen. When guests arrive and you begin to serve treats and drinks, that’s the time to sequester your pets for the duration. Place them in a quiet room with water, treats and their favorite toys. is keeps them calm and allows you to focus on your guests. It also prevents them from getting into foods that could harm them.
Finally, for pet owners who think, “My big dog ate a chocolate bar once and it didn’t hurt him at all” -- it’s a bad idea to assume that your dog isn’t at risk because they got through an earlier scare unscathed Protect your pets this holiday season, and enjoy many more to come. Most states don’t have a licensing requirement for pet sitters, but you can get a certification through Pet Sitters International (petsit.com), or the National Association of Professional Pet Sitters (petsitters.org). ese organizations offer a wealth of information -including how to find clients. Most of all, have fun! You’re getting into this business because you love pets, and that will be a big factor in your success Send your tips, comments or questions to ask@pawscorner.com.

Hair loss can affect quality of life and self-esteem. While it is common to lose some hair each day as part of the natural growing cycle, hair lost during the growth cycle will grow back for most people. But with hormonal conditions, illnesses, aging, and inherited conditions, new strands of hair don’t always grow back. The Cleveland Clinic says that a person cannot prevent all types of hair loss, but there are steps to keep hair healthy and minimize loss. Coping with stress in a healthy way; managing thyroid diseases or other hormonal conditions that could lead to hair loss; eating a diet that includes enough calories, protein and iron; avoiding hairstyles that pull tightly; and trying a cooling cap during chemotherapy treatment are steps one can take to minimize hair loss.

Skin cancer is the most common type of cancer worldwide. In the United States, one in five people will develop skin cancer by the age of 70, and more than two people die of skin cancer in the U.S. every hour, says the Skin Cancer Foundation. Roughly 90 percent of non-melanoma skin cancers are associated with exposure to UV radiation from the sun. Melanoma also is inextricably linked to UV exposure. In the United Kingdom, nearly nine in 10 cases of melanoma can be prevented by staying safe in the sun.The American Cancer Society estimates about 100,000 new melanomas will be diagnosed in 2024, when roughly 8,290 people are expected to die of melanoma. The number of people getting skin cancer in Canada also has risen at a fairly constant rate over the last 30 years.
One in 73 Canadian women and one in 59 Canadian men will develop the disease, according to Canada.ca. Early detection of skin cancer can improve prognosis.

CLASSIC WINE AND FOOD PAIRINGS
Congruent pairings, according to Wine Folly, create balance in the mouth by amplifying shared flavor compounds. For example, earth wines paired with earthy mushrooms. Contrasting pairings create balance through contrasting flavors. Therefore,a salad with acidity and bitterness may be complemented with a wine that is sweeter and not so acidic, or so may be the case with a fatty, salty food like foie gras.

Perhaps more surprisingly, a greater number of respondents indicated they prefer to dine out on Monday (23 percent) than Friday or Saturday. Despite these preferences, experienced diners know that it’s still wise to bookareservationwhendining out on Friday, Saturday and Sunday. WHAT TO EXPECT AT YOUR NEXT EYE EXAM Tonometry measurement: Glaucoma occurs when intraocular pressure is high. Some eye doctors will conduct tests to determine the pressure. One way is with a non-contact tonometry test, which is a puff of air squirted at each eye. With applanation tonometry, the doctor first administers a mild numbing agent to the eye, and then uses a pressure-sensitive instrument to gently tap on the surface of the eye to get a pressure reading. Dangerous E-Bike Riders
I left my wallet/phone in a cart at Henrietta Wegmans. When I realized it, I went into major panic mode! I drove back to Wegmans, nearly hyperventilating. At the Customer Service Desk, I saw it on their back counter, and we were reunited. I can’t even explain the relief I felt. To the person who turned it in - you are the best that ever lived!! You probably can imagine how panicked I was. I wish I could give you a big hug!! Thank you, thank you, thank you SOOO much!! God bless you!
Lately, I’ve come close to hitting e-bike riders riding on the wrong side of the road, in the dark, without lights and going way too fast. Bikes in their purest form are designed to help the rider get an assist going up hills. Some e-bikes are nothing more than simple motorcycles going way too fast and, in this case, riding on the wrong side of the road. You are a danger to yourself and others.
Help the Reindeer and The Birds
Simple act of kindness
Thank you to the gentleman who stopped November 14 on Summers Street, Livonia to check on my welfare. I had just given blood and felt faint which prompted me to sit down on the ground. His kindness and concern warmed my heart and reminded why I love small town people. A simple act of kindness changes everything!
Around this time of year there is a craft project often done with young kids to make reindeer food. This mix of seed, grains and glitter is sprinkled outside to feed Santa’s reindeer. It is mostly eaten by birds and other wild animals. Adding glitter to this food endangers the life of animals. The chemicals in glitter can damage immune and reproductive systems. Also, most glitter is made from PET plastic. It can break down and release chemicals that disrupt animal hormones. When making reindeer food, please leave out this toxic ingredient and make a product that is safe for all. Your kids won’t miss the glitter and you’ll be helping out wildlife.

Safety Council analysis of data from the National Highway Traffic Safety Administration indicates that motorcyclists accounted for 14 percent of all traffic fatalities, 17 percent of all occupant fatalities and 3 percent of all occupant injuries in 2021. More than 5,900 motorcyclist deaths occurred in 2021, a figure that far surpassed any other year between 2007 and 2021. And it wasn’t just fatal injuries that rose, as the NSC reports that nonfatal injuries among motorcyclists increased by 5 percent between 2020 and 2021. Recognition of these figures is important, and it’s equally notable that 83 percent of motorcyclist fatalities occurred in good weather, and a little more than half (53 percent) involved two vehicles.
