FRIDAY, JANUARY 22, 2016
TO YOUR GOOD HEALTH By Keith Roach, M.D.
Can Heart Surgery Cause Hot Flashes? DEAR DR. ROACH: I have had hot flashes, similar to those experienced by post-menopausal women, ever since my coronary bypass in 1990. I can be sitting quietly in a chair reading or at my computer, and suddenly I find myself dripping with perspiration. I also have had panic attacks, with increased heart rate and other physical symptoms. A few years ago, a 24-hour EKG showed heartrhythm irregularities and that my heart stopped briefly while asleep. If these idiosyncrasies of my autonomic nervous system are a consequence of my time on the heart-lung machine for the bypass, I’m not complaining -- just curious. You said that studies have been done on almost everything. Is this one? -- W.S.B. ANSWER: I found that a lot of people complain of similar symptoms. I also found that indeed, the autonomic nervous system (the part of the nervous system that regulates temperature, sweat, blood pressure and many other critical aspects of body function without our being consciously aware of it) can be adversely affected by cardiac surgery. Some authors have suggested that the autonomic nervous system changes are one mechanism by which people are more likely to have depression after cardiac surgery. I found several possible explanations why the ANS changes with bypass surgery, but no clear consensus. I would want to be sure that there are no other causes of these symptoms. It sounds like you have had an extensive evaluation, but I certainly would be concerned about a sudden fast heart rhythm, such as atrial fibrillation. The booklet on abnormal heart rhythms explains atrial fibrillation and the more common heart rhythm disturbances in greater detail. Readers can obtain a copy by writing: Dr. Roach -- No. 107W, 628 Virginia Drive, Orlando, FL 32803. Enclose a check or money order (no cash) for $4.75 U.S./$6 Canada with the recipient’s printed name and address. Please allow four weeks for delivery. *** DEAR DR. ROACH: I suffered a long time with GERD. I was diagnosed with Barrett’s esophagus, a precancerous disease. My doctor recommended a procedure called a Nissen fundoplication. Since the procedure, I have not had heartburn again. I would do it again, after what I went through. I get an endoscopy every two years to keep an eye on my disease. -- G.D.B. ANSWER: Barrett’s esophagus is a complication of longstanding reflux. It is diagnosed when the lining of the esophagus starts to look like the lining of the stomach. Barrett’s esophagus can progress to cancer of the esophagus. About 0.2 percent of those with Barrett’s per year will develop cancer, which sounds low, but if you live with the condition for 25 years, that’s about a 5 percent risk. Surgery for reflux disease is done only rarely. However, it is very effective (85 percent to 90 percent). The most common reason to consider surgery is inadequate relief from medications and lifestyle changes. However, severe erosions from acid in the esophagus, inability to take medications, a stricture (partial closure) of the esophagus and Barrett’s esophagus are all reasons to consider surgery. Surgery appears to reduce the risk of cancer more than other treatments.
*** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Readers may email questions to ToYourGoodHealth@med.cornell.edu. Kendall Students Learn About Immigrant Experience Day Teachers on the Kendall Eighth-Grade Junior High Team recently organized an Immigrant Experience Day for their students. Eighth-grade students had read stories by immigrants from other countries and studied the history of immigration in the United States. They had an opportunity to hear a first-hand account of immigration from a Kendall staff member and sampled ethnic food from many countries. The Immigrant Experience Day is part of the eighth-grade team’s method of finding new ways to incorporate what the students are learning into their lives. Interdisciplinary units allow the students to better retain ideas and concepts that they are learning in the classroom, and it gives the students the opportunity to expand their knowledge and apply it to different subjects. Students listened to the personal story of Mirjam Bauer, a counselor in the high school, who became a naturalized United States citizen after her time spent here as an exchange student. She shared her experiences of adapting to the culture of the United States and her ties with her family in Holland. Bauer also shared the process involved in becoming a citizen and the citizenship test that she had to take. Eighth-graders in Marlene MaglioccoMorrow’s ELA class read “Inside Out & Back Again” by Thanhha Lai, a critically acclaimed novel about a young Vietnamese girl who flees Saigon during its fall. Students in Morgan Engelbert’s social studies class studied immigration and the social and

Students sample immigrant foods political ramifications experienced by those who come to the United States. Gretchen Rosales’ Spanish 8 students read “The Circuit: stories from the life of a migrant child” by Francisco Jimenez, which details the story of a young child trying to find his voice in society and highlights his struggle to belong. Members of the eighth-grade team also brought in food from cultures that represent their ancestry and heritage. Math teacher Kaitlyn Curtis made German chocolate cookies; MaglioccoMorrow prepared antipasto; Rosales made Mexican rice; and science teacher David Heffron provided tortilla chips and salsa. The students were excited to try ethnic foods and many were surprised that they enjoyed the different tastes from around the world.

Preparing for hospice care Sometimes patients succumb to a disease, in spite of the best treatments and support. When the time comes for a family to confront a loved one’s declining health, hospice care can be a dignified way to make that person’s final days as comforting as possible. Hospice is a type of care designed to make the final moments of a person’s life as pain-free and manageable as possible. It can take place in a medical facility, but very often hospice care occurs at home or wherever the ill person feels most comfortable. Hospice care is usually the last step when all other options have been exhausted. Making the decision to move a loved one to hospice care can be an emotionally wrenching time. Decisions such as these may be better made at a time in a person’s life when he or she is not sick. That is why living wills that spell out details for end-of-life care can be quite valuable. Decisions are made with a level head and not wrought with emotions. Such living wills also can take the pressure off of family members who may not feel comfortable making such decisions on their own or in concert with relatives. Hospice is a type of care and a philosophy that focuses on the palliative care of terminally ill patients. Rather than providing medication to try to treat the illness, medicine is offered to make a person more comfortable and remove any pain. Psychological therapy may also be offered to help the person come to terms with the end of life. Although hospice is a concept that has gradually evolved since the 11th century, the principles of modern hospice care can trace their roots to the 1950s and Dame Cicely Saunders, a nurse and social worker who is known as the creator of the hospice movement. If hospice care is outlined in a dying person’s living will or expressed wishes, there are some guidelines that can be followed by the sick individual. • Visit various medical care centers to see if away-fromhome options provide the care and environment you desire. Be sure to establish what type of hospice care you prefer. This may

include care within a nursing home or hospital, or in the comfort of your own home. • Sign a medical durable power of attorney. This signed document gives authority to an adult age 18 or older, who then has the right to make necessary medical and healthcare decisions for you in the event you become incapacitated. • Clearly indicate your wishes regarding resuscitation. A signed document may alert emergency healthcare personnel or others of your wishes not to be resuscitated. This document must be signed by you and a doctor and witnessed by others. • Decide on the duration of care. Hospice care generally lasts six months. This enables a long stretch of palliative care. Should you live beyond the time period, hospice care can continue. Studies have shown that patients who receive hospice care for at least 30 to 60 days gain greater benefit than those who are placed in hospice only in their final days. • Investigate financial options for hospice. Medicare and Medicaid provide hospice coverage in 44 states. Many private insurance plans have a hospice benefit as well. Understand how payment is made to figure out if private funding will be necessary. Not all hospice programs are the same. Certain core models of care are followed by many hospice facilities, but options vary. Hospice can provide comfort in a person’s final days and help sufferers better enjoy life and time spent with loved ones in these last days.
MOMENTS IN TIME The History Channel • On Feb. 2, 1980, details of ABSCAM, an FBI sting operation to uncover political corruption in government, are released. The Memorial Day Detachment # 468 of the Marine Corps League would like to thank everyone who participated in the Toys For Tots campaign for 2015. We appreciate the organizations and businesses who participated as drop off points and the generosity of everyone in Ontario and Livingston Counties who donated toys. We were able to collect and distribute 4,000 toys through our participating agencies. A lot of children had a great Christmas because of all of you. Thank you and we look forward to 2016. They left before I did; they glanced over and gave me a nice smile. After they left the waitress came over and told me they had paid for my breakfast. I don’t know if they are local or just passing through. I just want to say Thank You to them. I will pass it on. A Grateful Senior Citizen The residents of Lakeville Estates want to give a huge THANK YOU to all the local merchants who were so generous with their donations of gift cards and merchandise to use as door prizes at our Community Christmas Party. Gratefully Yours, Residents of Lakeville Estates Hats off to Caledonia Mayor Debbie Davis for securing a third grant for the culverts on Mill Street. Today’s college kids are well aware of the job market that awaits them upon graduation. With an economy still on the mend and unemployment rates still high, college students across the country know that their post-graduation dream job is no guarantee. As daunting as entering the job market amidst the ongoing economic woes might seem, college students or even recent graduates can take steps to make themselves more attractive to prospective employers. Perhaps no such step has been more popular over the years than securing an internship. Internships are designed to give current students or recent graduates an entry into a field that interests them, providing an opportunity to gain hands-on experience and pad their resumes in the meantime. As valuable as a good internship can be, it can also prove a lost opportunity for those students who don’t recognize the opportunity at hand. To make the most of an internship, students or recent graduates should consider the following suggestions: • Find one that’s the right fit. Some students accept an internship simply because it looks good on a resume. While it’s true an internship stands out to hiring managers or other human resources personnel, students must find the right fit to make the experience truly valuable. For example, students who accept an internship outside of their major might not approach it with the same work ethic they would if they were to find an internship in a field they want to work in. That indifference, coupled with the lack of financial compensation commonplace with most internships, might lead to a less inspired effort, which won’t lead to a strong recommendation from bosses down the road. The recommendation could prove as important as the internship itself when it comes time to look for full-time work. So when seeking an internship, students should not simply settle on one because it’s offered. Rather, students should choose an internship that’s in a field they’re genuinely interested in. • Routinely meet with supervisors. Because most internships are during the summer, interns have a way of getting lost in the shuffle while their supervisors are away on vacation. Interns should meet, or at least discuss via the telephone or e-mail, regularly with supervisors to review their performance. This exhibits enthusiasm and a willingness to learn, while also putting interns in a position to get the most out of their internships. • Treat the internship like a lucrative full-time position. At times, being an intern can prove pretty awful. Most interns don’t get paid, despite working long hours and wearing many hats for their employers. As difficult as it may seem at times, interns should treat their internships as if they’re getting paid lots of money. That means showing up on time, working hard, staying late if necessary, and most of all, not complaining. Negative attitudes have ruined many an intern. Interns must remember that an internship is not a right, but a privilege, one that can lay the foundation for a successful career. Treating an internship like a lucrative position helps interns stay focused and work hard, even when it’s tempting to kiss that nonexistent paycheck and long hours goodbye. • Don’t be pigeon-holed. Some internships are better than others, and the best ones often rotate interns throughout several departments. That enables interns to learn more than one aspect of the business, possibly helping them choose the area they like most and the area they would prefer to avoid when it comes time to job hunt. Even if a company does not rotate its interns, that doesn’t mean it’s impossible for an intern to learn more than his or her job entails. Before going it alone, interns should discuss with their immediate supervisor if it’s possible to learn additional aspects of the business. Offer to come in on off days or stay late to see how the whole operation works. This also allows interns to meet more people, which can only help when the time comes to find fulltime work. • Remember it’s only temporary. Some interns love their internships and don’t mind working free for long hours. Others are not so lucky and fight the urge to quit on a daily basis. For the latter, the best advice is to remember an internship is only temporary. As hard as it can be to get out of bed and go work for no pay all day, it’s imperative interns keep a positive attitude regardless of how little they like their internship. Internships last roughly three months, so even the worst one with the worst boss will be over soon and students will still be able to make a valuable addition to their resume.
